Abstract:
Within general psychiatry, awareness of autism spectrum disorder (ASD) and the use of the terms ‘autism’, ‘autistic disorder’, ‘Asperger syndrome’, the ‘autism spectrum’, and ‘high functioning autism’ (HFA) are growing. However, autism has yet to become part of the accepted mainstream, core curriculum of general psychiatry. Psychiatrists are now expected to be able to recognize autism and consider its effects on their adult patients, particularly those showing signs of comorbid mental disorder, for example, schizophrenia, personality disorder, mood disorder, or attention-deficit/hyperactivity disorder (ADHD). From childhood through to adulthood and old age there is a failure in autism to develop skills in reciprocal social interaction, understanding, and flexibility. This can profoundly affect behaviour in the community, personal independence, employability, and social relationships, including marriage and parenting. Most cases of autism in adults are unrecognized and undiagnosed, both within the general population and in adults using psychiatric services. This book gives a comprehensive introduction to autism and Asperger syndrome written to fit the adult clinician’s perspective. It will assist with autism recognition and diagnosis in adulthood. It is designed to enhance the clinician’s role in treating patients with co-morbid mental disorder, while understanding and taking account of the autism component. It will also help in signposting patients with autism to appropriate care and support, as family involvement diminishes, or ceases in adulthood and old age, and in the psychiatrist’s role in providing advice to the courts and in the context of detention in accordance with mental health law.

Abstract:
There is very good evidence for a strong genetic component to the autism spectrum disorders (ASDs), which include autistic disorder, Asperger syndrome, pervasive developmental disorder not otherwise specified, and Rett syndrome. At the same time, identifying the loci contributing to ASD risk has proven difficult because of extreme heterogeneity. However, in spite of these difficulties, many ASD loci have been identified and, even using current clinical measures, an etiological diagnosis can be given in upward of 20% of cases. With the introduction of “second-generation” sequencing, gene discovery in ASDs will accelerate. As genes are being discovered, functional analyses are leading to potential novel therapeutics, and there is great optimism for more effective treatments in ASDs arising from gene discovery. In the current review, some of the important findings in ASD genetics will be outlined, as will the next steps in ASD genetics.

Gordon Peterson—early music specialist, professional musician, and former tenured music professor—was diagnosed with Asperger’s syndrome at the age of forty-five, by which time he had suffered through decades of misdiagnoses, misguided psychiatric treatments, and despair. “For the first time ever,” he recalls, “I felt the correctness of the diagnosis.” Gordon attributes many of the hardships he has endured to being “wired all funny” with Asperger’s, but he sees the condition as the primary source of his musical talent, intellectual prowess, and fertile imagination as well. “In my mind,” he says, “there is an impossibly complex web of musical and non-musical cultural connections across time and geographic location . . . . I see a ribbon-like time line [that] stretches back from today all the way to the beginning of recorded history, and before . . . and I can walk around in it, looking at the instruments, hearing the sounds, hearing the language.” Asperger’s is “my superpower,” Gordon attests, but as the chapter chronicles, that superpower exacts a very high toll on him in terms of the anguish and turmoil it brings.

When we first meet ten-year-old Zena Hamelson, she is sitting in a chair staring blankly at the wall, flapping her hands, repeatedly straightening and bending her legs, compulsively twisting and pulling on her fingers as her Artism Ensemble bandmates make joyful music all around her. Zena is stimming, that is, she is practicing a personal repertoire of self-stimulatory behaviors that align precisely with the symptomatic profile of her diagnosed autism spectrum disorder: Asperger’s syndrome. Stimming, autism researchers tell us, is associated with some dysfunctional system in the brain; its reduction or elimination is a target goal of many therapeutic interventions and autism studies. Yet as the chapter unfolds, Zena’s stimming is revealed as something else entirely: a meaningful mode of music-making, creative expression, and social experience unto itself.
