Full textThe exclusively biomedical approach to dealing with diseases is a paradigm that has proven insufficient on many occasions, especially in those pathologies that carry a burden of stigma and discrimination, such as infection with the human immunodeficiency virus (HIV) or -in certain population subgroups such as intravenous drug users - that caused by the hepatitis C virus (HCV). For this reason, identifying the impact of biopsychosocial factors on the health status of people with HIV and / or HCV can be a tool capable of favoring the paradigm shift that would imply moving from the exclusively biomedical approach to the multidisciplinary approach that would involve a biopsychosocial approach. The first part of the research in this thesis focused on identifying biopsychosocial factors that condition the response to antiretroviral treatment and / or the clinical symptoms of HIV monoinfection or HIV and HCV coinfection in a sample of patients treated in two hospitals in Barcelona and the metropolitan area. The second part -developed through an online survey with participants from all Spain- analyzed the impact of biopsychosocial factors on a subgroup of people with HIV particularly sensitive as is the case of older people due to, among other reasons, comorbidities and social support needs that accumulate during the aging process. The last part -also carried out through an online survey with the participation of people from all over Spain- focused on the role of biopsychosocial factors in the field of hepatitis C. The findings obtained during the three phases of research have allowed to determine those profiles of people with HIV and / or HCV with a greater tendency to have a worse state of health based on various biopsychosocial factors. Thus, people over 45, immigrants, coinfected people, those who have experienced discrimination, those with chronic pain, those who are not involved in decisions about their health, those with a pessimistic approach, people with symptomatic HCV infection or those who experienced treatment failure with a treatment against HCV -and, especially, those who accumulated several of these conditions- would be the profiles, in light of the findings of this thesis, in which socio-health professionals should focus its attention to detect and treat early conditions associated with a worse state of health and, therefore, a worse quality of life.

Full textEl ejercicio proporciona una amplia gama de beneficios en la prevención de enfermedades en mujeres menopáusicas, aunque el impacto de la participación medida por el bienestar psicosocial es confuso, partiendo de ello surge la necesidad de investigar lo siguiente :”Eficacia del Taller de Gimnasia en Climaterio Mejorando Estilos de Vida Como Soporte Biopsicosocial en Mujeres en la Etapa del Climaterio en el Instituto Nacional Materno Perinatal-2008”,que tuvo por objetivo describir los beneficios percibidos de un grupo de mujeres menopáusicas incorporadas al taller de gimnasia en climaterio El presente estudio tiene un diseño cuasiexperimental analítico, teniendo como muestra cincuenta mujeres del taller de gimnasia en climaterio que cumplían los criterios de inclusión. Se recogieron variables antropométricas (edad, peso, talla, índice de masa corporal). Se administró el Cuestionario de Salud SF-36 para medir los beneficios percibidos mediante entrevista personal pre y post intervención. Se calcularon medidas de tendencia central y dispersión de las puntuaciones, por grupos etarios. Teniendo como resultados que todos los indicadores del Cuestionario de Salud SF-36 experimentaron modificaciones tras la intervención. Los beneficios percibidos se agruparon en 8 indicadores principales: dolor corporal, rol físico ,vitalidad, rol emocional ,salud general, función social , función física y salud mental ,lo cual indica un mejor estado de salud y una mejor calidad de vida. Llegando a la conclusión que el taller de gimnasia en climaterio “Mejorando estilos de vida” se ha mostrado eficaz ya que se evidencio el incremento favorable en los 8 indicadores del estado de salud, que fueron evaluados a través del cuestionario SF-36,lo cual indica un mejor estado de salud y una mejor calidad de vida.

Full textSexual compulsivity -intense and repetitive preoccupation with sexual fantasies, impulses and behaviors that causes adverse consequences and discomfort- is a pathology that reached increasing attention. However, it still raises many questions. For this reason, the present thesis had a double objective: to estimate the prevalence of sexual compulsivity among young population and to characterize the biopsychosocial profile of those with and without subclinical scores in this dimension. For that purpose, a two phases research was developed: during the first one, around 1,600 young people between 18-27 years old (684 boys and 904 girls) completed a screening battery for sexual impulse control; during the second one, 400 (200 controls and 200 with some traits of sexual compulsivity) were selected in order to complete an additional evaluation. The results of this thesis allow knowing in depth the reality of sexual impulses control among Spanish young population.

Full textPost-surgical pain is a major health issue from the perspective of patients, health professionals and the society as a whole. This thesis has the aim to identify psychological and psychosocial variables associated with chronic post-surgical pain, and to understand how these factors influence pain trajectories over time. In the first chapter, complexity of post-surgical pain is explored. Starting from the definition of this phenomenon, conceptualization by the biopsychosocial model is presented. To understand how predictors at different levels might influence the patient’s pain experience, pathophysiology of post-surgical pain is scrutinized, with a particular focus on the physiological substrate that can explain how psychological and psychosocial predictors can influence pain perception and modulation. Physiological, psychological and social risk factors for post-surgical pain are then presented. For each, potential mechanisms explaining association with this phenomenon are reviewed and discussed. The second chapter presents a systematic review of the literature with meta-analysis on psychological and psychosocial risk factors of chronic post-surgical pain. Methodological adjustments were adopted to ensure that the synthesis of research results was based on a comprehensive sets of studies, and to take into account the effect of non-significant estimates in case they were not reported by original studies. After a selection from 6329 records, 63 articles were included in a narrative synthesis and 34 studies were employed to perform a meta-analysis. The narrative synthesis of the literature showed that evidence about the effect of psychological predictors is heterogeneous, with few predictors, such as optimism, mental health and surgical fear, consistently associated with chronic post-surgical pain. In contrast, the meta-analysis showed that depression, anxiety, catastrophizing and, to a lesser extent, kinesiophobia, optimism and self-efficacy, have a weak but significant association with chronic post-surgical pain. Results are discussed in the context of the available literature. The third chapter presents the results of a longitudinal prospective study aimed at describing individual trajectories of pain intensity in patients who underwent knee or hip arthroplasty. The focus of this study was the analysis of pain as a process and in its relationship with central sensitization processes, executive functions and trait and state psychological variables. This study was performed with 145 patients listed for surgery at the hospital Humanitas Pio X, which were asked to complete a pre-surgical assessment, to fill a pain diary during the first seven days after surgery and to provide data about their pain after one and three months from surgery. Statistical analyses were performed employing a multilevel growth curve analysis for acute pain trajectories and multiple regressions for follow-up data. Multiple imputation procedures were employed to account for missing information. Results showed that, after controlling for sex, surgical procedure and pain intensity, central sensitization was a predictor of the intercept of pain trajectories, scores on the Trail Making Test Part B were associated with their slope, and that daily post-surgical catastrophizing was a significant covariate of pain intensity. Analyses of follow-up data confirmed the predictive role of central sensitization and showed that emotional distress, along with executive functions, is related with pain intensity and interference. Finally, research and clinical implications of the findings of these studies are discussed.
