Abstract:
Abstract Background Over time human skin thins and loses elasticity; topical treatments attempt to reverse this process. Objectives The aim of this study was to assess the efficacy of TransFORM Body Treatment (TFB) in skin rejuvenation compared to a bland moisturizer on the extensor and volar forearms. Methods Blinded participants were given 2 products to apply on the designated forearms with follow-up at 4, 8, and 12 weeks. Measurements included skin thickness, photography, dermatopathology, cutaneous elasticity determined by 2 different methods, and patient-reported outcomes. All were compared to baseline. Results Changes between bland moisturizer and TFB were recorded for the following parameters. (1) Roughness: extensor –0.09 mm for bland moisturizer and –0.26 mm for TFB (P = 0.174); volar 0.01 mm for bland moisturizer and –0.23 mm for TFB (P = 0.004). (2) Recoil velocity: volar –56°/sec for bland moisturizer and –24°/sec for TFB (P = 0.61); extensor –95°/sec for bland moisturizer and –63°/sec for TFB (P = 0.57). Retraction speed: volar –3.25 ms for bland moisturizer and –20.08 ms for TFB (P = 0.33); extensor –2.17 ms for bland moisturizer and –10.83 ms for TFB (P = 0.66). Histologically, TFB resulted in an increase in mucopolysaccharide content, new collagen, and number of elastin fibers in the papillary dermis. Changes in the Rao-Goldman score were also observed: volar –0.17 for bland moisturizer and –0.33 for TFB (P = 0.25); extensor –0.08 for bland moisturizer and –0.17 for TFB (P = 0.36). Conclusions Histology showed production of new collagen and elastin. Quantification of changes in skin thickness, skin retraction speed, and skin recoil velocity showed trends that agree with the visual data. Level of Evidence: 4

On July 10, 2015, Sandra Bland, a Black woman, was pulled over by Officer Brian Encinia for failure to signal a lane change in Prairie View, TX. An altercation ensued and Bland was arrested and jailed. Three days later, her body was found in her jail cell and the week following, the dash camera footage was released of her controversial stop and arrest. Utilizing a qualitative content analysis based on a triangulated sample of social media posts in response to the arrest video of Sandra Bland, the current work explores how social media users understand the case of Sandra Bland. Specifically, we focus on where they cast blame and how they explain that blame.

A pervasive misconception in infection control circles is that simple bland soap handwash reliably prevents hand transmission of transiently acquired bacteria, disregarding the level of hand contamination. Often cited to support this widespread misapprehension is a bigger misconception-the efficacy of bland soap handwash is rooted in the epidemiological research of Ignaz Semmelweis. As emphasized by Walter and Beck, Semmelweis did no such thing. Indeed, it was the observation by Semmelweis of failure of bland soap handwash to prevent healthcare workers from spreading puerperal sepsis that led to his investigations of chlorine hand antisepsis for control of cross-infection. These studies ultimately resulted in his demonstrating that effective hand antisepsis (and not bland soap handwash) could prevent transmission of the agents of postpartum endometritis, sepsis, and death.

Laboratory studies have shown that the resistance of isolated LDL-cholesterol or linoleic acid to oxidation is increased in incubations with chilli extracts or capsaicin – the active ingredient of chilli. It is unknown if these in vitro antioxidative effects also occur in the serum of individuals eating chilli regularly. The present study investigated the effects of regular consumption of chilli on in vitro serum lipoprotein oxidation and total antioxidant status (TAS) in healthy adult men and women. In a randomised cross-over study, twenty-seven participants (thirteen men and fourteen women) ate ‘freshly chopped chilli’ blend (30g/d; 55% cayenne chilli) and no chilli (bland) diets, for 4 weeks each. Use of other spices, such as cinnamon, ginger, garlic and mustard, was restricted to minimum amounts. At the end of each dietary period serum samples were analysed for lipids, lipoproteins, TAS and Cu-induced lipoprotein oxidation. Lag time (before initiation of oxidation) and rate of oxidation (slope of propagation phase) were calculated. There was no difference in the serum lipid, lipoproteins and TAS at the end of the two dietary periods. In the whole group, the rate of oxidation was significantly lower (mean difference −0·23 absorbance ×10−3/min; P=0·04) after the chilli diet, compared with the bland diet. In women, lag time was higher (mean difference 9·61min; P<0·001) after the chilli diet, compared with the bland diet. In conclusion, regular consumption of chilli for 4 weeks increases the resistance of serum lipoproteins to oxidation.

Currently, there is no standardized method to evaluate operator reliability in nerve ultrasound. A short prospective protocol using Bland–Altman analysis was developed to assess the level of agreement between operators with different expertise levels. A control rater without experience in nerve ultrasound, three novices after two months of training, an experienced rater with two years of experience, and a reference rater performed blinded ultrasound examinations of the left median and ulnar nerve in 42 nerve sites in healthy volunteers. The precision of Bland–Altman agreement analysis was tested using the Preiss–Fisher procedure. Intraclass correlation coefficients (ICC), coefficients of variation, and Bland–Altman limits of agreement were calculated. The sample size calculation and Preiss–Fisher procedure showed a sufficient precision of Bland–Altman agreement analysis. Limits of agreement of all trained novices ranged from 2.0 to 2.9 mm2 and were within the test’s maximum tolerated difference. Ninety-five percent confidence intervals of limits of agreement revealed a higher precision in the experienced rater’s measurements. Operator reliability in nerve ultrasound of the median and ulnar nerve arm nerves can be evaluated with a short prospective controlled protocol using Bland–Altman statistics, allowing a clear distinction between an untrained rater, trained novices after two months of training, and an experienced rater.

We cross validated the 20-m shuttle run test of aerobic capacity in children and adolescents with mild and moderate mental retardation (MR) using the population specific formula of Fernhall et al. (1998). Nine boys and 8 girls (age = 13.7 yr) completed a maximal treadmill protocol (measured V̇O2peak) and a 20-m shuttle run (predicted V̇O2peak). The measured peak oxygen consumption (V̇O2peak) was 39.4 ml kg-1 min-1. The relationship between measured and predicted V̇O2peak was r = .86 with an SEE of 6.2 ml kg-1 min-1. Multiple regression and Bland-Altman analyses showed that there was little bias, but the Bland-Altman analysis indicated highly variable limits of agreement (Bland & Altman 1986). Thus, the traditional approach (regression analysis) to concurrent validity revealed that the 20-m shuttle run is a valid indicator of V̇O2peak in these participants. The accuracy of prediction (Bland & Altman, 1986), however, was lower than expected in a population without MR.
