Abstract:
IntroductionTemporomandibular disorders (TMDs) are considered a collection of musculoskeletal conditions involving the masticatory muscles, the temporomandibular joint and associated structures. The myogenous group appears to represent the most frequently diagnosed category. In the context of a multimodal approach, splint therapy and musculoskeletal physiotherapy are often considered as a preferred therapy. The purpose of this study will be to investigate the effects of musculoskeletal physiotherapy combined with occlusal splint and education versus occlusal splint and education alone in the treatment of chronic myogenous TMD on pain and mandibular range of motion.Methods and analysisAll consecutive adults complaining of TMDs presented to the Department of Biomedical and Neuromotor Sciences of the University of Bologna will be considered eligible. Inclusion criteria shall be based on the presence of myogenous TMDs, as diagnosed through clinical examination in reference to the international diagnostic criteria of TMDs. Randomisation, concealed allocation, blinded assessment and intention-to-treat analysis will be employed. The splint therapy will consist of the use of the splint every night and concurrent delivery of an educational programme; the protocol shall have a duration of three consecutive months. The combined musculoskeletal physiotherapy, splint therapy and education will additionally consist of manual therapy techniques and exercise; such protocol shall consist of a duration of three consecutive months, inclusive of 10 sessions for the enhanced elements. All outcome measures will be collected at baseline, after treatment and at a 6 months follow-up.Ethics and disseminationEthical approval has been obtained from the Independent Ethic Committee in Clinical Research of AUSL Bologna-Italy (47/2018/SPER/AUSLBO). Pursuant to applicable rules,we will obtain informed consent from each participant and collect data anonymously to maintain privacy. Results will be disseminated to clinicians and researchers through peer-reviewed publications and conferences.Trial registration numberNCT03726060

Abstract:
With the aim of understanding the insertion of higher education into a new context of organization of society and State, which is managed according to the New Public Management, this work presents a comparative historical study of the organization of French and Brazilian higher education. It is concluded that the French adherence to the New Public Management is based on the knowledge economy, while the Brazilian one is based on State size reduction along the lines of the Washington Consensus; in addition, higher education institutions in both countries are organized to participate in competitions: in France, the international competition promoted by rankings, and in Brazil, the market competition.ResumoCom o objetivo de compreender a inserção do ensino superior dentro de um novo contexto de organização da sociedade e do Estado, gerido pela Nova Gestão Pública, o trabalho apresenta um estudo histórico comparativo da organização do ensino superior brasileiro e o francês. Abstract:
Abstract Background Notification of confirmed or suspected cases of Malaria is compulsory in Italy. It is essential to optimize control measures in case of imported and indigenous events, considering that the main local vector, Anopheles labranchiae, is still present in Italy. Methods Descriptive analysis of malaria cases notified from Bologna local health authority (AUSL) from January 2000 to December 2019. Results In the period 2000-2019, out of the 248 Malaria cases notified to the AUSL, 161 were males, and 87 were females, with median age of 33 (IQR: 42-25). All the cases were sporadic and travel-related. The main reasons for travelling were: return to the country of origin (n = 139, 56,0%), tourism (n = 39, 15,7%), work (n = 30, 12,1%), and immigration (n = 13, 5,2%). 86 (34,7%) cases were born in Italy, 43 (17,3%) in Nigeria, 27 (10,9%) in Cameroon, and 17 (6,9%) in Ghana. The cases were imported from Nigeria (n = 56, 22,9%), Cameroon (n = 30, 12,2%), Ghana (n = 26, 10,6%), Ivory Coast (n = 24, 9,8%). Most of the cases were not taking any drug for Malaria chemoprophylaxis (n = 214, 86,6%), 20 cases (8,1%) properly completed the prophylaxis, while 13 cases (5,3%) started chemoprophylaxis but they didn't complete it. The etiological agent identified was Plasmodium falciparum in 176 cases (71,0%), P. vivax in 26 cases (10,5%). Among the former cases 49 (32,2%) were contracted in Nigeria, 29 (19,1%) in Cameroon, 23 (15,1%) in Ghana, 20 (13,2%) in Ivory Coast, 10 (6,6%) in Senegal. 11 (68,8%) cases of P. vivax were contracted in Pakistan. Conclusions Epidemiological data collected from Bologna AUSL are consistent with those described from Istituto Superiore di Sanità: the majority of cases was comprised of foreigners, the great majority of cases was imported from Africa. P. Falciparum was prevalent in Africa while P. vivax was prevalent in Asia. Questo progetto nasce con l’idea di elaborare una strategia organizzativa/professionale che possa favorire un’omogeneizzazione dei comportamenti dei professionisti sanitari, determinando una riduzione dei costi in termini di risparmio di risorse umane ed economiche, ma soprattutto favorire un miglioramento della qualità di vita dei soggetti portatori di lesioni cutanee. Le lesioni cutanee, ma soprattutto le ferite di difficile guarigione, si caratterizzano per una lunga durata e un’elevata incidenza di complicanze, che spesso si traducono in un considerevole onere economico. L’infermiere Specialista Wound Care, collaborando attivamente con i professionisti sanitari attraverso attività di consulenza, formazione on work, supervisione dei processi operativi, si pone come un’importante figura di riferimento in termini di miglioramento degli obiettivi di cura, nonché degli esiti di cura. Il progetto, di dimensioni Aziendali, si sviluppa in setting assistenziali specifici Ospedale-Territorio e lo Specialista Wound Care opera in collaborazione con gli infermieri di quei setting attraverso una metodologia di Coaching. The aim of this is project is the development of an organizational/professional strategy that can help the homogenization of the behavior of healthcare professionals, determining a reduction of human resources and costs, but above all promoting an improvement of the quality of life of people with skin lesions. Some skin wounds are characterized by a long duration and a high incidence of complications, which often result in considerable economic expenses. The Wound Care Specialist Nurse, in collaboration with healthcare professionals by consultancy, training and supervision of operational processes, is an important reference for the improvement of the care and the treatment goals and outcomes. The project involves Hospital/Health Care Home and the Wound Care Specialist that works in collaboration with the nurses of the area.

Abstract:
Nel 2015, l’Azienda USL di Bologna ha intrapreso un percorso di riorganizzazione ed ottimizzazione del Servizio Sanitario: in ambito gastroenterologico, è nato il progetto Gastropack. Esso propone un nuovo paradigma per la presa in carico dell’intero percorso di cura del paziente, con accesso diretto alle prestazioni sanitarie erogabili all’interno di pacchetti giornalieri. Il lavoro di tesi ha riguardato lo sviluppo di modelli di simulazione, con l’ausilio del software AnyLogic, rappresentativi delle sperimentazioni in atto presso cinque Nuclei di Cure Primarie, afferenti a due Distretti del territorio Bolognese. Sono state analizzate tre diverse politiche di assegnamento dei pazienti alle strutture e alle date in cui effettuare le prestazioni, e valutate configurazioni alternative del sistema. Complessivamente, gli esperimenti hanno fornito una panoramica su alcuni scenari di interesse strategico per il progetto Gastropack, supportando le decisioni intraprese e proponendo possibili soluzioni alternative da implementare presso le realtà sperimentali.

L’aumento dei costi in sanità, l’aumentata prevalenza delle patologie croniche, le disuguaglianze attuali, evidenziano la domanda crescente di una popolazione fragile che richiede una risposta globale ai bisogni della persona nel suo insieme, attraverso la costruzione di un sistema sanitario integrato che ne garantisca una presa in carico efficace. Riuscire a gestire le patologie croniche in modo appropriato è la sfida a cui sono chiamati i professionisti socio-sanitari; ma quali sono gli elementi per riuscirci? Le evidenze scientifiche dimostrano che è fondamentale l’integrazione tra i professionisti e lo sviluppo dei Percorsi Diagnostici Terapeutici Assistenziali (PDTA). In quest’ottica, in Italia e in particolare in Emilia-Romagna e nell’Azienda USL di Bologna si sono succeduti, e ancora si stanno evolvendo, diversi modelli di organizzazione per migliorare la gestione appropriata delle patologie croniche e l’aderenza alle linee guida e/o ai PDTA. Il ruolo del medico di medicina generale (MMG) è ancora fondamentale e il suo contributo integrato a quello degli gli altri professionisti coinvolti sono imprescindibili per una buona gestione e presa in carico del paziente cronico. Per questo motivo, l’Azienda USL di Bologna ha sviluppato e implementato una politica strategica aziendale volta a disegnare i PDTA e incoraggiato la medicina generale a lavorare sempre di più in gruppo, rispetto al modello del singolo medico. Lo studio ha individuato nelle malattie cardiovascolari, che rimangono la causa principale di morte e morbilità, il suo focus prendendo in esame, in particolare,lo scompenso cardiaco e il post-IMA. L’obiettivo è verificare se e quanto il modello organizzativo, le caratteristiche del medico e del paziente influiscono sul buon management delle patologie croniche in esame valutando la buona adesione alla terapia farmacologica raccomandata dalle linee guida e/o PDTA dello scompenso cardiaco e post-IMA.
The rising health care costs, the increasing prevalence of chronic conditions, the existing inequalities in access to care highlight the growing demand for a response to the needs of a fragile population, through the construction of an integrated health system that could ensure an effective provision of care. Managing chronic diseases appropriately is a challenge for health care professionals, but which are the key elements to achieve it? Scientific evidence indicates that the integration between professionals and the development of Integrated Care Pathways are fundamental. In Italy and especially in Emilia Romagna and in Local Health Authority of Bologna, different models of organization have been implemented and are still evolving to improve the management of chronic diseases, the appropriateness and adherence to guidelines and/or pathways. The role of the general practitioner is still vital and his/her contribution together with that of other health care professionals are essential for the appropriate treatment of chronic diseases. . For this reason, the Local Health Authority of Bologna has pursued a policy aimed at developing integrated care pathways and encouraged general practitioners to work together rather than running individual practices. The study has chosen cardiovascular diseases, and specifically heart failure and acute myocardial infarction, as its focus because these diseases are the leading cause of death and morbidity. The goal is to determine if and how the organizational models, the characteristics of the physician and of the patient are associated with the good management of chronic cardiovascular diseases and whether the different collaborative organizational models of general practitioners are also associated with adherence to the pharmacological treatments recommended by the guidelines and/or the integrated care pathways for heart failure and acute myocardial infarction.

La ricerca in oggetto ha analizzato le relazioni tra migrazione e salute mentale nel Distretto di Pianura Est dell'AUSL di Bologna. Attraverso un dispositivo d’indagine multi-disciplinare basato sui quadri teorici dell'Antropologia Medica Critica, della Salute Pubblica e della Psichiatria, la ricerca si è inserita nell’ampio contesto di sperimentazione di un innovativo modello di assistenza per pazienti migranti, denominato Centro di Consultazione Socio- Culturale. L'architettura dello studio si rifà a un modello di Ricerca-Azione Partecipata e Multi-Situata fondato su un approccio analitico e auto-riflessivo, il quale ha consentito di problematizzare, oltre alle azioni e alle traiettorie dei vari soggetti che operano nel campo della ricerca, anche le categorie oggetto della ricerca stessa. L'analisi, profondamente radicata nel dato empirico, è stata condotta a partire dall'esperienza degli attori sociali coinvolti. Le esperienze, le informazioni e le rappresentazioni reciproche sono state co-costruite in forma partecipativa attraverso l'uso combinato di metodologie quali-quantitative proprie sia delle discipline sanitarie sia di quelle sociali. Come materiali della ricerca sono stati utilizzati: dati primari e secondari prodotti dalle istituzioni e dalle organizzazioni del territorio stesso; informazioni provenienti dall'osservazione partecipante; colloqui con informatori-chiave; interviste semi-strutturate con decisori politici, amministratori, organizzazioni del territorio, operatori dei servizi, cittadini e pazienti. La ricerca ha dimostrato la validità delle prospettive teoriche utilizzate e delle strategie di lavoro proposte. Il modello di lavoro multi-disciplinare e multi-metodologico si è rivelato produttivo nell'indagare congiuntamente le prospettive degli attori coinvolti insieme alle loro traiettorie, alle reciproche interconnessioni e alle relazioni tra processi locali e globali. L’analisi auto-riflessiva ha consentito di analizzare le attività del Centro di Consultazione evidenziandone vantaggi e limiti. Infine, la collaborazione tra Salute Pubblica e Antropologia Medica Critica ha dimostrato una grande potenzialità e produttività sia sul versante della ricerca scientifica sia su quello dell'assistenza sanitaria.
This research has analysed the relationship between migration and mental health in the Distretto di Pianura Est of Bologna AUSL. Through a multi-disciplinary investigation tool based on the theoretical frameworks of Critical Medical Anthropology, Public Health and Psychiatry, the research was included in the broad context of testing an innovative model of care for migrant patients, called Socio-Cultural Advisory Centre. The study design relates to a model of Participatory and Multi-Situated Action Research, analytical and self-reflective, which made it possible to problematize, in addition to the actions and trajectories of the various actors of the research, also the categories addressed by the research. The analysis, deeply rooted in empirical data, was conducted starting from the real experience of the social actors involved. Experiences, mutual information and representations have been co-constructed in a participatory way through a combination of qualitative and quantitative methodologies well known both to social and health disciplines. The following research material was used: primary and secondary data gathered from institutions and organizations in the field; information from participant observation, interviews with key informants, semi-structured interviews with policymakers, administrators, local organizations, service personnel, citizens and patients. The research has demonstrated the validity of the theoretical perspectives that were applied and the strategies proposed. The working model of multi-disciplinary and multi-methodological approach has proved effective in investigating the perspectives of those involved as well as their own trajectories, the mutual interconnections and relationships between local and global processes. The self-reflective analysis has allowed the exploration of the activities of the Advisory Centre, highlighting advantages and limitations. Finally, the collaboration between Public Health and Medical Anthropology Critique has shown a great potential and productivity both in the field of scientific research and of health care.
A pesquisa tem analisado as relações entre o fenômeno da migração e a saúde mental no Distrito Sanitário de Pianura-Est da AUSL de Bolonha. Através de um dispositivo de indagação multi-disciplinar e multi-metodológico baseado na perspectiva da Antropologia Médica Crítica, da Saúde Pública e da Psiquiatria, a pesquisa tem se inserido na experimentação de um modelo de atendimento aos pacientes migrantes, chamado Centro de Consultação Sócio-Cultural. O esquema do estudo se inspira ao modelo da Pesquisa-Ação-Participativa e Multi-situada, baseada em um abordagem analítica e auto-reflexiva, que tem permitido problematizar, além das ações e trajetórias dos diferentes atores que operam no campo da pesquisa, também as categorias-objeto de investigação. A análise, fortemente baseada nos dados empíricos, tem sido desenvolvida a partir da experiência dos atores envolvidos. As experiências, as informações e as mútuas representações dos diferentes atores foram co-construídas de forma participava por meio de uma combinação de metodologia qualitativas e quantitativas pertencente às ciências sociais e de saúde. Como materiais de pesquisa foram utilizados: dados primários e secundários produzidos por instituições e organizações do próprio território; informações obtidas a partir da observação participante, entrevistas com informantes-chaves; entrevistas semi-estruturadas com gestores, administradores, o organizações locais, operadores dos serviços, cidadãos e os doentes. A pesquisa têm demonstrado a validade do quadro teórico e das estratégias utilizadas nos trabalho. A abordagem multi-disciplinar e multi-metodologia tem se demonstrado produtiva na investigação das perspectivas de todos os atores envolvidos, em conjunto com as suas trajetórias, as mútuas interconexões e as relações entre os processos locais e globais. A análise auto-reflexiva permitiu analisar as atividades do Centro de Consultação, destacando vantagem e limitações. Finalmente, a colaboração entre Saúde Pública e Antropologia Médica Crítica tem mostrado um grande potencial em termos de produtividade, tanto no campo da pesquisa científica tanto quanto no dos serviços de saúde.

Die nun anstehende Rückkehr zum Staatsexamen stellt nach Aussagen und Forderungen des Kultusministeriums keine Rückkehr zu alten Strukturen dar, sondern soll diese Veränderungen aufgreifen und die Qualität der universitären Lehrerbildung erhöhen (Pressemitteilung des sächsischen Kabinetts vom 19.10.2010). Wir sehen es daher als notwendige Bedingung für eine Weiterentwicklung der Qualität der Lehrerbildung an, die Erfahrungen der Dozierenden mit dem reformierten Lehramtsstudium in den Entwicklungsprozess i. ZusammenfassungUntersuchungsgegenstand des Forschungsprojekts, dessen Ergebnisse in diesem Buch dargestellt werden, sind die Auswirkungen der Reformen des deutschen Hochschulsystems („Bologna“, „Exzellenzinitiative“ und „New Public Management“) auf die berufliche Identität von ProfessorInnen. Welche Identitätsbedrohungen, aber auch -chancen gehen von den Reformmaßnahmen, insbesondere von vielfältigen Leistungsbewertungen, aus, und mit welchen Praktiken der Identitätsbehauptung und -darstellung reagieren ProfessorInnen darauf? In diesem Kapitel werden die Relevanz der Fragestellung, ein theoretischer Bezugsrahmen zu ihrer Analyse sowie die empirische Herangehensweise geschildert. Zentrale Datengrundlage der Untersuchung sind Leitfadeninterviews mit ProfessorInnen aus fünf verschiedenen Wissenschaftsgebieten (Zellbiologie, Nachrichten- und Hochfrequenztechnik, Vergleichende Regierungslehre, Strafrecht und Neuere Deutsche Literaturwissenschaft).
