Abstract:
The Master Plan for the Loughborough University of Technology is a 143-page document that gathers the work undertaken by the institution to become a university, thus benefiting from the educational policies derived from the 1963 Robbins report in Britain. Arup Associates authored in 1966 a proposal whose main characteristic is its ascription to an infinite grid strategy and a systematized project. The different diagrams and growth schemes represent the geometric synthesis of some compositional and constructive rules: three grids overlap to produce a germ drawing to which a growth pattern is added for its territorial extension. For the sake of flexibility and adaptability, the project tries to avoid architectural obsolescence through the achievement of a “universal space unit”. Hence, a “discipline” is established whose definition turns out to be a succession of limitations. Through the reconstruction of the design process for the Loughborough University, the multiple meanings of the limit concept are portrayed in parallel to its idea of ​​a continuous and endless campus. A strict internal order, an intentionally open reading of the territory and a constructive standardization produce a kind of visual exhaustion of the whole that could be understood as a limit of spatial nature.

SOUTH JAVA CURRENT. Indian Ocean is influenced by the monsoon cycle. Monsoon cycle that occurs in the Indian Ocean influences the current patterns. Since a very strong westerlies occurs in The Tropical Convergence Zone (TCZ) at the equator, wyrtki jet is formed in the 40° - 80° W Indian Ocean on a transitional season. Its branchs are possibly formed when the wyrtki jet is in the equatorial west coast of Sumatra to the north and south along the western coast of Sumatra which eventually creates South Java Current (SJC). SJC flows southeastward during December–April and northwestward during June–October, when it is associated with coastal upwelling. SJC develop upwelling on a seasonal basis.

Data from audited financial statements in year 2006 and 2007 weretaken from Indo-exchange files. The statistic method used to test hypothesis is a liner multipleregression Two models were considered: levels and return models.The statistical result of this shows that accounting income, cash flows and components ofcash flows are significantly associated with stock price in levels model, in contrast,the resultof this study shows that all independent variables (total cash flows, components of cashflows,and accounting income) are not associated with stock return in return model, exceptcash flows operation is significantly associated with stock return.Keywords : Information content, Cash Flows, Component of cash flows, Levels model, Return model

Abstract:
ABSTRACT An ampicillin enrichment strategy following transposon insertion mutagenesis was employed to obtain NaCl-sensitive mutants of a gltBD (glutamate synthase [GOGAT]-deficient) strain of Escherichia coli. It was reasoned that the gltBD mutation would sensitize the parental strain even to small perturbations affecting osmotolerance. Insertions conferring an osmosensitive phenotype were identified in the proU, argP (formerly iciA), and glnE genes encoding a glycine betaine/proline transporter, a LysR-type transcriptional regulator, and the adenylyltransferase for glutamine synthetase, respectively. The gltBD+ derivatives of the strains were not osmosensitive. The argP mutation, but not the glnE mutation, was associated with reduced glutamate dehydrogenase activity and a concomitant NH4 + assimilation defect in the gltBD strain. Supplementation of the medium with lysine or a lysine-containing dipeptide phenocopied the argP null mutation for both osmosensitivity and NH4 + assimilation deficiency in a gltBD background, and a dominant gain-of-function mutation in argP was associated with suppression of these lysine inhibitory effects. Osmosensitivity in the gltBD strains, elicited either by lysine supplementation or by introduction of the argP or glnE mutations (but not proU mutations), was also correlated with a reduction in cytoplasmic glutamate pools in cultures grown at elevated osmolarity. We propose that an inability to accumulate intracellular glutamate at high osmolarity underlies the osmosensitive phenotype of both the argP gltBD and glnE gltBD mutants, the former because of a reduction in the capacity for NH4 + assimilation into glutamate and the latter because of increased channeling of glutamate into glutamine.

Sungai Sambas merupakan suatu sungai yang bermuara di perairan Kabupaten Pemangkat Kalimantan Barat. Perairan tersebut mempunyai banyak peran terhadap aktivas jalur pelayaran dan pelabuhan kapal yang mempengaruhi kondisi dinamika perairan. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh debit aliran sungai terhadap pola arus pasang surut di Muara Sungai Sambas. Kajian pemodelan pola arus terhadap pengaruh debit aliran sungai ini dilakukan menggunakan Surface Water Modeling System (SMS) pada modul Resources Management Associates-2 (RMA-2), software ini not free access sehingga dalam penelitian ini linsensi yang digunakan berupa uji coba (trial) dengan batas waktu dua minggu penggunaan. Simulasi model pola arus laut dilakukan dengan empat skenario yaitu debit aliran sungai 0 m3/s, 100 m3/s, 500 m3/s dan 900 m3/s. Hasil dari simulasi pada skenario dengan nilai debit aliran 0 m3/s pola arus yang terjadi di Sungai Sambas merupakan arus harmonik dimana pengaruh debit aliran sungai masih rendah. Pada skenario dengan debit aliran sungai 100 m3/s pergerakan pola arus minimum yang terjadi dua arah yaitu pergerakan arus pada hulu sungai dan di muara sungai sama dan bertemu pada satu titik di arus maksimum. Skenario dengan debit aliran yang lebih besar yaitu 500 m3/s dan 900 m3/s menunjukkan pola arus yang terjadi merupakan arus non-harmonik dimana pada saat arus maksimum dan minimum terjadi pergerakan pola arus yang sama yaitu dominan menuju muara sungai.

Abstract:
Background: This study aimed to compare phenotype–genotype correlation in patients with Usher syndrome (USH) to those with autosomal recessive retinitis pigmentosa (NS-ARRP) caused by genes associated with Usher syndrome. Methods: Case notes of patients with USH or NS-ARRP and a molecularly confirmed diagnosis in genes associated with Usher syndrome were reviewed. Phenotypic information, including the age of ocular symptoms, hearing impairment, visual acuity, Goldmann visual fields, fundus autofluorescence (FAF) imaging and spectral domain optical coherence tomography (OCT) imaging, was reviewed. The patients were divided into three genotype groups based on variant severity for genotype-phenotype correlations. Results: 39 patients with Usher syndrome and 33 patients with NS-ARRP and a molecular diagnosis in an Usher syndrome-related gene were identified. In the 39 patients diagnosed with Usher syndrome, a molecular diagnosis was confirmed as follows: USH2A (28), MYO7A (4), CDH23 (2), USH1C (2), GPR98/VLGR1 (2) and PCDH15 (1). All 33 patients with NS-ARRP had variants in USH2A. Further analysis was performed on the patients with USH2A variants. USH2A patients with syndromic features had an earlier mean age of symptom onset (17.9 vs. 31.7 years, p < 0.001), had more advanced changes on FAF imaging (p = 0.040) and were more likely to have cystoid macular oedema (p = 0.021) when compared to USH2A patients presenting with non-syndromic NS-ARRP. Self-reported late-onset hearing loss was identified in 33.3% of patients with NS-ARRP. Having a syndromic phenotype was associated with more severe USH2A variants (p < 0.001). Eighteen novel variants in genes associated with Usher syndrome were identified in this cohort. Conclusions: Patients with Usher syndrome, whatever the associated gene in this cohort, tended to have an earlier onset of retinal disease (other than GPR98/VLGR1) when compared to patients presenting with NS-ARRP. Analysis of genetic variants in USH2A, the commonest gene in our cohort, showed that patients with a more severe genotype were more likely to be diagnosed with USH compared to NS-ARRP. USH2A patients with syndromic features have an earlier onset of symptoms and more severe features on FAF and OCT imaging. However, a third of patients diagnosed with NS-ARRP developed later onset hearing loss. Eighteen novel variants in genes associated with Usher syndrome were identified in this cohort, thus expanding the genetic spectrum of known pathogenic variants. An accurate molecular diagnosis is important for diagnosis and prognosis and has become particularly relevant with the advent of potential therapies for Usher-related gene

Purpose of the studyTo explore which factors increase the likelihood of being deemed appointable to core anaesthesia training in the UK and whether those factors subsequently predict performance in postgraduate training.Study designObservational study linking UK medical specialty recruitment data with postgraduate educational performance, as measured by Annual Review of Competence Progression (ARCP) outcomes. Data were available for 2782 trainee doctors recruited to anaesthesia core training from 2012 to 2016 with at least one subsequent ARCP outcome.ResultsBoth higher interview and shortlisting scores were independent and statistically significant (p≤0.001) predictors of more satisfactory ARCP outcomes, even after controlling for the influence of postgraduate exam failure. It was noted that a number of background variables (eg, age at application) were independently associated with the odds of being deemed appointable at recruitment. Of these, increasing age and experience were also negative predictors of subsequent ARCP rating. These influences became statistically non-significant once ARCP outcomes associated with exam failure were excluded.ConclusionsThe predictors of ‘appointability’ largely also predict subsequent performance in postgraduate training, as indicated by ARCP ratings. This provides evidence for the validity of the selection process. Our results also suggest that greater weight could be applied to shortlisting scores within the overall process of ranking applicants for posts.

(1) Background: NR2E3 encodes a nuclear receptor transcription factor that is considered to promote cell differentiation, affect retinal development, and regulate phototransduction in rods and cones. This study aimed to analyze the clinical characteristics and observe the prognosis of autosomal dominant retinopathy (ADRP) and autosomal recessive retinopathy (ARRP) associated with NR2E3; (2) Methods: NR2E3 variants were collected from our exome sequencing data and identified per the American College of Medical Genetics and Genomics criteria. Data from our cohort and a systemic literature review were conducted to explore the NR2E3 variants spectrum and potential genotype-phenotype correlations; (3) Results: Nine pathogenic variants/likely pathogenic variants in NR2E3, including five novel variants, were detected in eight families (four each with ADRP and ARRP). Follow-up data showed schisis/atrophy in the macula and retinal degeneration initiation around the vascular arcades with differences in ADRP and ARRP. A systemic literature review indicated patients with ADRP presented better visual acuity (p < 0.01) and later onset age (p < 0.0001) than did those with ARRP; (4) Conclusions: Macular schisis and retinal degeneration around vascular arcades may present as the prognosis of NR2E3-retinopathy, dominant, or recessive. Our data might further enrich our understanding of NR2E3 variants and associated inherited retinopathy.

The aim of surgical training across the 10 surgical specialties is to produce competent day 1 consultants. Progression through training in the UK is assessed by the Annual Review of Competency Progression (ARCP).ObjectiveThis study aimed to examine variation in ARCP outcomes within surgical training and identify differences in outcomes between specialties.DesignA national cohort study using data from the UK Medical Education Database was performed. ARCP outcome was the primary outcome measure. Multilevel ordinal regression analyses were performed, with ARCP outcomes nested within trainees.ParticipantsHigher surgical trainees (ST3–ST8) from nine UK surgical specialties were included (vascular surgery was excluded due to insufficient data). All surgical trainees across the UK with an ARCP outcome between 2010 and 2017 were included.ResultsEight thousand two hundred and twenty trainees with an ARCP outcome awarded between 2010 and 2017 were included, comprising 31 788 ARCP outcomes. There was substantial variation in the proportion of non-standard outcomes recorded across specialties with general surgery trainees having the highest proportion of non-standard outcomes (22.5%) and urology trainees the fewest (12.4%). After adjustment, general surgery trainees were 1.3 times more likely to receive a non-standard ARCP outcome compared with trainees in trauma and orthopaedics (T&O) (OR 1.33, 95% CI 1.21 to 1.45, p=0.001). Urology trainees were 36% less likely to receive a non-standard outcome compared with T&O trainees (OR 0.64, 95% CI 0.54 to 0.75, p<0.001). Female trainees and older age were associated with non-standard outcomes (OR 1.11, 95% CI 1.02 to 1.22, p=0.020; OR 1.04, 95% CI 1.03 to 1.05, p<0.001).ConclusionThere is wide variation in the training outcome assessments across surgical specialties. General surgery has higher rates of non-standard outcomes compared with other surgical specialties. Across all specialties, female sex and older age were associated with non-standard outcomes.

Model Hidrodinamika Resources Management Associates- 2 (RMA-2) diaplikasikan untuk mempelajari kondisi arus di perairan pantai Kijing. Tujuan penelitian ini adalah untuk mengkaji bagaimana kondisi arus pada kondisi puncak musim barat. Masukan model adalah data elevasi pasang surut yang diberikan pada bagian batas model, data batimetri perairan serta data angin. Verifikasi model dilakukan dengan membandingkan hasil model terhadap pengukuran di lapangan yang diperoleh nilai Root Mean Square Error (RMSE) elevasi 0,192 dan arus 0,019. Nilai RMSE untuk elevasi menunjukkan perkiraan yang sesuai dengan data pengukuran, sedangkan pada kecepatan arus hasil model memiliki perkiraan yang lebih kecil dari data pengukuran. Berdasarkan hasil simulasi model, elevasi muka air laut terendah dan tertinggi pada musim barat sebesar -0,7105 dibawah tinggi muka air laut rata-rata dan 0,9306 m diatas tinggi muka air laut rata-rata. Kecepatan dan arah arus saat surut menuju pasang berkisar antara 0,004 s.d. 0,016 dengan arah cenderung ke barat laut hingga timur dan saat pasang tertinggi berkisar antara 0,033 s.d. 0,034 dengan arah bergerak ke timur tenggara. Selanjutnya kecepatan dan arah arus saat pasang menuju surut berkisar antara 0,044 s.d. 0,048 dengan arah bergerak ke timur tenggara, dan saat surut terendah 0,046 s.d. 0,053 juga arah timur tenggara.

The Association for Respiratory Technology & Physiology (ARTP) last produced a statement on the performance of lung function testing in 1994. At that time the focus was on a practical statement for people working in lung function laboratories. Since that time there have been many technological advances and alterations to best practice in the measurement and interpretation of lung function assessments. In light of these advances an update was warranted. ARTP, therefore, have provided within this document, where available, the most up-to-date and evidence-based recommendations for the most common lung function assessments performed in laboratories across the UK. These recommendations set out the requirements and considerations that need to be made in terms of environmental and patient factors that may influence both the performance and interpretation of lung function tests. They also incorporate procedures to ensure quality assured diagnostic investigations that include those associated with equipment, the healthcare professional conducting the assessments and the results achieved by the subject. Each section aims to outline the common parameters provided for each investigation, a brief principle behind the measurements (where applicable), and suggested acceptability and reproducibility criteria.

Abstract:
The elucidation of mechanisms regulating the regeneration and survival of pancreatic beta cells has fundamental implications in the cell therapy of type 1 diabetes. The present study had the following three aims: 1. to investigate whether pancreatic ductal epithelial cells can be induced to differentiate into insulin-producing cells by exposing them to hepatocyte growth factor (HGF); 2. to characterize some of the molecular events leading to their differentiation toward a beta-cell-like phenotype; 3. to evaluate the susceptibility of newly differentiated insulin-secreting cells to cytokine-induced apoptosis, a mechanism of beta-cell destruction occurring in type 1 diabetes. We demonstrated that HGF-treated rat pancreatic ductal cell line (ARIP) cells acquired the capability to transcribe the insulin gene and translate its counterpart protein. HGF-treated cells also exhibited a glucose-dependent capability to secrete insulin into the cultured medium. Expression analysis of some of the genes regulating pancreatic beta-cell differentiation revealed a time-dependent transcription of neurogenin-3 and Neuro-D in response to HGF. Finally, we determined the susceptibility to proinflammatory cytokine (PTh1)-induced apoptosis by incubating HGF-treated and untreated ARIP cells with a cocktail of interleukin-1 beta (IL-1β), tumor necrosis factor-alpha (TNF-α) and interferon-gamma (IFN-γ). Such treatment induced apoptotic death, as determined by the TUNEL technique, in about 40% of HGF-treated, insulin-secreting ARIP cells, while untreated ARIP cells were resistant to PTh1-induced apoptosis. In conclusion, we showed that HGF promotes the differentiation of ARIP cells into pancreatic beta-cell-like cells, and that the differentiation toward an insulin-secreting phenotype is associated with the appearance of susceptibility to cytokine-induced apoptosis.

Existentialism, a very important literary movement of the twentieth century, occupies a very important place in the world literature. The major proponents of this theory are Soren Kierkegaard, Friedrich Nietzsche, Jean Paul Sartre, Albert Camus and others. The key features associated with the existential literature are the depiction of sense of alienation, detachment, loneliness, aloofness, anxiety among the denizens of the modern world. Although, to begin with it was a European phenomenon, it soon spread to other parts of the world and impacted the writers all over the world. India also came under its influence. Many Indian writers exhibited existential traits in their works. Arun Joshi was the most prominent of them. His works, especially all novels, very beautifully incorporates existential elements. The present paper intends to study and exhibit the existential traits in the novels of Arun Joshi.

Abstract PURPOSE: The purpose is to study the anatomical outcome of eyes in aggressive retinopathy of prematurity (AROP), treated with laser photocoagulation alone and to evaluate factors affecting outcomes. METHODS: Records of consecutive babies diagnosed with AROP, undergoing laser photocoagulation treatment in rural tertiary care centers from October 2016 to January 2021 were reviewed retrospectively. Anatomical outcome at 6 months follow-up was grouped as good in eyes with complete regression and poor in those who developed retinal detachment (stage IV a, IV b, and V). Both groups were compared with respect to the period of gestation, birth weight (BW), age at screening, age at treatment, zone of disease, presence of retinal fibrovascular proliferation (FVP), tunica vasculosa lentis, preretinal bleed, need for supplement laser, and associated systemic risk factors. RESULTS: Of the total of 2468 babies screened, 124 (5.02%) were diagnosed with severe retinopathy of prematurity (ROP), of which 54 (43.5%) lasered AROP babies were analyzed. Mean BW and gestation period of the AROP cohort were 1.43 kg and 31.1 weeks, respectively. Eighty-six eyes (79.6%) had good outcomes with laser photocoagulation alone. Posterior location of disease, presence of FVP, neonatal sepsis, shock, and late screening for ROP were found to be factors associated with poor outcomes. CONCLUSION: Adequate and timely treatment with laser photocoagulation in AROP can achieve good treatment outcomes in a significant proportion of babies. Although a combined approach using laser, anti-vascular endothelial growth factor and early vitrectomy is better, laser remains a viable treatment option in AROP, especially with limited resources and high risk of loss to follow-up.

Clonal outbreaks due to azole-resistant Candida parapsilosis (ARCP) isolates have been reported in numerous studies, but the environmental niche of such isolates has yet to be defined. Herein, we aimed to identify the environmental niche of ARCP isolates causing unremitting clonal outbreaks in an adult ICU from a Brazilian cancer referral center. C. parapsilosis sensu stricto isolates recovered from blood cultures, pericatheter skins, healthcare workers (HCW), and nosocomial surfaces were genotyped by multilocus microsatellite typing (MLMT). Antifungal susceptibility testing was performed by the EUCAST (European Committee for Antimicrobial Susceptibility Testing) broth microdilution reference method and ERG11 was sequenced to determine the azole resistance mechanism. Approximately 68% of isolates were fluconazole-resistant (76/112), including pericatheter skins (3/3, 100%), blood cultures (63/70, 90%), nosocomial surfaces (6/11, 54.5%), and HCW’s hands (4/28, 14.2%). MLMT revealed five clusters: the major cluster contained 88.2% of ARCP isolates (67/76) collected from blood (57/70), bed (2/2), pericatheter skin (2/3), from carts (3/7), and HCW’s hands (3/27). ARCP isolates were associated with a higher 30 day crude mortality rate (63.8%) than non-ARCP ones (20%, p = 0.008), and resisted two environmental decontamination attempts using quaternary ammonium. This study for the first time identified ARCP isolates harboring the Erg11-Y132F mutation from nosocomial surfaces and HCW’s hands, which were genetically identical to ARCP blood isolates. Therefore, it is likely that persisting clonal outbreak due to ARCP isolates was fueled by environmental sources. The resistance of Y132F ARCP isolates to disinfectants, and their potential association with a high mortality rate, warrant vigilant source control using effective environmental decontamination.

Abstract Cellular changes in the adaptive immune system accompanies the aging process and contributes to an age-related immune phenotype (ARIP) characterized by decrease in naïve T (TN) cells and increase in memory T (TM) cells. However, a population level marker of ARIP associated with biological aging and age-related chronic conditions has not been evaluated previously. We developed two ARIP measures based on well understood age related changes in T cell distribution: TN/ (TCM (Central Memory) + TEM (Effector Memory) + TEFF (Effector)) or TN/ TM in CD4+ and CD8+ T cells. We compared them with more commonly used ARIP measures such as CD4/CD8 ratio and CD8+ TN cells by evaluating associations with chronological age and phenotypic age using linear regression and association with multimorbidity using multinomial logistic regression. CD8+ TN and TN/TM had the strongest inverse association with chronological age (beta estimates: -3.41, -3.61; p-value&lt; 0.0001). CD4+ TN/TM had the strongest inverse association with phenotypic age (beta estimate = -0.74; p-value &lt; 0.0001) after adjustment for age, sex, race and CMV serostatus. CD4+ TN/TM was inversely associated with co-occurring chronic conditions (odds ratio for 2 conditions and 3 conditions vs. 0 conditions: 0.74 (95%CI: 0.63-0.86) and 0.75 (95% CI: 0.63-0.90), respectively) after adjustment for age, sex, race, CMV serostatus, smoking, and BMI. CD4+ TN/TM had a stronger association with phenotypic age and age-related morbidity compared to other ARIP measures. Future longitudinal studies can help us evaluate if CD4+ TN/TM can predict risk of aging related outcomes.

Introduction: An appendectomy is the surgical removal of the inflamed vermiform appendix organ system. Patients who have had an appendectomy will be treated in hospital inpatient rooms for varying lengths of time, which is affected by several factors. This study aims to analyze the factors associated with the length of hospital stay post an appendectomy in Arun Lhokseumawe hospital, Aceh between 2019 and 2020. The factors analyzed are age, gender, type of appendicitis, duration of surgery, and comorbidity. Method: A retrospective study with an analytical descriptive cross-sectional study design. The sampling technique used was purposive sampling with a total of 251 samples. All data analysis used SPSS 26. Results: The characteristics of patients were as follow: age 17 – 25 years old (25.15), female (58.2%), acute appendicitis (57.8%), and no comorbidity (66.1%). The statistical analysis by Chi-square α =0.05 provides the factors associated with the length of hospital stay after an appendectomy, such as gender (p = 0.000), type of appendicitis (p = 0.000), and comorbidity (p = 0.000). Whereas age (p = 0.059) and the duration of surgery (p = 0.052) were not related to the length of hospital stay after an appendectomy. Conclusion: There were significant relationships between the following factors, such as age, type of appendicitis, comorbidity, and the length of hospital stay after an appendectomy procedure in Arun Lhokseumawe hospital, Aceh.

The coastal area of Serangai Village is directly facing the open sea; as a result it is vulnerable to coastal abrasion. Longshore currents can accelerate the process of coastal abrasion. The study was aimed to determine the speed and direction of current as well as the existence of the longshore current at the Serangai. Field observation was conducted on 5-7 November 2018. The results showed that the frequency distribution of current at speed of 0-10 cm/s reached 69%, and at speed of 10-20 cm/s reached 25%. The highest speed of 20-30 cm/s has a frequency of 6 %. Moreover, the direction of the majority of ocean currents is to the southeast (120o-150o), showing that the current is more likely to be parallel to the shoreline called longshore current that can accelerate shoreline Serangai Village, Bengkulu. However, further research is needed to see variability of current associated with the season (monsoon). Wilayah pesisir Desa Serangai berhadapan langsung dengan laut lepas; sehingga rentan terhadap abrasi pantai. Arus sejajar pantai dapat mempercepat proses abrasi pantai. Penelitian bertujuan untuk mengetahui kecepatan dan arah arus serta keberadaan arus sejajar pantai di Serangai. Pengamatan lapangan dilakukan pada tanggal 5-7 November 2018. Hasil penelitian menunjukkan bahwa distribusi frekuensi arus pada kecepatan 0-10 cm/s mencapai 69%, dan pada kecepatan 10-20 cm/s mencapai 25%. Kecepatan tertinggi 20-30 cm/s memiliki frekuensi 6%. Selain itu, arah arus laut mayoritas ke arah tenggara (120o-150o), menunjukkan bahwa arus lebih cenderung sejajar dengan garis pantai yang disebut arus sejajar pantai yang dapat mempercepat garis pantai Desa Serangai, Bengkulu. Namun, penelitian lebih lanjut diperlukan untuk melihat variabilitas arus yang terkait dengan musim (monsun).

Background Insecticide resistance is challenging the effectiveness of insecticide-based control interventions to reduce malaria burden in Africa. Understanding the molecular basis of insecticides resistance and patterns of gene flow in major malaria vectors such as Anopheles funestus are important steps for designing effective resistance management strategies. Here, we investigated the association between patterns of genetic structure and expression profiles of genes involved in the pyrethroid resistance in An. funestus across Uganda and neighboring Kenya. Methods Blood-fed mosquitoes An. funestus were collected across the four localities in Uganda and neighboring Kenya. A Microarray-based genome-wide transcription analysis was performed to identify the set of genes associated with permethrin resistance. 17 microsatellites markers were genotyped and used to establish patterns of genetic differentiation. Results Microarray-based genome-wide transcription profiling of pyrethroid resistance in four locations across Uganda (Arua, Bulambuli, Lira, and Tororo) and Kenya (Kisumu) revealed that resistance was mainly driven by metabolic resistance. The most commonly up-regulated genes in pyrethroid resistance mosquitoes include cytochrome P450s (CYP9K1, CYP6M7, CYP4H18, CYP4H17, CYP4C36). However, expression levels of key genes vary geographically such as the P450 CYP6M7 [Fold-change (FC) = 115.8 (Arua) vs 24.05 (Tororo) and 16.9 (Kisumu)]. In addition, several genes from other families were also over-expressed including Glutathione S-transferases (GSTs), carboxylesterases, trypsin, glycogenin, and nucleotide binding protein which probably contribute to insecticide resistance across Uganda and Kenya. Genotyping of 17 microsatellite loci in the five locations provided evidence that a geographical shift in the resistance mechanisms could be associated with patterns of population structure throughout East Africa. Genetic and population structure analyses indicated significant genetic differentiation between Arua and other localities (FST>0.03) and revealed a barrier to gene flow between Arua and other areas, possibly associated with Rift Valley. Conclusion The correlation between patterns of genetic structure and variation in gene expression could be used to inform future interventions especially as new insecticides are gradually introduced.

Blood based CRC assessment biomarkers, on a triage concept, can lead to improved selection to colonoscopy and cost-effective CRC care. Methods: Innovative multi-omics approaches, with global and targeted LCMS data production (metabolomics, lipidomics, and 2D proteomics) and integrative data analytics, were applied to discover serological biomarkers to assess nonadvanced adenoma and identify stage I/II colorectal bowel lesions. A cohort of 2396 normal, 660 adenoma, 953 stage I, and 101 stage II blood samples, was constructed to discover screening biomarkers to support case finding of patients at high risk for nonadvanced adenoma and stage I/II cancer for subsequent diagnostic colonoscopy. Results: A three-analyte mProbe panel was constructed which outperformed the commercial assays of plasma methylated septin 9 and fecal Cologuard tests. Sensitivity: (1) nonadvanced adenoma–Cologuard 17.2%, mProbe 76.0%; (2) stage I-III-Cologuard 93.3%, stage I-II Septin 9 (ARUP laboratories) 77%, stage I-II mProbe: 92.3%. Specificity–Cologuard 89.8%, Septin 9 (ARUP laboratories) 88%, mProbe 90.7%. Conclusions: mProbe triage concept of a blood-based protein biomarker panel promises the precision to allow future CRC screening, and reduce the low-risk utilization of unnecessary, unpleasant and risk-associated bowel examinations.

AbstractThe seasonal variability of strong afternoon winds in a northern Himalayan valley and their relationship with the synoptic circulation were examined using in situ meteorological data from March 2006 to February 2007 and numerical simulations. Meteorological observations were focused on the lower Rongbuk valley, on the north side of the Himalayas (4270 m MSL), where a wind profile radar was available. In the monsoon season (21 May–4 October), the strong afternoon wind was southeasterly, whereas it was southwesterly in the nonmonsoon season. Numerical simulations were performed using the Weather Research and Forecasting Model to investigate the mechanism causing these afternoon strong winds. The study found that during the nonmonsoon season the strong winds are produced by downward momentum transport from the westerly winds aloft, whereas those during the monsoon season are driven by the inflow into the Arun Valley east of Mount Everest. The air in the Arun Valley was found to be colder than that of the surroundings during the daytime, and there was a horizontal pressure gradient from the Arun Valley to Qomolangma Station (QOMS), China Academy of Sciences, at the 5200-m level. This explains the formation of the strong afternoon southeasterly wind over QOMS in the monsoon season. In the nonmonsoon season, the colder air from Arun Valley is confined below the ridge by westerly winds associated with the subtropical jet.

As a proxy of investment decisions is stock returns in banking companies at the time of publication of financial statements. The technique applied in this research is purposive sampling, obtained as many as 114 data (from 38 companies for 3 years). Using the SPSS tool. The method used in this research is non-linear regression test. The results of the study show that accounting earnings have an effect on investment decisions (stock returns), meanwhile operating cash flows doesn’t have any effect on investment decisions (stock returns). Oleh karena itu dilakukan analisis mengenai pendangkalan kolam pelabuhan dan abrasi pantai di lokasi ini dengan menggunakan data arus stasioner, trayektori arus, pasang surut dan hindcasting gelombang. Data penelitian lapangan selama satu bulan memperlihatkan telah terjadi proses pendangkalan dan abrasi pantai di sekitar area Pelabuhan Jetty Pertamina. Proses ini terjadi akibat terganggunya laju sedimen yang berasal dari selatan ke utara dan sebaliknya oleh aliran arus sejajar pantai dan arus pasang surut karena keberadaan Pelabuhan Jetty Pertamina (terganggunya kesetimbangan suplai sedimen). Kecepatan arus pasang surut pada tiga kedalaman berbeda rata-rata berkisar antara 0.168 – 0.215 m/s dan kecepatan arus terbesar sebesar 0.371 m/s terjadi pada saat spring tide. Arus pasang surut dan arus sejajar pantai secara bersinergi mempercepat terjadinya abrasi pantai dan pendangkalan kolam Pelabuhan Jetty. Laju abrasi pantai di perairan ini berdasarkan data PPPGL tahun 2003 adalah sebesar 1 – 4 m per tahun. Salah satu upaya untuk menanggulangi abrasi dan pendangkalan di kolam Pelabuhan Jetty terlebih dahulu harus di lakukan studi pemodelan. Studi pemodelan ini digunakan untuk melihat gambaran secara dinamis kondisi hidro dinamika perairan yang berhubungan dengan proses terjadinya pendangkalan dan abrasi, serta untuk menentukan tipe bangunan pantai yang sesuai. Therefore an analysis of the shoaling pool of port and coastal abrasion in this location was conducted by using the stationary current data, trajectory current, tide and wave hindcasting. Data of one-month field observation shows there has been shoaling and coastal abrasion processes in the areas sorrounding port Pertamina Jetty. This process occurs due to disruption of the rate of sediment derived from south to north and vice versa by the current flow parallel to the coast and tidal current because of the presence of Pertamina Jetty port (disturbance of equilibrium sediment supply). The velocity of tidal currents on three different average depths ranging from 0.168 – 0.215 m/s and the largest flow velocity of 0.371 m/s during spring tide. The direction of static current measurement and float tracking south-southeast trending dominant at low tide and northwest-north at high tide. Tidal current and longshore current sinergies to accelerate the occurence of coastal abrasion and shoaling pool of Harbour Jetty. The rate of coastal abrasion in this water based on PPPGL data of 2003 that is 1 – 4 m/s per year. One effort to overcome abrasion and shoaling in pool Harbour Jetty should be done prior modeling studies. This modeling study is used to portraya water hydro dynamics associated with the process of shoaling and abrasion, as well as determine the appropriate types of coastal structures. Only a small number of aroids are examined for their symbiosis with glomeromycotean arbuscular mycorrhizal (GAM) fungi and the ascomycetous dark septate endophytic (DSE) fungi. Therefore we examined the aerial and terrestrial adventitious roots of Epipremnum aureum for the endophytic association and the soils for GAM spores. The aerial roots of E. aureum were free from fungal structures, whereas the terrestrial roots were colonized by GAM, fine root endophyte (FRE), and DSE fungi. The major portion of the terrestrial roots was colonized by FRE fungi followed by GAM and DSE fungi. The colonization pattern was a complex of Arum-Paris and intermediate types. Spores of Acaulospora, Funneliformis, Rhizophagus, Rhizophagus and Sclerocystis were isolated from the root zone soils. The results show that E. aureum can establish symbiosis with a wide range of endophytic fungi and FRE symbiosis is reported for the first time in aroids.

Abstract:
Background Anthrax is a zoonotic disease that can be transmitted to humans from infected animals. During May–June 2017, three persons with probable cutaneous anthrax were reported in Arua District, Uganda; one died. All had recently handled carcasses of livestock that died suddenly and a skin lesion from a deceased person tested positive by PCR for Bacillus anthracis. During July, a bull in the same community died suddenly and the blood sample tested positive by PCR for Bacillus anthracis. The aim of this investigation was to establish the scope of the problem, identify exposures associated with illness, and recommend evidence-based control measures. Methods A probable case was defined as acute onset of a papulo-vesicular skin lesion subsequently forming an eschar in a resident of Arua District during January 2015–August 2017. A confirmed case was a probable case with a skin sample testing positive by polymerase chain reaction (PCR) for B. anthracis. Cases were identified by medical record review and active community search. In a case-control study, exposures between case-patients and frequency- and village-matched asymptomatic controls were compared. Key animal health staff were interviewed to learn about livestock deaths. Results There were 68 case-patients (67 probable, 1 confirmed), and 2 deaths identified. Cases occurred throughout the three-year period, peaking during dry seasons. All cases occurred following sudden livestock deaths in the villages. Case-patients came from two neighboring sub-counties: Rigbo (attack rate (AR) = 21.9/10,000 population) and Rhino Camp (AR = 1.9/10,000). Males (AR = 24.9/10,000) were more affected than females (AR = 0.7/10,000). Persons aged 30–39 years (AR = 40.1/10,000 population) were most affected. Among all cases and 136 controls, skinning (ORM-H = 5.0, 95%CI: 2.3–11), butchering (ORM-H = 22, 95%CI: 5.5–89), and carrying the carcass of livestock that died suddenly (ORM-H = 6.9, 95%CI: 3.0–16) were associated with illness. Conclusions Exposure to carcasses of animals that died suddenly was a likely risk factor for cutaneous anthrax in Arua District during 2015–2017. The recommendations are investigation of anthrax burden in livestock, prevention of animal infections through vaccinations, safe disposal of the carcasses, public education on risk factors for infection and prompt treatment of illness following exposure to animals that died suddenly.

ObjectivesThe use of peripherally inserted central catheters (PICCs) are an integral part of caring for hospitalised children. We sought to estimate the incidence of and identify the risk factors for complications associated with PICCs in an advanced registered nurse practitioners (ARNP)-driven programme.DesignRetrospective cohort study.SettingSingle-centre, large quaternary children's hospital.ParticipantsHospitalised children who had PICC inserted from 1 January 2010 to 31 December 2016.InterventionsNone.Measurement and main resultsA total of 2558 PICCs were placed during the study period. Mean age at PICC insertion was 8.7 years, mean dwell time was 17.7 days. The majority of PICCs (97.8%) were placed by ARNP. Most were placed in a single attempt (79.6%). Mean PICC residual external length outside was 2.1±2.7 cm. The rate of central line-associated bloodstream infection (CLABSI), thrombosis and significant bleeding were 1.9%, 1% and 0.2%, respectively. The CLABSI rate in infants and early childhood was higher than those aged ≥5 years (2.8%, 3.1%, respectively vs 1.3%). In a multivariate analysis after adjustment of confounding effects of race and gender, infants (OR= 2.24, CI=1.14 to 4.39, p=0.02) and early childhood cohort (OR=2.37, CI=1.12 to 5.01, p=0.02) were associated with significantly higher odds of developing CLABSI compared with ≥5 years old. In the early childhood cohort, PICCs with longer residual external catheter length (OR=1.30, 95% CI=1.07 to 1.57, p=0.008) and those placed in the operating room (OR=5.49, 95% CI=1.03 to 29.19, p=0.04), were associated with significantly greater risk of developing CLABSI.ConclusionsThe majority of PICCs were successfully placed by ARNPs on the first attempt and had a low incidence of complications. Infants required more attempts for successful PICC placement than older children. The presence of residual external catheter length and placement in the operating room were independent predictors of CLABSI in younger children.

Hepatitis C virus (HCV) Core has been implicated in immune-mediated mechanisms associated with the development of chronic hepatic diseases. Discovery of different alternative reading frame proteins (ARFPs) expressed from the HCV Core coding sequence challenges properties assigned to Core. This study was designed to evaluate the immunomodulatory functions of Core and ARFPs in monocytes, dendritic cells (DCs), macrophages (Mφ) and hepatocytes, cells that are all capable of supporting HCV replication. THP-1 cells, monocyte-derived Mφ and DCs, and Huh7 cells were infected by using adenoviruses (Ad) encoding Core, CE1E2 and a Core sequence modified so that the Core protein is wild type, but no ARFPs are expressed (CΔARFP). THP-1 cells and DCs infected with Ad encoding Core or CE1E2 produced significant levels of interleukin-6 (IL-6), IL-8, MCP-1 and MIP-1β, whereas production of these chemokines with AdCΔARFP was reduced or abolished. Similar effects on IL-8 production were observed in Huh7 cells and on IL-6 and MIP-1β in Mφ. Wild-type Core sequence, but not CΔARFP, could trans-activate the IL-8 promoter and this activation was not associated with activation of p38/p42–44MAPK. This study illustrates, for the first time, the critical importance of ARFP expression in immunomodulatory functions attributed to Core expression and suggests a potential involvement of ARFP in mechanisms associated with HCV pathogenesis.

It becomes problematic because value of beauty for each culture and people's opinions are so diverse. Responding to the issue, Charisma wrote the song King to respond with her perspective. This study discusses the representation of mainstream beauty standards rejection in the song King by Charisma. The purpose of this study is to explain the aspects represented in the song about the rejection of mainstream beauty standards. Primary data consists of parts of the song, namely lyrics and music. While secondary data obtained from scientific articles for analysis purposes. This research is a descriptive qualitative type with analysis based on Saussure's semiotic method. The data is mapped into a signifier consisting of lyrics, melodic elements, and rhythmic elements. The next mapping is the signified which is the interpretation of the signifier aspect. This study findings consist of three things, the song King represents criticism and rejection of beauty standards, the intention of rejecting beauty standards, and the reality of beauty standards. Penelitian ini bertipe kualitatif deskriptif dengan analisis yang didasarkan pada metode semiotika Saussure. Data dipetakan menjadi signifier yang terdiri dari lirik, unsur melodis, dan unsur ritmis. Pemetaan selanjutnya adalah signified yang merupakan interpretasi dari aspek signifier. Background. Anthrax is globally recognized as an important public health and economic challenge in many agricultural communities. A cross-sectional study was conducted in three subcounties in Arua district to assess the community’s awareness, cultural norm, and practices regarding anthrax. This followed a report of active cases of human cutaneous anthrax in the district. Methods. The study was conducted in subcounties of Pawor, Rigbo, and Rhino Camp, Arua district, using focus group discussion. Results. The affected communities had limited knowledge about anthrax, especially its clinical manifestation and modes of transmission both in humans and animals. The community also had no knowledge of the anthrax vaccine or treatment and where they can be accessed from. Poor practices associated with anthrax outbreaks included poor disposal of carcasses and ruminal wastes, occupational hazards (butchers, slaughter men, and herdsmen), consumption of meat from infected animals, communal herding, and cultural norms encouraging consumption of dead animals. Conclusion. This study shows that there is a knowledge gap about anthrax among the people in the affected communities. Key drivers for the anthrax outbreak such as poor cultural beliefs and practices and wildlife-livestock-human interactions were observed in all the three subcounties studied. All these findings could imply a high risk of outbreak of anthrax in Arua and Ugandan agricultural communities where the public health programs are less standardized and less effective.

Introduction: Mother-to-child transmission is the leading cause of HIV infection in children. Without treatment, half of these HIV infected children die before their second birthday.Purpose: To establish the contribution of mother-baby-pair points approach to eMTCT outcomes in Arua District Uganda.Material and Methods: A descriptive and analytical cross sectional study design which employed mixed methods approach was used. 196 HIV positive breast feeding mothers from three government hospitals (Arua Regional Referral Hospital, Adumi Health Center IV and Oli Health Center IV) were interviewed. Data was collected using structured questionnaires, focus group discussions and interviews guides.Results: Majority 168 (85.7%) of the respondents and their babies were enrolled to eMTCT care immediately after delivery and up to 17(8.7%) were enrolled beyond two weeks after delivery. Reasons for enrolment into care included; counseling before, during and after pregnancy on the importance of adherence to ART, early enrolment into care and delivery in a health facility. Most of the babies enrolled in mother-baby-points were HIV sero-negative (75.5%) with only 6.6% of them being sero-positive, thus signifying the contribution of mother-baby-points towards eliminating mother-child-transmission of HIV. Age of mothers was associated with retention on eMTCT care (χ2(5) =11.19, p=0.048). Again, having had any form of education on eMTCT was associated with retention on eMTCT care (p=0.001).Conclusion: The degree of enrollment in to eMTCT care was high but some mothers who still enrolled late. This affects early infant diagnosis, uptake of eMTCT services and retention of mothers and their infants.

Aruh religion is a system of ideas for Loksado people to work on the environment and give meaning to all activities in daily life. Religious knowledge is transformed from generation to generation through oral traditions which are considered sacred literature. A number of myths found in Aruh religious teachings underlie various religious actions such as in rice harvest rituals, rites of passage, healing rituals (babalian) and also social, economic and political actions in their daily lives. The development of a system of oral teachings among local people such as the Loksado people can be associated with the concept of symbolism in anthropology. Humans are symbolic creatures (animal symbolicum), that symbolic thinking and symbolic behavior are human characteristics . Various hopes, dreams, and thoughts that are sometimes difficult to accept by the human mind itself, manifested in various actions and symbolic works. Among the forms of symbolization carried out by humans are myths. As a symbolic expression, myths can almost be found in various cultural symptoms in all societies.

The Escherichia coli PAIusp is a small pathogenicity island encoding usp, for the uropathogenic specific protein (Usp), a genotoxin and three associated downstream imu1-3 genes that protect the producer against its own toxin. Bioinformatic analysis revealed the presence of the PAIusp also in publically available Salmonella bongori and Salmonella enterica subps. salamae genome sequences. PAIusp is in all examined sequences integrated within the aroP-pdhR chromosomal intergenic region. The focus of this work was identification of the usp promoter and regulatory elements controlling its activity. We show that, in both E. coli and S. bongori, the divergent TyrR regulated P3 promoter of the aroP gene, encoding an aromatic amino acid membrane transporter, drives usp transcription while H-NS acts antagonistically repressing expression. Our results show that the horizontally acquired PAIusp has integrated into the TyrR regulatory network and that environmental factors such as aromatic amino acids, temperature and urea induce usp expression.

Arum maculatum is traditionally used for the control of many diseases and illnesses such as kidney pain, liver injury, hemorrhoids. However, the detailed biomedical knowledge about this species is still lacking. This study reports on the bioactive components and the possible mechanisms underlying the antioxidant, anti-inflammatory and cytotoxic activity of A. maculatum leaf extract. Gas chromatography-mass spectrometry (GC-MS) was used for phytochemical analysis. Assay of 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide ) (MTT) was used to determine the cytotoxicity in the murine cell line L20B upon exposure to different extract concentrations for 24 h. Enzyme-linked immunosorbent assay (ELISA) was used to detect pro-inflammatory cytokines and tumor necrosis factor-α (TNF-α). GC-MS analysis identified the presence of important phytochemical components, e.g., 9-octadecenoic acid, methyl ester, (E), hexadecanoic acid, methyl ester, followed by benzenepropanoic acid, 3,5-bis(1,1-dimethylethyl)-4-hydroxy-, methyl ester (17.74%), heptadecanoic acid, 16-methyl-, methyl ester and dibutyl phthalate. The results indicated a significant dose-dependent decrease in L20B cell growth at a dose of 400 μg/ml (IC50) that is associated with a significant 2,2-diphenyl-1-picrylhydrazyl (DPPH) scavenging activity. The results suggested that the aqueous extract of A. maculatum leaves have potent antioxidant activity and cytotoxicity against L20B cell line with potential pro-inflammatory activity.

Community antiretroviral groups (CAGs) is one of the innovative and efficient differentiated service delivery models (DSDM) for reaching persons needing human immunodeficiency virus (HIV) treatment in the community. Since DSDM adoption in Uganda, evidence suggests better care outcomes for patients in DSDM compared to counterparts in routine health facility care. However, uptake of CAG models for eligible community groups of persons living with HIV (PLHIV) has been slow in Arua district, Uganda and stakeholders’ perceptions regarding its implementation unexplored. The objective of the study was to determine the uptake, barriers and facilitators influencing CAG model implementation in Arua district, Uganda. We conducted a parallel convergent mixed-methods study from March 2020 to December 2020 at Adumi health centre IV and Kuluva hospital in Arua district. We enrolled and extracted data for every fifth virally suppressed participant on antiretroviral therapy (ART) at the two health facilities. Data were analysed using STATA 13.0. Uptake was determined as the proportion of eligible PLHIV that were enrolled into a group. We performed logistic regression to determine factors associated with uptake. We conducted one focus group discussion per facility among healthcare workers involved in the management of PLHIV. We also conducted 7 focus group discussions among PLHIV across the two facilities. Thematic analysis was used to describe the data. A total of 399 PLHIV were eligible for CAG, 61.6% were female, and 44.9% were on dolutegravir (DTG) based regimen. Uptake was 6.8%, 95% CI (4.7–9.7) and was found to be significantly associated with being divorced or separated in a marriage (OR; 0.14, 95%CI; 0.02–0.92, P = 0.014). Members picking drugs in turns, comforting and encouraging others to take the drugs, and health workers advising them to join and stay with other group members were perceived as facilitators to uptake of community antiretroviral group delivery model. Having few and distant eligible members in the local area to form a group, lack of transport among the member to pick the drugs when it’s their turn, misunderstandings and lack of confidentiality amongst the members, and lack of partner disclosure were perceived as barriers to uptake of community antiretroviral group delivery model. Uptake of community antiretroviral group delivery model in Arua district is very low. There may be a need to support community antiretroviral group delivery models with income- generating activities, transport facilitation, closer community drug pick-up points and improved partner disclosure support mechanisms among married group members.

The INPEX-led Ichthys Joint Venture has established strong partnerships with the Northern Territory Government (NTG), cultural organisations and Traditional Owner groups in the Northern Territory to deliver social and environmental offset programs. The key governance model for delivery of the offset programs is the Ichthys Project Voluntary Offset Agreement (IPVOA), which commits $91 M AUD over a 40-year period through a suite of offset programs. This paper focuses on the journey to partner with the NTG-run Aboriginal Ranger Grants Program (ARGP). Through the IPVOA, INPEX and its Ichthys Joint Venture participants will contribute $24 M AUD over 22 years, to the ARGP, for Aboriginal Ranger programs addressing specific species and their associated habitat throughout the Top End. This is the first time where industry has contributed to the ARGP to deliver positive outcomes in the NT. This has been achieved by fostering collaborative, engaging and trustworthy relationships with the Commonwealth and NT governments; engaging with the Aboriginal Land Management Advisory Group; and meeting with Aboriginal Ranger groups. The key has been finding a common ground between the perspectives and the priorities of all stakeholders for how best to deliver conservation management on country in the NT. This common ground is likely to evolve over the next 22 years as the INPEX-led Ichthys Joint Venture and NTG deliver environmental outcomes, promote a connection to country and empower Aboriginal Ranger groups to better manage their country for future generations.

ArsP is predicted to be a membrane permease containing eight transmembrane helices, distinct from other known arsenic transporters. Analysis of multipleC. jejuniisolates from various animal species revealed that the presence of an intactarsPgene is associated with elevated resistance to roxarsone. In addition, inactivation ofarsPinC. jejuniresulted in 4- and 8-fold reductions in the MICs of roxarsone and nitarsone, respectively, compared to that for the wild-type strain. Furthermore, cloning ofarsPinto aC. jejunistrain lacking a functionalarsPgene led to 16- and 64-fold increases in the MICs of roxarsone and nitarsone, respectively. Neither mutation nor overexpression ofarsPaffected the MICs of inorganic arsenic, including arsenite and arsenate, inCampylobacter. Moreover, acquisition ofarsPin NCTC 11168 led to accumulation of less roxarsone than the wild-type strain lackingarsP. Together, these results indicate that ArsP functions as an efflux transporter specific for extrusion of organic arsenic and contributes to the resistance to these compounds inC. jejuni.
