If a major city, the major administrative capital of the south and an integral part of how the system reproduces itself, is invisible beyond an occasional passing reference in the familiar narrative of the history of a country, something has gone amiss. Such is the situation of Asyut, a city so invisible that even the English spelling of its name is elusive—variably Asyut, Assuit, Assiut, Siout, or even Essiout—sometimes appearing to be a deliberate attempt to retain an ancient pharaonic or biblical rendering. So why is there this invisibility and how can it be overcome?

Abstract:
Abstract Background Different techniques for aortic root enlargement (ARE) have been reported in the literature. Each technique comes with its own advantages and disadvantages. We report our outcomes of Nick's technique for ARE. Methods A single-center retrospective data analysis of 31 patients was performed. Patients were operated between May 2015 and November 2017 at Assuit University Heart Hospital, Assuit, Egypt. Results The median cardiopulmonary bypass time was 125 minutes (range: 90.0–160.0 minutes), with 90 minutes of cross-clamp (range: 60.0–110.0 minutes). Altogether 59% of the patients had mixed aortic valve diseases. Median intensive care unit and total hospital stay were 2 and 5 days, respectively. Patient-prosthesis mismatch was reported in one patient only (3.25%). Two patients died within 30 days. Median pressure gradient across the aortic valve was 20 mm Hg at 3 years of follow-up. Conclusion The benefits of Nick's technique for ARE can be demonstrated in populations with younger patients and complicated pathology. Further research is required in larger patient populations.

AbstractThree hundred and seventeen patients with hypopharyngeal tumours presented at Assuit University Hospital and were studied retrospectively with regard to sex and age incidence. Postericoid tumours form the majority (50.1 per cent) followed by pyriform fossa tumours (26.5 per cent) and finally posterior pharyngeal wall tumours (23.4 per cent). Men form the majority (211 out of 317, i.e. 69.7 per cent). The age incidence shows two peaks at 31–35 years and 56–60 years. The first peak is formed mainly of women while the second peak is formed mainly of men.

In addition, one third of the world's population is latently infected with Mycobacterium tuberculosis (Mtb) and up to 10% of infected individuals develop tuberculosis (TB) in their lifetime. In this investigation, the incidence of urinary tuberculosis among renal patients was studied. Three hundreds urine samples were processed for detection of Mtb by Ziehl-Neelson (ZN) smear examination, Lowenstein Jensen (LJ) medium, radiometric BACTEC460 system as well as polymerase chain reaction (PCR) and DNA Enzyme Immunoassay (DEIA) test. Out of 300 urine samples, 2 were positive by both ZN smears and LJ medium with incidence rate of 0.66 %, 3 positive samples by BACTEC460 culture system with incidence of 1%. PCR assay gave more positive results than smear and culture examination (i.e. 8 positive samples with incidence rate of 2.6%). The specificities were 25% for both ZN smears and LJ medium, 37.5% for BACTEC460 culture system, and 100% for PCR test, while sensitivities of all assays were 100%. Thus PCR is a rapid and sensitive method for the early diagnosis of urinary tuberculosis.
