Abstract:
Imali smo 13 (24,5%) otvorenih i 40 (75,4%) zatvorenih preloma. Ni u jednom slučaju nije došlo do resorbcije alotransplantata. Kombinacija ovakvog vida koštane nadoknade sa transosealnom osteosintezom lečenja ovih preloma u velikoj meri minimizira postoperativne komplikacije i daje dobre koštane i funkcionalne rezultate.  Ključne reči: visokoenergetski prelomi proksimalne tibije, smrznuti spongiozni alograft, transosealna osteosinteza, ASAMI klasifikacija

Abstract:
Abstract Objective: The aim of this study was to evaluate the clinical and radiological results of patients who underwent bone transport with Ilizarov external fixator in the treatment of tibial defects due to traumatic causes, and to investigate the factors that have a positive or negative effect on the results. Patients and Methods: From a total of 30 patients with tibial segment loss due to traumatic causes treated with bone transport with an Ilizarov external fixator between 2013 and 2020 in the Department of Orthopedics and Traumatology of the Faculty, 25 with adequate examinations and follow up were included in this study and were evaluated retrospectively. The patients were grouped according to the etiology of bone loss, demographic characteristics, Paley’s pseudoarthrosis classification and Solomin’s long bone defect classification. The results were evaluated according to the Association for the Study and Application of the Method of Ilizarov (ASAMI) outcome criteria. Results: The 25 patients in the study comprised 20 males and 5 females with an average age of 44.80 ± 18.51 years. According to the Paley pseudoarthrosis classification, 16 (64%) patients were Type B1, 5 (20%) were Type B2, and 4 (16%) were Type B3. According to the Solomin long bone defect classification, 4 (16%) patients were Type B2, 14 (56%) were Type C1, and 7 (28%) were Type C2. In some patients, due to the fixation problem, early fixation was achieved with minimally invasive plate osteosynthesis or intramedullar nailing by removing the Ilizarov fixator after the necessary length and alignment was achieved for the purpose of early rehabilitation. Early fixation was achieved before consolidation was completed in 16 of 25 patients. Fixator duration was 360 ± 130.77 days in the group without additional fixation and external fixation index (EFI) was 61.63 ± 29.81 days/cm, while the fixator duration was 273.33 ± 116.82 days in the early fixation group, and EFI was 57.22 ± 29.05 day/cm. Complications were evaluated according to Paley’s criteria. A total of 39 complications were recorded, the most common being pin tract infection. The ASAMI bone results were found to be excellent in 7 patients, good in 9, moderate in 4, and poor in 5. The functional results were excellent in 5 patients, good in 11, moderate in 4, and poor in 5. In addition, 5 (20%) of 25 patients whose results were evaluated had depressive illness. Moreover, the results of these patients were found to be poor according to the ASAMI criteria. Conclusion: The Ilizarov method is a very safe and successful option in the treatment of open fractures with tibial segment loss resulting from high-energy traumas. Nevertheless, when making the decision for treatment with this method, the patient’s current depressive disorder should be taken into consideration in addition to the clinical problems of the patients.

Introduction: Infected non-union needs prolonged treatment and repeated surgeries which significantly impacts patient’s social, financial, physical and mental life. In relation to Ilizarov technique monolateral external fixator is less bulky, more comfortable and better tolerated by patient. In this study we aim to assess the union rates, infection control, alignment, complications and the functional outcome of infected femoral non-union managed with monolateral external fixator. Material and methods: This is a prospective interventional study carried out between June 2017 and June 2021 at tertiary care centre operated by a single surgeon. Patients diagnosed as infected non-union of femur clinically and radiologically and operated with monolateral external fixator were assessed. Functional and radiological assessment was done using ASAMI classification. Results: In our study 21 patients were operated which included 20 males (95.2%) and one female with mean follow up is 29.85 ± 11.42 months. The most common site of non-union is mid-shaft of the femur. The mean bone defect after debridement is 3.52 ± 2.23cm (1 -10cm). The mean time of bony union in our study is 180.65 days (range 95-740 days). The ASAMI classification of bone results is Excellent to good in 90% of patients. Conclusion: Our study suggested that the Limb reconstruction system can be successfully used in the management of infected non-union of femur as a stable fixation modality and achieve satisfactory outcome in terms of bony union and functional results. Pre-operatively planning of the LRS frame, radical debridement and maintaining stability of the LRS frame are key steps of the treatment.

Abstract:
Introduction. High-energy distal tibia fractures are characterized by great comminution and extensive lesions of the surrounding soft tissues. The risk of developing complications during treatment is high. The application of the Ilizarov's technique reduces the risk of complications significantly, and provides good final treatment results. Methods. In our prospective observational study 41 patients were included. Clinical characteristics were evaluated by Gustillo-Anderson, Checketts-Otterburns and AO/OTA classification. The ASAMI protocol was used to assess bone union. Functional treatment results were represented using the modified Karlström-Ollerud scoring system. Analysis and monitoring time (follow up) was 18 months. Results. Using radiographic and clinical parameters, we recorded complete healing in all fractures. In the group of 43B intra-articular fractures, circular fixator was removed after 16 weeks (range 13 -31), while in the 43C fracture type group, it was removed after 18 weeks (range 13 -29). The ASAMI evaluation of bone healing showed 31 (75%) excellent, 76 (15%) good, 3 (8%) satisfactory, and 1 (2%) poor result. Functional recovery results obtained after 6 months showed the mean value of 24.7 using the modified Karlström-Ollerud scoring system in three follow-up periods, which represents recovery. The results present 12 months after surgery showed the mean value of 27.6, representing satisfactory recovery, while during the last parameter measurement performed after 18 months, the value was 29.5, which indicates good functional recovery. Conclusion. Transosseous osteosynthesis Ilizarov treatment applied in closed and open comminuted distal tibia fractures showed good final bone and functional results with minimal complications, and contributed to better patient's quality of life.

Abstract:
Aims The aims of this study were to establish whether composite fixation (rail-plate) decreases fixator time and related problems in the management of patients with infected nonunion of tibia with a segmental defect, without compromising the anatomical and functional outcomes achieved using the classical Ilizarov technique. We also wished to study the acceptability of this technique using patient-based objective criteria. Patients and Methods Between January 2012 and January 2015, 14 consecutive patients were treated for an infected nonunion of the tibia with a gap and were included in the study. During stage one, a radical debridement of bone and soft tissue was undertaken with the introduction of an antibiotic-loaded cement spacer. At the second stage, the tibia was stabilized using a long lateral locked plate and a six-pin monorail fixator on its anteromedial surface. A corticotomy was performed at the appropriate level. During the third stage, i.e. at the end of the distraction phase, the transported fragment was aligned and fixed to the plate with two to four screws. An iliac crest autograft was added to the docking site and the fixator was removed. Functional outcome was assessed using the Association for the Study and Application of Methods of Ilizarov (ASAMI) criteria. Patient-reported outcomes were assessed using the Musculoskeletal Tumor Society (MSTS) score. Results The mean age of patients was 38.1 years (sd 12.7). There were 13 men and one woman. The mean size of the defect was 6.4 cm (sd 1.3). the mean follow-up was 33.2 months (24 to 50). The mean external fixator index was 21.2 days/cm (sd 1.5). The complication rate was 0.5 (7/14) per patient. According to the classification of Paley, there were five problems and two obstacles but no true complications. The ASAMI bone score was excellent in all patients. The functional ASAMI scores were excellent in eight and good in six patients. The mean MSTS composite score was 83.9% (sd 7.1), with an MSTS emotional acceptance score of 4.9 (sd 0.5; maximum possible 5). Conclusion Composite fixation (rail-plate) decreases fixator time and the associated complications, in the treatment of patients of infected nonunion tibia with a segmental defect. It also provides good anatomical and functional results with high emotional acceptance. Cite this article: Bone Joint J 2018;100-B:1094–9.

Abstract:
This prospective study compared bifocal acute shortening and relengthening (ASR) with bone transport (BT) in a consecutive series of complex tibial infected non-unions and osteomyelitis, for the reconstruction of segmental defects created at the surgical resection of the infection. Patients with an infected tibial segmental defect (>2 cm) were eligible for inclusion. Patients were allocated to ASR or BT, using a standardized protocol, depending on defect size, the condition of soft tissues and the state of the fibula (intact or divided). We recorded the Weber–Cech classification, previous operations, external fixation time, external fixation index (EFI), follow-up duration, time to union, ASAMI bone and functional scores and complications. A total of 47 patients (ASR: 20 patients, BT: 27 patients) with a median follow-up of 37.9 months (range 16–128) were included. In the ASR group, the mean bone defect size measured 4.0 cm, and the mean frame time was 8.8 months. In the BT group, the mean bone defect size measured 5.9cm, and the mean frame time was 10.3 months. There was no statistically significant difference in the EFI between ASR and BT (2.0 and 1.8 months/cm, respectively) (p = 0.223). A total of 3/20 patients of the ASR and 15/27 of the BT group needed further unplanned surgery during Ilizarov treatment (p = 0.006). Docking site surgery was significantly more frequent in BT; 66.7%, versus ASL; 5.0% (p < 0.0001). The infection eradication rate was 100% in both groups at final follow-up. Final ASAMI functional rating scores and bone scores were similar in both groups. Segmental resection with the Ilizarov method is effective and safe for reconstruction of infected tibial defects, allowing the eradication of infection and high union rates. However, BT demonstrated a higher rate of unplanned surgeries, especially docking site revisions. Acute shortening and relengthening does not reduce the fixator index. Both techniques deliver good functional outcome after completion of treatment.

Abstract:
Introduction: Closed complex tibial shaft fractures are frequently encountered fractures in lower limb injuries. Various surgical methods for these fractures include primary or delayed fixation with intramedullary nails, minimal invasive plates, and primary definitive external stabilization using ring fixators. We conducted this study to evaluate the radiological and functional outcomes of primary definitive fixation of these fractures using an all-wire Ilizarov circular fixator. Objectives: The objectives of this study were to evaluate the radiological and functional outcomes of primary definitive fixation of closed complex tibial mid-shaft fractures in patients with compromised soft tissue status. Methodology: We defined Arbeitsgemeinschaft für Osteosynthesefragen/Orthopedic Trauma Association (AO/OTA) 42-B1 to AO/OTA 43-C3 fractures as complex tibial shaft fractures for this prospective observational study, and evaluated all 26 patients for the functional and radiographical progression according to ASAMI classification. Results: Twenty-six patients were stabilized with an all-wire circular ring fixator. The union time on average was 27.61 weeks. Among all patients, 7 (37.5%) cases had minor pin tract infections, and one (2.7%) patient landed a non-union, which required bone graft. Conclusion: With the use of an all-wire Ilizarov circular frame, there is a good mean time to union, fewer chances of re-operations, and good functional outcomes without any major complications.

Abstract:
Abstract Introduction: Non-union is a complex, multifactorial orthopaedic problem that requires multiple treatment modalities for its management. It can result from infection, segmental bone loss, soft-tissue loss, and associated host factors. External fixators as management of gap non-union are bulky and give rise to a number of complications In our study, we have described a technique where we have used extramedullary fixation in the form of a tibial locking plate and transportation of part of the tibial bone after corticotomy by a monorail fixator. Materials and Methods: The procedure was done in two stages where internal and external fixation was done after creating a gap at the non-union site. The bone was transported after corticotomy and fixed to the internal fixation device in the second stage. The external fixator was removed in the second stage and the patient was then followed up till the bony union. The evaluation was done by functional and radiological assessment along with the complications noted with the described procedure. Results: Ten patients were enrolled in the study with a mean age of 33 years. The mean age of the patients was 33.7 ± 11.32 years with a mean size of the defect was 4.8 ± 1.7 cm. At 30 weeks out of nine patients, three patients had excellent and six had good functional results on the Association of Surgeons for Application and Methodology of Ilizarov (ASAMI) scale. The composite Musculoskeletal Tumor Society (MSTS) score was 76.66 % at the end of 30 weeks of follow-up. The mean time of consolidation was 134.4 days, whereas the mean union time was 145 days from index surgery. Ankle stiffness was the most common complication affecting 50% of the patients. Following closely was pin tract infection, which was present in 40% of the patients. According to Paley’s classification, there were 11 obstacles, two problems and none were true complications. Conclusion: The integrated fixation is both safe and effective and has the advantage of early removal of the external fixator and a low complication rate as compared to use of a bulky conventional fixator alone. Moreover, it gives protection to the regenerated bone for a long period. So, this technique can be recommended for the management of segmental tibial defects.

Abstract:
Objective: The Ilizarov external fixator (EF) is a widely used orthopedic device for limb lengthening, deformity correction, and fracture treatment. It operates on the principle of distraction osteogenesis, stimulating new bone formation through gradual mechanical traction. This method allows simultaneous correction of axial deviations and joint contractures while preserving bone vascularization. Despite its advantages, the technique is associated with prolonged treatment durations and potential complications such as pin-site infections, joint stiffness, and delayed union. In this study, we aimed to evaluate the clinical and radiological efficacy of the traditional Ilizarov EF in the treatment of lower extremity shortening. Material And Methods: This retrospective study was conducted in the Department of Orthopedics and Traumatology at Çukurova University Faculty of Medicine. A total of 51 bones from 48 patients diagnosed with lower extremity shortening and treated with Ilizarov EF were analyzed. The cohort included 21 males and 27 females, with a mean age of 13.13 years (range: 2–18 years). The extent of shortening was determined using orthoroentgenograms. The severity of shortening was classified according to Paley’s difficulty scale, while postoperative bone and functional outcomes were assessed using Paley’s and the Association for the Study and Application of the Method of Ilizarov (ASAMI) scoring systems. Complications were categorized according to Paley’s classification. Postoperative quality of life was evaluated using the SF-36 questionnaire. Results: The study analyzed 20 femurs and 31 tibias. The mean lengthening achieved was 5.2 cm (range: 2–10 cm), with a mean follow-up period of 31 months (range: 12–120 months). The mean duration of fixator use was 34.65 weeks (range: 18–72 weeks). Corticotomy was performed at the metaphyseal region in 42 bones and the diaphysis in 9; however, the difference in union times was not statistically significant (p = 0.876). The mean external fixator index (EFI) was 54 days/cm (range: 24–120 days/cm). Complications were observed in 45 patients, with 105 classified as minor and 21 as major. According to Paley’s criteria, excellent and good outcomes were achieved in 75% of femoral cases, 82.7% of tibial functional assessments, and 94.3% of tibial bone evaluations. Conclusion: The Ilizarov EF is an effective and modern treatment approach that enables simultaneous correction of limb length discrepancies and deformities in patients of all age groups and etiologies. A multidisciplinary approach, including comprehensive patient education and effective complication management, may further enhance clinical and radiological outcomes.

Abstract:
Background:Although methotrexate is the anchor drug, it took some days to check contraindications, such as interstitial pneumonia, hepatitis B virus infection, and latent tuberculosis infection. Therefore, we sometimes start rheumatoid arthritis (RA) treatment with other disease-modifying antirheumatic drug (DMARD) in daily clinical setting. Iguratimod (IGU) is a newly developed DMARD approved in Japan in September 2012, and the efficacy of IGU for DMARD naïve patients has not been thoroughly evaluated. Objectives:The aim of this prospective single-center study was to demonstrate the efficacy and safety of IGU when used as a first-line daily DMARD for patients with RA in a clinical setting. Methods:Enrolled patients included those diagnosed with RA according to the 2010 American College of Rheumatology/European League against Rheumatism (ACR/EULAR) classification criteria who took IGU as a first-line DMARD at Niigata Rheumatic Center between April 2016 and December 2018 (IGU group). There were no constraints regarding the addition or withdrawal of other DMARDs. Details of the patients’ background, clinical parameters, and laboratory findings were obtained, including C-reactive protein (CRP) level, erythrocyte sedimentation rate (ESR), matrix metalloprotease-3 (MMP-3), rheumatoid factor (RF), Disease Activity Score for 28-joint counts based on the erythrocyte sedimentation rate (DAS28-4[ESR]), Clinical Disease Activity Index (CDAI), and Health Assessment Questionnaire Disability Index (HAQ-DI). The efficacy of IGU was evaluated at week 24. The IGU group’s data were then compared with 64 patients who took salazosulfapyridine or bucillamine as first-line DMARD in our hospital (other DMARD group). The data were expressed as median values. Comparisons between the parameters were performed using nonparametric method.Results:Forty-three patients (15 males, 28 females) received IGU as a first-line DMARD for RA. The age was 69.0 years and the duration of disease was 2.0 months. Twenty patients received prednisolone (PSL) concomitantly at a median dose of 5.0 mg/day. At 24 weeks medications were utilized in patients as follows: IGU in 36 (83.7 %), methotrexate in 18 (41.2%), PSL in 17 (39.5%), BUC in 10 (23.2 %), and biological DMARD in 7 (15.2 %) patients. Although 7 patients discontinued taking IGU due to liver enzyme elevation (n=4), nausea (n=1), creatinine elevation (n=1), and skin rash (n=1) during the 24-week period, serious adverse events requiring hospitalization were not observed. Clinical parameters that improved from baseline after 24 weeks of treatment included: ESR from 42.0 mm/h to 16.0 mm/h with p < 0.001; CRP from 0.70 mg/dL to 0.10 mg/dL with p< 0.001; MMP-3 from 136 ng/mL to 91.5 ng/mL with a p value of 0.11; RF went from 18.0 IU/mL to 8.00 IU/mL with p < 0.001; DAS28(4)-ESR dropped from 5.13 to 2.73 with p< 0.001; CDAI dropped from 20.0 to 5.00 with p< 0.001, and HAQ-DI decreased from 0.55 to 0.00 with p< 0.001. Patients who achieved a good or moderate response based on EULAR response at 24 weeks, included 90.7% in the IGU group versus 70.5% in the other DMARD group (Fisher`s exact test, p=0.046). The retention rate of the IGU group was 81.4 %, roughly equivalent to the 81.2% retention rate in the other DMARD group (Log-rank test, p=0.733).Conclusion:Our study indicates IGU is safe and effective for DMARD naïve RA patients. Starting treatment with IGU might be a new and effective strategy for RA patients without previous use of a DMARD.Disclosure of Interests:Daisuke Kobayashi: None declared, Eriko Hasegawa: None declared, Yoko Wada: None declared, Satoshi Ito Speakers bureau: Abbvie,Eisai, Asami Abe: None declared, Kiyoshi Nakazono: None declared, Akira Murasawa: None declared, Ichiei Narita: None declared, Hajime Ishikawa: None declared

Abstract:
Abstract In response to challenges in land cover classification (LCC), many researchers have experimented recently with classification methods based on artificial intelligence techniques. For LCC mapping of the vegetated Asahi River in Japan, the current study uses deep learning (DL)-based DeepLabV3+ module for image segmentation of aerial photographs. We modified the existing model by concatenating data on its resultant output port to access the airborne laser bathymetry (ALB) dataset, including voxel-based laser points and vegetation height (i.e. digital surface model data minus digital terrain model data). Findings revealed that the modified approach improved the accuracy of LCC greatly compared to our earlier unsupervised ALB-based method, with 25 and 35% improvement, respectively, in overall accuracy and the macro F1-score for November 2017 dataset (no–leaf condition). Finally, by estimating flow-resistance parameters in flood modelling using LCC mapping-derived data, we conclude that the upgraded DL methodology produces better fit between numerically analyzed and observed peak water levels.

Abstract:
The suffixes ~kin, ~ryou, and ~hi have different kanji but the same meaning and usage, which often makes it difficult for Japanese learners to distinguish their use when writing Japanese sentences. Therefore, the purpose of this study is to find out how the formation process and the meaning and ability of these suffixes in substituting each other. The method used in this research is descriptive qualitative. The data sources are taken from Asahi Shibun and Yomiuri Shinbun newspapers published on March 24 to 28, 2023. Based on the analysis, it is found that the suffixes ~kin, ~ryou, and ~hi belong to the classification of compound words formed from content morpheme + setsuji. Based on the word class, they are categorized as kin'in hyouji suffixes, which are nouns that show currency. Based on the vocabulary classification, the suffix can be attached to wago, kango, and konshugo type words but cannot be attached to gairaigo. Based on their semantic objects, the suffix ~kin mean 'money' which has a definite monetary value, the suffix ~ryou means 'rate' which is to give or get something in return, and the suffix ~hi means 'cost' as it is to the word to which it is attached. Then based on the substitution results, from the 20 data found, only 7 data can replace each other in terms of kanji formation, and from the 7 data only 3 data can substitute each other in terms of meaning.

Abstract:
Visual Question Answering (VQA) is an emerging field of deep learning that combines image and question features and generates collaborative feature rep resentations for classification by uniquely fusing the components. To enhance the effectiveness of models, it is crucial to fully utilize the semantic information from both text and vision. Some researchers have improved the accuracy of the model?s training by either adding new features or enhancing the model?s ability to extract more detailed information. However, these methods have made experimentation more challenging and expensive. We propose a model called asynchronous self-attention model (ASAM) that makes use of an asynchronous self-attention component and a controller, integrating the asynchronous self-attention mechanism and collaborative attention mechanism effectively to leverage the rich semantic information of the underlying visuals. It realizes an end-to-end training framework that can extract and exploit the rich representational information of the underlying visual images while performing coordinated attention with text features, as it does not over-emphasize fine-grained but finds a balance within it, thus allowing the model to learn more valuable information. Extensive ablation experiments were conducted on the proposed ASAM using the VQA v2 dataset to verify its effectiveness. The results of the experiments demonstrate that the proposed model outperforms other state-of-the-art models, without increasing the model complexity and the number of parameters.

Abstract:
The relation between the functions investigation of the National Human Rights Commission and the Attorney General leaves a space for debate that holds hostage to completion of gross human rights violations. At the same time, the Constitutional Court Decision No. 75/PUU-XII/2015 which examines the phrase "incomplete..." Article 20 (3) of the Law on Human Rights Courts, emphasizes that back and forth files are the implications of problems in applying norms in practice. While the consequences of these phrases target the scope of proofs and event examinations. This study seeks to unravel the practice of investigating human rights violations by analyzing the relations between the two authorities. Legislative approaches, conceptual approaches, historical approaches are the scope of analysis which helps to address legal issues. This study found that the use of phrases and their interpretations gave birth to friction in the application of norms. To parse this, it is done by mapping the practice of back and forth files. Differences in interpretation were found in the widening of the results of investigations to prosecution, which led to differences in the classification of criminal types. The findings in the form of incompatibility of norms are a consequence of the practice of hierarchical models faced with separate authority. As a result, the form of procedure is questioned, dwelling on existing procedures without seeing the special character of the situations, events, and the crimes. Continuing from that, anomalies in practice make the use of hierarchical models and coordination models in investigations obscure the process of resolving gross human rights violations. Reflecting on the process at the ICC, there are several things that can be taken to enrich the discourse on the process of completion gross human rights violations.

Abstract:
Abstract This study aims to develop a rapid method for assessing the quality of coconut oil and palm oil based on peroxide value and free fatty acid (FFA) content, using a reflectance–fluorescence-based computer vision system. The background of this research lies in the need for a more efficient analytical method compared to conventional laboratory testing, which is often time-consuming and costly. In this study, oil samples were subjected to heating treatments at 180 and 200 °C to simulate frying conditions. The peroxide value and FFA were analyzed using standard laboratory methods, while the computer vision system acquired image data of the oil samples using a reflectance and fluorescence imaging setup. A classification model was developed using a Convolutional Neural Network (CNN) algorithm to automatically extract color features contributing to oil quality classification. The results showed that the *a* (redness) and *b* (yellowness) values increased in samples treated at 180 and 200 °C, indicating Maillard reactions and thermal degradation. Conversely, the *L* value (lightness) decreased due to prolonged exposure to high temperatures. The CNN model was able to classify coconut oil quality with accuracies of 57.07% and 57.41%, while for palm oil, the model achieved accuracies of 85.77% and 94.22%. These findings suggest that the reflectance–fluorescence computer vision approach holds potential as an alternative method for evaluating the quality of cooking oils.

Abstract:
ABSTRACT Purpose: This research aims to (1) explore the local wisdom values of the Special Region of Yogyakarta (DIY), which can constitutionally strengthen human rights values in Indonesia, and (2) analyze the judge's consideration of the Constitutional Court Decision No. 88/PUU-XIV/2016 from the perspective of universalism versus particularism of human rights in Indonesia. Methodology: This normative legal research employed a philosophical and statutory approach. This research used only secondary data consisting of primary and secondary legal materials refined through Focus Group Discussion. The data were then analyzed by descriptive qualitative philosophy to find the meaning behind the object under study through data reduction, classification, interpretation, display, and drawing conclusions. Results: The study revealed that (1) constitutional human rights values in DIY’s local wisdom are explicitly stated in the DIY Regional Regulations (Perda) and Special Regional Regulations (Perdais). DIY’s local wisdom can philosophically strengthen human rights values in Indonesia based on Pancasila, confirming that Pancasila as a constitutional identity crystallizes cultural customs and religious values throughout Indonesia. (2) The judges’ opinion for the Constitutional Court Decision No. 88/PUU-XIV/2016 is relative-particular; other than human rights is universal as there is no differentiation between the male and female to be the Governor of Yogyakarta Special Region, in the particular case of Yogyakarta, it remains upholding the local wisdom values as the requirement to be the Governor of Yogyakarta Special Region, where he shall be the reigning monarch of Sultan Hamengku Buwono, and the requirement to be the Sultan relies on the hereditary internal law of the Kasultanan Ngayogyakarta Hadiningrat (Yogyakarta Palace) since 1755. Applications of the study: This study is applied (1) to maintain and preserve the local wisdom values of the Special Region of Yogyakarta (DIY) for the upholding of human rights in Indonesia as mandated by the 1945 Constitution and (2) to support the government’s program in the 2019-2024 RANHAM so that every Indonesian’s human rights receive perfect protection, in which the administrators of state power uphold human rights values in carrying out their duties to serve the community. Novelty/Originality: This research explored the local wisdom values of the DIY which can strengthen human rights values in Indonesia from various existing regulations in DIY with a broader approach, including raising local wisdom from human rights enforcement cases in Indonesia. Abstract:
This study aims to analyze the conformity of the age-based classification of prison inmates in Indonesia with human rights (HR) principles, evaluate the state's responsibility, and assess the potential use of Public Private Partnership (PPP) schemes in the development of age-based correctional facilities. The background of the research is based on the ongoing practice of housing juvenile, adult, and elderly inmates together in Indonesian correctional institutions, which contradicts Law Number 22 of 2022 on Corrections and international standards such as the Nelson Mandela Rules and the Convention on the Rights of the Child. The urgency of this study lies in the need to protect vulnerable groups and provide equitable, humane, and rehabilitative correctional services. This research uses a normative legal method with statutory, human rights, and international comparative approaches, supported by qualitative data analysis. The findings reveal a significant gap between legal norms and field practices, inadequate fulfillment of state obligations, and high potential for PPP schemes as an alternative solution. The novelty of this study lies in its multidimensional approach and its proposal to apply PPP in age-based correctional institution management. The study concludes that the Indonesian state has yet to fully fulfill its constitutional and international obligations concerning the treatment of vulnerable inmate groups. Therefore, it recommends strengthening implementing regulations, enhancing institutional capacity, reforming legal culture, and applying a transparent, human rights-based PPP model as a strategic solution.

The Covid-19 pandemic is a transnational threat that requires a global response, but the outbreak has laid bare divergent national approaches to exposed broader structural weaknesses in the governance system. The challenges of governance of the state amidst the Covid-19 pandemic is not only on the public health approach but also must face the risk of economic recession. In the present report, the government of the Republic of Indonesia has taken anticipation steps to prevent and overcome Covid-19 through legislation which is then implemented to the public. The focus of this paper is to review how the Republic of Indonesia maintains national economic resilience using the Indonesian tax law approach. Income tax is part of tax classification in Indonesia has rights and obligations attached to the state as well as taxpayers. With using the doctrinal legal research method, this papers analyze the perspective of the Republic of Indonesia's tax laws on opportunities for corporate taxpayers to get incentives in their income tax because they have contributed to the need to overcome the pandemic Covid-19 and explained the relationship between human rights and taxes on the case. As the papers make clear, the tax revenue paradigm is considered important because it impacts on economic security and national development. The government must be careful in carrying out taxation policies by considering the economic conditions of democracy, globalization, and the synergy of the center and the regions as long as the Covid-19 pandemic continues. Human rights and taxes are related to the realization of the right to the social-economic and social justice in society because Indonesia taxes has rights and obligations attached to the state as well as taxpayers.

Abstract Asam gelugur fruit has economic value. The government’s reforestation program empowers farmers through planting asam gelugur seeds at 3 altitudes, The problem is that not all of asam gelugur trees produce fruit. Characterization of morphology needs to be carried out to obtain basic data for selecting prospective mother trees as a source of seed supply that guarantees reforestation trees bear fruit. This study aims to characterize the morphology of asam gelugur based on classification of local wisdom of farmers of North Sumatra, namely classifying Asam gelugur based on the success or failure of the pollination process followed by the formation of fruit at 3 altitudes. This study was conducted in three altitudes: lowlands 1-<400 m above sea level (asl), medium (400- < 800 m asl and highlands (800- 1300 mdpl), exploration using survey method with IPGRI Descriptor guide and purposively sampling. The results found three types of asam gelugur in North Sumatra: Female, Male and Shemale (muzzy) at the three altitudes. There is no effect of altitude on the morphological characters of the three types of asam gelugur, except for the sharpness of the leaves color of the shoots.People namely Female asam gelugur tree, if pollination is successful and produces fruit. the tree routinely produces fruit. The name of a tree that has never been successfully pollinated and never bears fruit is called a male tree. The shemale tree is called a tree whose pollination occurs sometimes successfully. There were five striking differences in the characteristics of the three types asam gelugur based on local wisdom. The different characteristics were the Character shape (canopy), the petal’s color, the shape fruit, the Color of the young leaves, and the thickness of the leaves.

Abstract:
BackgroundThe pathogenesis of SSc consists of three aspects: fibrosis, angiopathy, and immune abnormalities. Nintedanib has been shown to be effective against interstitial lung disease (ILD) as an antifibrotic drug, but it may also have effects on vascular disorders due to its inhibitory effects on FGF, PDGF, and VEGF. While nailfold capillary abnormalities in patients with systemic sclerosis (SSc) may improve with immunosuppressive therapy, no reports have evaluated whether nintedanib improves nailfold capillary abnormalities.ObjectivesTo clarify whether nintedanib improves nailfold capillary abnormalities in patients with SSc complicated by ILD.MethodsThis study is a prospective observational study. Among patients diagnosed as having SSc according to the 2013 ACR/EULAR classification criteria, those who started to treat with nintedanib for ILD at our hospital from March 2020 to July 2022 were consecutively registered. The nailfold capillaries were evaluated using a nailfold videocapillaroscopy (NVC), capable of observation at 200x magnification. The evaluation was carried out using the current standardized method.[1]NVC was performed before drug induction and 1 year after drug induction. Only patients who had NVC testing up to 1 year later were included in the evaluation. At the same time, blood tests such as KL-6, SP-A, and SP-D, and respiratory function tests were evaluated. This study has been pre-approved by the Hiroshima University Hospital Ethics Committee. (OySTeR study: Optimal strategy in SSc Treatment for Recovery, approval number: E-1940-1)ResultsEight patients were analyzed. The mean age was 59 ± 17 years. Female were 7 (87.5%). Six patients (75%) were positive for anti-Scl-70 antibodies. Details of other cases are listed in Table 1. Seven patients (87.5%) had a history of treatment with immunosuppressive therapies. The disease duration was less than 6 years in 5 patients, and 6 years or longer in the remaining 3. Five patients were able to continue nintedanib for 52 weeks: 1 patient discontinued nintedanib after 1 month due to abnormal liver function, 1 patient with diarrhea discontinued after 10 months. All of the patients who were able to continue oral administration were also reduced to 200 mg/day due to diarrhea. Baseline mean FVC, KL-6, SP-A, and SP-D are described in Table 1. The results by NVC pattern were Normal in 1 case, Early in 2 cases, Active in 1 case, Late in 3 cases, and Non-specific in 1 case. Quantitative baseline results for the NVC tests were capillary density (number of capillaries) 5.8 ± 2.2/mm, enlarged capillary score 0.91 ± 0.83, giant capillary score 0.31 ± 0.30, and hemorrhage score 0.22 ± 0.27. 1 year later, the NVC pattern deteriorated from early to active in only one case, but the others remained unchanged. Quantitative results for the NVC test were capillary density 5.8 ± 2.3/mm, enlarged capillary score 0.98 ± 0.95, giant capillary score 0.38 ± 0.48, and hemorrhage score 0.20 ± 0.33. Mean FVC, KL-6, SP-A, and SP-D after 1 year are also shown in Table 1.ConclusionNintedanib tended to improve ILD biomarkers and pulmonary function test results in patients with SSc, but did not improve nailfold capillary abnormalities. Based on this result, combination therapy is preferable to nintedanib alone for improving the pathology of SSc.Reference[1]Smith V, Herrick AL, Ingegnoli F, Damjanov N, Angelis R, Denton CP et al. Abstract:
BackgroundDifferent diagnostic interviews are used as reference standards for major depression classification in research. Semi-structured interviews involve clinical judgement, whereas fully structured interviews are completely scripted. The Mini International Neuropsychiatric Interview (MINI), a brief fully structured interview, is also sometimes used. It is not known whether interview method is associated with probability of major depression classification.AimsTo evaluate the association between interview method and odds of major depression classification, controlling for depressive symptom scores and participant characteristics.MethodData collected for an individual participant data meta-analysis of Patient Health Questionnaire-9 (PHQ-9) diagnostic accuracy were analysed and binomial generalised linear mixed models were fit.ResultsA total of 17 158 participants (2287 with major depression) from 57 primary studies were analysed. Among fully structured interviews, odds of major depression were higher for the MINI compared with the Composite International Diagnostic Interview (CIDI) (odds ratio (OR) = 2.10; 95% CI = 1.15–3.87). Compared with semi-structured interviews, fully structured interviews (MINI excluded) were non-significantly more likely to classify participants with low-level depressive symptoms (PHQ-9 scores ≤6) as having major depression (OR = 3.13; 95% CI = 0.98–10.00), similarly likely for moderate-level symptoms (PHQ-9 scores 7–15) (OR = 0.96; 95% CI = 0.56–1.66) and significantly less likely for high-level symptoms (PHQ-9 scores ≥16) (OR = 0.50; 95% CI = 0.26–0.97).ConclusionsThe MINI may identify more people as depressed than the CIDI, and semi-structured and fully structured interviews may not be interchangeable methods, but these results should be replicated.Declaration of interestDrs Jetté and Patten declare that they received a grant, outside the submitted work, from the Hotchkiss Brain Institute, which was jointly funded by the Institute and Pfizer. Abstract:
Abstract Coal is a conventional energy source found in tropical countries (e.g., Indonesia) with varying distribution and thickness in the South Sumatra Basin. However, the thickness of the coal seams was not evenly distributed. This affected the estimation of coal resources in the mining area of PT Bukit Asam Tbk in Tanjung Enim, Muara Enim Regency. We used field observation methods and borehole data analysis to determine the geological conditions and estimate the coal resources. The geological condition seems to indicate a claystone unit with tuffaceous sandstone interbeds and a sandstone unit with claystone interbeds. These two units might host coal seams (i.e., A1, A2, B1, B2, and C), where the coal seam thickness may determine increased towards the south and west in the study area. The coal resource estimation based on the SNI 5015: 2019 classification, under moderate geological conditions, can result in a total of 26.034.991 tonnes of measured resources, 25.209.688 tonnes of indicated resources, and 11.189.554 tonnes of inferred resources. Therefore, to ensure efficient coal mining in accordance with the measured reserves, the stripmining method may be used by the company.

Asadi F, Sharifnia F, Salimpour F, Majd A. 2019. Using micro-morphological fruit characters in resolving some of ambiguities in Iranian Acer L. (Sapindaceae) species. Biodiversitas 20: 297-304. Acer L. belongs to the Sapindaceae family consists of 126 species which are distributed in the temperate regions of the northern hemisphere. There are 12 taxa (including Acer monspessulanum with four subspecies and Acer velutinum with two varieties) in Iran. In this study 21 fruit characteristics are partially investigated using Scanning Electron Microscope (SEM) and Stereomicroscope. According to the obtained results, four types of faveolate, folded, wrinkled and pavement wing surface ornamentations as well as four types of faveolate, folded, verrucous, and wrinkled achene surface ornamentations were observed. Hair of wing margin, achene hair, wing ornamentation, inside angle of wing, and outside angle of wing have taxonomic value. According to fruit morphology, separation of 4 subspecies of A. monspessulanum can be verified. Also varieties of A. velutinum var. velutinum, and A. velutinum var. glabrescens were totally separated due to differences in characteristics such as wing ornamentation, achene ornamentation, hair density of achene, wing length, wing width, the ratio of length to width of wing, total fruit length, total fruit width, the ratio of length to width of fruit, inside angle of wing, outside angle of wing. The results showed that micro-morphological fruit characters of Acer genus are valuable in terms of taxonomy and classification of subspecies level.

Abstract:
Post induction hypotension (PIH) is a largely debated topic in anaesthesia , without any definitive consensus on ideal method for its accurate identification. We hypothesized that inferior venae cava collapsibility index (IVCCI)measured pre-operatively before general anaesthesia could predict this hypotension .The patients were categorized on the basis of their American Society of Anaesthesiologists physical status classification(ASA) to find the optimal cutoff value and sensitivity of this index in predicting PIH.In this prospective observational study, 157 patients (79 ASA I and 78 ASA II) undergoing routine general anesthesia for surgical procedures were enrolled and their preoperative IVCCI measurements were correlated with non-invasive hemodynamic parameters measured till 12 minutes post induction. The overall incidence of PIH was 46% (12.7% in ASA I and 79.49% in ASA II patients). The optimal cutoff value of IVCCI for predicting hypotension was 47% .The sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) at 95%CI was 95% (87-98%), 97% (90-99%), 100% and (90%- 97.1%) with ROC (Receiver operating characteristic) of 0.972 (95%CI, P<0.0001). ASA status (β=2.924, OR (Odd ratio) 18.611;P<0.0001), baseline DBP (β=0.056, OR-0.946; p<0.004) and IVC-CI <50 (β=0.410, OR -1.506; p<0.0001) had significant correlation with PIH in linear regression logistic model.Preoperative IVCCI measurements, baseline DBP and ASAII physical status of patients could significantly and accurately predict PIH before general anaesthesia.

Abstract:
BackgroundRecent studies have shown that complement C5a receptor antagonist is highly effective for treating microscopic polyangiitis (MPA), suggesting that complement is substantially involved in the pathogenesis.ObjectivesIn this study, we aimed to examine whether serum C3 levels were associated with clinical characteristics, disease severity, treatment, and prognosis in the Japanese multicenter cohort.MethodsPatients with MPA whose C3 levels were measured at diagnosis were enrolled. The diagnosis of MPA was based on the 2012 revised Chapel Hill consensus criteria. Patients were divided into three groups: low C3 (<86 mg/dL), normal C3 (86-130 mg/dL), and high C3 (>130 mg/dL) groups to compare clinical characteristics. Analysis of variance or the Kruskal–Wallis test was used when the variates were numeric, while the chi-squared test or Fisher’s exact test was used when variates were categorical.ResultsA total of 158 patients (low C3: 28, normal C3: 90, high C3: 40 patients) were enrolled. Baseline characteristics did not differ between the three groups in age at onset, sex, smoking history, and MPO-ANCA positivity. Serum C3 levels (P<0.001) as well as C4 (P<0.001) and CH50 levels (P=0.0013) were significantly lower in the low C3 group. In addition, hemoglobin (P=0.0058), platelets (P<0.001), and serum CRP levels (P=0.002) were significantly lower in the low C3 group, while serum creatinine levels (P=0.001), BVAS (P=0.039), and five factor scores (P=0.005) were significantly higher in the low C3 group. Notably, the proportion of patients who fulfilled the 2022 revised ACR/EULAR classification criteria for MPA was significantly lower in the low C3 group (P=0.039). Initial prednisolone dose and survival did not differ between the three groups.ConclusionLow C3 levels are associated with severe disease and renal dysfunction, while high C3 levels are associated with elevated CRP levels. Serum C3 levels are closely associated with disease pathology.References[1]Jane DRW, Merkel PA, Schall TJ, Bekker P; ADVOCATE Study Group. Avacopan for the treatment of ANCA-Associated Vasculitis.
