Abstract:
Some of the women who, in their letters, claim to be fulfilled by motherhood and keeping house, display such a rude and caustic aggression that it casts doubt over the happy balance of which they boast. Others reproach me more moderately for seeing motherhood as a servitude; but without a doubt, in France today, it is one. I understand that one can choose it deliberately; I am aware of the joy that children can bring when they have been wanted. But for me, who did not wish to have any and who wanted above all to accomplish an oeuvre, I was lucky to not have any. I am not someone who wishes to impose my manner of living upon all women, since on the contrary, I am actively fighting for their freedom: freedom to choose motherhood, contraception, or abortion. The fanatics are those mothers who refuse to accept that someone might follow a path other than their own....

Wiersinga, W. Joost, and Tom van der Poll. The host response to infection in the critically ill. Oxford University Press, 2016. http://dx.doi.org/10.1093/med/9780199600830.003.0303.

Full text
Abstract:
Infection continues to be a leading cause of intensive care unit death. The host response to infection can be seen as a pattern recognition receptor (PRR)-mediated dysregulation of the immune system following pathogen invasion in which a careful balance between inflammatory and anti-inflammatory responses is vital. A measured and rapid response to microbial invasion is essential to health. The same immunological and coagulation systems that protect against localized infection can act to our disadvantage when these systems are activated systemically during generalized microbial infection. Toll-like receptors (TLR), the inflammasomes and other PRRs initiate the host response after recognition of pathogen-associated-molecular-patterns (PAMPs) or endogenous danger-associated-molecular-patterns (DAMPs). The systemic host response to infection will result in activation of coagulation, downregulation of physiological anticoagulant mechanisms, and inhibition of fibrinolysis. Further dissection of the role of host–pathogen interactions, the cytokine response, the coagulation cascade and their multidirectional interactions in sepsis should lead towards the development of new therapeutic approaches in the critically ill who are faced with infection.

Abstract:
President Xi’s “China Dream” is underpinned by the country’s expanding trade and aid initiatives. While many welcome such opportunities, Beijing’s assertive foreign policy has exacerbated tensions. Meanwhile, America’s rebalancing toward Asia is seen by China as a containment strategy. China’s response has been to launch its “One Belt, One Road” initiative to support infrastructure development along historic land and maritime routes across Asia and the Middle East to Europe. Beijing’s plans have been substantiated by large amounts of lending and a new multilateral institution. China is no longer willing to accept the United States as the dominant regional power, yet it is unclear whether America is prepared to surrender any portion of its primacy. It remains to be seen whether the disputes over the sovereignty of regional islands will lead to conflicts or a conciliatory process that will allow the region to remain stable and prosper.

Smith, Robert C., Stefan Leucht, and John M. Davis. Maximizing response to first-line antipsychotics in schizophrenia. Oxford University Press, 2018. http://dx.doi.org/10.1093/med/9780198828761.003.0003.

Full text
Abstract:
The choice of first-line antipsychotic treatment for patients with schizophrenia should balance considerations of differential efficacy of antipsychotics against the relative risk of different side effects. In terms of efficacy, recent meta-analyses have shown that antipsychotics are not equivalent in efficacy. Clozapine, amisulpride, olanzapine, and risperidone show small to moderate, but statistically significant, differences, indicating greater efficacy compared to a number of other antipsychotics on some primary efficacy outcome measures. Amisulpride and cariprazine have the strongest evidence for greater efficacy for treating negative symptoms relative to other antipsychotics. In terms of side effects, clozapine and olanzapine have among the highest weight gain potential and amisulpride has more effects on QTc prolongation and prolactin elevation than other commonly used antipsychotics. Adjunctive treatment with an antidepressant drug may improve response in patients with schizophrenia who also have severe depressive or negative symptoms. For a patient with an inadequate response to an adequate dose and duration of the initial antipsychotic choice, switching to another antipsychotic with a different receptor profile may improve response, although evidence is limited. There is little evidence to support using doses above the therapeutic range other than in exceptional circumstances.

Raju, Raghavan, and Irshad H. Chaudry. The host response to hypoxia in the critically ill. Oxford University Press, 2016. http://dx.doi.org/10.1093/med/9780199600830.003.0305.

Full text
Abstract:
The hypoxic response of the host is complex. While the oxygen-sensing intracellular machinery attempts to restore cellular homeostasis by augmenting respiration and blood flow, events such as severe haemorrhage lead to whole body hypoxia and decreased mitochondrial function. Immunological perturbations following severe haemorrhage may result in multiple organ dysfunction and sepsis, while impaired perfusion may lead to microvascular injury and local hypoxia. Trauma-haemorrhage or hypoxic exposure in animals causes a systemic inflammatory response, decreased antigen presentation by peritoneal macrophages, hypoxaemia and initiation of endoplasmic reticulum stress. In response, the protein level of the oxygen-sensing transcription factor, hypoxia inducible factor (HIF)-1 increases; this leads to the regulation of expression of a number of genes resulting in decreased mitochondrial ATP production, but enhanced glycolytic processes, thus shifting the energy balance. In addition, sustained tissue hypoxia leads to increased free radical production and cellular apoptosis. Though the initial host response to hypoxia may be protective, sustained hypoxia becomes detrimental to the tissues and the organism as a whole.
