Full textBackground: Breast cancer is a global disease that with technology can be discovered at an early stage. Advanced breast cancer is the last stage (IV) of breast cancer, which means the cancer has been spread in to other parts of the body. The registered nurse has an important role in alleviating the suffering women with breast cancer undergo. Therefor it is important to have knowledge in order to provide the necessary care that is based on each women’s needs. Aim: To describe womens’ experience of living with advanced breast cancer. Method: A literature review of eight qualitative articles and one article that had combined both qualitative and quantitative approach. Result: Two main categories identified; changed life situation and left by themselves with six subcategories. Conclusion: There is a need to get an appropriate information and an increased support from healthcare, in managing the disease. The women wish for establishment of group supports that has its focus on advanced breast cancer.

Full textAnhörigas närvaro vid avancerad hjärtlungräddning (A-HLR) har på senare år varit ett internationellt diskuterat ämne. Flera undersökningar har gjorts som undersökt anhörigas syn på sin egen närvaro vid denna akuta vårdsituation. Dessa studier har visat att anhöriga i stor utsträckning önskar få möjlighet att närvara vid en A-HLR situation. Av denna anledning var det av intresse att studera akutsjuksköterskors syn på detta. Syftet med studien var att undersöka och jämföra attityder och erfarenheter av anhörigas närvaro vid A-HLR hos sjuksköterskor inom ambulanssjukvården, akutmottagningen och hjärtintensivvårdsavdelning. Studien bygger på en enkätundersökning som lämnats till samtliga sjuksköterskor på dessa enheter (n=101). Data har analyserats statistiskt med chitvåtest samt Fishers exakta test. I resultatet framkommer tydligt att ambulanssjuksköterskorna är de sjuksköterskor som har mest erfarenhet av anhörigas närvaro under A-HLR. De är också den grupp sjuksköterskor i denna jämförelse som ser mest positivt på att anhöriga finns närvarande. För övrigt är de den grupp som i minst utsträckning aktivt kan välja att utesluta anhörigas närvaro i jämförelse med slutenvårdsarbetande sjuksköterskor. Då tidigare forskning visar att anhöriga önskar och bör ges möjligheten att närvara vid A-HLR situationen kan resultatet av denna studie leda till mer strukturerade utbildningsinsatser för akutsjuksköterskan.

Aim: The purpose of thestudy was to focus on the experience of patients with amyotrophic lateral sclerosisregarding advanced home care. Method: The method for the research was anintegrated literature study where articles searches and manual searches were made insix databases covering medicine and medical care. The data analysis was performedaccording to Whittemore and Knalfs four step process. Results: The study is based on21 scientific articles and reported in three categories and six sub-categories. Patients’with amyotrophic lateral sclerosis experience of advanced medical care in the homewas that they felt security and support in their everyday life from the professional caregiven by their family and friends. Multi professional teams around the patient createdcontinuous and customized symptoms relief and eased the physical and mentalsymptoms. Involvement and self-determination brought purpose and autonomy in thepatients’ daily life. Implication: In the future, an increase of advanced home care isto be expected, hence further research is recommended in the field and the patientsexperience thereof.

Full textIntroduction: Nurses working in an acute medical care setting sometimes participate in the treatment of patients diagnosed with anorexia nervosa. Anorexia nervosa is a serious disease with a high mortality-rate. The disease affects the body as well as the mind. Patients diagnosed with anorexia nervosa describe nurses' lack of knowledge about the diagnosis. Nurses in medical care settings describe several challenges in the nursing process involving patients diagnosed with anorexia nervosa. Purpose: To examine the experiences of nurses working in an acute medical care setting when treating and nursing patients diagnosed with anorexia nervosa. Method: A qualitative interview study. Semi-structured interviews were conducted with eight nurses working in an acute medical care setting. The text material was analyzed with thematic analysis. Results: The nurses experienced strong feelings when nursing patients diagnosed with anorexia nervosa. The nurses described unclear treatment structures and insufficient knowledge about the diagnosis. A well-established cooperation with the psychiatric unit as well as improved knowledge about the diagnosis were described as requirements in order to offer the patients advanced nursing. Conclusion: Nurses in a medical care unit experienced several challenges when nursing patients with anorexia nervosa. The nurses described a constant lack of time, unclear treatment structures and insufficient knowledge about the diagnosis. Tutorials, education and cooperation with the psychiatric unit could support nurses in medical health units to improve in order to understand the complex needs of patients diagnosed with anorexia nervosa.

Full textBackground: Cancer is, globally, one of the most common diseases. Half a million Swedes recive a cancer diagnosis during their lifetime. In one day, approximately 3 Swedish children experience the loss of their parent due to cancer. To help these vulnerable children the community needs to increase knowledge of the subject in order to create better conditions for the childrens future. Aim: The aim of this litterature study was to illuinate the experiences of children and adolescents living with a parent diagnosed with advanced cancer. Metodhs: A literature study with qualitative articles was selected. The articles were found in Cinahl, PubMed and PsycINfO. Eight scientific articles conformed to the aim. The result in the studies were analyzed with help of a 5-stepmodel as described by Friberg. Results: The analysis resulted in 5 categories and 13 subcategories. The categories are: emotional reactions, how children cope with the situation, the main support, changes in family- and social life and growing with the experience. Conclusion: An open communication and to be involved in the parent´s desease is important for the children. The nurse should work in a family-centred approach to provide good care for the children.

Full textBackground: At present, there are gaps in the knowledge of how the perioperative process is seen from the point of view of the operating room nurse. When it comes to the operating theater nurse to provide quality care to the patient, it is important that the operating theater nurse gets a good contact with the patient. The operating room nurse has to share the patients’ world. Then, it will be easier to give a patient-centered care. Aim: The purpose of this study is to investigate how the operating theater nurse experienced the perioperative process from her/his point of view. Method: This study is a literature review of eleven articles that have a qualitative approach and one article which has a quantitative approach. This study has a more qualitative approach. Its focus is describing and interpreting the surgery nurses’ experience of the perioperative process. Result: The surgical nurses had, to a large extent, a favorable attitude to the perioperative process. Surgical nurses felt that the perioperative process contributed to increased continuity and increased safety for patients, but above all for themselves. Surgical nurses developed professionally, but also as individuals, they felt that their work was meaningful and that there was a caring relationship between the patient and the surgical nurse. Conclusion: This paper emphasizes that if the perioperative process would be used on a larger scale, the work for operative nurses would be more efficient, provide a more secure care, give more continuity and be very favourable to nurses as well as patients. The result for the patients would be a good care and well-being.

Full textPandemics have occurred several times around the world and the covid-19 pandemic has hit Sweden's healthcare really hard. The spread of the virus was discovered in 2019 and reached Sweden in 2020. Sweden's healthcare and population have previously had four pandemics since the beginning of the 20th century: the Spanish flu in 1918, the Asian in 1957, the Hong Kong flu in 1968 and the 2009 pandemic called the swine flu. But pandemic preparedness and planning to reduce the negative consequences of a pandemic have not been fully structured. Working patient safety, relieving suffering for patients and working to promote health is part of the district nurses work. The district nurses work has for a long time been strained and especially in-home care with understaffing, very independent work and patient’s multiple health conditions. Home care is one of the fastest growing areas in health care and more and more sick patients are being cared for at home. Even before the covid-19 pandemic the workload has been very mentally and physically stressful for the district nurse. The purpose of this study is to investigate the district nurses experience of caring for covid-19 patients during an ongoing pandemic. Semi-structured interviews with ten district nurses were performed and the material was transcribed and analyzed with qualitative content analysis with an inductive approach. The result resulted in three main categories and seven subcategories. The district nurses expressed how important it was to have strong leadership in an organization, anxiety, fear and apprehension of taking the infection home to their private lives. The feeling of down priority of protective materials compared to other care chains. No routine and structure in the work and an increased care sufferer related to increased workload on the district nurses.

Full textBackground: As the population grows older, dementia also increases. About 160,000 people currently live with a dementia disease in Sweden. Dementia is a complex disease with both mental and physical symptoms, and more knowledge is required in several areas to fulfil the nursing care required by the nurse. The disease cannot be cured but its symptoms can be relieved with the help of various efforts. Patients with advanced dementia have a greater need for care depending on the course of the disease. The nurse is the one responsible for the care of patients with dementia. Nursing should be guided by a person-centered approach.Purpose: The purpose of this study was to highlight the nurse´s experiences of nursing palliative patients with advanced dementia.Method: Literature study with qualitative approach was chosen as a method. The databases which were used was CINAHL and Pubmed. Ten articles were selected which were assessed and analyzed through quality review and thematic analysis.Result: The result shows that the nurse´s experience in the care of palliative patients with advanced dementia is based on four different categories, communication, the nurse´s care, shortcomings and ethical dilemmas. The categories were selected based on various themes such as needs, communication, family and relatives, pain/symptoms and nurses perspective.Conclusion: The conclusion shows that education, good teamwork, person-centered care and resources from different directions are required for the care to be as good as possible for palliative patients with advanced dementia. It requires the nurse to create a good relationship with the patient with advanced dementia. It is important to take into account the individual needs of the patients and to take into account the ethical of every decision that is made regarding the care.Keywords: Advanced dementia, literature study, palliative care, nursing, qualitative.

Full textPalliative care in Sweden takes place both in hospital, hospice, municipal nursing homes, and in the patient's home. Regardless of where, family caregivers play a central role and are entitled to individualized support measures. In order to provide this, we need knowledge about their experiences and in which domains of life they need support. The purpose of this study was to describe and interpret the experiences of being next-of-kin to a person who, at the end of life, was cared for in advanced palliative home care, and died in Hospice. Eleven qualitative interviews were conducted with next-of-kin to patients in an advanced palliative care setting in southern Sweden. The data were analyzed using a phenomenological-hermeneutic method inspired by Ricoeur. The phenomenon of being next-of-kin to a person at the end of life meant participating in a drama with an unknown script. Three main themes emerged - change, vulnerability and support. The phenomenon was interpreted in the light of Eriksson's theory of suffering, where suffering is described as a drama. When caregivers were co-actors to the next-of-kin and confirmed her/his suffering, suffering was alleviated. Support meant helping next-of-kin and the ill person to be directors of their own drama, even when the ill person was in Hospice. This support can be developed by using family focused communication models.

Full textBackground: A nurse graduate’s transition period in her profession is well illustrated from previous studies and theories. However, what has been overlooked is how the newly graduate perceives her ability in practice, especially in relation to what is actually required in good nursing care. Aim: The aim of this study was to elucidate the newly examined nurse’s experience of her conception of what is demanded of her in relation to a safe care. Method: This study was carried out as a literature review from nine scientific articles. The analysis method was a deductive content analysis (template analysis; Malterud, 1998). The structure of the analysis was based on the model Man -Technique-Organisation of Ödegård (2006). Results: in the analysis, originated from the categories – man– technique – organisation after the model of Ödegård, emerged eight subcategories: physical stress, distraction, emotional inadequacy, inability to objective thinking regarding nursing care; need of support in urgent situations, frustration in dealing with complicated and difficult technology; a guest in a dysfunctional reality, and the weak link – an imbalance in staffing. Conclusion : The analysis of nine scientific articles, with support of Ödegård’s and Benner’s theoretical concepts, has shown that the transition from a student nurse to novice and advanced beginner, i.e. a transition from the theoretical knowledge to the knowledge in practice, subsequently includes experiences of restraining aspects which leads to problem issues. The nurse’s inability in practice is manifested clearly according to the category person and moderately to the categories technology and organisation. Regarding the newly examined nurses’ experience of knowledge issues in relation to the demand in giving a good and safe nursing care, the results originate primarily from the concepts Man and Organisation, and only hardly from the concept, technology.

Full textBackground: Breathlessness is a symptom that makes breathing difficult which can lead to experiences of anxiety and fear. It is one of the most common symptoms experienced by patients with advanced stage disease. Several diseases such as COPD, cancer and heart failure can cause breathlessness. Palliative care aims to alleviate suffering by treating and preventing symptoms, such as breathlessness. Nurses need to be aware of patients´ experiences in order to provide and individually adjust symptomatic relief. Aim: Describe patients´ experiences of breathlessness, while receiving palliative care or receiving care for an advanced disease, including their strategies for managing breathlessness. Method: A literature review was carried out with data collection from five databases that resulted in 12 scientific articles of which 11 were qualitative and one was quantitative. The data analysis was made with Friberg (2017) method and resulted in two themes. Results: The two main themes that emerged were Experiences of breathlessness and Strategies for managing breathlessness. The result showed that breathlessness can cause anxiety and fear, physical fatigue and severe coughing. In addition, breathlessness implied limitations of patients´ daily life and could lead to patients being isolated in their homes, intended or unintended. Different strategies for managing breathlessness are presented. Discussion: In the discussion the pros and cons regarding the authors' joint work, challenges and considerations were addressed. The main findings of the result were discussed using Sister Callista Roy’s adaptation model.

Full textAbstract Background: For ambulance nurses work often involves long journeys to the hospital. Journey times vary between 20-60 minutes. When the mechanical compression equipment doesn`t exist, the ambulance team is performing advanced cardiopulmonary resuscitation with manual compressions, unbelted at high speeds. Aim: The aim of this study was to investigate the ambulance nurses experience of cardiac arrest situations with following longer transports to the hospital. Method: Ten ambulance nurses were interviewed. For the analysis of the collected data, qualitative content analysis with inductive approach was used. Results: The results showed that the situations when ambulance nurses need to perform advanced cardiopulmonary resuscitation during longer transports to the hospital, causes feelings of being unsafe and deserted. Mechanical compression equipment is not standardized in all ambulances, and manual compressions during transport requires that ambulance nurses are unbelted. The security risks this implies for the ambulance nurse and the patient, lead to feelings of vulnerably and insecurity. In the prehospital environment that often requires quick decisions that can be life changing, ambulance nurses believe that a higher competence in ambulance service would give them a more secure profession. Ambulance nurses value the interaction with rescue and police, and see them as well functioning. Conclusion: The results show that there is a need for standardized mechanical compression equipment in the ambulances, measures to strengthen the teamwork, and competence in the daily work. These measures could contribute to increased efficiency and greater security in the professional role of ambulance nurses during longer transports to the hospital. In order to create safety and confidence in the ambulance teams, experience should be considered as an important aspect when recruiting ambulance nurses. Result: The improvement work resulted in an even flow of referrals and shortened admission times to ASIH. Patients’ sense of security were rated to 76%. Seven Seven patients had the possibility towere be re-admitted 1-7 times 1-7 times during the project. Casestudy summary: ”Conditions for cooperationfor a common goal”. A clear pattern with the following significant conditions emerged from the focusgroups interviews: continuity, learning, cooperation, and communication. Discussion: ASIH can be an option to facilitate the transition home for elderly patients with multiple illnesses. ASIH provides a sense of security and may prevent re-hospitalization. This model of cooperation requires clear, common goals and opportunities for feedback to enable learning contributes to measurable results.

Full textThe proportion of older people is increasing in Sweden and the world, which means that the demand for care is increasing, but all care need not executed in hospitals. Efforts that can prevent avoidable outpatient care should be important to work on from a sustainability perspective, but also important to reduce the stress, confusion and deteriorating health status a hospital stay can cause for the older fragile patient. Mobile local health care teams executed from the local hospital are a relatively new form of care in Sweden. Mobile local health care teams were introduced in the Västra Götaland region about 10 years ago. Mobile local health care teams makes efforts for fragile patients with complex needs in their home. The registered nurse’s expertise includes being able to independently be responsible for the care of patients, having the ability to see the entire patient's needs and working for patient's safety. The aim of this study is to illustrate the nurse's work with fragile older patients in mobile local health care teams. This study has been conducted as a qualitative method with an inductive approach and is based on interviews as a data collection method. We have interviewed nurses working on three different mobile local health care teams. The result has shown that the registered nurse in mobile local health care teams plays an important role as they carry out advanced care in the home through collaboration in teams and with other healthcare players. The registered nurse has a holistic perspective with the patient in focus, unnecessary hospitalizations and re-admission can be reduced. This study shows that fast follow-up in the home of fragile patients who are discharged from hospitals creates security and reduces re-admissions. In addition, proper care of fragile elderly patients is an organizational issue and care needs to be conducted by the right expertise. Registered nurse's work in mobile local health care teams means working with sustainable development by reducing the consumption of inpatient care.

Full textResearch has proven that a care of high quality is connected with a high degree of nursing competence. Although nurse specialists with advanced education have responsibilities to implement evidence-based research in practice, in addition to improve the quality of nursing care, there is a lack of a comprehensive and clear description of what this actually means and how their skills best should be applied. The purpose of this study is to examine the roles of nurse specialists and the impact their professional knowledge has on quality of care and health care development. A literature review was performed. Eighteen articles, of which twelve with qualitative method and six with quantitative method, were included in the result. A thematic analysis was made of the results of the articles. The skills of specialist nurses proved to be important for several patient groups as well as for other healthcare professions. The advanced competence made a difference for both quality of care and healthcare improvement. Thus, several challenges like organizational resistance and ambiguity in role descriptions can lead to a lack of development opportunities and that the competence of specialist nurses are not being fully utilized. The skills of nurse specialists need to be seen by healthcare organizations as a prerequisite for the improvement of care and for the development of care quality.
