Van, Duker Heather L. "The Effect of Birth Order on Infant Injury." BYU ScholarsArchive, 2007. https://scholarsarchive.byu.edu/etd/1306.

Abstract:
Pediatric injury is both common and expensive. Finding ways to prevent pediatric injury is a major public health concern. Many studies have investigated various aspects of pediatric injury, and some suggest that birth order may be an important risk factor for pediatric injury. This study further examined the relationship of birth order with pediatric injury, specifically studying the association of birth order with emergency department-attended infant injury while adjusting for other important family and individual covariates. Data for analysis included Utah birth certificate, death certificat

Abstract:
Objectives: 3D/4D translabial ultrasound (4D TLUS) is used to image the levator ani muscle and the anal sphincter to diagnose maternal birth trauma. We tested the validity of these methods. Secondly, we studied the pelvic floor of Nepali women. Finally, we undertook a medium to long-term outcomes in women after OASI. Methods: The first study was a retrospective analysis of 172 nulliparae. All had an interview, clinical examination and 4D TLUS. For the Nepal study, 129 consecutive women attending a gynaecology clinic were offered the same. The long-term follow-up of 146 women after OASI include

Abstract:
Obstetric anal sphincter injuries (OASIS) are serious complications of vaginal birth with a reported average worldwide incidence of 4%-6%. They are a recognised major risk factor for anal incontinence resulting in concern amongst women who sustain such injuries when considering the most suitable mode of birth in a subsequent pregnancy. This thesis contains three studies; a systematic review and meta-analysis of the published literature exploring the impact of a subsequent birth and it’s mode on bowel function and/or QoL for women with previous OASIS, a follow-up study on the long-term effects

Abstract:
Abstract Traumatic brain injury (TBI) is often the leading cause of death and the most common cause of permanent disability in children and young adults. The hospital admission rates as well as the incidence and mortality rates of TBI vary enormously in different countries and populations. Even though alcohol misuse is a well-known modifiable risk factor for TBI and other injuries, few studies have been carried out on drinking patterns in relation to TBI, alcohol's role in recurrent brain injuries as well as TBI in relation to alcohol use in children and adolescents. Background and aim: Persons sustaining different types of brain injury may experience difficulties with advanced mobility. Both persons with traumatic brain injury (TBI) and persons born with very low birth weight (VLBW) have similar brain abnormalities, such as reduced white matter and connectivity, and may thus experience similar mobility problems. However, few studies have assessed advanced motor abilities, and none have compared mobility functions in adult TBI and VLBW populations. Maternal birth trauma in particular pelvic floor trauma (PFT) is of increasing concern in recent years, mainly due to its association with long term morbidities affecting women’s quality of life. Prediction is difficult and likely to raise ethical, moral and health economic questions, and attempts at primary and interval reconstruction have had only limited success. This work was designed to explore multiple aspects of PFT with a particular focus on prevention and its effect on pelvic organ support through one prospective multicentre randomised controlled trial, two cross-sectional and four re

Preterm birth is the leading cause of neonatal morbidity and mortality worldwide. Preterm newborns require special care for surviving and may develop severe diseases related to prematurity, especially those born very early. The fetal inflammatory response (FIR) to intraamniotic infection is characterized by high levels of cytokines in umbilical cord (UC) blood and it is diagnosed by the identification of pathological vasculitis of vessels of fetal origin in the umbilical cord and the placenta. The FIR jointly with neonatal diseases with systemic inflammation increase the risk of perinatal brai

Infants with hypoxic-ischaemic encephalopathy (birth asphyxia) have a high risk of death or disability. Those with poor prognosis are sometimes allowed to die after withdrawal of intensive care. In recent years, doctors have used new types of brain scan, magnetic resonance imaging (MRI), to predict the type and severity of impairment if the infant survives and to help with such decisions. In this thesis, I analyse the issues arising from the use of MRI for prognostication and decision-making in newborn infants.
