Abstract:
Behcet’s Disease (BD) is a multiorgan disorder characterized by oral and genital ulceration, uveitis, and dermatological symptoms. BD is most prevalent in the Mediterranean countries and East Asia, but also occurs in Europe and North America. The etiology remains unknown. Evidence suggests that BD is an autoimmune disorder with complex traits. Neuro-Behcet’s Syndome (NBS) develops in about 5% to 30% of patients with BD and presents with parenchymal or nonparenchymal pathology. The course of NBS is highly variable. Treatment strategies include modulations of the immune response and tissue degeneration, along with symptomatic medications. Main directions of current research include genomic studies, biomarker discovery, and inventive drug- development strategies.

Abstract:
This chapter reviews evidence for an autoimmune etiology in a subset of pediatric OCD cases. The sudden onset of OCD and related symptoms following an infectious illness in youth has been termed Pediatric Autoimmune Neuropsychiatric Disorder Associated with Streptococcus, or PANDAS. By analogy with the better-understood syndrome of Sydenham Chorea, it has been hypothesized (as the name implies) that this syndrome derives from the development of brain-reactive autoantibodies after infection. The specifics of this pathophysiology have been difficult to establish, and the syndrome remains somewhat controversial. Nevertheless, treatment with antibiotics and immune-modulating therapies has shown promise in these cases.

Myasthenia gravis is in most cases an autoimmune disorder of the neuromuscular junction in which antibodies are directed at nicotinic acetylcholine receptors or other synaptic proteins, such as the MusK protein that is involved in the formation of the formation and maturation of the motor endplate. Less commonly, myasthenia gravis can result from antibodies directed to presynaptic calcium channels as a side effect of paraneoplastic antibodies (Lambert-Eaton syndrome) or from a developmental paucity of acetylcholine receptors in the neonatal form of the disease. Treatment is usually a combination of aceetylcoholinesterase inhibitors such as pyridostigmine to prolong the life of acetylcholine released at the neuromuscular junction and/or drugs such as corticosteroids aimed at reducing inflammation.

Advanced maternal age and increasing numbers of women of childbearing age with endocrine and autoimmune disorders have become the challenge for both anaesthetists and obstetricians. Genetic studies have provided new insight into underlying causes of endocrine disorders and prenatal prediction of inheritance. The expression of endocrine disease may influence the interpretation of diagnostic laboratory testing during pregnancy. Better understanding of the pathophysiological mechanisms enables new therapeutic approaches which can compromise pregnancy outcome. Although only a small number of drugs have been shown through clinical studies to be safe for use in pregnancy, intensive therapy for chronic disease is usually needed. Thus, anaesthetic management of women with endocrine disorders in pregnancy has become more complex. The most frequently encountered endocrine disorders during pregnancy include gestational diabetes mellitus and thyroid and adrenal disorders. Gestational diabetes has become increasingly common in pregnant women. Not only does it influence pregnancy outcome, but it also carries a risk for mother and offspring of developing type 2 diabetes later in life. Intensive glucose control may prevent maternal and fetal complications and improve long-term outcome. Pregnancy itself has been found to influence the course of autoimmune diseases, such as rheumatoid arthritis and systemic lupus erythematosus. However, autoimmune diseases may have adverse consequences for maternal, fetal, and neonatal health. There is a relative paucity of literature concerning anaesthetic management of autoimmune diseases. Early recognition and immediate treatment of the common complications have been the key elements to achieving the ultimate goal—good pregnancy outcome.
