Abstract:
BACKGROUND: Action plans are recommended for most patients with persistent asthma to reduce the morbidity associated with this chronic disease. Unfortunately, despite these recommendations, this tool remains underused.METHODS: The authors conducted a descriptive study at the asthma clinic of a tertiary care centre to determine the number of asthmatic patients presenting to a respiratory physician (new reference or follow-up visit) who possessed an individualized, written action plan, and to evaluate the patients’ level of confidence and perceived efficacy toward their plans. In addition, for all patients in the study, the level of confidence in and the perceived efficacy of three different action plans (two traditional tools versus a simplified tool) were compared.RESULTS: A total of 92 asthmatic patients were included in the study. Overall, 46% of the patients possessed an action plan. The patients’ average level of confidence and perceived efficacy toward their action plans were high (4.1 out of five and 3.3 out of four, respectively). When the three different action plans were compared, the level of confidence in and perceived efficacy of the traditional tools were similar, both being superior to the simplified tool.CONCLUSION: The number of asthmatic patients who presented to the asthma clinic and who possessed an action plan was higher than the reported Canadian mean of 10%; however, most of the patients were treated by specialized respiratory physicians, which may explain this improvement. Considering that most patients with persistent asthma should have an individualized, written action plan, the present study confirms that this tool is still not used for all asthmatic patients.

National guidelines recommend that all patients with asthma are offered a personal asthma action plan (PAAP), which contains potentially lifesaving information should a patient experience a decline in symptoms. A PAAP was created for use within a primary care setting based on a review of currently available plans. It was based on a colour-coded system: green for when the patient feels well, amber for a decline in symptoms, and red for an asthma attack. It was designed to fit onto one side of A4 paper, with guidance for the patient to take a photograph of it with their mobile phone. The PAAP was only recently introduced into practice, so its impact on reducing asthma related complications or hospital admissions has not yet been reviewed.

Objective: To summarize and evaluate existing literature regarding the impact of mobile asthma action plans (MAAPs) versus written asthma action plans (WAAPs) on degree of asthma control. Data Sources: PubMed, EMBASE, Web of Science, and ClinicalTrials.gov were searched (2000-January 2019) using the term asthma action plan with each of the following: smartphone, computers, handheld, mobile applications, portable electronic application, portable software application, tablet, or technology. Study Selection and Data Extraction: The search was limited to cohort and randomized controlled trials examining MAAP versus WAAP data. Data extracted included the following: study design, population, intervention, control, outcomes related to asthma control, and potential biases assessed using Cochrane Collaboration’s Risk of Bias Assessment Tool. Data Synthesis: Four of the 41 studies identified were included, each of which were randomized control trials. One study showed significant improvement using a non–asthma-specific assessment tool, 1 study showed improvement only for patients with uncontrolled asthma at baseline, and 2 studies showed no difference in asthma control scores. Overall risk of bias across all studies was low to moderate. Relevance to Patient Care and Clinical Practice: Health care providers should select an asthma action plan (AAP) format based on what the patient is most likely to understand and consistently use. Conclusions: Because of conflicting published data regarding the use of MAAPs versus WAAPs and risk of bias, it is unclear at this time whether one format of AAP is superior to the other for either adolescents or adults.

Abstract:
Asthma is a chronic respiratory disease affecting over 300 million individuals worldwide (Akinbami et al., 2012; Brashers, 2010). Despite increased knowledge regarding the mechanisms and pathophysiology of asthma and increased treatment options, the prevalence of asthma in the United States continues to rise (Akinbami et al., 2012). Evidence-based guidelines on the treatment and management of asthma have been available for over 20 years and the use of Asthma Action Plans (AAP) has been consistently recommended to provide patients with the self-management skills to control asthma symptoms. However, research has consistently demonstrated underutilized AAPs as a method to decrease asthma exacerbations. The purpose of this DNP project was to develop and implement a quality improvement (QI) initiative with Internal and Family Medicine in Mesa-Gilbert, Arizona that will improve asthma clinical management by providing the patient self-management skills needed to control the symptoms, prevent complications, and improve outcomes through the implementation of AAP standards for patients diagnosed with asthma. Using the Plan-Do-Study-Act (PDSA) method to promote quality improvement (QI), a root cause analysis was conducted to evaluate the current processes within the clinic. The QI team agreed upon a process change, which was implemented to promote the identification of patients with asthma so that an AAP could be implemented. The low rates of patients identified during the time period in which this QI project was conducted presented a limitation as to whether the process change was truly effective. This project discussed the process of QI using the PDSA Model for Improvement and need for evidence-based practice to promote improved patient outcomes. Further PDSA cycles and additional time are required to fully incorporate process change and to determine the effectiveness of the intervention.

Bakgrund: Astma är en folksjukdom som ökar kraftigt bland den svenska befolkningen. Sjukdomens svårighetsgrad kan påverka livskvalitet och arbetsförmågan.För att kunna hantera sjukdomen och minska besvären kan sjuksköterskan lära ut handlingar för egenvård. Syfte: Att belysa hur sjuksköterskor undervisar vuxna patienter med astma bronkiale. Metod: Litteraturstudien grundade sig på tidigare forskning och användes 10 vetenskapliga artiklar från Cinahl, PubMed och Psykinfodatabas. Artiklarna har varit både kvalitativa och kvantitativa. Resultat: Litteraturstudie visade sig på tre olika huvudkategorier. Sjuksköterskan förmedlar kunskap om egenvård genom att göra patienten delaktig, utforma individuellt anpassad utbildning och involvera anhöriga. Att upprätta handlingsplan görs tillsammans med patienten med hjälp av sjuksköterskan. Att kommunicera och ge stöd är en viktig komponent i patientens behandling. Slutsats: Inom vården kan man utgå från Orems och Suzie Kims domäner för en god egenvård. Egenvård bör utföras så gott som möjligt för att främja patientens välbefinnande.
Background: Asthma is a widespread disease that is increasing dramatically among the Swedish population. Diseases severity may affect quality of live and working ability. Inorder to cope with the disease and improve health nurse can teach acts of self-care. Aim: To illuminate how nurses teach adult patients with asthma bronchial. Method:The literature review is based on previous research and used 10 scientific articles from database, Cinahl, PubMed and PsycInfo. The review has encompassed both qualitative and quantitative reference. Results: The analysis of the articles revealed three different main categories. The nurse conveys knowledge about self-care by making patient involvement, develop individualized education and involve relatives. The preparation of action plan with the patient is positive for the treatment. To communicate and provide support is an important component of patient treatment. Conclusion: In healthcare it can be assumed Orems and Suzie Kims domains for a good self-care. Self-care should be performed as best as possible to promote patient comfort.

Abstract:
Evidence-based practice is an integral part of multi-disciplinary healthcare, but its routine clinical implementation remains a challenge internationally. Written asthma action plans are an example of sub-optimal evidence-based practice because, despite being recommended, these plans are under-issued by health professionals and under-used by patients/carers. This thesis is a critical analysis of the generation and implementation of evidence in this area and provides fresh insight into this specific theory/practice gap. This submission brings together, in five published papers, a body of work conducted by the candidate. Findings report that known barriers to action plan use (such as a lack of practitioner time) are symptomatic of deeper and more complex underlying factors. In particular, over-reliance on knowledge derived from randomised controlled trials and their systematic review, as the primary and sole source of evidence for healthcare practice, hindered the implementation of these plans. A lack of evidence reflecting the personal experience of using these plans in the real world, rather than in trial settings, contributed to a mismatch between what patients/carers want from asthma action plans and what they are currently being provided with by professionals. This submission illustrates the benefits of utilising a broader range of knowledge as a basis for clinical practice. The presented papers report how new and innovative research methodologies (including meta-ethnography and cross-study synthesis) can be used to synthesise individual studies reporting the personal experiences of patients and professionals and how such findings can then be used to better understand why interventions can be implemented in trial settings rather than everyday practice. Whilst these emerging approaches have great potential to contribute to evidence-based practice by, for example, strengthening the ‘weight’ of experiential knowledge, there are methodological challenges which, whilst acknowledged, have yet to be fully addressed.

Background: Tennessee made a commitment to improve the quality of care provided to children with asthma, which affects approximately 10% Tennessee's public school population. Asthma is chronic respiratory condition that can be managed with medications and behavioral and environmental interventions. Asthma action plans (AAP), which document an individual's asthma management plan, are a proven strategy associated with improved asthma outcomes. To improve the utilization of AAPs among children with asthma enrolled in public schools, the Tennessee Department of Health (TDOH) established the Tennessee Asthma Learning Collaborative (TALC), a multidisciplinary, multiagency stakeholder group that engaged in quality improvement activities at selected local sites across the state. This dissertation is an evaluation of TALC and its activities. Methods: TALC members and their community partners from local sites were interviewed about their experience participating in the learning collaborative. The Consolidated Framework for Research (CFIR) guided this qualitative data collection and analysis. AAPs were reviewed at local sites. A run chart tool was used to analyze quantitative data obtained from the review of AAPs to determine if a change in the number of forms collected occurred. Results: Participant interviews yielded information about barriers and facilitators to AAP collection in selected schools. Barriers included school nurse availability, inconsistent AAP forms across school systems, and lack of school nurse leadership at the state level. Facilitators included purposeful communication and collaboration between community healthcare providers and local school nurses. Analysis also revealed strengths and weaknesses in implementing the learning collaborative. Translation: This project informed two public health practice products. A manuscript was prepared for submission to the Journal of School Health that addresses the original state objective to reach 100% AAP compliance in schools and describes challenges related to implementing improvement strategies. A state report was produced for use by the TDOH which included recommendations for continued quality improvement efforts. Conclusion: CFIR framework provided a useful tool for evaluating the complex challenges and opportunities associated with implementing a state level multidisciplinary quality improvement effort and generated information that will inform future public health efforts to improve school-based management of chronic health conditions.

After formulating the problem and reviewing relevant research literature, a study was performed that compared reverse engineering with traditional scenario-based techniques in usability evaluation. In this case study, an online tool for creating asthma action plans was created and evaluated through questionnaires and focus groups. The tool was then tested in a controlled study using both a traditional scenario-based approach and the reverse engineering method. A group of twelve users built asthma action plans using each method in a randomized order. Results concerning usability, efficiency and the types of usability problems found were reported, along with recommendations for further research in the use of reverse engineering as a method of usability evaluation.

Asthma is the most common chronic medical condition to affect pregnancy. Asthma may adversely affect maternal quality of life and perinatal outcomes, as well as the course of asthma. Management of asthma during pregnancy should optimize the health of both the mother and her baby. Adherence to treatment, specifically inhaled corticosteroids, has been a problem for many pregnant asthmatic women and is usually due to concerns regarding the safety of these medications during pregnancy. Symptoms and pulmonary function need to be monitored on a monthly basis in pregnant asthmatic women so that any change in course can be matched with an appropriate change in therapy. Education is an important part of managing the pregnant asthmatic patient and includes explaining the relationship between asthma and pregnancy, identifying asthma triggers, providing training on correct use of inhalers, and establishing an asthma action plan. One of the most important needs for the future is the availability of further safety information for asthma medications used during pregnancy that can also account for asthma control.

Herbal medicines are utilized as traditional and alternative therapy to precisely restore declining metabolic functions. Herbal bioactive claims its rewards for their effectiveness, safety, and acceptability. Evolvulus alsinoides (Linn.) Linn. is a Virya herb from Convolvulaceae with Tridoshhara, Majjadhatu Rasayan, Vajikarak, Chedan, and Nidrajanan properties. In Ayurvedic medicine, the whole plant is used in the treatment of neurodegenerative diseases as brain tonic, amnesia and asthma, epilepsy and as a hepatoprotective. The phytochemical analysis has reported the existence of biomolecules such as β-sitosterol, scopolin, scopoletin, umbelliferon, triacontane, shankpushpine and betaine. A thorough review of classical as well as contemporary literature study has been done on Evolvulus alsinoides (Linn.) Linn. to validate the pharmacological actions and cytotoxicity effects on various cell lines.

In the summer of 2009, I participated in a rally against mountaintop removal coal mining in Appalachia. The rally was held on the grounds of Marsh Fork Elementary, a school situated between the Coal River and Route 3 in Raleigh County, West Virginia. Sitting immediately below a slurry impoundment (a giant reservoir of toxic coal sludge produced by the coal preparation process and retained by an earthen dam), Marsh Fork Elementary also sat at the center of many debates surrounding the safety and justness of mountaintop removal. Activists cited increased health problems for Marsh Fork students due to their proximity to an active strip mine, such as abnormally high rates of asthma, and worried about the potentially disastrous consequences of any stresses or failures in the earthen dam retaining the slurry. The nearby mine and processing plant were owned and operated at the time by Massey Energy, one of the most controversial coal companies in the region. It was led by Don Blankenship, an outspoken and active opponent of labor unions and environmental regulations. Both Blankenship and his company were frequent targets for environmentalist outrage, and for his part, Blankenship seldom passed an opportunity to denounce “tree huggers” and others who, so he claimed, would destroy the jobs of hard-working Appalachian miners. In 2012 a new elementary school was built several miles from the original site, thanks to donations and ongoing political pressure; but in June 2009 these issues remained unsettled....

The movie Erin Brockovitch, starring Julia Roberts, opened in the spring of 2000 to excellent reviews and immediate popular success. Advertised as being “based on a true story,” it describes how an uneducated but feisty young woman discovers a cluster of diseases in a small California town, including uterine and breast cancer, Hodgkin’s disease, brain cancer, colon cancer, asthma, heart disease, and disorders of the immune system, which she attributes to contamination of the town’s drinking water with chromium, a toxic metal, due to negligent waste disposal practices of a large corporation. She then initiates a class-action lawsuit on behalf of the victims, which is settled for $333 million, the largest sum yet won in such a suit. The true story on which the case was based involved the Pacific Gas and Electric Corporation, which really did settle the lawsuit by paying out $333 million. Whether the willingness of the corporation to settle for this amount proves that the chromium did cause all the diseases claimed is an interesting question, which this book may shed some light on, but not directly answer. The movie is one of a number of recent films reflecting a widespread fear that the environment is being polluted by hazardous chemicals and harmful radiation that cause cancer and other diseases, and a need to identify and seek restitution from whoever is responsible. People living in the advanced industrialized countries of the world suffer from mixed feelings about their high standard of living. They are fully aware of its advantages, both for health and for the quality of life generally: greater freedom from infectious diseases, better medical care, longer life span, a higher standard of living, more choice in work and play. But there is a sense of disadvantages as well: concerns about a loss of community and of cultural diversity, about an obsession with material possessions, about pollution and destruction of the environment.
