Abstract:
Objective. To estimate the diagnostic accuracy of the anti-CCP test in JIA and to evaluate factors associated with higher accuracy.Methods. Two investigators performed an extensive search of the literature published between January 2000 and January 2014. The included articles were assessed by the Quality Assessment of Diagnostic Accuracy Studies tool. The meta-analysis was performed using a summary ROC (SROC) curve and a bivariate random-effect model to estimate sensitivity and specificity across studies.Results. The bivariate meta-analysis yielded a pooled sensitivity and specificity of 10% (95% confidence interval (CI): 6.0%–15.0%) and 99.0% (95% CI: 98.0%–100.0%). The area under the SROC curve was 0.96. Sensitivity estimates were highly heterogeneous, which was partially explained by the higher sensitivity in the rheumatoid factor-positive polyarthritis (RF+ PA) subtype (48.0%; 95% CI: 31.0%–65.0%) than in the other subtypes (17.0%; 95% CI: 14.0%–20.0%) and the higher sensitivity of the Inova assay (17.0%; 95% CI: 14.0%–20.%%) than the other assays (0.05%; 95% CI: 2.0%–11.0%).Conclusions. Anti-CCP antibody test has a high specificity for the diagnosis of JIA. The sensitivity of this test is low and varies across populations but is higher in RF+ PA than in other JIA subtypes.

Abstract:
Summary Juvenile pilocytic astrocytoma (JPA) is a low-grade glioma, a most common astrocytoma in young patients. It is a tumor with relatively well-defined margins. Pilocytic astrocytomas (PA) comprise approximately 5-6% of all gliomas. Gross total resection ensures a radical cure of patients and long-time survival. In the literature, the data on the survival rate of more than 20 years is scarce. A 5.5-year old boy with a history of 3-month complaints of headache, dizziness, and vomiting was diagnosed after CT to harbour a hypodense cerebellar tumor mass, situated in the midline-right hemisphere. The compression of the fourth ventricle resulted in rostral hydrocephalus with transependymal resorption. Within a week, a VP shunt was applied, followed by a radical Nafziger-Town operation. Gross total resection of the tumor was achieved. Profound clinical improvement was observed immediately after the operation. Postoperative CT scans, including the ones 22 years after the operation, remained practically normal. The patient is now 28-year old and is a perspective economist now. He leads a healthy working life. In general, the prognosis is excellent. If the tumor is completely removed by surgery, the chances of being “cured” are very high. Pilocytic astrocytoma has a five-year survival rate in over 96 percent in children and young adults, which is one of the highest survival rates of any brain tumor. However, there is even a small percent possibility for malignant transformation (1-4%).

Abstract:
Pilomyxoid astrocytoma (PMA) is a recently described histological type of pilocytic astrocytoma (PA), but the tumors show histological differences. PMA has more aggressive malignant behavior than PA. Magnetic Resonance Imaging (MRI) may play a crucial role in the preoperative setting and also help to establish an appropriate therapeutic regimen. In this case report, we illustrated MRI findings of a hypothalamic-chiasmatic PMA in a 15-year-old female patient presenting with extensive leptomeningeal seeding. The patient was operated for total tumor resection, but could not survive the second post-operative day. We have comprehensively discussed the clinical, imaging, and histopathological features of these relatively rare tumors and also reviewed the recent literature.

"DECHLORINATION OF CARBON TETRACHLORIDE BY NANOSCALE IRON PARTICLES IN AQUEOUS SOLUTION HSING-LUNG LIEN WEI-XIAN ZHANG Department of Civil and Environmental Engineering, Lehigh University, Bethlehem, PA 18015 INTRODUCTION Recently, a method for the generation of very small (nanoscale) bimetallic particles has been reported [1,2]. These nanoscale metal particles typically have a diameter on the order of 1-100 nm and feature 0.06% by weight of palladium deposited on the surface of iron. Advantages of the nanoscale bimetallic system for treatment of chlorinated organic pollutants include: (1) High specific surface area. The nanoscale metal particles have a specific surface area around 35 m2/g. Tens to hundreds times higher than those of the commercial grade iron particles (used in conventional iron walls). (2) High surface reactivity. For example, values of surface-area-normalized rate coefficient (KSA) for the transformation of chlorinated ethylenes were about one to two-orders of magnitude higher than those reported in the literature for commercial grade iron particles [3]. Due to their small particle size and high reactivity, the nanoscale metal particles may be useful in a wide array of environmental applications. In the aqueous phase, the nanoscale iron particles remain suspended, almost like a homogenous solution. Theoretical calculations indicate that, for colloidal particles less than about 1 micrometer, gravity of the metal particles is insignificant to influence the particle movement. Brownian motion (thermal movement) tends to dominate the transport process in groundwater. Thus, we believe that the metal particles could be injected directly into contaminated soils, sediments and aquifers for in situ remediation of chlorinated hydrocarbons, offering a cost-effective alternative to such conventional technologies as pump-and-treat, air sparging or even conventional iron reactive walls.
