Abstract:
As the Hispanic community becomes increasingly important in American politics there are competing views about whether they can be converted to the Republican Party. One perspective argues that Hispanics’ religion and traditional social values makes them natural constituents of the Republican Party. Alternatively, Hispanics are primarily concerned about issues promoting their well-being and topics such as moral values or religion are private. We use a novel approach to test whether traditional social values might attract Hispanic voters to the Republican Party. Using exit poll results for ballot propositions on moral issues from Arizona, Colorado, and Florida we find weak evidence that traditional values will convert Hispanics to the Republican Party. Instead, our results indicate that traditional social values issues reinforce the polarization between the two parties.

Abstract:
Cannabis is traversing an extraordinary journey from an illicit substance to a legal one, due in part to an unprecedented wave of bottom-up law reform through successful citizen ballot initiatives. Yet, even in states that have legalized recreational cannabis, there is substantial geographic variability in support of cannabis legalization. Geographic variability in voter support for cannabis legalization is impactful (e.g., county moratoriums/bans) yet poorly understood. This paper demonstrates the consequences of county-level population demographics, sociopolitical factors, and community differences in experience with criminalization of cannabis possession for understanding county-level variation in support of recreational cannabis law reform on (un)successful ballot measures in California (2010), Colorado (2012), Washington (2012), and Oregon (2014). OLS regression analyses of nearly 200 counties indicate that differences in racial and ethnic composition (% Black, Hispanic), political affiliation (% Republican), past criminalization, gender composition, and higher education level of residents all predict county-level variation in support for liberalization of cannabis law. Stronger Republican political leanings and/or higher percentages of Black or Hispanic residents were associated with reduced support, whereas higher education, male sex composition, and greater past criminalization were associated with increased support for cannabis legalization across counties. Religiosity and rurality were inconsequential as predictors of county-level voting patterns favoring recreational cannabis. The substantial geographic variability in voter support for cannabis legalization has significant implications for policy implementation and effectiveness.

Abstract:
ObjectiveTo assess the feasibility of collecting, examining and reporting observational, real-world evidence regarding the novel use of the GORE EXCLUDER Iliac Branch Endoprosthesis (IBE) in conjunction with the GORE VIABAHN VBX Balloon Expandable Endoprosthesis (IBE+VBX stent graft).DesignMulticentre retrospective cohort study.SettingFour real-world data sources were used: a national quality improvement registry, a statewide clinical research network, a regional quaternary health system and two tertiary academic medical centres.ParticipantsIn total, 30 patients with 37 IBE+VBX stent graft were identified. Of those, the mean age was 72±10.2 years and 90% were male. The cohort was 77% white, 10% black, 3% Hispanic and 10% other.Main outcome measuresOutcome measures included: proportion of percutaneous vs open surgical access, intensive care admission, intensive care unit (ICU) length-of-stay (LOS), total LOS, postoperative complications, discharge disposition and 30-day mortality.ResultsThe majority (89%) of cases were performed percutaneously, 5% required surgical exposure following failed percutaneous access and 6% required open surgical exposure outright. Nearly half (43%) required intensive care admission with a median ICU LOS of 1 day (range: 1–2). Median total LOS was 1 day (IQR: 1–2). There were zero postoperative myocardial infarctions, zero reported leg embolisations and no reported reinterventions. Access site complications were described in 1 of 28 patients, manifesting as a haematoma or pseudoaneurysm. Ultimately, 97% were discharged to home and one patient was discharged to a nursing home or rehabilitation facility. There were no 30-day perioperative deaths.ConclusionsThis project demonstrates the feasibility of identifying and integrating real-world evidence, as it pertains to an unapproved combination of endovascular devices (IBE+VBX stent graft), for short-term outcomes analysis. This new paradigm of evidence has potential to be used for device monitoring, submission to regulatory agencies, or consideration in indication expansions and approvals with further efforts to systematise data collection and transmission mechanisms.

Abstract:
Background: Acute Myocardial Infarction (AMI) care and outcomes disparities have been reported worldwide in women and racial-ethnic minorities. In the U.S., some have attributed disparities in Hispanics and other minorities to lower quality of services at hospitals where they seek care. Current information from hospitals with large Hispanic representation and equitable updated resources is needed. Methods: Retrospective observational study of 839 AMI patients discharged in 2013 from three Southern California Community Hospitals (A, B, C) with tertiary cardiac care level. Non-Hispanic Whites (NHW) and Hispanics (H) were the larger racial-ethnic groups (68.3%). Mortality, 30 day readmissions, medication/performance measures (PRx); Aspirin, Statins/anti-lipids, Beta-blockers, Angiotensin Converting Enzyme Inhibitors and Receptor Blockers for Left Ventricular Systolic dysfunction, <90 minute door-balloon time, and coronary percutaneous interventions and revascularization procedures were compared between Hospitals, NHW and H, and gender using Chi-squared tests (χ 2 ), Odds Ratios (OR) with 95% confidence intervals (CI), and Z tests for proportions -independent groups. Results: No significant differences in hospital, 30 day mortality, or PRx were observed between hospitals, NHW and H. Hospital C had 47.3% Hispanics, Hospitals A+B 14.6% (η 2 =.430, p<0.001). AMI performance measures exceeded 2013 national rates across all facilities. NHW had more private insurance (52.5% vs. 25.4%, OR 3.24, 95% CI 2.19-4.80, p<.001) than Hispanics. Women had similar PRx, mortality, and readmissions, yet less CABG (6.7% vs. 14.1%, OR 0.38, 95% CI 0.21-0.67, p = .001) and Non-STEMI PCI procedures than men (39.1% vs. 51.2%, z = 2.65, p = 0.008) after full data adjustment. Conclusions: Equitable standards of care offset previously reported disparities in AMI management in Hispanics but not in women’s coronary revascularization procedures.

Introduction: In patients with ST-Elevation Myocardial Infarction (STEMI) treated with PCI, door to balloon time (DTB) of ≤ 90 minutes is associated with improved outcomes. We sought to compare DTB between patients of different race/ethnicity undergoing primary PCI in Get With The Guidelines (GWTG) hospitals. Methods: We analyzed the GWTG registry to identify patients treated with primary PCI for STEMI within 24 hours of presentation between 2005-2009. Patients treated with fibrinolytics, transferred to the hospital for PCI, or those missing DTB were excluded. Baseline demographics, median DTB, proportion of patients with DTB ≤ 90 minutes, and in-hospital mortality were compared between groups: White, African American, Hispanic. Results: Of the 7824 patients, 85% were White (n=6666), 8% were African American (n=607) and 7% were Hispanic (n=551). When compared with Whites and Hispanics, African American were younger but had higher rates of smoking, hypertension, renal disease, prior myocardial infarction, heart failure, or stroke. The Hispanic group had a higher proportion of patients with diabetes, hyperlipidemia, and no insurance. Median DTB times and the proportion of patients treated ≤ 90 minutes were similar between the three groups (Figure). In-hospital mortality was similar in the groups: white (3.8%), African American (3.0%), and Hispanic (3.8%); p=0.62. Conclusion: Within the context of a national quality improvement initiative, race/ethnicity was not associated with either DTB or in-hospital mortality among STEMI patients undergoing PCI. This suggests that programs such as GWTG may reduce race/ethnicity based disparities in the care of patients with STEMI.

Abstract:
Introduction: Despite the high prevalence of chronic kidney disease (CKD) in the Hispanic/Latino population, most Hispanic/Latino adults with CKD are not aware. This study aims to develop a tool for the Hispanic/Latino population to determine whether an individual has CKD. Methods: Baseline (2008-2011) data on 13,636 participants (aged 18 – 74 years; 60.8% female) from the Hispanic Community Health Study/Study of Latinos were analyzed to develop a tool using routinely available information to identify individuals with an increased likelihood of having CKD. CKD was defined as a low eGFR (<60 ml/min per 1.73 m 2 ) or the presence of albuminuria [urine albumin to creatinine ratio (UACR)≥30 mg/g]. Based on prior literature, several CKD risk factors were examined using several prediction algorithms (RIDGE regression, LASSO regression, classification and regression tree, and random forest). Results: A total of 364 (2.7%) individuals had low eGFR and 1346 (10.1%) had albuminuria. The LASSO model was selected due to its greater accuracy and parsimony. Variables predictive of CKD were weighted to develop a model-based numeric scoring system. Age, self-reported health conditions (i.e., hypertension, diabetes, prevalent coronary heart disease/stroke, balloon angioplasty/stent/bypass surgery), physical functioning limitations, and having health insurance were predictive of low eGFR (Table). Diabetes, hypertension, and pulse were predictive of albuminuria (Table). Multivariate models were internally validated using 10-fold cross-validation (area under the receiver operating characteristic curve of 0.87 and 0.70 for low eGFR and albuminuria, respectively). A score of ≥9 on the eGFR model yielded sensitivity (SN) of 84%, specificity (SP) of 78%, positive predictive value (PPV) of 9%, and negative predictive value (NPV) of 99%. A score of ≥4 on the albuminuria model yielded SN of 62%, SP of 67%, PPV of 18%; and NPV of 94%. Conclusion: This community tool may facilitate the identification of undiagnosed CKD in Hispanics/Latinos.

Questo progetto di ricerca ha lo scopo di ampliare le conoscenze di alcune piante appartenenti alla flora Mediterranea. Le piante da studiare sono state scelte in seguito ad un’attenta analisi bibliografica tra specie poco sudiate. La scelta è quindi ricaduta su nove specie appartenenti al genere Anthemis, due specie appartenenti al genere Pulicaria e sulle specie di Tetraclinis articulata, Salvia argentea, Ajuga tenorei, Ballota hispanica, Moluccella Spinosa e Thapsia garganica. Di esse sono stati preparati ed identificati i componenti degli oli essenziali e successivamente effettuati su di essi i saggi antibatterici. Dagli estratti delle specie di Tetraclinis articulata e Ajuga tenorei sono invece stati isolati e caratterizzati i metaboliti secondari non volatili. Di alcune delle molecole identificate da questi estratti è stata determinata la loro potenziale attività antitumorale attraverso l’utilizzo di metodi computazionali. Le specie appartenenti al genere Anthemis collezionate in Sicilia rappresentano una grande fonte di dati per la classificazione chemotassonomica e per considerazioni di biodiversità. I dati di composizione di tre di loro, riconosciute appartenenti alla sezione Hiorthia, quindi strettamente collegate tra loro, sono stati comparati con i dati disponibili in letteratura di tutte le specie appartenenti al genere Anhemis usando l’analisi statistica dei cluster. I risultati ottenuti mostrano che questi taxa appartengono alla stessa sezione sulla base delle classi di composti in esse contenuti, soprattutto sesquiterpeni. Una estensione di questo lavoro, già programmata ed in corso, include tutti i dati di composizione collezionati per le alter specie di Anthemis analizzate. Inoltre, i loro oli essenziali hanno mostrato una moderata attività antibatterica contro batteri gram+ e gram-. Di due specie siciliane di Pulicaria (P. vulgaris var. graeca and P. sicula) è stata identificata la composizione dell’ olio essenziale. L’analisi della componente principale (PCA) Introdution 7 di P.sicula, rispetto alle altre specie di Pulicaria fino ad ora studiate, mostra una peculiare biodiversità di questa pianta siciliana. Il confronto, tra i dati di composizione di P. vulgaris var. graeca e i dati relativi a P. vulgaris Gaertner, mette in evidenza un profilo chimico completamente differente. Quindi i due taxa potrebbero essere considerati due entità distinte. L’olio essenziale di P. vulgaris mostra una media attività antimicrobica contro i batteri Bacillus cereus e B. subtilis. L’analisi della composizione dell’olio essenziale di Salvia argentea, collezionata in Sicilia, mostra un profilo chimico differente rispetto a quello di Salvia argentea collezionate in altri paesi. inoltre è stata studiata la composizione degli estratti ottenuti con solventi a bassa polarità. Anche la composizione dell’olio essenziale di Ballota hispanica mostra un profilo caratteristico, comparato a quello degli altri taxa. Inoltre il suo olio essenziale mostra una bassa attività antibatterica mentre l’attività antiossidante è veramente alta, tale che si potrebbe supporre l’uso di B. hispanica come fitoterapico. Lo studio dell’olio essenziale di Moluccella spinosa, una pianta mai investigata in precedenza, ha mostrato differenti marker nella sua composizione rispetto all’olio essenziale di altre specie collezionate in Turchia, tuttavia si è riscontrata una buona correlazione con l’olio di M. laevis. Anche in questo caso si è osservata una moderata attività antibatterica dell’olio essenziale. Infine i risultati ottenuti per l’olio essenziale di Thapsia garganica, indicano un profilo chimico completamente differente rispetto a quelli delle altre specie di Thapsia studiate fino ad oggi, indipendentemente dal metodo di estrazione utilizzato (SPME o idrodistillazione). Lo studio dei metaboliti non volatili riguarda Tetraclinis articulata e Ajuga tenorei. Introdution 8 Solo un articolo sull’investigazione di T. articulata collezionata in Marocco è stato pubblicato. T. ariculata analizzata in questo lavoro è stata collezionata in Tunisia e ha mostrato un profilo metabolico simile a quello della specie marocchina. Infatti sono stati isolati derivati Δ15-pimaranici dall’estratto esanico di T. articulata; quasi tutti i composti isolati da questa pianta erano già stati in precedenza isolati ad eccezione del composto 5. I tre estratti (esano, diclorometano e metanolo) di T. articulata mostrano una buona attività antiproliferativa contro cellule tumorali. Gli estratti in diclorometano e metanolo sono stati cromatografati, ma non sono state ottenute frazioni pure. L’ottenimento di composti puri rappresenta il passo successivo della ricerca. La possibile attività antitumorale delle molecole 3, 4 e 5 verso differenti target è stata valutata attraverso metodi computazionali, utilizzando l’ Inverse Virtual Screeneing (IVS). Da un’accurata analisi delle interazioni, le tre molecole sembrano altamente correlate con la proteina fxr (Farnesoid X receptor, Cod. PDB: 1OSV). La purificazione dell’estratto metanolico di Ajuga tenorei ha portato all’isolamento di due iridoidi (arpagide e 8-O-acetilarpagide) e un fitosteroide (ajugalattone), prodotti di cui erano note le loro proprietà biologiche (antibatterica, anti-infiammatoria e antivirale). Per esplorare nuovi target per l’attività antitumorale, questi composti erano soggetti a IVS. Le molecole 6, 7, 9 e 10 mostravano la migliore correlazione con target differenti, ciò può essere spiegato dal cambiamento dei pattern di sostituzione nella struttura che porta a una differente interazione con la proteina. Sfortunatamente non sono state possibili modificazioni chimiche dei composti isolati a cause delle basse quantità del materiale puro isolato.
This research project aims to broaden the knowledge on Mediterranean plants as the genus Anthemis and Pulicaria and as species Tetraclinis articulata, Salvia argentea, Ajuga tenorei, Ballota hispanica, Moluccella Spinosa and Thapsia garganica. These plants are widely used in folk medicine and their extracts will be analyzed for the isolation of metabolites. Appropriate biological target will be selected by computational methods. The biological activity of these molecules and chemically modified derivatives will be evaluated in order to obtain structureactivity relationships.

Carmen and the Staging of Spain explores the Belle Époque fascination with Spanish entertainment that refashioned Bizet’s opera and gave rise to an international “Carmen industry.” Authors Michael Christoforidis and Elizabeth Kertesz challenge the notion of Carmen as an unchanging exotic construct, tracing the ways in which performers and productions responded to evolving fashions for Spanish style from its 1875 premiere to 1915. Focusing on selected realizations of the opera in Paris, London, and New York, Christoforidis and Kertesz explore the cycles of influence between the opera and its parodies; adaptations in spoken drama, ballet and film; and the panorama of flamenco, Spanish dance, and musical entertainments. Their findings also uncover Carmen's dynamic interaction with issues of Hispanic identity against the backdrop of Spain's changing international fortunes. The Spanish response to this now most-Spanish of operas is illuminated by its early reception in Madrid and Barcelona, adaptations to local theatrical genres, and impact on Spanish composers of the time. A series of Spanish Carmens, from opera singers Elena Sanz and Maria Gay to the infamous music-hall star La Belle Otero, had a crucial influence on the interpretation of the title role. Their stories provide a fresh context for the book's reappraisal of leading Carmens of the era, including Emma Calvé and Geraldine Farrar.

Abstract The United States has a history of either welcoming immigrants or rejecting them. At many periods the country welcomed immigrants, but at other times it has not. It always has struggled with the treatment of minorities, from African American enslavement to native Americans placed on reservations to Asian Americans put into camps to intolerance of Hispanics and to rejection of gays, lesbians, Muslims, Jews, and people who do not work with their hands. This trend has been prominent in American politics throughout its history but has become more prominent as American parties have polarized. White evangelicals now dominate the Republican party and have attempted to institute regulations on access to the ballot. Such actions are aimed at the minorities who support Democrats.
