Abstract:
Dalam hukum positif Indonesia, autopsi forensik diatur dalam Kitab Undang-Undang Hukum Pidana, Undang-Undang No. 8 Tahun 1981 tentang Kitab Undang-Undang Hukum Acara Pidana dan Undang-Undang Nomor 36 Tahun 2009 Tentang Kesehatan. Ketiga undang-undang tersebut membenarkan melakukan tindakan autopsi dengan tujuan untuk menegakkan keamanan, dan keadilan bagi masyarakat. Hasil pemeriksaan autopsi forensik akan tertuang dalam sebuah laporan tertulis dalam bentuk visum e repertum dapat digunkan sebaga alat bukti di pengadilan. Dalam hukum islam, autopsi forensik dilarang karena dapat merusak mayat dan melanggar kehormatan mayat. Namun, ada beberapa ulama membenarkan autopsi forensik dengan alasan untuk mewujudkan kemaslahatan ummat (mashalih mursalah) baik di bidang keamanan, keadilan, dan kesehatan. Berdasarkan Fatwa Majelis Ulama Indonesia Nomor 6 Tahun 2009 tentang Autopsi Jenazah menyebutkan bahwa pada dasarnya autopsi dilarang atau haram, namun dalam keadaan Darurat atau mendesak maka diperbolehkan. In Indonesia positive law, a autopsy is regulated in the Criminal Code, Law No. 8 of 1981 concerning the Criminal Procedure Code and Law Number 36 of 2009 concerning Health. The three laws justify carrying out autopsy actions with the aim of enforcing security, and justice for the community. The results of a forensic autopsy examination will be contained in a written report in the form of a post mortem report that can be used as evidence in court. In Islamic law, forensic autopsies are prohibited because they could damage a corpse and violate the honor of a corpse. However, there are some scholars who justify the forensic autopsy on the grounds of realizing the benefit of the ummah (mashalih mursalah) in the fields of security, justice and health. Based on the Fatwa of the Indonesian Ulema Council Number 6 of 2009 concerning the Body Autopsy states that basically autopsies are prohibited or haram, but in an emergency or urgent condition it is permissible.

AbstrakPeningkatan resiko penularan infeksi di ruang autopsi semakin meningkat. Hal ini terjadi karena peningkatan jumlah kasus korban meninggal yang terinfeksi penyakit terutama di negara berkembang. Beberapa studi menyatakan terjadinya peningkatan prevalensi HIV, hepatitis B, C, D dan G, tuberkulosis, penyakit Prion, Hantavirus, campak, infeksi bakteri atau HTCV pada pekerja di ruang autopsi. Penerapan kewaspadaan universal, termasuk standar OSHA, pemilihan disinfektan dan pengolahan limbah medis sangat penting diperhatikan untuk mencegah resiko terjadinya penularan infeksi. Tujuan penulisan tinjauan pustaka ini adalah mengetahui risiko penularan infeksi pada pekerja di ruang autopsi dan penerapan kewaspadaan universal. Penulisan ini berdasarkan studi kepustakaan yang terkait dengan dua topik ini. Penerapan kewaspadaan universal sangat diperlukan dalam pencegahan penularan penyakit infeksi pada pekerja di ruang autopsi yaitu meliputi penggunaan alat pelindung diri yang tepat, perilaku, tindakan mencegah infeksi, disinfeksi dan penanganan sampah medis yang sesuai aturan.AbstractThe risk of infection transmissions in autopsy room is increasing. This happens because the increase of the number of cases died affected by infectious disease, especially in developing countries. Several studies found an increase on the prevalence of HIV , Hepatitis B , C , D and G, Tuberculosis , Prion Disease , Hantavirus , Measles , Bacterial Infection or HTCV on workers in autopsy room .The application of universal precaution , including OSHA standards , the selection of disinfectant, medical waste management is very important to prevent the risk of the infection transmission. The aim of this review was to explain the risks of infection transmissions on workers in autopsy room and the application of universal precaution. Literatures on these two topics were evaluated. Application of universal precautions are very necessary in the prevention of transmission of infectious disease in workers in the autopsy room which includes the use of appropriate personal protective equipment, behaviors, actions to prevent infection, disinfection and treatment of medical waste according to the rules.

Objective: The study was aimed to find out thepercentage of negative autopsy against the total cases of medicolegal autopsies conductedduring the study period in Lakki Marwat KPK. Material and methods: The study was carried outin DHQ Lakki Marwat based upon five years autopsy data from 1st January 2013 to 31st December2017. All the medico legal autopsies were included in the study whereas; the cases of allegedcustodial deaths / police encounters were excluded. The cases where gross examination atinitial autopsy, histopathalogical examination & toxicological analysis failed to detect the causeof death were labeled as negative autopsies. Study design: Descriptive, Retrospective Study.Setting: The study was conducted at DHQ hospital Lakki Marwat. Period: 1st January 2013to 25th June 2017. Results: Out of the total 200 autopsies, there were 39 negative autopsiescomprising of 26 (60.46%) males and 17(39.53%) females. Conclusion: The cases of negativeautopsy are higher at lakki marwat as compared to international perspectives.the negativeautopsies are higher because of the inadequate training of doctors or limited resources likeavailability of histo-pathological labs, analytical services and radiological facilities. Postmortemunits and toxicology laboratories are not equipped according to the modern standards in thelight of recent advances in the field of medico legal death investigation.

This dissertation is a response to the most uncomfortable tasks in the medical examiners work, the tasks required to prepare an autopsy report. This is one of their biggest and most frequent limitations. The new technological tools of person-machine interface in the market motivated the realization of this project. This project is developed in Java, and allows a medical examiner to perform an autopsy report without requiring physical contact with the computer. Thus, this work has the following advantages: it does not require that the medical examiner removes his gloves whenever he needs to make an annotation, also, there is no need to transcribe information from paper format to a digital format, furthermore, it makes the process of developing the autopsy report simple having less likelihood of contaminate the material. This application was designed and developed in order to facilitate their use through voice commands, using the Java Speech and gestures detected by Leap Motion. The application operation is very satisfactory and was a source of enthusiasm to the medical examiners that accompanied the project, since this work has created a useful tool for their daily work.

Les expériences de physique quantique ont atteint un niveau de contrôle permettant de préparer avec précision l'état quantique de nombreux systèmes physiques. Cela a mené à la naissance de l'optique quantique électronique, un sujet émergent qui vise à préparer, manipuler et caractériser l'état de courants électriques contenant quelques excitations électroniques se propageant dans un conducteur quantique ballistique. Ceci est un défi conséquent qui se heurte à la difficulté de caractériser un état quantique à N corps.Le sujet de cette thèse sera le développement de méthodes de traitement du signal quantique permettant d'accéder à une connaissance partielle d'un tel état pour des courants électriques quantiques. Une première méthode consiste à les analyser à nombre d'excitations fixé au travers des cohérences électroniques. Pour cela, nous élaborons une analyse de la cohérence à un électron en termes d'atomes de signaux électroniques. En combinant cela au protocole de tomographie par interférometrie HOM, nous présentons la première autopsie, fonction d'onde par fonction d'onde, d'un courant électrique quantique.Une autre approche consiste à examiner des indicateurs sondant directement l'état à N corps. Nous étudions le rayonnement émis par un conducteur quantique ainsi que la décohérence électronique d'une excitation à un électron. Ensuite nous analysons la distribution de probabilité de la chaleur dissipée par un système quantique mésoscopique. Dans ce cadre, nous développons une théorie de l'effet Joule en régime quantique et à explorons comment celle-ci pourrait permettre de sonder l'état à N corps
Quantum physics experiments have reached a level of precision and control that allows quantum state engineering for many systems. This has led to the birth of electron quantum optics, an emerging field which aims at generating, manipulating and characterizing quantum electrical currents built from few-electron excitations propagating within ballistic quantum conductors. This is challenging since it is generically impossible in practice to fully characterize the many-body state of a beam containing indistinguishable electrons. The thesis presents new quantum signal processing approaches for accessing, at least partially, to the quantum many-body state of quantum electrical currents.A first approach is to access such a state at few-particle levels through electronic coherences. We will thus present a new representation of single-electron coherence in terms of electronic "atoms of signal". Combining this signal processing algorithm to HOM tomography enables us to present the first autopsy, wavefunction by wavefunction, of an experimental electrical quantum current. Another method is to look for indicators giving information directly at the many-body level. We will investigate the radiation emitted by a quantum conductor and address the problem of decoherence of a general single-electron excitation. Finally, we will look at the heat deposited by a mesoscopic quantum system, leading to a quantum version of Joule heating and discuss how it gives an insight on the many-body state of the electron fluid

Abstract:
The main purpose of this study was to assess the value of the autopsy in clinical medicine today. This has been achieved by analysing several variables. Clinical diagnostic accuracy was determined by assessing the concordance/discrepancy of clinical and autopsy diagnoses. In addition, several variables were assessed, namely whether or not the size of the hospital, type of ward, autopsy rate, age of the patient, and degree of confidence with which the clinical diagnosis was made had any bearing on the accuracy of the diagnosis. The impact of the newer diagnostic tests on clinical diagnosis was also assessed to confirm or refute the widely held opinion amongst clinicians that diagnostic advances have indeed reduced the need for autopsies. The attitudes of pathology registrars and consultants towards the autopsy were analysed, using a questionnaire proposed by Stubbs et al.

Abstract:
The aim of this study was to determine the extent to which the cause of death in sudden natural death in adults could be ascertained without autopsy, based on information provided in the police report, and to evaluate any degree of error between that and the officially certified cause of death. Two methods were carried out: 1. A questionnaire-based retrospective survey of sudden natural deaths distributed to practitioners. The predicted cause of death was compared with the actual cause of death as determined after autopsy. The difference between the two causes of death was classified into 'no difference', minor and major difference according to the classification scheme developed for this study. 2. A prospective study of adult sudden natural deaths referred by the procurators fiscal in Lothian and Borders to the department during 2009 and 2010. The police reports were reviewed and a cause of death was ascribed by the author. This cause was compared with the actual cause of death in the final autopsy report and any difference assessed according to the classification scheme. In the view of the retrospective study results, it is possible to determine the cause of death which is with no or minor difference to that determined by autopsy if, in addition to provide sufficient information regarding deceased’s medical history and circumstances of death, the participants are given the option to select which cases should undergo external examination only. The prospective study results indicate that in certain circumstances the cause of death could be ascribed correctly based on the available information prior to autopsy. Causes of death such as ischaemic heart disease, hypertensive heart disease and alcohol related death are more frequently ascribed correctly. However, ischaemic and hypertensive heart disease were over-predicted. Results indicate that there are criteria to select which cases might be subject to a "view and grant" procedure with no significant loss of accuracy. The implications of the study for the development of medico-legal services are considered with specific reference to Kuwait.

Abstract:
Quality management is of the utmost importance for providing the best patient care in our healthcare system. Patients rightfully expect the care that they receive to be of sound quality. The medical autopsy has been used as the gold standard of diagnostic medicine and continues to provide insights into strategies for healthcare quality improvement. The discordance and concordance rates between autopsy and clinical diagnoses have been used to identify areas of improvement and provide education opportunities to healthcare professionals. Discordance between autopsy and clinical diagnoses has revealed several areas in which clinicians need to improve their diagnostic skills and implement systemic changes to detect and mitigate diagnostic errors. Unfortunately, the rate of hospital autopsies has declined over the past several decades. The purpose of this study was to expand our previous work and combine the analyses of discordance and concordance between autopsy and clinical diagnoses across a 10-year period from 2002 to 2011 and to assess the role of the medical autopsy as a quality improvement tool in modern healthcare systems. Within our study, the autopsy rate of all in-patient deaths was approximately 6%. Our study indicates that the concordance rate between clinical and autopsy diagnoses was 77.5%, the discordance rate was 19.4%, and 3.1% were inconclusive. The discordance rate varied from as low as 9.7% in 2007 to as high as 27.1% in 2002. These findings suggest that overall, approximately 1 in 5 autopsies revealed discordance between autopsy and clinical diagnoses. This is a significant number of cases for which there exists both quality improvement and educational opportunities, thus, supporting the continued use of autopsy. We suggest these data should be used to encourage residents and physicians to continue using autopsy as an important quality tool to extend our understanding of disease processes. Hospital autopsies are closely associated with quality improvement and better patient care.

Prescription opioid deaths have tripled since 1999, and currently opioid overdose kills 115 Americans per day on average (1). Prior to 2014, prescription opioids have been the primary driver of opioid-related mortality. In recent years, the United States has seen a steady decline in the rate of opioid prescription. At the same time, there has been a significant increase in the number of deaths attributed to non-prescription opioids such as heroin, illicitly manufactured fentanyl, and fentanyl analogues. In 2017, among 70,237 drug overdose deaths nationally, 47,600 (67.8%) involved opioids, with increases across age groups, racial/ethnic groups, and county urbanization levels in multiple states (2). The opioid epidemic is especially profound in Tennessee, which had the 3rd highest opioid prescription rate in the country in 2017 and an opioid-related death rate of 19.3 deaths per 100,000 persons, compared to the national average of 14.6 (3). This retrospective study analyzes autopsy data from West Tennessee Regional Forensic Center (WTRFC) from 2007 to 2017 to gain a better understanding of the effects of the opioid epidemic on West Tennessee and the surrounding areas. Data from opioid-related accidents and suicides were analyzed in order to identify trends in race, age, gender, location, types of opioids, and drug combinations involved in opioid-related deaths.

Lorsque Neal Stephenson a introduit le terme « métavers » dans son roman de science-fiction Snow Crash, en 1992, il était loin de se douter que le mot allait susciter autant de discussions. La notion d’une réalité d’un type nouveau, qui serait synthétisée, puis virtualisée et librement socialisée, est fascinante par ce qu’elle exigerait aux plans scientifique et technique. Fascinante surtout par ses retombées éventuelles aux niveaux culturel et social, y compris de nature éthique (qui ne sont pas abordées ici). Ce texte rappelle brièvement l’origine du concept avant de se consacrer à ses requis et défis techniques, abordés en l’examinant comme un système avancé d’information et communication. Le métavers revêt une complexité inédite alors que les capacités cognitives de l’humain et de la machine sont appelées à se fusionner avec synergie. L’analyse – qui demeure succincte compte tenu du format d’un article court – permettra de comprendre comment et pourquoi le métavers, dans la mouture originale proposée par Stephenson, demeure une utopie. Mais aussi comment l’élimination de certains requis peut permettre d’en retenir une saveur intéressante, laquelle apparait déjà dans une multitude d’applications.
