Abstract:
This book describes much of what has been learned about antisocial personality disorder (ASPD) and traces the continuing search for answers. Perhaps the best, most concise definition of ASPD comes from psychiatrists Donald Goodwin and Samuel Guze: “Sociopathy (sociopathic personality or antisocial personality) is a pattern of recurrent antisocial, delinquent, or criminal behavior that begins in early childhood or early adolescence and is manifested by disturbances in many areas of life: family relations, schooling, work, military service, and marriage.” Thus, the diagnosis of ASPD relies on a mental health professional’s ability to uncover this pattern and trace it through a patient’s life. The book explores the possibility that ASPD may be at the root of a substantial amount of the troubles that plague society, and that learning more about the disorder might help in fighting crime, violence, and other social ills. It also highlights how ASPD is more common in men than women.

This chapter provides an overview of the incidence, presentation, assessment, diagnosis, and management of the four main circadian rhythm sleep disorders: advanced sleep phase disorder (ASPD), delayed sleep phase disorder (DSPD), free-running (non-24-hour) sleep disorder (FRSD), and irregular sleep–wake rhythm disorder (ISWRD). Following a brief discussion of the daily entrainment of the human circadian clock to the light cycle, and the shifting effects of light and melatonin on the clock, each of the four different disorders are considered in turn. The aim of this chapter is to provide a concise overview of the disorders and the potential treatment strategies for each. The chapter is extensively referenced for further information.

Abstract:
This chapter examines ethical issues that arise when diagnosing and treating dangerous and self-destructive patients. In particular, we look at sex offenders and those diagnosed with antisocial personality disorder (ASPD), who primarily present a danger to others, and those with borderline personality disorder (BPD), who primarily present a physical danger to self. We identify four questions that arise within this population: (1) when should patients be restrained and secluded; (2) what ethical justifications can be employed to analyze the use of restraint and seclusion in these populations; (3) what special considerations need to be taken into account in the use of restraint and seclusion; and (4) what metaphysical and epistemological challenges are involved with this population? Each of these questions is linked to ethical concerns and responsibilities in the use of confinement measures when patients are dangerous to themselves or others.

Abstract:
Personality disorders (PDs) and substance use disorders (SUDs) frequently co-occur both in the general population and in clinical settings. The authors review the recent literature that documents high comorbidity between these two classes of disorders, discuss possible mechanisms of comorbidity, and describe the clinical implications of this comorbidity. Although most attention on comorbidity between PDs and SUDs has focused on antisocial personality disorder (ASPD) and borderline personality disorder (BPD), it is also clear that other PDs (in particular, paranoid, avoidant, and obsessive compulsive PD) are prevalent among those suffering from SUDs. The effect of SUD on PD expression appears to be one of exacerbating PD symptomatology and, in turn, contributing to chronicity. This has important treatment implications in that clinicians must keep in mind the challenges present when planning and implementing treatment for those with both SUD and PD.

Abstract:
This chapter argues that, in order to distinguish between good and bad defiance, it is inadequate merely to rely on legal or clinical definitions of vice and disorder. Instead, a realistic account of some of the features of phronesis (practical reasoning) is needed. The question examined in this chapter is what qualities or characteristics would make defiant behavior wrong and why. The author claims that sometimes defiant behavior should be seen as virtuous. In order to show that this is not the case, the author begins with a discussion of the features of Antisocial Personality Disorder (ASPD). By examining what harms are relevant to vicious (bad) defiance and why antisocial behavior shows flawed reasoning, the author enriches her theory of defiance as a virtue, identifies constraints on what can count as virtuous defiance in a person with a mental disorder, and indicates what defiance as a virtue would look like by examining the case of Henry, a person diagnosed with schizophrenia.
