Abstract:
Despite concerted efforts from our educators, administrators, and government, American education continues to struggle. The author of this work argues that the decline can be traced to censored curricula, inaccurate textbooks, test-driven evaluations, and increasing poverty among the student population. Under the definition of “failure” specified in the No Child Left Behind Act, more than 80 percent of American schools could currently be labeled as failing, while the quality of American education overall and our students’ performance continue to rank unfavorably against international competiti

Abstract:
Adult patients are presumed to possess decision-making capacity, but when they are unable to make their own decisions—which is especially frequent in the context of serious illness—ideally a surrogate decision-maker will be able to determine what the patient would have wanted (i.e., substituted judgment). Only when this is not possible is it necessary to fall back on what seems to be in the patient’s best interests. To foster patient autonomy, goals and values should be identified and documented in advance, such as in an advance directive, as well as a surrogate decision-maker named. This help

Abstract:
The concept of ‘sovereignty as responsibility’ is without question one of the foundations for the concept of the responsibility to protect (R2P). As United Nations Secretary-General Ban Ki-moon observed in 2008, R2P is built on the ‘positive and affirmative concept of sovereignty as responsibility—a concept developed by . . . Francis Deng, and his colleagues at the Brookings Institution more than a decade ago’. This chapter discusses how the concept of sovereignty as responsibility developed from discussions about governance in Africa and from the application of human rights standards to the p

Abstract:
The chapter asks if the British state is being hollowed out. It identifies four trends that are redrawing the boundaries of the state: privatization; alternative service delivery systems; the impact of the EU; and the new public management. The resulting problems include: fragmentation; accountability; catastrophe; central capability. It discusses the case for a return to bureaucracy. The Afterword shows the trends in public spending and public employment, the emergence of the ‘franchise state’, and austerity narrative mean that the pressures redrawing the boundaries of the state persist. Abstract:
Through a discussion of the effects of cigarette smoking on both pain perception and other substance use, this chapter outlines the presentation of the benefits to the patient and reviews smoking cessation strategies. It begins with an epidemiological review of the higher prevalence of smoking among those with chronic pain syndromes, such as fibromyalgia, headache, and low back pain. The many adverse consequences of cigarette smoking for general health are identified, from enhanced osteoporosis to prolonged wound repair. Most strikingly, smoking is demonstrated to both exacerbate the experienc

Abstract:
The Crisis of Israel’s Democracy, 1948 - 2025: Origins, Developments, and Consequences focuses on Israeli democracy, its deep historical origins, development over seven decades, long-term consequences, and possible resolutions. While most commentaries on Israel’s crisis focus on the proposals for judicial reform by the Netanyahu government, this study focuses on what it identifies as the fundamental problems of the Israeli regime—its ethnonational (rather than liberal-democratic) form, its lack of a written constitution undermines the country’s deeply divided society, the greatly dysfunctional
