First, the effects of CCK-8 and proglumide upon passive avoidance behavior was investigated. Operant conditioning interventions for clinical pain involve the modification of environmental and social stimuli that are (1) antecedents (discriminative stimuli or setting events) to maladaptive pain behaviour or (2) consequent stimuli that maintain the maladaptive pain behaviour through positive reinforcement (social attention or assistance from others) or negative reinforcement (escape or avoidance of uncomfortable, effortful activity, or of social, academic, or work responsibilities). While aspects of operant conditioning theory have been included in the design of interventions to manage acute paediatric medical procedure-related pain (see Slifer et al., 1995 , 2002 , 2011 ), this chapter will not include these studies but will focus on operant conditioning based interventions for illness-and injury-related chronic or recurrent pain.

The role of conditioning in the development and maintenance of fear and avoidance is well-researched and well-established. Learning theory and conditioning principals form the basis of various behavioral therapies. Indeed, even the process of nociception is susceptible to conditioning. Pharmacological therapy is not likely to be effective and may, in fact, be detrimental to the patient. There are a variety of behavioral strategies that can be implemented in a primary care setting as well as in a pain clinician. Utilizing the expertise of a behavioral therapist may be ideal, but most interested clinicians can acquire a basic understanding of the principle and techniques. There are a variety of educational materials suitable for patient use that can be incorporated into treatment. Functional restoration should be considered as a primary outcome, perhaps even more so, than a reduction of subjective pain rating.

Post-Traumatic Stress Disorder (PTSD) is characterized by substantial physiological and/or psychological distress following exposure to trauma. Intrusive fear memories often lead to persistent avoidance of stimuli associated with the trauma, detachment from others, irritability and sleep disturbances. Different key structures in the brain are involved with fear conditioning, fear extinction and coping. The limbic system, namely, the amygdala complex in close relationship with the hippocampal hub and the prefrontal cortex play central roles in the integration and in coping with fear memories. Serotonin acting both as a neurotransmitter and as a neurohormone participates in regulating the normal and pathological activity of these anatomic structures. We review the literature analyzing how the different actors of the serotoninergic system (5-HT receptors, transporters and anabolic and catabolic pathways) may be involved in regulating the sensitivity to highly stressful events and hopefully coping with them.
