Full textBackground: Asthma is a widespread disease that is increasing dramatically among the Swedish population. Diseases severity may affect quality of live and working ability. Inorder to cope with the disease and improve health nurse can teach acts of self-care. Aim: To illuminate how nurses teach adult patients with asthma bronchial. Method:The literature review is based on previous research and used 10 scientific articles from database, Cinahl, PubMed and PsycInfo. The review has encompassed both qualitative and quantitative reference. Results: The analysis of the articles revealed three different main categories. The nurse conveys knowledge about self-care by making patient involvement, develop individualized education and involve relatives. The preparation of action plan with the patient is positive for the treatment. To communicate and provide support is an important component of patient treatment. Conclusion: In healthcare it can be assumed Orems and Suzie Kims domains for a good self-care. Self-care should be performed as best as possible to promote patient comfort.

Full textAnxiety is a common reponse in patients undergoing cardiac surgery. The source of this anxiety is attributed to a multitude of factors. Research findings support the relationship between preoperative preparation and a reduction in anxiety. However, a lack of information exists as to the role of critical care nurses in this process. This purpose of this study was to examine the effect of a structured preoperative education/orientation visit by the critical care nurse on the level of patient anxiety. A sample of 21 patients were drawn from the population of cardiac surgical patients and randomly assigned to the experimental or control group. Subjects in the experimental group received a structured preoperative education/orientation visit by a critical care nurse the evening prior to surgery. Subjects in the control group did not receive the visit. Subjects in both groups completed the State-Trait Anxiety Inventory preoperatively and the A-State Scale plus an additional questionaire postoperatively. Data were analyzed using analysis of covariance on repeated measures and a t-test comparison of mean anxiety scores. Findings revealed no significant differences in postoperative state anxiety scores between the experimental and control group. Results from the second questionaire indicated that subjects in the experimental group felt the preoperative visit was helpful. Furthennore, those patients who had the continuity of the same nurse conducting the preoperative visit and assuming primary care postoperatively in the intensive care unit responded more favorably on the questionaires. There were however, significant pre-existing differences in age and trait anxiety scores between the groups. This fact, in addition to the small sample size limit the ability to evaluate the effectiveness of the experimental intervention. Thus, further research is warranted to determine the effect of a preoperative education/orientation visit by critical care nurses utilizing a larger sample size.

Full textSyfte: Syftet med denna kvalitativa intervjustudie var att beskriva sjuksköterskans yrkesroll samt patientrollens förändring under olika tidsperioder. Metod: Data inhämtades genom semistrukturerade intervjuer, där respondenterna hade en koppling till sjuksköterskeyrket, antigen som student, nu arbetande sjuksköterska eller pensionerad sjuksköterska. Datan analyserades med innehållsanalys. Resultat: De äldre respondenterna anser att sjuksköterskan har en hög status idag och ses som en påläst och välinformerad person. Samarbetet mellan sjuksköterska och läkare anser de flesta har förbättrats genom tiden. Flertalet äldre sjuksköterskor påpekar att nyexaminerade sjuksköterskor idag har en god teoretisk kunskap, men att den praktiska kunskapen inte är lika bra. Idag arbetar sjuksköterskan mer självständigt fast med mer administrativa uppgifter. Att hälso- och sjukvårdens yrkesgrupper har en gemensam klädsel ses som någonting positivt, dock kan det vara svårt för patienterna att se skillnad på yrkesgrupperna. Patienten får idag mer information och är även mer välinformerad, framför allt genom internet. Idag finns även ett större patientinflytande, särskilt inom barnsjukvården och psykiatrisk vård. Slutsats: Genom att få en övergripande syn på vad förändringarna har medfört kan arbetet med vidareutveckling inför framtiden ske.

Full textWithin the health care system, patient safety outcomes have been criticized for many years. Medical malpractice, common errors, and nosocomial infections (i.e., hospital-acquired infections) are safety concerns, and represent a public health problem. Since the Institute of Medicine (1999) published To Err is Human: Building a Safer Health System in 1999, changes have been made to improve the use of technology and leverage advancements in research that improve patient safety. Nurse leaders can also help to facilitate process improvements in the patient safety culture. The purpose of this capstone project was to explore the nursing leader role in improving patient safety in a hospital setting. The method utilized for this study was a literature review. Prominent articles identifying the role of nursing leadership were included. Seven drivers of patient safety were identified (Sammer, Lyken, Singh, Mains, & Lackan (2011), and subsequently informed this project. The targeted populations were patients, families, nurses, nurse administrators, and medical personnel. Findings regarding the nurse leader role, patient improvements, and barriers to improvements were reviewed. Nurse leaders were found to be of critical importance to patients, medical personnel, and the health care system. The limitations of this review and implications for policy and practice are discussed.

Full textEffective communication ensures patient safety and quality care. Shift report is multi-faceted, and if not conducted in a consistent manner, can be a source of errors. Implementing bedside report places focus on the patient’s values and needs and meets Quality and Safety Education for Nurses (QSEN) competencies, including patient-centered care. Bedside reporting with an established template improves patient safety, as well as nurse and patient satisfaction. The investigator expected that by implementing nurse and patient education while establishing a bedside reporting template, there would be an increase in nurse and patient satisfaction, improved bedside reporting frequency, and a decrease in patient safety alerts.
This advanced scholarly project was conducted at a Midwest hospital including approximately 160 nurses in various departments. A nonrandomized control pretest and posttest assessed nurse satisfaction with bedside report frequency. The requirements of the reporting template were established by the Council for Nursing Excellence (CFNE). Education on the template included nurse training and patient brochures before implementation. Additional assessments examined patient satisfaction scores and safety alerts. Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) scores provided details on patient satisfaction, while Iris alerts offered information on patient safety as it related to communication or a lack thereof.
HCAHPS scores revealed inconsistencies throughout the various units while Iris alerts provided minor quantities of data. Nurse survey results demonstrated a decrease in nurse satisfaction with the new template, as well as a reduction in the verification of report; however, there were improvements in satisfaction for all other areas examined including the frequency of report while including the patient, sufficiency of report, the ease of including the patient in report, as well as the belief that information is not lost during the reporting process.

Full textExcerpt Chapter 2 - A review of the literature has revealed that asthma management practices in the Australian community are currently suboptimal resulting in significant morbidity and mortality. In adolescent asthma there are added challenges, with problems of self-image, denial and non-adherence to therapy where self-management skills assume a greater importance (Forero et al 1996, Price 1996, Brook and Tepper 1997, Buston and Wood 2000, Kyngäs et al 2000). In rural and remote areas in Australia, asthma management practices have been shown to be poorer and mortality rates from asthma are considerably higher compared to metropolitan areas (AIHW ACAM 2005, AIHW 2006). Limited access and chronic shortages of specialist services in rural areas are shifting the burden more and more towards the primary sector (AIHW 2006). It becomes paramount that people with asthma in rural settings become involved in self-management of their asthma and that community based health care providers be more proactive in facilitating these self-management behaviours by appropriate education and counselling. Health promotion activities, which are a broad range of activities including health education, have been acknowledged as having the potential to improve the health status of rural populations (National Rural Health Alliance 2002). Community pharmacy settings have been shown to be effective sites for the delivery of health promotion, screening and education programs (Anderson 2000, Elliott et al 2002, Cote et al 2003, Hourihan et al 2003, Watson et al 2003, Boyle et al 2004, Goode et al 2004, Paluck et al 2004, Sunderland et al 2004, Chambers et al 2005, Saini et al 2006). In the case of asthma, outreach programs have been shown to have beneficial effects in terms of reducing hospital admissions and emergency visits and improved asthma outcomes (Greineder et al 1995, Stout et al 1998, Kelly et al 2000, Legorreta et al 2000, Lin et al 2004). We proposed to extend the role of the community pharmacist beyond the traditional realm of the “pharmacy” into the community in rural Australia with the first asthma outreach programs designed for community pharmacy. The outreach programs were designed to include two health promotion strategies, the first targeting adolescents in high schools and the second targeting the general community. The project aimed firstly, to assess the feasibility of using community pharmacists to deliver two asthma outreach programs, one targeting adolescents and one for the wider community in a rural area and secondly, to assess the programs’ impact on adolescent asthma knowledge and requests for information at the community pharmacy. Excerpt Chapter 3 - Patient education is one of the six critical elements to successful long-term asthma management included in international and national asthma management guidelines, which have emphasised education as a process underpinning the understanding associated with appropriate medication use, the need for regular review, and self-management on the part of the person with asthma (Boulet et al 1999, National Asthma Council 2002, National Asthma Education and Prevention Program 2002, British Thoracic Society 2003, NHLBI/WHO 2005). The ongoing process of asthma education is considered necessary for helping people with asthma gain the knowledge, skills, confidence and motivation to control their own asthma. Since most health care professionals are key providers of asthma education, their knowledge of asthma and asthma management practices often needs to be updated through continuing education. This is to ensure that the education provided to the patient conforms to best practice guidelines. Moreover, health care professionals need to tailor this education to the patients’ needs and determine if the education provided results in an improvement in asthma knowledge. A review of the literature has revealed that a number of questionnaires have been developed that assess the asthma knowledge of parents of children with asthma (Parcel et al 1980, Fitzclarence and Henry 1990, Brook et al 1993, Moosa and Henley 1997, Ho et al 2003), adults with asthma (Wigal et al 1993, Allen and Jones 1998, Allen et al 2000, Bertolotti et al 2001), children with asthma (Parcel et al 1980, Wade et al 1997), or the general public (Grant et al 1999). However, the existing asthma knowledge questionnaires have several limitations. The only validated asthma knowledge questionnaire was developed in 1990 and hence, out of date with current asthma management guidelines (Fitzclarence and Henry 1990). The shortcomings of the other knowledge questionnaires relate to the lack of evidence of the validity (Wade et al 1997, Grant et al 1999, Bertolotti et al 2001), being outdated 81 with current concepts of asthma (Parcel et al 1980) or having been tested on small or inadequately characterised subject samples e.g. subject samples consisting of mainly middle class and well educated parents (Brook et al 1993, Wigal et al 1993, Moosa and Henley 1997, Allen and Jones 1998, Allen et al 2000, Ho et al 2003). Furthermore, most of the published asthma knowledge questionnaires have been designed to assess the asthma knowledge of the consumer (i.e. a lay person with asthma or a parent/carer of a person with asthma). There is no questionnaire specifically developed to assess the asthma knowledge of health care professionals, who are key providers of asthma education. It is hence important to have a reliable and validated instrument to be able to assess education needs and to measure the impact of training programs on asthma knowledge of health care professionals as well. An asthma knowledge questionnaire for health care professionals might also be used to gauge how successful dissemination and implementation of guidelines have been. Excerpt Chapter 4 - Asthma self-management education for adults that includes information about asthma and self-management, self-monitoring, a written action plan and regular medical review has been shown to be effective in improving asthma outcomes (Gibson et al 1999). These interventions have been delivered mostly in a hospital setting and have utilised individual and/or group formats. Fewer interventions have been delivered in a primary care setting, usually by qualified practice nurses and/or general practitioners or asthma educators and, to date, their success has not been established (Fay et al 2002, Gibson et al 2003). Community pharmacy provides a strategic venue for the provision of patient education about asthma. Traditionally, patient education provided by community pharmacists has been individualised. However, group education has been shown to be as effective as individualised education with the added benefits of being simpler, more cost effective and better received by patients and educators (Wilson et al 1993, Wilson 1997). While small group education has been shown to improve asthma outcomes (Snyder et al 1987, Bailey et al 1990, Wilson et al 1993, Yoon et al 1993, Allen et al 1995, Kotses et al 1995, Berg et al 1997, de Oliveira et al 1999, Marabini et al 2002), to date, no small-group asthma education provided by pharmacists in the community pharmacy setting has been implemented and evaluated.

Full textIn Guadeloupe, a French Overseas Territory, the population uses French and Creole linguistic codes. The main goals of this thesis are to understand and to explain how a better use of French and Creole, within the framework of educational medical exchanges, can improve the understanding of the mechanisms of the disease by the patient, its acceptance and its management.The bronchial asthma is a chronic disease, which affects an important part of the Guadeloupian population. In spite of the recommendations promulgated by the medical scholars, the care of the patients can be improved in Guadeloupe. Certain patients have difficulty understanding the terms used in French by their doctor during the announcement of the diagnosis, as well as by the educators in charge of bringing them the tools of good management of this disease what is not without incidents in terms of morbidity and mortality.The author elaborates his research around five main themes:- The thorough description of the space studied in this thesis; the geography of the land, the history of the population, the impact of the economic; the presentation of the social and health care infrastructures; and furthermore the birth of a new cultural era generating a language shared by more than 95 % of the population and passed on within families and communities.- The presentation of the therapeutic education, the approach considered by the scientific community as essential to improve the management of the patient and the self-management of the disease, the successive governmental plans dealing with this subject.- The state of the knowledge on the bronchial asthma in France, in the world and in Guadeloupe more particularly.- The impact of the Creole language in the process of understanding of the disease within the healthcare relationships and its consequences at the bilingual natives in terms of acquisitions of the tools of self-management in comparison to the use of French.- The formulation of proposals of didactic contextualization regarding the training of the healthcare professionals; the recommendation to the healthcare workers to use the Creole language; the adaptation of the psycho-pedagogical tools to the local context; the encouragement to the patients to use their native language in the expression of their feelings.- Researches show that the Creole language used during the consultation by the regular doctor, but also during the educational sessions therapeutics by the educators influence the indicators of management of this disease. In conclusion, the author defends the hypothesis according to which, the use of the Creole language in the educational medical exchanges at the bilingual asthmatic teenagers French-Creole would favor the understanding of the mechanisms of the asthmatic disease, the awareness of its gravity and the necessity of looking after oneself to avoid the complications, corollary of a good quality of life

Full textBackground In 2018, 754 310 surgical procedures were performed on patients over 20 years old in the inpatient care in Sweden. For many patients, surgical procedures are associated with a feeling of anxiety. Studies have shown that 60-80 per cent of surgical patients suffer from anxiety. As a nurse, relieving the patients ́ anxiety prior to a surgical procedure and increasing patient participation requires time, knowledge and commitment. Aim The aim was to highlight nursing interventions that may encourage increased patient participation in preoperative care. Method A literature review where searches for scientific articles were made in PubMed and CINAHL. Eighteen scientific articles were selected and reviewed using an integrated analysis, examination of quality and then sorted by themes according to their findings. Result The results of the included scientific articles showed the importance of clear information and a person-centred approach from the nurses preoperatively to create participation and reduce anxiety. Patients who held adequate and clear information as well as when the nurse had a person-centred approach in both treatment and communication, entailed that the patients felt well prepared for the surgical procedure. Conclusion A person-centred approach and clear information to patients preoperatively are the nursing interventions shown to be most effective from the patient's perspective. This is the result of this literature review. As a nurse, establishing a relationship with the patient based on the patient's story and identifying the patient's need for care regarding the degree and content of information given prior to the surgical procedure, may be the key to increased patient participation.

Full textHuman beings who are in need of caring in nursing requires pedagogical learning in the form of support from the nurse facing the issues and challenges they are receiving. How pedagogical learning is designed in different care situations is not yet sufficiently scientifically explored, making it important to map the problem of the lack of pedagogical learning in nursing. The aim was to identify what pedagogical learning is in nursing. The method is a literature review of 8 scientific articles that formed the basis for the result. The results showed six different sub-themes; pedagogical flaws, pedagogical innovation, the nurse, the patient's life-world, pedagogical mutuality and pedagogical care and who formed the main theme "With the eyes of others”. Pedagogic is one of the most important key aspects in nursing in order to initiate a proper care to the patient. In order to integrate pedagogy as part of the care process, more training, knowledge and research in pedagogy is needed and learning how unique nursing situations for the unique patient should be applied.

Full textTodays patients face difficulties in choosing between methods of treatment and care. Therefore it is very important that nurses give the patient an opportunity to handle their daily life and support them in their choises. Patient education is of central importance and should be integrated in the daily care of patients and their relatives. This literature review aims to investigate the factors that influence the nurse role in patient education and in what way Internet can be used. Peplau’s theory of Interpersonal relations underlie this study. The sudy is based on the results of ten scientific publications. The results indicate that nurses not always have enough knowledge to provide good patient education. More education is needed in the nursing education programme. With todays technical evolution Internet can function as an excellent tool in patient education.

Sexuality is affected by both illness and treatments, which is a reason why nurses ought to incorporate sexuality into nursing practice. Despite being aware of the importance of patients’ sexuality, many nurses choose not to address the issue, which could possibly cause the patient unnecessary suffering as sexual dysfunction can be the only symptom indicating severe illness. The aim of this study was to explore nurses’ attitude towards patients’ sexuality throughout life. The study was conducted as a literature review based on eleven scientific articles. Qualitative as well as quantitative articles were included in the study. Results showed that there are a number of barriers that affect nurses’ attitudes towards patients’ sexuality. The majority of all informants were well aware that addressing patients’ sexuality formed part of their duties as nurses, however, only a small number of nurses actually did address the issue. The most prominent barrier iden-tified by both nurses and nursing student was their self-reported lack of education in sexology.

Full textHow patients and nurses experience participation in care is an important aspect to advance and improve the patient’s participation in care. The aim with the study was to investigate experiences of the patient’s participation from a patient and nurse perspective. The study was performed as a systematic literature study and is based on 14 scientific articles. The results showed that patients’ experiences of patient participation included to be seen and respected as an own individual. An inviting environment where the nurse was sensitive and where the relation was equal between the parts was experienced as important. Nurses experienced that it was important to have an empathic approach where establishing of a functional relation was essential. This meant a respectful manner where patients were encouraged and recognized. A good communication, a giving and taking between the parts and individual and adequate information was important. Both patients and nurses experienced lack of time, lack of continuity and limited choices as hindrances for participation. It is important to raise patient participation in nurse education. Daily reflection in nursing care can be one way for nurses to maintain awareness of the importance of patient participation. It would be desirable with further research which can explain why there are shortages with patient participation and the benefits with patient participation.

Full textBackground: Today's research shows that mental illness is increasing globally. In society, we can see a stigmatizing and discriminatory attitude towards people with mental illness. It appears that this patient group has a greater need for care, but that they feel unfairly treated and cared for in the health and medical care. Aim: The literature review aims to describe attitudes of nurses in somatic care towards people with mental illness. Method: This literature review has an integrative method which means that the result is based on both qualitative (9) and quantitative articles (2) from databases MEDLINE, CINAHL and PsycINFO. All articles have been read thoroughly to correlate to the studys’ aim and their quality has been assessed with guidelines from the School of Health and Welfare in Jönköping. Result: Three main themes emerged: Having a judgmental attitude towards the patient, avoiding attitudes and empathetic attitudes. Conclusion: Discriminatory and positive attitudes can be found in nurses. In between, there are nurses who prioritize caring for patients with mental illness related to external and internal circumstances such as perception of responsibility, care culture, lack of assets, powerlessness and hopelessness. Education and experience can, according to the results of the literature review, give positive attitudes to the nurse under the right conditions, but society's view of persons with mental illness seems to be a strong factor that influences nurses' attitude. The nurse's attitude towards people with mental illness correlated with society lived in.

Full textBackground: Living with long term illness means losses for the individual and demands for adaptation. An important part of the nurse's work on strengthening the health of people in need of care is to support learning. An important part of learning is interaction and participation. As a nursing-science basis Dorothea Orem’s "Theory of nursing system" has been used and this theory describes the interaction between the nurse and the person she cares for. Aim: The purpose was to gain knowledge about how nurses can work with learning in a group of persons with long-term illness with basis in Dorothea Orem’s "Theory of nursing systems." Method: A literature review was carried out where ten quantitative studies have been compiled through analysis with Dorothea Orem’s "theory of nursing system" as a basis. Results: The results how how the various programs of the studies worked with learning in a group among people with long-term illness, where similarities and differences in the roles of the participants and the nurses emerged. Similarities were found in the programs focus on the interaction between participants as well as on the facilitating role of the nurse. An important difference was revealed in the extent of participants' influence on the content of the group sessions, where some programs let participants' needs and interests guide while some programs previously had determined the content. Discussions: In the discussion of the results it is argued that man's self-identified learning needs should direct learning. Learning should encourage the development of the person’s self-direction. The results show that participants in the interaction between each other have inherent resources used to promote learning among themselves and others. Strengths and weaknesses regarding the chosen method for this literature review are highlighted in the method discussion.

Full textUnhealthy lifestyle habits increase the risk of illness and death. Cardiovascular disease, cancer and type-2 diabetes are examples of diseases that can be avoided by making positive lifestyle changes. An important part of the nurse's job is to teach patients how they can change their lifestyle and take responsibility for their own self-care. The purpose of this study was to highlight the nurse's role in patient education with a focus on lifestyle changes. The study was done in the form of a literature review in which the basis for the results consisted of 11 scientific articles. The result showed that the nurse's assessment of the patient's need for knowledge was an important part of the teaching process. Factors of importance for the teaching results were information and the selection of appropriate methodology and model. The nurse's pedagogical approach was another factor that influenced their teaching. The study shows that nurses need to adjust their role based on the patient's needs. It has also been found that there are deficiencies in the nurse's competence in terms of pedagogical and didactic skills. By expanding patient education for nursing and prioritize it in health care, there are profits to be made from both a patient perspective and socioeconomically. More scientific research is needed in the subject of how the practices of planning and implementation of patient education can be improved.

Full textBakgrund: Den senaste ebolaepidemin i västafrika har gjort att blödarsjukdomar hamnat i rampljuset. Uganda har genom åren drabbats av flera utbrott som framgångrikt hanterats. Sjuksköterskors patientutbildning spelar en viktig roll i arbetet för att öka allmänhetens medvetenhet om blödarsjudkomar. Syfte: Att undersöka hur sjuksköterskor på akutmottagningen utbildar patienter och anhöriga om blödarsjukdomar. Metod: En explorativoch deskriptiv kvalitativ studie med kvalitativ ansats användes. Sjuksköterskor som arbetar på akutmottagningen har djupintervjuats med 18 öppna frågor. Data har analyserats med en kvalitativ innehållsanalys och analyserats utifrån Peplaus ”interpersonal relationship theory”. Resultat: Genom dataanalysen utvecklades fyra kategorier; Stor variation på utbildningstekniker, Upplevda hinder, Hur man ska nå en bredare publik och Önskvärda egenskaper som utbildande sjuksköterska. Sjuksköterskorna använder många olika metoder för utbilda patienter och anhöriga om blödarsjukdomar, ofta i unika kombinationer. Tidsbrist och alltför instabila patienter på akutmottagningen sågs som de största hindren för att utbilda. Metoder för att nå allmänheten och att anställa en speciell utbildningssjuksköterska på avdelningen var några av förslagen för att förbättra det förebyggande arbetet mot blödarsjukdomar. Slutsats: Sjuksköterskorna är medvetna om vikten av patientutbildning och använder de pedagogiska metoder de anser vara mest effektiva. Då patienterna kommer till akutmottagningen är det dock ofta för sent och preventiva åtgärder borde prioriteras. Att öka sjuksköterskornas kunskap i utbildningstekniker och patientutbildning kan vara en viktig del i det preventiva arbetet för att minska risken för kommande utbrott.

Full textBackground: Each year approximately 100,000 patients in Sweden suffer from healthcare injuries due to the insufficiency of documentation. Lack of communication between healthcare staff is one of the main factors for healthcare injuries. In view of this, this literature review aims at highlighting the nurse's experiences and attitudes towards nursing documentation. Aim: The aim was to highlight nurses' experiences of nursing documentation. Method: A literature review with qualitative articles and inductive approach. Results: Time consuming and difficult to manage computer systems with inadequate training of the new systems and large demand from the community made it difficult to document. Although the nurses felt that it was time-consuming, they understood the importance of a complete documentation because of the patient safety it provides. Nurses on the other hand, consider that the lack of support from the organization made it difficult for them to complete documentation. Conclusion: More research focusing on the essentials of increased supports when implementation of a new system or learn the electronic health system from scratch is needed. To allow nurses to receive more personalized assistance with regard to further education, could have a positive impact on a more professionally and accurate documentation.
