Full textBackground: Lesbian, gay, bisexual and transgender (LGBT) parents sometimes experience heteronormative values within the child- and maternal health care. Assumptions are made that parental couples will be of different sex instead of same sex. Transgender parents experience that they have to educate health personnel. Still, how specialist trained nurses within child health care experience meeting with LGBT-families is understudied. Aim: The aim of this study it to explore the experiences among specialist trained nurses within the child health care when it comes to meeting LGBT-parents seeking heath care for their children. Method: This was a qualitative interview study with a descriptive design including nine specialist trained nurses within the child health care. Individual interviews were conducted, recorded, transcribed verbatim and analyzed through a qualitative content analysis. Result: The content analysis of the study generated five categories: LGBT-families are more vulnerable; Un unclear documentation-system and attitudes from LGBT-parents create insecurity; Approach of LGBT-families requires competence; There has been an increased acceptance and tolerance to LGBT-families; There has been an increased awareness about the value of an including health environment within child health care. Specialist trained nurses in child health care experience that they approach LGBT-parents in the same way as other parents, but that it can be differences that can create insecurity, for example about documentation. Conclusion: Specialist trained nurses within the child health care expressed that there are no differences in the meeting with LGBT-families and other families. Still, strategies to make the families feel more welcomed, for example by showing extra care, were used.

Full textVaccination as a part of the preventative public health project has for many years been one of the important tasks of the children's health care. The vaccination program has been adjusted to current research and the health situation in the country. Having their children vaccinated is not something obvious for all parents, though. Especially the vaccination against measles, whooping cough and German measles have been questioned. The purpose of this literature study was to investigate the reasons for the resistance to vaccinate and to illuminate the security creating advice of the nurse at the public health centre. The study showed that there were varying ways to look at the MPR-vaccination among the parents. It could be considered as something injuring while childhood illnesses were sometimes considered as something positive. The social circumstances and a lack of trust were also factors that influenced the parents. It was also obvious that the parents need a district nurse that is a good listener and who can give advice individually and also give information that creates security. This information consists of telling the parents that the MPR-vaccine does not cause autism or chronic intestine disease. Furthermore, there is a need to inform of the illnesses and their risks. This information is objectively given by a well informed nurse, whose task it is to work for a vaccination program with the widest coverage possible.

Full textBakgrund:Barnvaccinationer är kostnadseffektivt och har stor betydelse för folkhälsan. I Sverige är täckningsgraden hög och förtroendet för vaccin stort. Dock är tveksamhet till vaccin ett växande problem globalt. Enligt WHO är tveksamhet till vaccin ett av de tio största hoten för hälsa i världen idag. Sjuksköterskan har en nyckelroll i avseendet att vaccinera barn och ge information till föräldrar. Syfte:Syftet var att undersöka varför vissa barn inte blir vaccinerade på BVC, samt vad BHV-sjuksköterskor har för strategier att bemöta föräldrars motstånd eller tveksamhet till vaccin. Metod:Semistrukturerade intervjuer utfördes med 12 barnhälsovårdssjuksköterskor som hade erfarenhet av att bemöta föräldrar kring frågor om vaccin. Analysmetod var induktiv innehållsanalys. Resultat:Ur analysen framkom sex kategorier och 11 underkategorier. Anledningar till tveksamhet eller motstånd till vaccin var att konsekvenserna av sjukdomarna glömts bort, att föräldrarna kände oro för hur vaccinet påverkade barnet samt brist på förtroende. Grundstrategier som BHV-sjuksköterskorna använde för att bemöta föräldrarna utgick från god evidensbaserad information. Det möjliggjorde högt förtroende för sjuksköterskan och det vaccin som erbjöds. Slutsats:I Sverige är det norm att vaccinera barn, något som inte bör tas för givet. Sjuksköterskan kan i ett tidigt skede ge evidensbaserad information till samtliga föräldrar på BVC och därigenom ge kunskap om vaccinets hälsofrämjande fördelar. Det kan förebygga tveksamhet till vaccinationer och få fler barn att vaccineras.

Full textBakgrund: Föräldrars upplevelser av att bli förälder är olika, eftersom ett nyfött barn medför en stor omställning i deras liv. Majoriteten av barnfamiljerna i Sverige besöker barnhälsovården under barnets första levnadsår. Syfte: Syftet var att belysa hur föräldrar i Sverige upplevde omvårdnad inom barnhälsovården under barnets första levnadsår. Metod: En litteraturstudie gjordes som baserades på åtta vetenskapliga artiklar från Sverige. Resultat Det var tre områden inom omvårdnaden på BHV framstod som viktiga för föräldrarna. Dessa var bemötande, stöd och råd och trygghet. I resultatet framkom att föräldrarna ville bli bemötta på ett respektfullt och trevligt sätt av sjuksköterskan, de ville känna sig trygga vid besök på BHV och de vill få råd och stöd. Det framkom även att vissa föräldrar var negativa till hur de blev bemötta, fick stöd och råd samt att de kände sig otrygga med sjuksköterskan. Papporna ansåg att sjuksköterskan borde träffa och se dem mer. Slutsats: Genom att föräldrarnas negativa upplevelser lyfts fram, kan den kunskapen användas för att förbättra omvårdnaden i framtiden. Det är viktigt för sjuksköterskor att tillfredsställa föräldrars behov av ett respektfullt bemötande, stöd och råd samt trygghet genom att lyssna på vad föräldrar efterfrågar.
Background: Parent’s experience of parenting in the child’s first year of life differs considerably as it makes a great change in their lives. The majority of families in Sweden visit the Child-Health Care units during the child’s first year. Aim: The aim of this study was to illuminate the parents’ experience of Swedish Child Health Care. Method: The method was based on a literature review of eight scientific articles from Sweden. Results: There were three areas of the Child Health Care which were considered important by the parents. These were the reception, advice and help and security. The result showed that parents wanted to be received respectfully by the nurse and to feel secure at the visit and get advice and help. Some parents were negative as to their reception advice and help and their security Fathers’ thought that the nurse should meet and see them more often. Conclusion: The negative experiences of the parents could be made use of to improve the Child Health Care in future. It is important that the nurse satisfies the parents’ needs of a respectful reception, proper advice and help and security by listening to parents’ wishes.

Full textBakgrund: En engagerad pappa är betydelsefull för barnets hälsa och utveckling. Genom att ge stöd i föräldraskapet kan barnavårdscentralen (BVC) öka barnets möjlighet till god hälsa. Syfte: Att belysa de svårigheter och utmaningar pappor till små barn upplever och hur papporna beskriver sitt behov av stöd från BVC. Metod: En kvalitativ metod användes. Tio pappor intervjuades och materialet analyserades med hjälp av innehållsanalys. Resultat: Det var en stor händelse och en mycket positiv upplevelse att bli pappa. Det innebar svårigheter och utmaningar framförallt att hinna med både arbete och familj och att ta ansvar för en annan individ. De flesta hade en positiv bild av BVC. De hade många tankar om verksamheten och hur de skulle vilja att den utformades. Papporna upplevde behov av att BVC kontrollerade barnets hälsa, gav rådgivning vid sjukdom och förmedlade kontakter med andra föräldrar. Det var viktigt att kunna komma i kontakt med BVC när behov uppkom liksom att sjuksköterskan kände till barnet. Slutsats: Pappor upplevde en svårighet att hinna med både arbete och familj. De önskade främst stöd från BVC när barnen var sjuka och ville att BVC skapade möjligheter för dem att träffa andra pappor. Pappors engagemang är betydelsefullt för barnets hälsa, utveckling och välbefinnande. Det är därför viktigt för BVC att verka för pappors delaktighet i barnens hälsovård. Ett område för framtida forskning är därför att hitta och utvärdera former för föräldrastöd som attraherar pappor.
Background: Involved fathers are important for childrens´ health and development. By giving parent support the child health care can increase childrens´ chance to good health. Aim: To illuminate the difficulties and challenges that fathers of small children experience and how they describe their need of support from the child health care. Method: A qualitative method was used. Ten fathers were interviewed and data was analysed using content analysis. Result: Becoming a father was a major event in life and a positive experience. It involved difficulties and challenges, especially being able to handle both work and family life and to be responsible of another individual. The majority had a positive image of the child health care. They had many opinions about it and how they would like it to be. The fathers experienced a need that the child health care controlled the child´s health, gave advice when the child was ill and mediated contacts with other parents. It was important to be able to reach the child health care when they needed as well as the nurse knowing the child. Conclusion: Fathers experienced difficulties managing work and family life. The fathers´ needs of support from the child health care was greatest when the child was ill and they would like them to provide opportunities to meet other fathers. Committed fathers are important for the child´s health, development and well- being. Therefore, it is important for the child health care to work for fathers´ participation in the health care of the child. A field for future research is therefore to find and evaluate forms of parent support that attract fathers.

Full textIn 2001 the World Health Organization (WHO) issued a revised global recommendation for exclusive breastfeeding for a child ´s first six months. The aim of this study was to investigate what information parents receiving from the Child Health Care about introduction of solids to breastfed infants and if they follow the recommendation. A further aim is to document reasons for introduction of solids at the selected time and describe what food parents start with.
The method has a descriptive and a qualitative design. Users of several Swedish parenting websites were asked to participate in the study. A total of 27 users completed the anonymous open-ended questions posted on the websites. Supplementary data (13 texts) were obtained from the forum discussions on parenting websites. Data were analyzed with content analysis.
Results: The new recommendation in Sweden from 2003 is introduction of solids at the age of 6 months. According to the parents the Child Health Care has recommended introduction of solids between 4 months and 6 months. A few of the parents said they got a double message, or received no recommendation. The timing when the first solids were given by the parents varied between 3 months and 10 months. The most common reason to introduce solids was that the “infant is interested in food”. Porridge made from commercial baby cereals, vegetarian purees or “finger food” was most commonly used for complementary food.

Full textBackground: Sweden today is a multicultural country and many families come here to seek asylum. In child health care district nurses and pediatric nurses are working, her they meet asylum -seeking children and their families. Child health care nurses have a central role for families with children and the work in child health care assume to see the family as a one unit, on the basis of a family- focused perspective. Purpose: To elucidate the child health care - nurses experiences in meeting the asylum -seeking children and their families. Method: For the study a qualitative design with semi-structured interviews was chosen. Ten child health care- nurses were interviewed. The material was analyzed based on a qualitative content analysis. Results: The authors came up with three categories communicate both difficult and easy, see the whole family's needs and ensure people with different cultural needs.By communicating without help from an interpreter child health care – nurses considered that talks with the family became deeper and the relationship became more secure. Cultural differences were a recurring topic of conversation during the interviews with both similarities and differences in families' lifestyles. All child health care - nurses talked about the need to see the whole family. Conclusions: Child health care - nurses have a central role in all families. As child health care - nurse, it is important to have a family- focused perspective of the meeting on child health care, to include the whole family. To make the whole family feel good it requires that everyone in the family achieve good health. Child health care should focus extra on families seeking asylum because they often have a more complex situation. There is a need for increased cultural competency of health professionals.

Many mothers are worried about the child´swellbeing and if the child acts normally, and needs support and confirmation thatthey take care of the child in the right way. This support is called the individualsupport and the parents get it through the child health nurse.Aim: The aim of the study was to illustrate first time mothers experiences ofsupport from the child health nurse during the child´s first year.Method: A qualitative semi-structured interviewstudy were conducted with sevenfirst time mothers. The data was analysed through thematic content analysis.Result: The analysis work were resulted in four categories: To be inbetween,Accessibility, CHC – an open arm and The social network.Conclusion: The child health nurses are good at supporting mothers in theirparenthood. It would be good to give more support the first week, so the familiesknow where to turn to, until the child health clinic fully taken over. It is a bigsafety to be able to turn to the child health clinic. To have a parent group means alot to mothers, to be able to support each other and keep company with mothers inthe same situation.

Full textInvandrare kommer till Sverige från världens olika hörn med en annan tradition och kultur. Det kan uppstå kommunikationsproblem vid kontakten med de svenska myndigheter, bland annat hälso- och sjukvården. Syftet med denna studie är att undersöka iranska mammors upplevelse av kontakten med BVC samt deras upplevelse av styrkor och brister i kontakt med svenska barnhälsovården. Metoden har en explorativ design med en kvalitativ ansats. Sammanlagt intervjuades sju mammor där samtliga hade iranskt ursprung och persiska som modersmål. Samtliga mammor har fött alla sina barn i Sverige och barnen är mellan 5 månader och 4 år. Intervjuerna har analyserats enligt innehållsanalysmetoden. Resultaten utgår från fyra huvud kategorier: upplevelse av bemötande, upplevelse av information, upplevelse av stöd från BVC och förväntningar. Resultaten visar att sjuksköterskans välbemötande räknas som en viktig faktor som kan ha en positiv påverkan på mammornas upplevelse av kontakterna på BVC. Att kunna ha tillgång till informationsbroschyrer som är lätt förståliga för invandrare med språksvårigheter eller skrivet på persiska språket, visar sig vara önskvärd. Barnsjuksköterskas bemötande och kunskap om olika kulturer kan ha positiv påverkan på mammornas tillfredställelse och vällbefinnande. Med kulturkompetens ökar sjuksköterskans förståelse av invandraremödrars tankar och värderingar, vilket kan leda till en ökad tillfredställelse hos både sjuksköterskan och mamman.

Immigrants are coming to Sweden from different corners of the world with a different traditions and cultures. It may come up some communication problems at the contact with the Swedish authorities, including health care. The purpose of this study is to investigate Iranian mothers' experience of contact with the BVC and their experience of strengths and weaknesses in contact with the Swedish child health care.The method has an exploratory design with a qualitative approach. Totaly seven mothers were interviewed and all had Iranian origin and Persian as their mother tongue. All mothers have given birth to all their children in Sweden and the children are all between 5 months and 4 years. The interviews were analyzed according to content analysis. The results are based on four main categories: experience of treatment, experience of information, experience of support from the child health center and expectations. The results show that the nurse nice treatment counted as an important factor that can have a positive impact on mothers' experience of contacts at the child health center. To have access to information brochures which are easily understandable for immigrants with language difficulties or written in Persian language, is found to be desirable. The Paediatric nurse treatment and her knowledge of different cultures may have positive effects on mothers' satisfaction and wellbeing. Cultural competence is increasing the nurse's understanding of immigrant mothers' thoughts and values, which can lead to greater satisfaction among both nurses and mothers.

Full textPurpose The purpose of the study was to examine different professional’s views and experiences of the collaboration work that is going on between preschool and child health care in the municipality of Karlskrona and what impact it has had for the local cooperation-, their professional development and competence, and how it has affected the work and its content. Method The study had a qualitative approach with focus group interview as method for the data collection. Four focus group interviews were carried out with 21 respondents from child health care centres and preschools in the municipality of Karlskrona. The respondents were represented by six different professions; district nurses, preschool teachers, special education teachers, child minders, department heads and headmasters. Qualitative content analysis was used as a method of analysis. Results: The analysis resulted in one theme and 13 different categories with focus on collaboration, cooperation and common foundations. The theme captures the respondents positive attitude and willingness to cooperate, while they at the same time wished better conditions in terms of enough time and structure and that the staff managers clearly shows that they support and give priority to cooperation. A bridge has started to be built over the gap between the two health promoting settings. Relationships have been created, the different professionals have exchanged experiences and competences, and have learned together. A common foundation that affects the daily work content with children and parents has been evolved. The cooperation has been established at different levels in the geographical areas Conclusion The conclusion is that the collaboration work between child health care and preschool in the municipality of Karlskrona has influenced the development of local cooperation, professional development and competence, the work and its content. The result gave important knowledge of the professionals’ views and experiences of the collaboration process. This knowledge is valuable for further development of sustainable collaboration with the aim to promote children´s health. The study, which is a part of a more extensive follow-up, was focused on the professionals. In further studies it is also important to take in consideration the parents' views and experiences of the cooperation between child health care and preschool.

Full textBackground. Involved fathers has been seen to reduce the incidence of psychological problems in adolescence girls and acting out behavior in boys of school age. Personnel working with children are hence recommended to actively encourage fathers to become involved in their children from an early age. Child health services in Sweden are offered to all parents free of charge and reaches almost all children. Aim. The purpose of the study was to examine the current state of knowledge about fathers and nurses experience of fathers involvement in child health care. Method. Ten relevant studies were synthesized and interpreted by means of meta-synthesis inspired by Roséns (2014) description. Result. Fathers involvement in child health care is dependent on, among other things, working life and the feeling of being welcome in child health care. The nurses express an awareness of what they can do to increase the participation of fathers in child health care, but says that it is up to the parents to determine how participation should take place. Conclusion. A deeper understanding of fathers experiences are needed to respond to and support fathers in child health care to increase their engagement and by doing so also conduce childrens health and development in the long term.

Full textBackground: The large number of families seeking asylum affect nurses work in child health care. All children in Sweden between 0-6 years, do regular check-ups, including children seeking asylum. Nurses health promotion and preventive care covers childrens health based on the physical, mental and social well-being. Health promotion develops empowerment of families where the goal is good health. Escape and trauma affect families seeking asylum and many suffer from mental illness. Nurses work requires special and individual efforts in meeting with families. Purpose: To describe nurses experience of work in child health care with families seeking asylum. Method: The interview study had a qualitative design with an inductive approach. Fifteen nurses from six different municipalities participated in the study, all with experience working with asylum-seeking families in child health care. Collected data were analyzed according to qualitative content analysis. Results: The results revealed three categories: Families needs and support affect the work, External factors affect the work, The complexity of the meeting with the families affect the work. Conclusion: The study shows that nurses in child health care adapts work to asylum seeking families needs and circumstances. Nurses experience and education is the foundation of his or her skills. Mental illness and a socially complex situation for families seeking asylum is a challenge for nurses working with health promotion.

Full textThe consumption of alcohol in Sweden is today higher than a hundred years ago. Many children grow up in families where adults have a risky or harmful drinking. A high parental consumption of alcohol may affect the children negatively. The national Risky Drinking Project (2004-2010) was a commission from the government aiming at the health care working actively to decrease the consumption of alcohol among Swedish inhabitants. Since 2008 the nurses in the Child Health Services were educated by the Risky Drinking Project. A new way of alcohol prevention in the Child Health Services has been developed. It means that the nurses in the Child Health Services discuss with the parents about alcohol in relation to parenthood. The aim of this study was to describe parents' thoughts about alcohol in relation to parenthood and parents' experiences of the alcohol preventive work within the Child Health Services. Ten parents from Stockholm were interviewed. The collection of data was performed in May 2011. The data was analysed with content analysis. The result displayed six categories: The importance of the company, The exposure of the child, The dialog at the homevisit; hard to remember, The group dialog creates awareness, Strengthened attitude and Unclear message. The findings showed that the parents had lots of thoughts about alcohol inrelation to parenthood and all of the parents thought that the alcohol issue was important and concerned them in various ways. They meant that alcohol in relation to parenthood should be discussed with parents in the Child Health Services. Many of the parents thought that it was hard to remember what has been said about alcohol at the home visit. The dialog connected with the parents group had increased their awareness about alcohol and had given them new perspective. The conclusion of this study is that parents have many thoughts about alcohol in relation to parenthood and it looks as if the alcohol preventive work in Child Health Services can give new thoughts and awareness of alcohol. It seems to be of significance to discuss alcohol with parents in the Child Health Services.

Full textBackground: Support in parenting is one of the fundamental tasks of child health care. Child health nurses should have a good knowledge of children's development, health promotion and attachment between children and parents. Support and affirmation to fathers in their parental role is important as their involvement in children's upbringing has positive health effects for children. Sweden's work on equal parenting is on the right track but there are still mostly mothers who participate in the child health care. Through trusting relationships with child health nurses, fathers' involvement in child health care can increase. Aim: The aim of the study was to describe the experiences of child health nurses in involving fathers in child health care. Method: Qualitative method has been implemented with an inductive approach. Semistructured interviews were conducted with ten child health nurses in southern Bohuslän and Fyrbodal. Data was analyzed based on content analysis. Results: Two categories emerged in the result, Seeing the whole family and Organizational prerequisites. The first category contains three subcategories; Child health nurses' approach highlights how child health nurses with their approach affect how fathers are involved in child health care, The family as a team highlights how important it is to see the family as a whole and Strengthen the father's parent role. The second category has two subcategories; Individual parental conversations proved to be significant and are supposed to affect the continued care relationship and Adapting the work to the father's needs was important to enable the father to participate. Conclusion: The conclusion is that the approach of child health nurses is crucial for the relationship with the fathers. The importance of seeing the family as a whole, paying attention to fathers' need for support in their parenting role and individual parental conversations increases fathers' involvement and strengthens the care relationship. More efforts from society and organizational prerequisites in child health care are needed.

Full textBackground: Obesity is an increasing problem in Sweden and large parts of the world, for both adults and children. Overweight and obesity in the mother before and during pregnancy increases the risk of overweight and obesity in the child. The risk of childhood obesity is increasing in proportion with parental obesity. Obesity at age four increases the risk of obesity later in life. An overweight / obese child generally have worse quality of life and are more often depressed, obesity also causes diseases such as hypertension, and diabetes. To breastfeed the child promotes its appetite control and prevents obesity. Purpose: The purpose of this study was to investigate whether enhanced interventions within the framework of regular base program during pregnancy and under the children’s first four years in women with a BMI over 25 affects weight gain / ISOBMI and lactation length. Method: The intervention study of 182children divided into an intervention group (94) and a comparison group (88). Surveys regarding the child's weight, height and dietary habits were collected and analyzed when the child was 10 months, 2.5 years and 4 years old Results: A significant difference was seen between the groups at age four, when more were overweight and obese in the comparison group. The average weight differed one kg. A trend was seen in all weight categories in which the control group weighed more than the intervention group. There was no significant difference in breastfeeding duration between the groups. Conclusion: Overweight / obese women have a greater degree of overweight / obese children. Our study shows that small interventions make a difference.

Full textThis study wants to identify how the early prevention work looks in the society as well as how it could be developed. The purpose with this study is to exam the early prevention work for children, also if an implementation of a social worker in the children health care could develop the early prevention work that exist today. The study has a qualitative approach and an inductive view. A general strategy has been used to reach the empirics, that is gathered through four semi structed interviews and a review of current research in the field. The problem perspective is a foundation for processing and coding the interviews in the study. The study uses the bioecological model as the theory to process the coding and analysis of the interview empirics. From the theory three of the systems are used, micro-, exo and macrosystems. The study reaches conclusions from the systems that are: Micro conclusion: Early prevention work must be taken more seriously and occur in a greater extent. Exo conclusion: The interprofessional team should exist, along with the social worker, to perform early prevention work of good quality. Macro conclusion: To focus on early prevention will reduce high costs in the future.

Full textThe prevalence rates of mental health problems in Swedish children and adolescents are increasing. The reasons behind the increase is still unclear, and there is a paucity of research on factors affecting children’s mental health. More knowledge is also needed about the instruments being used to measure children’s mental health. Previous research has shown that parents’ and that the mental health problems can affect how they perceive their children’s health. It is also known that children’s mental health may affect parents’ health. The aim of this study was to investigate if there is an association between parents’ self-rated mental health, and their perception of their children’s mental health. The sample was drawn from a large ongoing study in Uppsala and consisted of 7094 3–5-year old children. Children’s mental health was measured by their parents through the Strengths and Difficulties Questionnaire (SDQ) and parents’ mental health was measured by the General Health Questionnaire (GHQ). Multiple linear regression was performed with SDQ as the dependent variable and GHQ as the independent variable. The analysis also included a number of control variables. The result revealed association between parents GHQ-ratings and their SDQ-ratings of their children. The conclusion was that parents’ mental health affects their understanding of their children’s mental health. This finding indicates that parents’ mental health needs to be considered when interpreting parent reports of children’s mental health.

Full textBackground: The aim of child health care is to promote and follow the child’s health, development, well-being and life situation. Through health dialogue with parents, the child health nurse lays the foundation for a good language development. Reading books and the linguistic interplay within the family are significant factors in the child’s language development.Purpose: The purpose was to describe parents' experiences of reading importance to children after visiting/language information in child health care. Method: Two semi-structured interviews consisting of focus groups of parents were carried out. The interviews were analysed using a qualitative content analysis with an inductive approach. The data analysis resulted in two categories: knowledge and commitment, as well as five sub-categories.Results: Parents do possess knowledge on how reading affects the child. Lack of time was presented as one concern and older siblings were being prioritised for bookreading. The child health nurse´s concrete examples of reading children’s books and the setup of the visit affect how the information is received. Language information leads to inspiration for reading to your child using age group-adapted books. Parents are under the impression that the health dialogue does not provide any new information, and that the child is not interested in being read to.Conclusion: Information about language development and reading inspires parents to start reading to the child. Lack of time, siblings and insecurities from the parents’ side on how to read in an age group-adapted way affect parents in their ability to maintain previous family reading habits.

Full textBackground: Breastfeeding is associated with many health benefits for both mother and child. In spite of that, almost a quarter of women stop breastfeeding within a couple of weeks for various reasons, such as breastfeeding problems and experiencing a lack of support. Women who have problems breastfeeding are primarily referred to paediatric healthcare, which plays an important role to promote breast feeding and to offer support with possible issues. This requires knowledge and enough time. Child health nurses’ experiences in this field can highlight possible areas of improvement regarding support for women with breast feeding problems. Purpose:To describe child health nurses’ experiences of working with breastfeeding difficulties. Method:A qualitative approach, in which semi-structured interviews were carried out with eight nurses. The selection was made strategically within two regions in southern and central Sweden. Qualitative contentanalysis was used as method for analysis. Results: Thechild health nurse has a varied and challenging job that relates to every unique patient encounter. They rely on their expertise and experience acquired through different specialist medical training, in-house training and number of years in their profession in a Child Health Centre. More education on breastfeeding is required. Mother’s desire to breastfeed is brought into focus, while the health of both the mother and her baby is taken into consideration. Collaboration such as handovers, counselling and meetings in interprofessional teams to exchange information and knowledge with other health care providers is an important aspect of child health nurses’ work. Conclusion: It is important to have a solid knowledge base in order to provide help and support when patients have breastfeeding problems. In addition to this, a child health nurse needs to be responsive and be able to meet different individuals in their particular situation. Collaboration with other health care providers plays a valuable role and can be improved.

Full textBackground: Attachment is a relationship created between a child and its parents. It involves an interaction between the child's need to receive care and the parent's willingness and ability to provide care. How the connection between the child and parent develops has been shown to be significant for the child's onward development and maturity, both intellectually and emotionally. Among children who do not develop a secure attachment to their parents, mental disorders, hyperactivity syndrome, attention deficit disorder and social problems are more common. All parents with children aged 0-6 years are offered general parenting support through their local child welfare centre. The aim of child healthcare is to promote overall positive development for the child and interactive communication with the parents. Currently there are no clear guidelines for how the advanced practice nurse can promote attachment between infant and mother and how attachment related problems can be handled in a health promoting way. Aim: The aim of this study is to describe advanced practice nurses' experience in promoting attachment between infant and mother in child healthcare. Method: A qualitative interview study with inductive approach was chosen to answer the study's aim. The interviews were processed using qualitative content analysis. Results: The result culminated in two categories, six sub-categories and an overall theme. The study shows that the advanced practice nurse promotes attachment between infant and mother by observing their interaction, individual counselling, group activities for parents and infants and screening for postnatal depression. All the advanced practice nurses describe that they observe attachment in all meetings with the family. They have an advisory and supportive role versus the mother Conclusion: Attachment is essential for the child's health and wellbeing. A lack of attachment results in unhealthy consequences for the child. The advanced practice nurse in child healthcare works to promote attachment through conversations, advice, support and group activities. Mothers with a lack of attachment are prioritized and can benefit from group activities designed to promote attachment, eg infant massage.

Full textBackground: The primary child health care services (PCHC services) plays an important part in the Swedish public health work. One fifth of the enrolled children have at least one parent born outside of Sweden. The majority of the PCHC nurses finds the interaction with the immigrant families stimulating but also recognizes difficulties. The aim: was to illuminate factors that influence the PCHC nurse's transcultural caring relationship with immigrant families in PCHC services. Method: The study was conducted as a literature review and based upon eleven scientific articles which were quality assessed and analyzed. The result was reported in three different themes. Result: Factors affecting the transcultural caring relationship in the PCHC services were the PCHC nurse's ability to communicate, the reception and approach and the cultural competence. Discussion: Communication difficulties affected the possibilities to create trust and confidence in the transcultural caring relationship. The PCHC nurse's reception and approach had implications not only for the outcome of the health care work but also for the immigrant family's attitude towards integration. Cultural competence affected the PCHC nurse's possibilities to see each immigrant family individually rather than as a representative of their cultural group. Conclusion: To enable a safe and trusting transcultural caring relationship in the PCHC services it’s necessary that the PCHC nurse has good communication skills, is able to use and incorporate cultural competence and understands the importance of providing a friendly, empathetic and individual approach to the immigrant family.
