Abstract:
ABSTRACT Background The importance of continuity of care in training is widely recognized; however, a broad-spectrum assessment across all specialties has not been performed. Objective We assessed the continuity of care provided by trainees, following patient consultations in the emergency department (ED) across all specialties at a large pediatric tertiary care center. Methods Medical records were reviewed to identify patients seen in consultation by a resident or fellow trainee in the ED over a 1-year period, and to determine if the patient followed up with the same trainee for the same condition during the next 6 months. Results Resident and fellow trainees from 33 specialties participated in 3400 ED consultations. Approximately 50% (1718 of 3400) of the patients seen in consultation by a trainee in the ED followed up with the same specialty within 6 months, but only 4.1% (70 of 1718) followed up with the same trainee for the same condition. Trainee continuity of care ranged from 0% to 21% among specialties, where specialties with resident clinics (14.4%) have a greater continuity of care than specialties without resident clinics (2.7%, P < .001). Continuity of care did not differ between fellows (4.2%) and residents (4.0%, P = .87), but did differ between postgraduate years for residents (P < .001). Conclusions Trainee continuity of care for ED consultations was low across all specialties and levels of training. If continuity of care is important for patient well-being and trainee education, efforts to improve continuity for trainees must be undertaken.

Abstract:
ABSTRACT Background Just-in-time (JIT) training refers to education occurring immediately prior to clinical encounters. An in situ JIT room in a pediatric emergency department (ED) was created for procedural education. Objective We examined trainee self-reported JIT room use, its impact on trainee self-perception of procedural competence/confidence, and the effect its usage has on the need for intervention by supervising physicians during procedures. Methods Cross-sectional survey study of a convenience sample of residents rotating through the ED and supervising pediatric emergency medicine physicians. Outcomes included JIT room use, trainee procedural confidence, and frequency of supervisor intervention during procedures. Results Thirty-one of 32 supervising physicians (97%) and 122 of 186 residents (66%) completed the survey, with 71% of trainees reporting improved confidence, and 68% reporting improved procedural skills (P < .05, +1.4-point average skills improvement on a 5-point Likert scale). Trainees perceived no difference among supervising physicians intervening in procedures with or without JIT room use (P = .30, paired difference −0.0 points). Nearly all supervisors reported improved trainee procedural confidence, and 77% reported improved trainee procedural skills after JIT room use (P < .05, paired difference +1.8 points); 58% of supervisors stated they intervene in procedures without trainee JIT room use, compared with 42% with JIT room use (P < .05, paired difference −0.4 points). Conclusions Use of the JIT room led to improved trainee confidence and supervisor reports of less procedural intervention. Although it carries financial and time costs, an in situ JIT room may be important for convenient JIT training.

Abstract:
<b><i>Introduction:</i></b> Numerous factors such as endothelial disease and hormonal disorder cause the development of erectile dysfunction (ED). However, the relationship between vitamin D deficiency (VDD) and ED is unclear. Moreover, the benefit of vitamin D replacement on ED patients with VDD is uncertain. As far as we know, there is no study yet in the literature regarding the addition of vitamin D to phosphodiesterase type 5 inhibitors in the treatment of ED patients with VDD. In this study, we investigated whether adding vitamin D to daily tadalafil treatment would be beneficial in ED patients with VDD. <b><i>Methods:</i></b> A total of 111 patients with VDD accompanying ED were retrospectively evaluated between January 2016 and December 2019. Patients were divided into 2 groups according to the treatment they received. Group 1 (<i>n</i> = 58) was treated with daily oral tadalafil 5 mg, while group 2 (<i>n</i> = 53) received oral tadalafil 5 mg and 4,000 IU vitamin D3. Total International Index of Erectile Function-15 (IIEF-15) scores and vitamin D levels of the groups were compared at the end of the study. <b><i>Results:</i></b> The mean vitamin D level was increased statistically significant in group 2, but no difference was seen in group 1 (<i>p</i> < 0.001 and <i>p</i> > 0.05, respectively). There was a significant increase in median erectile function, orgasmic function, sexual desire, sexual satisfaction, and overall satisfaction scores in both groups (<i>p</i> < 0.001). However, the increase in median erectile function and sexual desire scores was significantly higher in group 2 compared to group 1 at the end of the study (<i>p</i> = 0.01 and <i>p</i> < 0.001, respectively). <b><i>Conclusion:</i></b> We found that adding vitamin D to 5 mg oral daily tadalafil treatment may have an additional positive effect on erectile function and sexual desire in ED patients with VDD.

Abstract:
OBJECTIVES/SPECIFIC AIMS: Translational researchers often require the use of informatics methods in their work. Lack of an understanding of key informatics principles and methods limits the abilities of translational researchers to successfully implement Findable, Accessible, Interoperable, Reusable (FAIR) principles in grant proposal submissions and performed studies. In this study we describe our work in addressing this limitation in the workforce by developing a competency-based, modular course in informatics to meet the needs of diverse translational researchers. METHODS/STUDY POPULATION: We established a Translational Research Informatics Education Collaborative (TRIEC) consisting of faculty at the University of Utah (UU) with different primary expertise in informatics methods, and working in different tiers of the translational spectrum. The TRIEC, in collaboration with the Foundation of Workforce Development of the Utah Center for Clinical and Translational Science (CCTS), gathered informatics needs of early investigators by consolidating requests for informatics services, assistance provided in grant writing, and consultations. We then reviewed existing courses and literature for informatics courses that focused on clinical and translational researchers [3–9]. Using the structure and content of the identified courses, we developed an initial draft of a syllabus for a Translational Research Informatics (TRI) course which included key informatics topics to be covered and learning activities, and iteratively refined it through discussions. The course was approved by the UU Department of Biomedical Informatics, UU Graduate School and the CCTS. RESULTS/ANTICIPATED RESULTS: The TRI course introduces informatics PhD students, clinicians, and public health practitioners who have a demonstrated interest in research, to fundamental principles and tools of informatics. At the completion of the course, students will be able to describe and identify informatics tools and methods relevant to translational research and demonstrate inter-professional collaboration in the development of a research proposal addressing a relevant translational science question that utilizes the state-of-the-art in informatics. TRI covers a diverse set of informatics content presented as modules: genomics and bioinformatics, electronic health records, exposomics, microbiomics, molecular methods, data integration and fusion, metadata management, semantics, software architectures, mobile computing, sensors, recruitment, community engagement, secure computing environments, data mining, machine learning, deep learning, artificial intelligence and data science, open source informatics tools and platforms, research reproducibility, and uncertainty quantification. The teaching methods for TRI include (1) modular didactic learning consisting of presentations and readings and face-to-face discussions of the content, (2) student presentations of informatics literature relevant to their final project, and (3) a final project consisting of the development, critique and chalk talk and formal presentations of informatics methods and/or aims of an National Institutes of Health style K or R grant proposal. For (3), the student presents their translational research proposal concept at the beginning of the course, and works with members of the TRIEC with corresponding expertise. The final course grade is a combination of the final project, paper presentations and class participation. We offered TRI to a first cohort of students in the Fall semester of 2018. DISCUSSION/SIGNIFICANCE OF IMPACT: Translational research informatics is a sub-domain of biomedical informatics that applies and develops informatics theory and methods for translational research. TRI covers a diverse set of informatics topics that are applicable across the translational spectrum. It covers both didactic material and hands-on experience in using the material in grant proposals and research studies. TRI’s course content, teaching methodology and learning activities enable students to initially learn factual informatics knowledge and skills for translational research correspond to the ‘Remember, Understand, and Apply’ levels of the Bloom’s taxonomy [10]. The final project provides opportunity for applying these informatics concepts corresponding to the ‘Analyze, Evaluate, and Create’ levels of the Bloom’s taxonomy [10]. This inter-professional, competency-based, modular course will develop an informatics-enabled workforce trained in using state-of-the-art informatics solutions, increasing the effectiveness of translational science and precision medicine, and promoting FAIR principles in research data management and processes. Future work includes opening the course to all Clinical and Translational Science Award hubs and publishing the course material as a reference book. While student evaluations for the first cohort will be available end of the semester, true evaluation of TRI will be the number of trainees taking the course and successful grant proposal submissions. Abstract:
Four glycoproteins (gD, gB, gH, and gL) are required for herpes simplex virus (HSV) entry into the cell and for cell-cell fusion in transfected cells. gD serves as the receptor-binding glycoprotein and as the trigger of fusion; the other three glycoproteins execute fusion between the viral envelope and the plasma or endocytic membranes. Little is known on the interaction of gD with gB, gH, and gL. Here, the interactions between herpes simplex virus gD and its nectin1 receptor or between gD, gB, and gH were analyzed by complementation of the N and C portions of split enhanced green fluorescent protein (EGFP) fused to the glycoproteins. Split EGFP complementation was detected between proteins designated gDN + gHC, gDN + gBC, and gHN + gBC + wtgD, both in cells transfected with two or tree glycoproteins and in cells transfected with the four glycoproteins, commited to form syncytia. The in situ assay provides evidence that gD interacts with gH and gB independently one of the other. We further document the interaction between gH and gB. To elucidate which portions of the glycoproteins interact with each other we generated mutants of gD and gB. gD triggers fusion through a specialised domain, named pro-fusion domain (PFD), located C-terminally in the ectodomain. Here, we show that PFD is made of subdomains 1 and 2 (amino acids 260–285 and 285–310) and that each one partially contributed to herpes simplex virus infectivity. Chimeric gB molecules composed of HSV and human herpesvirus 8 (HHV8) sequences failed to reach the cell surface and to complement a gB defective virus. By means of pull down experiments we analyzed the interactions of HSV-HHV8 gB chimeras with gH or gD fused to the strep-tag. The gB sequence between aa residues 219-360 was identified as putative region of interaction with gH or critical to the interaction.

Pisani, Federico. "Knowledge workers management. Concorrenza e invenzioni nel rapporto di lavoro subordinato: il modello statunitense." Doctoral thesis, Università degli studi di Padova, 2019. http://hdl.handle.net/11577/3425914.

Abstract:
This work addresses the issues of competition and inventions in the U.S. employment relationships. The research was carried out in part at the Boston University School of Law of, under the supervision of Micheal C. Harper, professor of Labour Law. The selection of the topic is justified in the light of its importance, given that in the new production organization, based largely on globalized knowledge, employees are now increasingly being asked for professionalism, innovation and creativity. The decision to examine this issue from the perspective of the "U.S. laboratory" is due to the primacy that this nation holds at international level on the economic, scientific and innovation of work processes, which bring out critical issues that in other Countries probably have not yet been raised. In order to frame the above-mentioned topics, it has become appropriate to give an account of the system of regulatory sources in the USA, with particular focus on the Restatement of Employment Law, i.e. the collection of fundamental principles developed over the years by common law in the field of employment relationships. The examination of the sources is followed by the definition of the concept of employee and self-employed worker (independent contractor), necessary for the assessment of the application of the obligations arising from the employment relationships, including the duty of loyalty, involved in the fiduciary law. In this context, the evolution of the case law has been observed, as well as the examination of the criteria relating to the distinction between employees and independent contractors, mainly concerning the judgement on the relevance of the factual elements determining the assessment of the existence of an employment relationship. Subsequently, this study addresses the issue of the typical form of the U.S. employment contract, the so-called employment-at-will. This peculiarity is originated from the principle that the parties are not bound by any obligation to provide reasons for termination. The third part of the work has as its object the discipline of competition of the worker carried out on the basis of the knowledge acquired, legally or illegally, during the relationship and the relative legal remedies for the employer, against the violation of the duty of loyalty, intended as an obligation of the employee to perform the work in the exclusive interest of the entrepreneur and, consequently, to refrain from engaging in prejudicial conduct against the company. About the remedies available in the event of breach of the obligations examined, the legal and equitable remedies that U.S. law offers the employer have been explained. The final part of this study deals with the rules governing the ownership of rights arising from inventions developed by employees in the course of their employment. The definitions of "invention" and "patent" and their relationship in the context of employment law has been examined and the difference between invention as a work of genius and intellectual property protected by copyright has been highlighted. In addition, the mechanisms underlying the basic rules governing the subject matter and their coexistence with the contractual freedom of the parties and their power to dispose of these rights have been observed.

Abstract:
Shou-Fei Zhu of Nankai University developed (Angew. Chem. Int. Ed. 2014, 53, 13188) an iron catalyst that effected the enantioselective cyclization of 1 to 2. Bypassing diazo precursors, Junliang Zhang of East China Normal University used (Angew. Chem. Int. Ed. 2014, 53, 13751) a gold catalyst to cyclize 3 to 4. Taking advantage of energy transfer from a catalytic Ir complex, Chuo Chen of University of Texas Southwestern carried out (Science 2014, 346, 219) intramolecular 2+2 cycloaddition of 5, leading, after dithiane formation, to the cyclobutane 6. Intramolecular ketene cycloaddition has been limited in scope. Liming Zhang of the University of California Santa Barbara found (Angew. Chem. Int. Ed. 2014, 53, 9572) that intramolecular oxidation of an intermediate Ru vinylidene led to a species that cyclized to the cyclobutanone 8. James D. White of Oregon State University devised (J. Am. Chem. Soc. 2014, 136, 13578) an iron catalyst that mediated the enantioselective Conia-ene cyclization of 9 to 10. Xiaoming Feng of Sichuan University observed (Angew. Chem. Int. Ed. 2014, 53, 11579) that the Ni-catalyzed Claisen rearrangement of 11 proceeded with high diastereo- and enantiocontrol. The relative configuration of the product 12 was not reported. Robert H. Grubbs of Caltech showed (J. Am. Chem. Soc. 2014, 136, 13029) that ring opening cross metathesis of 13 with 14 delivered the Z product 15. Mn(III) cyclization has in the past required a stoichiometric amount of inorganic oxidant. Sangho Koo of Myong Ji University found (Adv. Synth. Catal. 2014, 356, 3059) that by adding a Co co- catalyst, air could serve as the stoichiometric oxidant. Indeed, 16 could be cyclized to 17 using inexpensive Mn(II). Matthias Beller of the Leibniz-Institüt für Katalyse prepared (Angew. Chem. Int. Ed. 2014, 53, 13049) the cyclohexene 20 by coupling the racemic alcohol 18 with the amine 19. Paultheo von Zezschwitz of Philipps-Universität Marburg added (Chem. Commun. 2014, 50, 15897) diethyl zinc in a conjugate sense to 21, then reduced the product to give 22. Depending on the reduction method, either diastereomer of the product could be made dominant. Nuno Maulide of the University of Vienna displaced (Angew. Chem. Int. Ed. 2014, 53, 7068) the racemic chloride 23 with diethyl zinc to give 24 as a single diastereomer.

Since the early 1980s, aldol condensations involving boron enolates have gain great importance in asymmetric synthesis, particularly the synthesis of natural products with adjacent stereogenic centers bearing hydroxyl and methyl groups. (Z)-Boron enolates tend to give a high diastereoslectivity preference for the syn-stereochemistry while (E)-boron enolates favor the anti-stereochemistry. Because the B-O and B-C bonds are shorter than other metals with oxygen and carbon, the six membered Zimmerman–Traxler transition state in the aldol condensation tends to be more compact which accentuates steric interactions, thus leading to higher diastereoselectivity. When this feature is coupled with a boron enolate bearing a chiral auxillary, high enantioselectivity is achieved. Boron enolates are generated from a ketone and boron triflate in the presence of an organic base such as triethylamine. Reviews: (a) Abiko, A. Acc. Chem. Res. 2004, 37, 387-395. (b) Cowden, C. J. Abstract:
Richard J. K. Taylor of the University of York has developed (Angew. Chem. Int. Ed. 2008, 47, 1935) the diasteroselective intramolecular Michael cyclization of phosphonates such as 2. Quenching of the cyclized product with paraformaldehyde delivered ( + )-Paeonilactone B 3. Roberto Fernández de la Pradilla of the CSIC, Madrid established (Tetrahedron Lett. 2008, 49, 4167) the diastereoselective intramolecular hetero Michael addition of alcohols to enantiomerically-pure acyclic sulfoxides such as 4 to give the allylic sulfoxide 5. Mislow-Evans rearrangement converted 5 into 6, the enantiomerically-pure core of Ethyl Deoxymonate B 7. The ellagitannins, represented by 10, are single atropisomers around the biphenyl linkage. David R. Spring of the University of Cambridge found (Organic Lett. 2008, 10, 2593) that the chiral constraint of the carbohydrate backbone of 9 directed the absolute sense of the oxidative coupling of the mixed cuprate derived from 9, leading to Sanguiin H-5 10 with high diastereomeric control. A key challenge in the synthesis of the solandelactones, exemplified by 14, is the stereocontrolled construction of the unsaturated eight-membered ring lactone. James D. White of Oregon State University found (J. Org. Chem. 2008, 73, 4139) an elegant solution to this problem, by exposure of the cyclic carbonate 11 to the Petasis reagent, to give 12. Subsequent Claisen rearrangement delivered the eight-membered ring lactone, at the same time installing the ring alkene of Solandelactone E 14. AD-mix usually proceeds with only modest enantiocontrol with terminal alkenes. None the less, Ian Paterson, also of the University of Cambridge, observed (Angew. Chem. Int. Ed. 2008, 47, 3016, Angew. Chem. Int. Ed. 2008, 47, 3021) that bis-dihydroxylation of the diene 17 proceeded to give, after acid-mediated cyclization, the bis-spiro ketal core 18 of Spirastrellolide A Methyl Ester 19 with high diastereocontrol.

Jin K. Cha of Wayne State University described (J. Org. Chem. 2009, 74, 5528) the diastereoselective intramolecular cyclopropanation of nitriles with homoallylic alcohols such as 1 . Valery V. Fokin of Scripps/La Jolla found (J. Am. Chem. Soc. 2009, 131, 18034) that the diazoimine derived from 4 could add with high enantioselectivity to aryl alkenes, including styrene 5. Andreas Gansäuer of the University of Bonn optimized (Angew. Chem. Int. Ed. 2009, 48 , 8882; Tetrahedron 2009, 65, 10791) the Ti catalyst to enable efficient cyclization of substrates such as 7 to the corresponding cyclobutanes. F. Dean Toste of the University of California, Berkeley, devised (J. Am. Chem. Soc. 2009, 131, 9178) a gold catalyst for the enantioselective ring expansion of a prochiral allene such as 9 to the cyclobutanone 10. David J. Procter of the University of Manchester developed (J. Am. Chem. Soc. 2009, 131, 15467) the SmI2 -mediated cyclization of a lactone such as 11 to the cyclopentanone 12. Shigeki Matsunaga and Masakatsu Shibasaki of the University of Tokyo designed (Chem. Commun. 2009, 5138) a Ni catalyst for the enantioselective condensation of 13 with formaldehyde. Some acyclic β-keto esters could also be hydroxymethylated with high enantiocontrol. Darren J. Dixon, also of the University of Manchester, devised (J. Am. Chem. Soc. 2009, 131, 9140) a Cu catalyst for the enantioselective Conia cyclization of 15 to 16 . K. C. Nicolaou, also of Scripps/La Jolla, reported (Angew. Chem. Int. Ed. 2009, 48, 6293) a Rh catalyst for the related cyclization of 17 to 18. Ryo Shintani and Tamio Hayashi of Kyoto University showed (J. Am. Chem. Soc. 2009, 131, 13588) that a Rh catalyst could effect enantioselective conjugate addition of a tetraaryl borate even to a 3-methyl cyclohexenone 19, to establish the cyclic quaternary center. Alexandre Alexakis of the University of Geneva established (Chem. Commun . 2009, 3868) that with the easily ionized allylic bromide 21, Cu-mediated coupling with the alkyl Grignard 22 proceeded with substantial asymmetric induction. Jon D. Rainier of the University of Utah devised (Organic Lett. 2009, 11, 38774) conditions for effecting Ti-mediated intramolecular metathesis between an alkene and a lactam carbonyl, giving the cyclic enamide 24.

Abstract:
Intramolecular cyclopropanation has long been a powerful strategy for stereoselective polycarbocyclic construction. For many years, this was limited to diazo carbonyl insertion. More recently, alternative strategies for intramolecular cyclopropane construction have been developed. The power of such an approach is illustrated by the total synthesis of echinopine B 3 reported (Angew. Chem. Int. Ed. 2012, 51, 7572) by Christopher D. Vanderwal of the University of California, Irvine. The starting material for the synthesis was the commercial keto ester 4. Enol ether formation followed by cyclopropanation set the stage for fragmentation to the enone 7. The cyclopentannulation reagent 9 was prepared in two steps from butynol 8. Following the precedent of Heathcock (J. Am. Chem. Soc. 1983, 105, 2354), conjugate addition proceeded with significant diastereocontrol to give the ketone 10. Exposure of the derived tosylate to less than one equivalent of lithium diisopropylamide gave clean cyclization to the bicyclic ketone 11. It is a measure of the brevity of this approach that the cyclization of 10 to 11 was reported on a 5-gram scale. Relatively little is known about conformational preference around seven-membered rings. Methylenation of 12 followed by reduction of the ester delivered the aldehyde 13. Epimerization of the aldehyde with methanolic K2CO3 gave a 4:1 mixture, with the new aldehyde dominating. Inclusion of the Ohira reagent 13 in the same pot with 11 gave the alkyne 15, also as an ~4:1 mixture of diastereomers. Alkylation with MOM-Cl then completed the assembly of 1. Warming of 1 with a catalytic amount of PtCl2 led to the cyclopropane 2. This reaction apparently proceeded through a series of steps leading to a Pt carbene that then underwent β-hydride elimination to give the enol ether. On oxidation, the enol ether was smoothly converted to the methyl ester, echinopine B 3 In this synthesis, the targeted natural product included a cyclopropane. Fused cyclopropanes are also versatile precursors to other polycyclic ring systems. Future developments in intramolecular cyclopropane construction will open efficient new strategies for polyalicyclic synthesis.

Abstract:
Abstract author: Virtually nothing is known about the author, although he locates himself on the Greek island of Lesbos. The plot of the novel conforms to the basic ancient Greek romance (see comments at entry 23). Although in most of these novels, the lovers are members of the elite classes, Daphnis and Chloe are simple shepherds living an idealized pastoral life. translation: Christopher Gill, in B. P. Reardon, ed., Collected Ancient Greek Novels (Berkeley: University of California Press, 1989). text: Budé (G. Dalmeyda, 1934); Teubner (M. D. Reeve, 1982). additional translation: Ronald McCail, Longus, “Daphnis and Chloe,” OWC (New York: Oxford University Press, 2002). additional text and translations: LCL (J. M. Edmond, 1916; rev. trans. by George Thornley); O. Schönberger, Longos, “Daphnis und Chloe” (Berlin, 1960; 2d ed., 1973). bibliography: See entry 23. 2.2 A few days later, the vines were harvested, the sweet new wine was in jars, and there was no longer any need for many hands. Daphnis and Chloe drove their flocks down to the plain and, in a happy mood, worshiped the Nymphs, bringing them bunches of grapes still on the shoots as firstfruits of the grape harvest. Not that they ever went past them and neglected them before; they always visited them when they went to pasture and worshiped them when they left the pasture and without fail brought them some kind of offering, a flower or a fruit or some green leaves or a libation of milk. For this, they received a reward from the goddesses later on.

Abstract:
Abstract The latest revision of NACE TM0177 standard allows notch-machining of Double Cantilever Beam (DCB) specimens for the determination of KISSC using Electro Discharge Machining (EDM-notch) and cutting tools (chevron notch) for the evaluation of Sulfide Stress Cracking (SSC) resistance of Low Alloy Carbon Steels. Chevron specimens are widely used and the obtained value has proved reliability; whereas utilization of EDM notch-specimens is more restricted. One of the reasons may be attributed to the fact that NACE TM 0177 does not specify tolerance for the height of the EDM notch (0.3 mm or 0.01 inch) and only gives a recommendation of a rounded or beveled for the slot end in order to avoid side-arm breakoff. The intention of this work was to study for which conditions non-precracked EDM notch-DCB specimens produced reproducible results and comparable to the ones obtained using chevron specimens; varying the machining parameters and also the configuration of the initial EDM notch in C110 grade. Since pre-cracking loads are considerable low for C110, the use of non-precracked EDM-notch specimens appears to be an attractive alternative for reducing testing time. Six EDM conditions were selected for the two configuration of EDM notch DCB specimens (one stated in NACE TM0177 standard and a second one for which the final EDM notch was line-up with the chevron base specimen). All EDM configurations as well as chevron specimens were tested at room temperature in SR39.3 solution saturated with 0.07 bar of H2S and in NACE A solution saturated with 1 bar of H2S. For the ED machining conditions used in this work, it was found that: a) the selection of inadequate ED parameters produced invalid tests due to lack of initiation; b) adequate reproducibility in mild and NACE A when notch height was between 0.26 and 0.32 mm, c) KISSC obtained with line-up EDM configuration was closer in mean value and dispersion than the ones obtained for the chevron in NACE A solution, d) for some ED machining conditions KISSC in SR39.3 was slightly more conservative than the one obtained with the chevron configuration.

Abstract:
IntroductionDigital assistant systems (DAS) can support tasks by mediating between complex data and users, promoting continuous learning and on-the-job training (Jwo et al., 2021; Longo et al., 2017; Prinz et al., 2017). Integrating a DAS into surgical instrument reprocessing, where instrument-specific and complex manual tasks need to be performed strictly according to the manufacturer's instructions, can be advantageous. In Germany, reprocessing by trained on-the-job personnel is common. In addition, human factors are often neglected in the instructions for instrument reprocessing (Choi et al., 2017). A cooperative robot can be a valuable addition to mitigate health risks associated with handling contaminated surgical instruments during reprocessing, resulting in a cyber-physical assistance system (Heibeyn et al., 2021). However, it is unclear how the transition from paper-based instructions to either a DAS or a DAS supplemented with a cooperative robot assistance (“cyber-physical assistance system” – CPAS) affects usability for complex and workpiece-specific tasks in instrument reprocessing. Therefore, this study aimed to investigate the differences in usability with different assistance systems for typical tasks in instrument reprocessing for untrained personnel.MethodsWe conducted an interaction-centered user study with 13 participants unfamiliar with the reprocessing tasks. The participants were asked to complete typical reprocessing tasks three times in a random order, using different assistance approaches each time. The reference process consisted of paper-based instructions that required stepping away from the workstation to retrieve information from the storage of guidelines, simulating common set-ups of current reprocessing processes. The first assistance approach presented digital instructions right at the workstation. The second assistant combined a cooperative robot with digital instructions. The robot performed simple processing steps, while the human operator focused on the complex tasks and verified the cleaning success. We measured the required time, counted user errors and rated the criticality, measured the perceived workload using the NASA-TLX questionnaire (NASA, 2020), and documented the remarks of the participants using the thinking-out-loud method for all assistance systems. Results The NASA-TLX did not reveal any significant differences among the three systems, however, the CPAS reduced the number of critical errors. The errors included omitted processing steps and deviations from required times, which could pose risks to patients. The DAS was perceived as a suitable way to check instructions, based on the participants’ comments. However, in the case of the CPAS, some participants missed or ignored messages provided by the user interface.Discussion and ConclusionIn summary, the CPAS improved usability the most, improving effectiveness (number of errors) while maintaining the same efficiency (total duration). Although our study found promising results for integrating a DAS or CPAS into on-the-job training assistance for novice personnel, future studies should compare the results obtained from inexperienced to those of experienced users to fully assess the usability of related approaches. This study contributes to the field of human factors by providing comparative data on usability across different levels of assistance for complex and workpiece-specific tasks in surgical instrument reprocessing.ReferencesChoi, J., Seraphina, S., &amp; Knudsen, K. Abstract:
Abstract:
Abstract:
Abstract:
The rock shelter of the Roc-aux-Sorciers at Angles-sur-lAnglin (Vienne, France) is one of the archaeological reference sites for the Upper Paleolithic. The sculpted, painted and engraved frieze was gradually brought to light in its archaeological context by Susanne Cassou de Saint-Mathurin and Dorothy Garrod from 1949 onwards (Saint-Mathurin, Garrod, 1950). A wealth of archaeological material was discovered alongside the parietal art, comprising numerous works of portable art, tools made of animal bone, jewellery, etc. It was very rapidly observed that the portable art and the parietal art shared the same graphic and thematic conventions. For example, images of female bodies are rendered in high relief, in a monumental way, but also in the form of small statuettes. Our research has demonstrated the undeniable intra-site links between the portable art and the parietal art (Pinon, 2012). Here we propose to broaden this analysis within a well-identified culture of the Middle Magdalenian known as the Magdalenian of Lussac-Angles spearpoints. At the time of the discovery of the Magdalenian site of La Marche in 1937 (Vienne) (Lwoff, Pricard, 1940), some similarities had been identified between this site and that of Le Roc-aux-Sorciers, where the discoveries dated back to 1927 (Rousseau, 1933). These similarities are also perceptible in shared techniques (Chehmana, Beyries, 2010), as well as in the production of objects in hard organic materials such as the Lussac-Angles spearpoints (Pinon, 1988), the jewellery in fossil mammoth ivory (Dujardin, Pinon, 2000), the engraved horse incisors (Mazire, 2009) and the figurative art (Bourdier et al., 2016 Fuentes, 2016). We propose to further explore the links between these two sites through the analysis of the dynamic processes of reworking images. In particular we examine the engraved plaquettes of La Marche and the parietal art of Le Roc-aux-Sorciers to bring these links into perspective. This could shed light on some common ways of seeing the world in this Magdalenian group. Bourdier, C., Pinon, G., Bosselin, B. (2016).
