Abstract:
Introduction Anthony Burgess’ novel ‘Clockwork Orange’ identifies the topical debates surrounding the use of aversion therapy (or aversive conditioning) as an effective treatment for addictive behaviours. Widely popularised in literature as ‘Ludovico’s Technique’, Burgess attempts to credit the misunderstanding and dramatization of its effects when the main protagonist is released from a prison sentence after undergoing this treatment. Objectives We aimed to highlight the depictions of aversion therapy in modern popular literature. Methods A narrative review of the current literature concerning aversion therapy and Anthony Burgess’s novel ‘A Clockwork Orange’ was conducted. Emphasis on the misinterpretation of aversive therapies was noted. Results Since the introduction of pharmacological alternatives and additional forms of psychological therapies, there has been a decline in the use of aversion therapy in recent decades. However, it is still effective when conceding the conditioning process. Likewise, its predecessor’ visual imagery’ is believed to be a more acceptable and effective form. Conclusions The depiction of aversion therapy in literature and media has played a role in shaping societal views on aversive conditioning techniques and the degree to which they are deemed acceptable forms of treatment. The “Ludovico Technique” featured in the novel ‘A Clockwork Orange’ and its film adaptation is arguably the most salient depiction of aversion therapy in popular culture. Abstract:
The sensitivity of an alcoholic's sensory systems (55) to alcohol therapy is a good indication of treatment efficacy in this disease. In this study, we compare pharmacotherapy (PHT) and acupuncture therapy (AP) on the sensitivity of visual (VL), acoustic (AC), olfactory (OL) and taste (TS) systems to alcohol irritants (AI). The results showed that PHT changed the threshold sensitivity of 55 to AI but did not cause aversion of SS to AI. In acupuncture treatment, results can be divided into four groups: Group 1, no aversion of AC was produced to AI; Group 2, no aversion of TS and OL were produced to AI; Group 3, no aversiori ofVL to AI; and group 4, no aversion was recorded to AI. Supposedly, one of the sensory of information, and other SS are subordinates. Therefore, before acupuncture treatment for alcoholism, to the leading sensory system of the patient.

Background Learning aversion symptoms have become increasingly common among college students, who may lose interest in their studies, lack motivation, and even experience avoidance and aversion to school life. These emotions and behaviors not only affect their academic performance but may also cause harm to their long-term development and mental health. As a psychotherapy that focuses on the present and cultivates self-awareness, and acceptance, mindfulness therapy has shown positive therapeutic effects on various mental health problems. Combining cross-cultural human-computer interaction and collaborative translation teaching may provide a novel approach to treating college students’ learning aversion.Subjects and MethodsThe study randomly divided 46 college students with learning aversion into experimental and control groups. The control group received routine treatment; The experimental group conducted cross-cultural human-computer interaction and collaborative translation teaching in combination with mindfulness therapy for treatment. The study used the self-made Academic Negative Emotion Scale to evaluate the academic aversion of college students.ResultsIn the measurement before the experiment, all students showed severe aversion to learning, but the difference between the two groups was not significant (P>0.05); After the experiment, the level of student aversion to learning in the experimental group was significantly improved and significantly lower than that in the control group (P<0.05).ConclusionsCross-cultural human-machine interaction collaborative translation teaching and mindfulness therapy can alleviate the degree of college students learning aversion and assist their regular learning and life.

This paper presents a single case study of a patient with compulsive hairpulling (trichotillomania) of 16 years' duration, who was successfully treated using electric aversion therapy. All previous attempts at treatment and self-help had been unsuccessful. A behavioural self-control programme was tried initially with some success, but the progress was not maintained following an unplanned break in treatment and this treatment was ineffective when reinstated. Electric aversion therapy eliminated hair pulling almost immediately and the results were maintained on follow-up, with the exception of a minor relapse at a time of stress.

Abstract Background Addiction of tobacco is both mental and physical. For many smokers, the best way to quit is a combination of medical, behavioral, and emotional support. The aim of the work was to study the effectiveness of aversion therapy using onion-powdered cigarette as a method of quitting compared with nicotine replacement therapy and conventional behavioral (5As) therapy in smoking cessation. Patients and methods A prospective, randomized study recruited 60 smokers from Ain Shams University Hospitals who were divided into three groups. Group I: included 20 participants subjected to a brief verbal (5As) therapy for smoking cessation; group II: included 20 participants subjected to a brief verbal (5As) with adding nicotine gum; group III: included 20 participants subjected to brief verbal (5As) with adding aversion therapy in the form of single onion-powdered cigarette by mixing tobacco with dried onion powder by an equal amount. Weekly interviews were conducted for follow-up, for at least 6 months from the beginning of treatment. Results All participants were male smokers within the age range of 21–70 years. There were significant statistical differences (P≤0.05) between the three studied groups as regards the outcome: the complete success rate of quitting was significantly higher among group 1 (30%) versus 15% in group II and 15% in group III. On other hand, the partial success was recorded in 45, 55, and 20% of group I, II, and III, respectively. Lastly failed attempts were recorded in 25, 30, and 65% of group I, II, and III, respectively. There were significance statistical correlations (P<0.05) found between the outcome and the age, pack/year, duration of smoking, craving time, presence of relapsing causes, and nicotine dependence score. However, there was no statically significant correlation (P>0.05) found between the outcome and age of initiation, occupation, presence of comorbidities, marital status, and previous quitting attempts. Conclusion Behavioral therapy (5As) is most effective in smoking cessation program. Nicotine replacement therapy and aversion therapy may be an adjuvant in the smoking cessation program. Aversion therapy is a simple procedure, economic, practical, easy to apply, and may be effective in smoking cessation.

Physical activity (PA) is a non-invasive, cost-effective means of reducing chronic disease. Most US citizens fail to meet PA guidelines, and individuals experiencing chronic stress are less likely to be physically active. To better understand the barriers to maintaining active lifestyles, we sought to determine the extent to which short- versus long-term PA increases stress- and aversion-related markers in wild-type (WT) and low voluntary running (LVR) rats, a unique genetic model of low physical activity motivation. Here, we tested the effects of 1 and 4 weeks of voluntary wheel-running on physiological, behavioral, and molecular measures of stress and Hypothalamic Pituitary Adrenal (HPA)-axis responsiveness (corticosterone levels, adrenal wet weights, and fecal boli counts). We further determined measures of aversion-related signaling (kappa opioid receptor, dynorphin, and corticotropin releasing hormone mRNA expression) in the basolateral amygdala (BLA), a brain region well characterized for its role in anxiety and aversion. Compared to sedentary values, 1, but not 4 weeks of voluntary wheel-running increased adrenal wet weights and plasma corticosterone levels, suggesting that HPA responsiveness normalizes following long-term PA. BLA mRNA expression of prodynorphin (Pdyn) was significantly elevated in WT and LVR rats following 1 week of wheel-running compared to sedentary levels, suggesting that aversion-related signaling is elevated following short- but not long-term wheel-running. In all, it appears that the stress effects of acute PA may increase molecular markers associated with aversion in the BLA, and that LVR rats may be more sensitive to these effects, providing a potential neural mechanism for their low PA motivation.

This study sought to investigate the relative efficacy of Multimodal aversion therapy on alcohol dependence behavior of College of Education students. The study used experimental research design involving randomized experimental and control groups. The population for the study consisted of 2,150 NCE 1 (100level (Nigeria Certificate in Education) students of the college. However, the target population was those identified with alcohol drinking dependent behavior. A sample of 42 NCE 1(100level) students constituted the subjects for the study. Both stratified and simple random sampling were adopted for selecting the sample, taking into cognizance, the students’ schools, departments and gender. Two instruments Alcohol Drinking Dependence Questionnaire (ADDQ) and Drinking History Questionnaire (DHQ) were used for data collection. Both instruments were validated by expert judgment and had reliability co-efficient of 0.73 and. 0.76 respectively. The data collected were analyzed by using both descriptive and inferential statistics to answer the research questions and test the postulated hypothesis respectively. The result indicated that the prevalence of alcohol drinking among FCT College of Education students was on the average. It also revealed that multimodal aversion therapy was effective in the cessation of alcohol drinking dependence behavior of both male and female students. Multimodal aversion therapy was recommended for both psychologists and Guidance Counselors for the modification of alcohol drinking behavior and abuse of drugs among students.

Penelitian ini dilatar belakangi oleh fenomena menurunnya tingkat kedisiplinan siswa kelas IXF SMPN 1 Semanu semester gasal tahun pelajaran 2019/2020. Penelitian bertujuan untuk mengetahui peningkatan kedisiplinan siswa melalui konseling kelompok behavioristik dengan teknik aversion therapy. Subyek penelitian siswa kelas IX-F SMP Negeri 1 Semanu dengan jumlah 8 siswa. Penelitian dilakukan berdasarkan metode penelitian tindakan kelas (Class Action Reasearch). Pelaksanaan penelitian ini menerapkan 2 siklus, masing-masing siklus terdiri dari perencanaan, pelaksanaan, pengamatan dan refleksi. Instrumen yang digunakan adalah observasi kedisiplinan siswa pra siklus, siklus I dan siklus II serta observasi keaktifan siswa. Hipotesis penelitian yaitu konseling kelompok behavioristik dengan teknik aversion therapy dapat meningkatkan kedisiplinan siswa. Data penelitian menyimpulkan keaktifan siswa pada aspek menyimak secara aktif sebesar 93% kategori sangat baik. Aspek mengajukan pertanyaan sebesar 85% kategori sangat baik. Aspek menjawab pertanyaan sebesar 88% kategori sangat baik. Aspek menghargai orang lain sebesar 85% kategori sangat baik. Aspek mengemukakan pendapat sebesar 83% kategori sangat baik. Frekuensi siswa meninggalkan kelas jam ke 1-2 sebanyak 1 kali atau 2%. Frekuensi siswa meninggalkan kelas jam ke 3-4 sebanyak 0%. Frekuensi siswa meninggalkan kelas jam ke 5-6 sebanyak 1 kali atau 2%. Frekuensi siswa meninggalkan kelas jam ke 7 sebanyak 1 kali atau 2 %. Rata-rata persentase frekuensi siswa membolos pada siklus I sebesar 30% dan pada siklus II sebesar 6% atau terjadi penurunan sebesar 24%. Kedisiplinan siswa pada siklus I 70% kategori sedang dan pada siklus II 94% kategori sangat tinggi, meningkat sebesar 24%. Abstract Growth faltering is a complex problem in the pediatric population and can result from multiple mechanisms, including inadequate caloric intake. Feeding difficulties are common among children with growth faltering and can present significant challenges for achieving adequate nutrition and weight gain, particularly when coupled with any degree of oral aversion. The patient in this case is a 21-month-old child with oral aversion, pediatric feeding disorder (PFD), and growth faltering who had failed to gain weight while receiving the standard of care. Upon transitioning his care to an osteopathic pediatrician, who incorporated osteopathic manipulative treatment (OMT) into his care, he demonstrated weight gain, which was sustained over the 8 months of this study. This case report demonstrates the potential benefit of OMT for children with PFD, oral aversion, and growth faltering.

Even when pain control is implemented, calves may experience pain for days after hot-iron disbudding. Whether calves seek pain relief post-disbudding offers a novel approach to assessing pain in these animals. By employing an approach-aversion paradigm, we explored the ability of cold therapy to provide immediate pain relief in disbudded calves. Calves were habituated to the manual placement of a cool or ambient pack on their forehead for a short duration simultaneous to milk reward consumption, prior to disbudding. Calves were then disbudded under local anaesthesia (procaine) and analgesia (meloxicam), and responses to the packs were observed over subsequent days. Individual calves were consistently exposed to either cool or ambient packs in different halves of a two-sided experimental pen, allowing for the testing of approach-aversion and conditioned place preference. We found calves approached milk rewards quicker and maintained contact for longer when receiving cold therapy compared to the ambient control. However, calves did not display any conditioned preference for the pen where they received the cool pack. These results add to the growing evidence of lasting pain following disbudding procedures and suggests that cold therapy provides some form of pain relief post-disbudding. Future studies should seek other ways to use cold therapy post-disbudding to reduce aversiveness and human involvement.

This paper explores how emotional cues from unexpected sports outcomes impact consumers’ perception of their experience at local businesses. Using nearly 1 million Yelp reviews from the Phoenix area, I empirically test for the presence of loss aversion and reference-dependent preferences in reviewer behavior. Consistent with loss aversion, unexpected losses lead to worse reviews while there is no effect for unexpected wins. The impact of unexpected losses is concentrated in home games, with no effect for away games. The results also reflect reference-dependent preferences since wins and losses in games predicted to be close do not impact reviewer behavior. Consumer services that cater to National Basketball Association fans (e.g., sports bars) experience pronounced effects.

Abstract Background Perampanel is a selective, noncompetitive amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid glutamate receptor antagonist indicated for management of partial-onset and primary generalized seizures in epilepsy patients aged ≥12 years. Patient History A 29-year-old, white female with significant history of medically refractory frontal lobe epilepsy, status post right frontal and temporal resections, was initiated on perampanel as an add-on therapy to phenytoin extended-release (330 mg/d) and clonazepam (2.5 mg/d). She previously failed several antiepileptic drugs because of inefficacy and/or intolerance. Perampanel was initiated at 2 mg/d and the dose was increased by 2 mg/d increments every 2 to 3 weeks. Following the first dose, nausea and drowsiness were reported but resolved the following day. Three days after titration to 6 mg/d, the patient developed complete food aversion and became more irritable and anxious while no seizure frequency improvement was noted. No change of sense of taste was reported. After reduction to 4 mg/d, adverse effects improved but did not completely resolve until 2 months following perampanel discontinuation. Review of Literature A PubMed search revealed no published literature or case reports of perampanel-induced food aversion or anorexia in a presence or absence of phenytoin and clonazepam. Conclusion In this report, a temporal relationship was observed between perampanel dose-increase and the development of food aversion. Return to baseline appetite and eating habits following perampanel discontinuation strongly suggest perampanel involvement. At this time, the exact mechanism(s) behind food aversion associated with perampanel is/are unknown.

We describe the limited availability of United States Food and Drug Administration (FDA)–approved antiobesity medications for adolescents and compare this to what is available for adults. We also highlight the need for more pharmacotherapy options and additional research into novel treatments for pediatric obesity. Case Description We describe a patient who is struggling with managing her weight and starting to develop complications of obesity. We offer a framework in which off-label prescribing may be beneficial to patients who have been engaging in lifestyle modification therapy yet fail to see improvement. Conclusion Lifestyle modification therapy is necessary but often insufficient in stimulating clinically meaningful weight loss when used alone in children and adolescents who struggle with weight management. Until more FDA-approved antiobesity medications are available, pediatricians may be able to help more patients achieve weight reduction goals by familiarizing themselves with the responsible use of off-label medications and implementing these tools to improve clinical outcomes. There is a critical need for more pharmacotherapy options to help pediatric patients in managing their weight and preventing or improving the insidious complications resulting from untreated obesity.

Starting point of this study was the investigation, which was performed through focus groups, clinical interviews and a playful approach, on how children and parents perceive the aerosol therapy. After having identified critical issues and spontaneous proposals to ameliorate aerosol administration, the Authors evaluated the results obtained by the association of the traditional aerosol therapy with a device equipped with educational games. An experimental environment was set up in a school together with a procedure aiming at evaluating the children behavior towards aerosol therapy administered with and without the device equipped with educational games. The results of this monocentric, randomized, cross-over study demonstrated a statistically significant increase of the compliance index when aerosol therapy was administered with the device equipped with educational games, increasing interest, satisfaction and pleasure during the aerosol administration, which was accepted for a longer time.

In this article, through a critical reading of the published diaries written by gay ‘patients’ who received aversion therapy in south China in the 1990s, I examine how the transformation of subjectivities from gay to straight was made possible by such ‘self-technologizing’ practices as writing and communication. I also consider the centrality of the body and affect in the process of subject (trans)formation, and ask how a new, coherent and authentic ‘self’ was fabricated through bodily and affective experiences. This discussion not only reveals the social construction of the self as central to China’s postsocialist governmentality, but also the central role that gender and sexuality play in processes of self-formation.

In Russia, as in the United States, there has been such a whole-hearted acceptance of the medical model in regard to deviance in general and substance abuse in particular that it can be categorized as a Kuhnian paradigm. A Russian deviant is labeled as either sick, crazy, or a parasitic counter-revolutionary. In the United States, the deviant can be either sick, mentally ill, or a politically-active ethnic or minority group member. The difference in treatment varies only slightly with the political system. Yet in both the United States and Russia the various medical treatments have been generally found to be unproductive in reducing recidivism. And, just as in the United States, the Russians have developed their own aversion therapy, which to a certain extent is based on the moralistic belief that a “true” Soviet man or woman should not drink to excess, take drugs, or hunt the rare Amanita mushroom. There is a difference, however, in the approaches of the two countries: in the United States, with some exceptions, aversion therapy for substance abuse requires the prior permission of the patient. In Russia, it is often administered by the security police as part of the campaign against “parasites” and “hooligans.” Offenders who fail to respond to medical treatment may be sent to a mental institution, or-if truly unresponsive-may be deported to work on the state farms in frontier provinces. In the United States, they are often merely returned to the streets.

Food Aversion (FA) is a strong refusing behaviour to the oral assumption of food that can affect children with Short Bowel Syndrome (SBS). Management includes behavioural and Messy Play treatments, with few reports on systematic strategies to return the patient to enjoyable eating. We conducted a systematic review to better understand this complex and vital issue. (1) Materials and Methods: We investigated publications using MEDLINE, Embase, and the Web of Science to include articles published up to July 2022. The inclusion criteria were original articles including paediatric patients (aged < 18 years old) affected by SBS and Intestinal Failure (IF) who underwent treatment for FA. (2) Results: A total of 24 patients received treatment—15 (62.5%) patients were male and 9 (37.5%) were female. The age range was from 1 month to 16 years. Treatment of FA was carried out by behavioural therapy in 2 patients and Messy Play Therapy in 12 patients already surgically and pharmacologically managed for SBS. The treatment results showed complete weaning from Parenteral Nutrition in 9/14 cases (64%) using the behavioural treatment and 7/12 cases using Messy Play Therapy. (3) Conclusions: FA is a rare but disabling condition that often affects SBS patients, worsening their overall health and quality of life. This condition should be addressed in an Intestinal Rehabilitation Centre context. Our review sheds light on the literature gap regarding FA, and further studies are required to understand better which treatment options best suit SBS paediatric patients.

ABSTRACTBackgroundIn chronically ill children who refuse to eat, surgery to correct anatomic problems and behavioral treatments to overcome oral aversion often succeed. A few patients fail with standard treatments. The aims of the study were to: 1) investigate motility and gastric sensory abnormalities and 2) describe treatment that was individualized based on pathophysiology in children who failed surgery and behavioral treatments.MethodsWe studied 14 patients (age 1.5–6; mean 2.5; M/F: 7/7). All had a lifelong history of food aversion and retching or vomiting persisting after feeding therapy and fundoplication. All were fed through gastrostomy or gastro‐jejunostomy tubes. We studied esophageal and antroduodenal manometry, and gastric volume threshold for retching. We identified when gastric antral contractions were associated with retching and pain. A multidisciplinary treatment program included a variable combination of continuous post‐pyloric feedings, drugs to decrease visceral pain, drugs for motility disorders, and behavioral, cognitive, and family therapy. We interviewed parents 2–6 months following testing to evaluate symptoms, mode of feeding and emotional health.ResultsWe found a motility disorder alone in 2, decreased threshold for retching alone in 5 and both motility and sensory abnormalities in 7. After treatment, 6 of 14 (43%) began eating orally and 80% had improved emotional health. Retching decreased from 15 episodes per day to an average of 1.4 per day (p <0.01).ConclusionsUpper gastrointestinal motor and/or sensory disorders contributed to reduced quality of life for a majority of children and families with persistent feeding problems. A multidisciplinary approach improved symptoms and problems in these children

Abstract This perspective piece gives an overview of the current situation of yoga therapy in Japan today. Traditional yoga in Japan suffered a serious setback in 1995 with a nerve gas terrorist attack on the Tokyo subway, which was carried out by a cult that recruited members through yoga classes. But with the increase in popularity with modern forms of yoga such as Iyengar yoga, Ashtanga yoga and hot yoga in the West, the general public in Japan today is forgetting its aversion to yoga and considers it to be something that can contribute to good health. In 2012, the Japan Yoga Therapy Society (JYTS) conducted a study on adverse events in yoga classes throughout Japan with the University of Kyushu School of Medicine, with support from the Ministry of Health, Labour and Welfare. This study indicated that more than half of people attending yoga classes have some form of chronic illness, with 42.3% receiving outpatient care. This survey was the beginning of growing interest from both the government and universities in yoga therapy. JYTS is beginning to make inroads into bringing yoga therapy into cancer and palliative care, senior citizen homes, alcohol and drug addiction rehabilitation, cardiovascular rehabilitation, and research on trauma and schizophrenia. While there are still limited opportunities for yoga therapists to work in mainstream healthcare services, there is growing interest among medical professionals in both physical and mental health. JYTS is beginning to make inroads into bringing yoga therapy into cancer and palliative care, senior citizen homes, alcohol and drug addiction rehabilitation, cardiovascular rehabilitation, and research on trauma and schizophrenia. Cognitive therapy was administered for the first experimental group and the second experimental group receives exposure therapy while the control group received no treatment. Subscales Madsly questionnaire was usedfor data gathering for OCD. For data analysis, multivariate analysis of covariance (MANCOVA) and least significant difference test to compare scores differences between pretest - posttest variables in the experimental and control groups was used. Findings showed that Cognitive and Aversion Therapy on the control group has a significant impact on the improvement of obsession, check out, washing, slowness and obsessive doubts. The results showed that Whittal Cognitive Therapy more impact on the reduction of obsessions in comparsion with Exposure therapy.

Median arcuate ligament syndrome (MALS), also known as celiac artery compression syndrome, is a rare gastrointestinal condition with an estimated incidence of 2 per 100 000 population. Predominantly in female patients, this syndrome is characterized by the compression of the celiac artery at its origin from the aorta by the median arcuate ligament, which at the same time is entrapping the celiac plexus, causing upper abdominal pain, notably postprandial pain, as well as nausea, vomiting, food aversion, and weight loss. We present a case of abdominal pain secondary to MALS that was appropriately diagnosed after requiring narcotic medication, which responded to surgical therapy.

Supervised administration of disulfiram is one of the very few treatments that can significantly reduce alcohol consumption, yet it is rarely used, despite being simple and cheap. Disulfiram is not ‘aversion therapy‘, but a technique for facilitating exposure and response prevention, and it fits comfortably into a behavioural, social-learning model of addiction (or dependence) and its treatment. The availability of other effective antagonists, notably to opiates, suggests that an approach that is demonstrably effective in alcohol dependence may be helpful in treating other addictions. The reasons for the unpopularity of this approach appear to be largely ideological.

This article relies on psychoanalytic theory and technique to examine the dynamics of a couple with a sexual aversion disorder in one partner and to provide treatment. It uses the language of object relations, including Ogden’s (1989a,b) reformulation of Freud’s notions about the oedipal complex. In doing so, it defies the popular trend in sex therapy to employ behavioural methods, as well as the less commonly used approach to integrate psychoanalytic with behavioural techniques. The outcome supports the prospect of psychoanalytically oriented therapists reclaiming sexual difficulties as entities that are within the domain of their clinical expertise. Additionally, it adds to the literature on sexual desire disorders and because the partner, in this case, who presents with the sexual problem is male, it counters commonly held myths about male sexuality and recognises the complexity of their sexual functioning.

During the past decade, the field of human–animal interaction(s) research has been characterized by a significant increase in scientific findings. These data have contributed to our current understanding of how humans may benefit from contact with animals. However, the animal experience of these interactions is still an under-researched area. This paper addresses the welfare of dogs who participate in animal-assisted interventions (AAIs) to improve health in human recipients. This paper builds on previous work by Glenk (2017) and provides an updated review of the literature on therapy dog welfare published from 2017–2021. New advances in scientific methodology, such as the determination of salivary oxytocin, breath rate and tympanic membrane temperature, are analyzed regarding their value and limitations for research in AAIs. Moreover, welfare-related social and environmental factors (e.g., freedom of choice, exploration of novel environments, inequity aversion, individual development, working experience, relationship with handler and handler skills) that profoundly influence dog perception and well-being are reviewed and discussed. Accounting for the globally increasing interest and the number of dogs utilized in AAIs, safeguarding therapy dog well-being, and identifying situations, circumstances and protocols that may challenge animal welfare remains an emerging and crucial area of scientific effort.

Long-standing, overt hypothyroidism-induced bilateral multiloculated ovarian cysts represent an infrequent occurrence. Our first case, presented with bilateral complex ovarian masses, exhibited overt hypothyroidism symptoms, including lethargy, weight gain and subfertility, prompting consideration for surgical intervention. Similarly, in the second case, a girl aged 11 years with stunting, delayed bone age and academic challenges was referred for surgical exploration due to bilateral complex ovarian masses. Both cases revealed elevated thyroid-stimulating hormone levels during preoperative workup. Commencing levothyroxine replacement therapy resulted in complete regression of ovarian cysts and substantial symptom improvement within an 8-week timeframe. The third case, a previously diagnosed patient with Hashimoto’s thyroiditis, benefited from the lessons gleaned in managing the initial cases, responding well to levothyroxine therapy, thereby averting the necessity for surgery in all three instances. These cases underscore the significance of considering thyroid function in the evaluation of ovarian masses and highlight the efficacy of levothyroxine replacement therapy in resolving both hypothyroidism and associated ovarian cysts, thereby obviating the need for surgical intervention.

Ambiguous data on both terminology, diagnostics, and treatment of testosterone deficiency in men prompted us to attempt a critical analysis of existing knowledge on this subject. Current guidelines of both American and European Association of Urology (AUA, EUA) define testosterone therapy as effective and safe. However, media reports and some scientific reports indicating negative effects of the abovementioned therapy arouse aversion to its use by doctors and potential patients for fear of developing prostate cancer or cardiovascular incidents. The peak of scepticism about testosterone therapy was observed after the publication in 2013 and 2014, respectively, two retrospective data analysis on this topic, which resulted in the discontinuation of therapy in many patients with long histories of benefits from testosterone therapy. In addition, in many men with indications for testosterone therapy, this treatment was not used for fear of patient safety. However, the latest data on these concerns do not confirm any negative effects. More than 100 recently published scientific studies have shown the beneficial effects of testosterone therapy on many aspects of health. The American Society of Clinical Endocrinologists (AACE) and the American College of Endocrinology (ACE) have jointly developed their own literature assessment, stating that there is no convincing evidence that testosterone therapy increases the risk of cardiovascular incidents. The same conclusions can be drawn from the current EAU and AUA guidelines.

Previous (mainly classical) conditioning and current (mainly operant) approaches to the treatment of gambling addictions are reviewed in the light of recent evidence of the central importance of heightened arousal in the experience of normal gambling. Within a general framework which views the central features of addiction as the phenomenology of arousal and the acquisition of altered states of consciousness as goals, the possible contributions of reversal theory are explored. A more detailed examination of operant and classical conditioning analyses of the maintenance and reinstatement of excessive gambling behaviour appears to point towards a return to interventions based upon classical conditioning theory, although to cue exposure and the extinction of “peak experiences” rather than to aversion therapy.
