Abstract:
ObjectiveEmergency general surgery (EGS) patients experience superior outcomes when cared for within an acute surgical unit (ASU) model. EGS structures in most Australian hospitals remain unknown. This study aimed to describe the national spectrum of EGS models. MethodsA cross-sectional study was performed of all Australian public hospitals of medium or greater peer group (>2000 patient separations per annum). The primary outcome was the incidence of each EGS model. Secondary outcomes were the relationship of the EGS model to objective hospital variables, and qualitative reasons for the choice of model. ResultsOf the 120 eligible hospitals, 119 (99%) participated. Sixty-four hospitals reported using an ASU (28%) or hybrid EGS model (26%), whereas the remaining 55 (46%) used a traditional model. ASU implementation was significantly more common among hospitals of greater peer group, bed number, surgeon pool and trauma service sophistication. Leading drivers for ASU commencement were aims to improve patient care and decrease after-hours operating, whereas common barriers against uptake were insufficient EGS patient load or surgeon on-call pool. ConclusionsASU or hybrid models of care may be more widespread than currently reported. The introduction of such structures is heavily dependent on hospital and staff size, trauma subspecialisation and EGS patient throughput. What is known about the topic?Traditionally, general surgical staff were rostered to elective operating and clinic duties, with emergency patients managed on an ad hoc basis. An ASU model, with a surgeon dedicated to EGS patients, has been associated with superior outcomes. However, the Australian uptake of this model is unknown. What does this paper add?This study enrolled 119 of 120 (99%) Australian public hospitals of medium or greater peer group (>2000 patient separations per annum). Uptake of the ASU or hybrid model was more widespread than expected, existing in 64 of 119 (54%) centres. Factors for and against ASU implementation were also assessed. What are the implications for practitioners?Hospitals considering implementing an ASU or hybrid model will be reassured by the common reports of improved patient outcomes and decreased after-hours operating. However, potential hospitals must assess the suitability of the ASU model to their surgeon pool and EGS patient load.

Abstract:
In this study, iron oxide (Fe3O4) was coated with ZrO2, and doped with three rare earth elements((Y/La/Ce), and a multi-staged rare earth doped zirconia adsorbent was prepared by using uniform design U14, Response Surface methodology, and orthogonal design, to remove As3+ and As5+ from the aqueous solution. Based on the results of TEM, EDS, XRD, FTIR, and N2-adsorption desorption test, the best molar ratio of Fe3O4:TMAOH:Zirconium butoxide:Y:La:Ce was selected as 1:12:11:1:0.02:0.08. The specific surface area and porosity was 263 m2/g, and 0.156 cm3/g, respectively. The isothermal curves and fitting equation parameters show that Langmuir model, and Redlich Peterson model fitted well. As per calculations of the Langmuir model, the highest adsorption capacities for As3+ and As5+ ions were recorded as 68.33 mg/g, 84.23 mg/g, respectively. The fitting curves and equations of the kinetic models favors the quasi second order kinetic model. Material regeneration was very effective, and even in the last cycle the regeneration capacities of both As3+ and As5+ were 75.15%, and 77.59%, respectively. Adsorption and regeneration results suggest that adsorbent has easy synthesis method, and reusable, so it can be used as a potential adsorbent for the removal of arsenic from aqueous solution.

Abstract:
ObjectiveTo assess the incidence and risks of common extra-articular manifestations (EAMs), that is, acute anterior uveitis (AAU), psoriasis and inflammatory bowel disease (IBD), in patients with ankylosing spondylitis (AS) compared with population-based controls.MethodsAll incident patients with AS (n=4101) from the UK Clinical Practice Research Datalink (1987–2012) were matched with up to seven control subjects without AS by year of birth, sex and practice (n=28 591). Incidence rates, cumulative incidence rates and adjusted (adj) HRs for the development of EAMs were calculated, with time-dependent adjustments for age, sex, comorbidity and medication use.ResultsAt diagnosis of AS, the proportion of patients with an EAM was 11.4% for AAU, 4.4% for psoriasis and 3.7% for IBD. Incidence rates of EAMs were 8.9/1000 person-years for AAU, 3.4/1000 person-years for psoriasis and 2.4 /1000 person-years for IBD in AS. The 20-year cumulative incidence was 24.5%, 10.1% and 7.5%, respectively. Risks of EAMs were 1.5-fold to 16-fold increased versus controls, with an adj HR of 15.5 (95% CI 11.6 to 20.7) for AAU, adj HR of 1.5 (95% CI 1.1 to 1.9) for psoriasis and adj HR of 3.3 (95% CI 2.3 to 4.8) for IBD. For psoriasis and IBD, the highest risks were found in the 1st years after diagnosis, while developing AAU continued to be increased also 10 years after diagnosis of AS.ConclusionsThe risk of, in particular AAU, but also of psoriasis and IBD, is significantly increased in patients with AS compared with controls. Hazard patterns are different for each of the EAMs.

ObjectivesExtra-articular manifestations (EAMs) are important systemic features of axial spondyloarthritis (axSpA), which may influence the choice of tumour necrosis factor-inhibitor (TNFi). We examined the cumulative incidence and predictors of EAMs and the influence of these on first TNFi choice in a ‘real-world’ cohort of patients with axSpA.MethodsClinical and patient-reported outcomes of 2420 patients with axSpA from 83 centres were collected by the British Society for Rheumatology Biologics Register in Ankylosing Spondylitis. Lifestyle factors for EAMs (acute anterior uveitis (AAU), inflammatory bowel diseases (IBD), psoriasis) were compared with those without EAMs. Also, the association between pretreatment EAMs and choice of first TNFi (adalimumab, etanercept, certolizumab) was analysed.ResultsAAU was directly associated with human leukocyte antigen (HLA)-B27 (incidence rate ratio (IRR) 1.95, 95% CI 1.40 to 2.73) and inversely associated with ever-smoking (IRR=0.71, 95% CI 0.55 to 0.92). For both psoriasis and IBD, there was an inverse relationship with HLA-B27 (IRR 0.54, 95% CI 0.36 to 0.79 and IRR 0.63, 95% CI 0.43 to 0.91, respectively). A diagnosis of either AAU (OR 3.79, 95% CI 2.11 to 6.80) or IBD (OR 5.50, 95% CI 2.09 to 14.46) was associated with preference for adalimumab versus others. In contrast, a diagnosis of either AAU (OR 0.14, 95% CI 0.06 to 0.33) or IBD (OR 0.17, 95% CI 0.05 to 0.57) was associated with less preference for etanercept over other TNFi.ConclusionThe higher occurrence of AAU and lower occurrence of psoriasis and IBD in HLA-B27-positive patients with axSpA are consistent with current pathophysiology. Patients with previous AAU and IBD are more likely to be prescribed adalimumab and less likely to receive etanercept, consistent with the superior efficacy of monoclonal TNFi for these indications. Future work will determine whether EAMs influence TNFi survival, or effectiveness, and whether this varies between agents.

Abstract:
BackgroundOut-of-hospital cardiac arrest, the leading cause of death among patients on hemodialysis, occurs frequently within outpatient dialysis centers. Practice guidelines recommend resuscitation training for all dialysis clinic staff and on-site defibrillator availability, but the extent of staff involvement in cardiopulmonary resuscitation (CPR) efforts and its association with outcomes is unknown.MethodsWe used data from the Cardiac Arrest Registry to Enhance Survival and the Centers for Medicare & Medicaid Services dialysis facility database to identify patients who had cardiac arrest within outpatient dialysis clinics between 2010 and 2016 in the southeastern United States. We compared outcomes of patients who received dialysis staff–initiated CPR with those who did not until the arrival of emergency medical services (EMS).ResultsAmong 398 OHCA events in dialysis clinics, 66% of all patients presented with a nonshockable initial rhythm. Dialysis staff initiated CPR in 81.4% of events and applied defibrillators before EMS arrival in 52.3%. Staff were more likely to initiate CPR among men and witness cardiac arrests, and were more likely to provide CPR within larger dialysis clinics. Staff-initiated CPR was associated with a three-fold increase in the odds of hospital discharge and favorable neurologic status on discharge. There was no overall association between staff-initiated defibrillator use and outcomes, but there was a nonsignificant trend toward improved survival to hospital discharge in the subgroup with shockable initial cardiac arrest rhythms.ConclusionsDialysis staff–initiated CPR was associated with a large increase in survival but was only performed in 81% of cardiac arrest events. Further investigations should focus on understanding the potential facilitators and barriers to CPR in the dialysis setting.

Este trabalho apresenta um estudo sobre a inspecionabilidade dos furos da junção asa/stub das aeronaves das famílias EMB 145, EMB 170 e EMB 190. O estudo ée baseado nos métodos de avaliação não-destrutiva (NDE) utilizados pela Embraer. Os furos em questão neste trabalho são inspecionados pelo método de Eddy Current, sendo este o mesmo método utilizado na linha de produção da Embraer. Corpos-de-prova simulam a região da aeronave estudada em termos de espessuras e ligas de alumínio utilizadas para a fabricação das peças. Defeitos como ovalizações, marcas de ferramentas e riscos de tamanhos diversos são reproduzidos nos corpos-de-prova e inspecionados por Eddy Current. Os resultados das inspeções são comparados a requisitos técnicos de projeto e produção e, finalmente, são apresentadas as conclusões e considerações finais do trabalho.

E-voting involves using different forms of electronic means like

This thesis concerns voting by telephone, or televoting, it starts by giving a brief overview and evaluation of various models and technologies that are implemented within such systems. Abstract:
The Ministry of Education in Abu Dhabi launched an extensive reform initiative developed by the Abu Dhabi Education Council (ADEC) in partnership with international operators. The reform has introduced innovative teaching methodologies, modern books, a new curriculum and the use of English as a medium of instruction (EMI) in mathematics and science classes. The research carried out for this thesis investigated 11 Emirati female teachers. The data was collected from interviews and several informal observations carried out in a public middle school in a town outside Abu Dhabi city, UAE. The rationale for this study is to critically investigate the effects that rapid reform is having on the performance of the female teachers in the classroom and the impact of this speedy reform on their professional and personal lives so as to highlight differences between reform theory and practice. This study is significant because few studies on female teachers in a Gulf educational reform environment have been undertaken and it is essential to uncover the foreign and non-egalitarian nature of the reform programme. Therefore, the study’s aims are to highlight teacher perspectives and teacher marginalization, EMI and consider whether the reform is being implemented successfully at the classroom level. Of course, an important aim of this critical study has also been to raise the consciousness of those participating in the reform. This study has revealed mostly negative perceptions regarding the Abu Dhabi education reform programme, in particular with teachers’ perceptions of the work environment and the new pedagogy. It has also disclosed several instances of teacher marginalization as the result of a top-down reform and has exposed a prevailing sentiment of teacher disempowerment because of the presence of foreigners operating in the country. Teacher perceptions regarding EMI have been numerous in particular with cultural issues relating to the use of EMI in mathematics and science classes. Surprisingly, the study revealed a few unexpected positive findings with certain aspects of reform.

Abstract:
Coal ash deposits were collected in a 160 kWth, down-fired oxy-coal reactor under staged and unstaged conditions for four different coals (PRB, Gatling, Illinois #6, and Mahoning). Concentration measurements of carbon, oxygen, sodium, magnesium, aluminum, silicon, phosphorus, sulfur, chlorine, potassium, calcium, titanium, chromium, manganese, iron, nickel, strontium, and barium were gathered from each deposit sample using scanning electron microscopy and energy dispersive spectroscopy (SEM-EDS). Backscattered electron micrographs for each deposit sample were analyzed to gather morphological data. Particle size and shape were studied for each deposit collected. The average particle sizes of the particles in upstream deposits were much larger than the average particle sizes of the downstream deposits. The downstream deposits consisted primarily of spherical particles while the upstream deposits consisted of round, irregular polygonal, and porous particles. Deposit particles are believed to have deposited at all stages of burnout; those depositing early during pyrolysis may have continued to react after deposition. Element maps for the aforementioned elements were collected with SEM-EDS and analyzed to quantify both average composition and composition of individual particles. These values were compared to ASTM ash analyses performed for each coal and ash collected from the flue gas stream with a cyclonic particle separator. It was found that sulfur concentrations of deposits do not correlate with corresponding sulfur concentrations of the coal. Comparison of similar experiments performed with air-combustion show that oxy-combustion deposits contain about twice as much sulfur as air-combustion deposits when burning the same coal. Deposition propensity of each coal was also examined, and the PRB and Gatling coals were found to have a moderately high deposition propensity whereas the deposition propensity of the Mahoning and Illinois #6 coals was fairly low.

Abstract:
This study is both quantitative and qualitative in nature. In order to support or discard the hypothesis that high-functioning, autistic upper secondary school students speak more freely in English than in their native language Swedish, an experiment with a narrative format was conducted with students both within and without Autism Spectrum Disorder (ASD). Measurements of lexical density, hesitations and long pauses in the narrative were applied to assess the quality of produced speech. A questionnaire survey for teachers of this age category of students was also carried out. The analyses of the results are primarily quantitative, but a portion of the questionnaire is also thematically analysed. The study shows that there is initial support for the hypothesis.
Denna studie är båda kvantitativ och kvalitativ i sin natur. För att stödja eller förkasta hypotesen att högfungerande, autistiska gymnasieelever talar engelska mer obehindrat än deras modersmål svenska, genomfördes ett experiment med ett berättande format med studenter både inom och utanför Autismspektrumtillstånd (AST). Mått på lexikalisk täthet, tvekan och långa pauser i berättandet användes för att utvärdera kvalitén av producerat tal. En enkätundersökning för lärare av denna ålderskategori av elever genomfördes också. Analyserna av resultaten är primärt kvantitativa, men en del av enkäten är också tematiskt analyserad. Studien visar att det föreligger ett initialt stöd för hypotesen.

It is generally thought that the extracellular matrix (ECM) is thrombogenic.However,one of us (MRB) has reported that the ECM is thromboresistant,and postulated that this was due to the release of endothelial cell (EC) 13-hydroxyoctadecadienoic acid (13-HODE) into the ECM. To test this possibility, we measured platelet adhesion (PLT ADH) onto cultured ECs and their ECMs exposed by 3 methods. We also extracted the ECMs for HPLC analysis of 13-HODE.PLT ADH was expressed as i)adhesion of 3H-adenine labelled platelets/mm2 of ECs or ECMs under static conditions, and ii) % surface^ area coverage measured morphometrically following 5"perfusion with citrated whole blood at 1300 sec-1 in the flat chamber.ECMs were prepared by removing the EC monolayers by freeze thawing , cellulose acetate stripping or NH4OH treatment. PLT ADH to ECs under static and flow conditions were 4700±240/mm2 and 0.1%, respectively, and were associated with 12,6± 1 pg of 13-HODE/mm2 of EC surface (M+SEM). Removal of the ECs by freeze thawing or stripping, resulted in a 18% and 25% increase in PLT ADH to the ECM,under static and flow conditions respectively, and a 80% decrease in ECM associated 13-HODE level. Removal of the EC by NH4OH resulted in a 380% and 770% increase in PLT ADH to the ECM in static and flow conditions. 13-HODE was undetectable.These data support the hypothesis that 13-HODE released from ECs influences the ECM thrombogenecity, and indicate that the residual amounts of components present in the ECMs following EC removal is influenced by the method of ECM preparation.

Abstract The Pressure Suppression Pool (PSP) in a Boiling Water Reactor (BWR) is served as a heat sink to prevent containment over-pressure. The steam can be injected through the multi-hole spargers. The development of thermal stratification where a thermocline with a large temperature gradient appears in the pool can lead to the higher pressure in the dry well compared with completely mixing pool conditions. Prediction of the thermal phenomenon in the pool is necessary for the support of system design and operation. Thus, the Effective Heat Source (EHS) and Effective Momentum Source (EMS) models have been proposed. The models can be applied to CFD code by using (i) source terms in the transport equations or (ii) using respective boundary conditions at the Steam Condensation Region (SCR). Previous validation against PPOOLEX and PANDA tests using the source terms approach faced challenges in momentum distribution. Therefore, a preliminary investigation of using the second method was performed. The encouraging results implied that it is possible to further develop this approach. The goal of this work is to further develop the EHS/EMS models for the steam injection through a multi-hole sparger through the SCR model (i.e approach (ii)) and to validate it against the experimental data obtained from PANDA HP5 tests. Modeling guidelines are proposed. The temperature evolutions and vertical velocity profiles of these tests are compared to the simulation results. The agreement suggests that this model can provide an adequate estimation of the pool behavior.

Abstract:
EMC/EMI is a source of concern from the beginning of planning a new railway line. High powered electrified railways such as HSR are usually AC electrified. This poses a hazard for nearby systems and users. Induced voltages can create electric shock risks or malfunction in signalling or other nearby systems. LRTs, metro lines or commuter systems are usually DC fed and can be located near electromagnetic sensitive equipment such as MRI equipment in hospitals or research equipment — such as electron microscopes — in campuses or laboratories. EMF calculation and impact assessment in early phases of the project rely on simulation and expertise in these complex multi system interrelations. Rail traction creates EM fields which tend to decrease rapidly as the distance from the track axis increases. However, considerations about passengers, workforce and public safety and system compatibility are always needed. As an example, transient currents in DC rail, especially due to short circuits or arcs can interfere with nearby equipment. Even semi steady state DC currents associated with normal operation of the line (traction and breaking currents) present EMC related hazards that need to be assessed in order to evaluate if mitigation measures for associated risks are needed. AECOM’s Madrid Transportation Design Center has developed a tool that provides 3D EMF calculations for railway lines, either AC or DC. This tool, called EMFRail, can deliver EMF estimations based on power load supply simulations considering alignment, separation between tracks, geometry of the electrification system (OCS, third rail), rolling stock mechanical and electric features as well as regenerative braking in order to provide decision support information to make educated project decisions from the beginning of the design. Regenerative braking is a common energy saving tool for railways these days, but has posed new EMF hazards since currents sent back to the OCS can be bigger than those related to traction and this process can occur in different locations of the line (usually when the train is decelerating from maximum speed). This tool can calculate both magnetic and electrical fields for traction frequencies. This tool has been used in several LRT and rail projects providing insightful information to adopt mitigation measures such as underground feeders or operational limitations. EMFRail can simulate mitigation measures for challenging situations such as EMF caused by semi steady state maximum currents (prior to the trip of protection systems). EMFRail can also provide EMF calculations for transmission or distribution power lines once the required current or voltage values (module and phase) are known as well as the position of the different conductors (OCS wires, feeders, rails) of the line under study. EMFRail is developed using MatLab programming suite taking advantage of matrix operation capabilities. Output results are isocurves that can be superimposed to a raised view drawing, 3D contours for specific magnetic field values or even time animated frames to understand worst case scenarios that sometimes are not easy to foresee based on common assumptions.

Electromyogram (EMG) consists on the recording and measurement of the electrical potential generated by the activation of muscle fibers [1]. Electromyographic signals (EMGs) are directly linked to the movement performed by a person. Thus, the study of EMGs for the control prosthesis and exoskeletons has become increasingly popular in the past years. To provide a real time control of a prosthesis or exoskeleton (assistive device) to the user, the time between the movement performed by a healthy arm and the movement of the exoskeleton should be small as possible. The main objective of this paper is to map different movements of the upper limb. Moreover, detect the onset of the EMGs to determine which muscle is producing movement. Surface electrodes were used to perform the experiments in order to insure the comfort of the subjects. The analysis of the signal to detect the onset was done using Matlab. After mapping eight movements, results show that the EMGs recorded from the Trapezius muscle can be used as a discriminative to differentiate between movements performed by the arm and movements performed by the forearm and hand. This will reduce the time and number of EMG channels needed to correctly identify the movement performed by the upper limb of a subject.

Tumor cell (TC) vessel wall adhesion is thougth to occur at specific sites of exposed extracellular matrix (ECM).To determine the role of fibronectin (FN) in TC/ECM adhesion,we measured; 1)TC adhesion to intact cultured endothelial cell monolayers (EC) or their ECMs,with or without incubation with a polyclonal antibody (Ab) to human FN, or a monoclonal antibody (Mab) to the cell binding site of FN (3E3);2)TC adhesion to EC or their exposed ECMs incubated with specific peptides against bacteria adhesion sites (I 133-79) or the peptide GRGDSP contained in the cell binding sites of several adhesive proteins.TC adhesion was measured as number of 111In-labeled A-549 adenocarcinoma cellsxlO /disc. ECMs were exposed by removing the ECs with N2 flow or EGTA treatment.There was 5.5±1x10 A-549/ EC covered disc (Table). Treatment of ECs with the Abs or peptides had no effect on TC adhesion.TC adhesion to the two ECM preparation were 42 ± 12 and 55±10 respectively.(Table).Blockage of the FN adhesive site by either Ab inhibited TC adhesion (p < 0.01). In contrast,blockage of the bacteria-adhesion site or incubation with GRGDSP had not significant effects in TC adhesion. The table shows the results (mearv±SEM (* p < 0.01).We conclude that TC adhesion to ECM but not to ECs,is dependent upon FN.

This paper documents results of fuel consumption and exhaust emission tests performed on a 1,500 kW EMD GP38-2 locomotive equipped with an auxiliary power unit (APU) designed to minimize main engine idling time by providing stand-by services normally provided by the main EMD 16-645-E engine at idle. The purpose of these tests was to perform an evaluation of the exhaust emissions and fuel consumption of both the EMD 16-645-E engine and the APU. The APU diesel engine was a 2.0L, 4-cylinder, turbocharged, Kubota model V2003-TEBG rated at 30.6 kW. The APU was tested using an external load box over a range of load conditions, ranging from unloaded (0 kW) through 16 kW, which was the maximum APU load expected as installed in the locomotive. Fuel consumption and exhaust emissions are compared between an idling EMD 16-645-E engine and the APU engine at a “typical” stand-by condition with the coolant and lubricating oil heaters operating and the locomotive control cab air conditioner turned off. Test results showed that the APU fuel consumption and exhaust emissions are dramatically lower than the idling EMD locomotive engine. Because the APU is designed to automatically start and stop as a function of the locomotive water temperature, and therefore operates only a portion of the time that the EMD engine would otherwise be idling. Reductions in fuel consumption and exhaust emissions over an extended period of time would be even more dramatic.

Abstract Advanced ASIC devices with multi-level metallization and high density DRAMs with lead-on-chip (LOC) packages are becoming more difficult to analyze using emission microscopy from the Front-side of the chip (Front-side EMS). In this paper, we have found that evaluation of the wavelength spectrum from the back-side of the chip (Back-side EMS) could be used as easily as Front-side EMS. We investigated the wavelength spectrum of light emitted from the backside of the chip and the front-side of the chip, and calculated a photon counting ratio of the Back-side EMS and Front-side EMS for several different failure mechanisms. The wavelength spectra also showed that all light emissions had a component of Infrared (IR) light with wavelength longer than 900nm. The minimum ratio in the various failure mechanisms is 40%. From these data, Back-side EMS can be applied to the analysis of various failure mechanisms. Although the thickness of silicon substrate using for the spectrum measurement is from 204um to 380um, the measured values are not affected by the thickness. Therefore, in the Back-side EMS, the light emission can be detected without strict control of chip thinning.
