Abstract:
La ventilation mecanique est un soutien vital qui permet dameliorer loxygenation lors dune detresse respiratoire mais qui peut aggraver les lesions pulmonaires dun poumon malade. Le syndrome de detresse respiratoire aigue (SDRA) lie au covid19 est caracterise par des lesions pulmonaires bilaterales et une compliance pulmonaire tres diminuee dans les formes severes . Lalteration des proprietes elastiques du poumon et la reduction de son volume aere induisent une forte elevation des pressions pour un faible volume insuffle dans les alveoles. La surdistensiondes alveoles induite par la ventilation peut provoquer un barotraumatisme lie a une rupture alveolaire. Le pneumothorax est la lesion la plus typique detectee sur la radiographie thoracique. Le barotraumatisme est favorise a la fois par la pathologie pulmonaire sous jacente et par lutilisation de pressions excessives delivrees par le ventilateur. La mesure de la pression de plateau en fin dinspiration reflete la pression alveolaire maximale. Cest le meilleur moyen de monitorage du risque de surdistension alveolaire : cette pression ne doit pas depasser 30 cmH2O.

Abstract:
SCUBA (self-contained underwater breathing apparatus) diving is an underwater activity that is practiced with a certain frequency in certain environments and which is very popular nowadays. In this thesis it is proved that the proposed technical modifications allow the necropsic diagnosis of arterial gas embolism following pulmonary barotrauma (PBt), through the isolation and manipulation of the carotid, vertebral and thoracic arterial systems; what the use of complementary studies such as the histomorphometric digital study has allowed to describe a characteristic emphysematous pattern in cases of PBt; and although drowning is the main cause of death other causes and multiple predisposition factors are described. Finally, this thesis shows that having a multidisciplinary team formed by dive police, specialists in forensic pathology and professionals in the field of underwater and hyperbaric medicine guarantees a high quality investigation of the cases, and to help avoid unsafe behaviors and prevent future SCUBA diving accident<br>El buceo con escafandra autónoma es una actividad subacuática que se practica con cierta frecuencia en determinados entornos y que goza actualment de gran popularidad. En esta tesis se demuestra que las modificaciones técnicas propuestas permiten el diagnóstico necrópsico del barotraumatismo pulmonar (BtP) seguido de una embolia gaseosa arterial, mediante el aislamiento de los sistemas arteriales carotídeo, vértebras-basilar y torácico; que la utilización de estudios complementarios como la histomorfometría digital permiten describir un patrón emfisematoso característico en casos de BtP; y que aunque el ahogamiento es la principal causa de muerte, se describen otras causas y múltiples factores de predisposición. Finalmente, esta tesis pone de manifiesto que disponer de un equipo multidisciplinar formado por policía judicial subacuática, patólogos forenses y especialistas en medicina subacuática e hiperbárica garantiza una investigación altamente cualificada y evita factores de riesgo para contribuir a la prevención de nuevos accidentes en el buceo con escafandra autónoma

Abstract:
Dans le contexte actuel d’attentats et autres conflits armées, le risque lésionnel thoracique par explosion est exacerbé sans que pour autant l’on sache l’efficacité réelle des moyens actuels de protection thoracique individuelle face à une telle menace. Ce travail de thèse, combinant expérimentations et simulations numériques, a traité de front l’ensemble de la chaine physique entre la détonation d’une charge explosive et l’apparition de lésions pulmonaires. Un des premiers objectifs a été de comprendre les différents phénomènes physiques mis en jeu lors de la propagation de l’onde de choc en champ libre. Le large éventail de données expérimentales acquis a permis de développer des outils simples permettant la détermination des caractéristiques de la menace ainsi qu’une approche numérique robuste sous LS-DYNA. L’objectif suivant a été d’étudier l’interaction d’ondes de choc avec des cibles de géométries simples et de compositions différentes. L’analyse des données expérimentales acquises a permis, en plus de la validation numérique de l’interaction fluide-structure et des MEF des structures, de déterminer de possibles candidats pour la définition d’un critère de lésions thoraciques. Enfin, des essais sur réacteurs biologiques post-mortem ont été réalisés, permettant d’obtenir la réponse cinématique du thorax du modèle porcin sous blast. Tout ceci a permis d’aboutir à des voies d’amélioration et à des outils prometteurs pour permettre l’évaluation et l’évolution des systèmes de protection thoracique dans un futur assez proche afin de limiter les risques face à cette menace qui a gagné en importance ces dernières années<br>With the increasing number of bombing attacks and armed conflicts, the risk of thoracic injuries caused by the blast threat is worsen, without knowing the efficiency of the current individual chest protection systems impacted by such a threat. This research, combining experiments and numerical simulations, dealt with the physics at play from the detonation of an explosive charge and the injury outcomes. One of the first objectives was to understand the different physical phenomena involved in the propagation of the shock wave in the open field. The huge set of data acquired allowed the development of simple tools for the determination of the blast characteristics as well as a robust numerical approach under LS-DYNA. The next objective was to study the interaction of shock waves with targets of simple geometries and different compositions. In addition to the numerical validation of the fluid-structure interaction and of the FEM of the structures, the analysis of the experimental data acquired allowed to determine possible candidates for the definition of a thoracic injury criterion. Finally, tests on biological post-mortem reactors have been carried out, which enabled to obtain the kinematic response of the swine’s thorax under blast. All this work has led to improvements and promising tools for the evaluation and the improvement of chest protection systems in the near future. The proposed tools should be used to limit the risks to this threat which has gained in importance in recent years

Trabalho final de mestrado integrado em Medicina área cientifica de otorrinolaringologia, apresentado á Faculdade de Medicina da Universidade de Coimbra<br>O mergulho recreativo tem vindo a crescer de popularidade, surgindo acompanhado por patologias relacionadas com variações de pressão, que na sua maioria ocorrem na área da otorrinolaringologia, especialmente, os barotraumatismos do ouvido. A prática saudável do mergulho recomenda uma vigilância periódica no otorrinolaringologista, com o objetivo de despistar as suas contraindicações, tratamento das patologias subjacentes, assim como, na orientação das manobras de compensação do ouvido, pois o aperfeiçoamento da técnica e treino diminuem significativamente a incidência deste tipo de acidente. Deste modo, este trabalho objetiva a revisão da literatura científica relativa ao barotraumatismo do ouvido em mergulhadores, permitindo não só, uma melhor compreensão dos mecanismos adaptativos do nosso organismo ao meio subaquático e da patofisiologia relacionada, assim como, fornecer uma ferramenta útil para o médico diagnosticar, tratar e orientar na prevenção dos acidentes de mergulho.<br>Recreational diving is growing in popularity, followed by emerging diseases related to pressure variations, which occur mostly in the field of otorhinolaryngology, especially the barotrauma ear. For a healthy diving practice is recommended periodic monitoring of the ear, nose and throat, in order to seek out contraindications, treat the underlying pathologies and orientate on the autoinflation techniques. Education and training of voluntary autoinflation techniques will significantly decrease the incidence of ear barotrauma. This study reviews the scientific literature related to ear barotrauma in divers, allowing, not only a better understanting of adaptive body mechanisms to subaquatic environment and its pathophysiology, but also, providing a useful tool for the physicians diagnose, treat and advice their patients to prevent this type of diving accident.
