At the turn of the twenty-first century, middle-class men and women formed non-governmental organizations (NGOs) and filed public interest litigation suits (PILs) in order to expand temple space, knock down buildings that block views of Kālīghāṭ’s façade, and remove undesirable materials and populations from its environs. Employing the language of cleanliness and order, they worked (and continue to work) to make Kālīghāṭ a “must-see” tourist attraction. Scholarship has shown that India’s new middle classes—those produced through India’s economic liberalization policies in the 1990s—desire highly visible forms demonstrating their modernity as well as their uniqueness on the international stage of urban space. The example of Kālīghāṭ indicates how India’s new middle classes build on the work of the old middle classes to deploy the temple as emblematic of both their modernity and their Indian-ness. In so doing, they read the idioms of public space onto sacred space.

Chapter 8 argues that the Relational Blockworld (RBW) account naturally admits a kind of neutral monism that simultaneously deflates the generation/hard problem and explains time as experienced. Thus, the claim that the block universe is incompatible with time as experienced is refuted. The first section sets the stage, the second focuses on the Passage of time, and the third focuses on the Direction of time. Section four argues that embodied, embedded, and extended cognitive science and phenomenology support the neutral monism of RBW. The fifth section focuses on freedom, spontaneity, and creativity in RBW. Objections to the block universe picture based on free will in human action and creativity and spontaneity in the universe writ large are refuted. It is shown that RBW has all the freedom, creativity, and spontaneity anyone could reasonably hope for. Section six characterizes Presence in detail, and its relation to time as experienced is discussed.

This chapter discusses neurological complications of the administration of epidural and spinal anesthesia in the obstetric population. It begins with a case report on a patient with a total spinal block, which occurs when large doses of local anesthetic intended for the epidural space are inadvertently injected into the subarachnoid space. The chapter reviews key points in recognizing and treating this potentially fatal complication. It discusses other complications, including epidural hematoma, epidural abscess, spinal cord injury, and meningitis, as well as complications from intravascular injection of local anesthetic. Lastly, it discusses how to recognize and treat the most common complication of neuraxial block, post dural puncture headache.

Abstract:
This chapter moves beyond viewing nodes as homogeneous dots set on a plane. To introduce more complicated underlying space, multiplex networks (which are defined with layers of interaction on the same underlying node set) and temporal (time-dependent) networks are discussed. It shown that despite the much more complicated underlying space, many of the techniques developed in earlier chapters can be applied. Heterogeneous nodes are introduced as an extension of the stochastic block model for community structure, then extended using methods developed in earlier chapters to more general (continuous) node attributes such as fitness. The chapter closes with a discussion of the intersections and similarities between the many alternative models for capturing topological features that have been presented in the book.

Thoracic nerve root blocks can be achieved by interlaminal epidural, transforaminal epidural, paravertebral, and selective nerve root injections. The interlaminal approach allows blocking multiple nerve roots bilaterally, while the transforaminal approach has the advantage of depositing the injectate primarily to the anterior epidural space on the side of the injection, closer to the pathology. The paravertebral approach is often used to block multiple nerve roots on the side of injection, and the selective nerve root block is used to target a specific nerve root using a small volume of injectate. Fluoroscopy-guided injection the most commonly used technique. Contrast materials are often used to confirm the appropriate needle placement and monitor the spread of the injectate. Thoracic nerve root block and transforaminal epidural block are perceived as technically demanding due to anatomic complexity of the thoracic spine, its proximity to the lungs and major vasculature, and potential complications.

High or total spinal/epidural blockade occurs due to excess spread of local anesthetic within the neuraxial space. While this is an infrequent complication, it can cause respiratory and hemodynamic instability in obstetric patients. If high/total spinal/epidural occurs prior to delivery, such derangements may lead to fetal intolerance and need for emergency delivery. Clinicians should suspect risk for high block when patients lose upper extremity motor function and complain of dysphonia or dyspnea. Intubation and respiratory and hemodynamic support along with adequate sedation should be given until the block recedes. Preventative measures include strict epidural catheter aspiration practice and incremental epidural dosing strategies.

Intra-articular facet joint injections commonly refer to the injection of a contrast media and local anesthetic solution, with or without corticosteroids, directly into the facet joint space. The purpose of this procedure is pain relief as well as to establish an etiological diagnosis for surgical interventions such as joint denervation or radiofrequency ablation. Medial branch block, or facet nerve block, refers to injection of local anesthetic and possible corticosteroids along the medial branch nerve supplying the facet joints. Cervical intra-articular and facet nerve block injections are often part of a work-up for general or focal neck pain, headaches, or cervical muscle spasms. There is limited evidence for short- and long-term pain relief with cervical intra-articular facet joint injections. Cervical medial branch nerve blocks with local anesthetics demonstrate moderate evidence for short- and long-term pain relief with repeat interventions, and strong evidence exists for long-term pain relief following cervical radiofrequency neurotomy.

The design of every aspect of the urban landscape—from streets and sidewalks to green spaces, mass transit, and housing—fundamentally influences the health and safety of the communities who live there. It can affect people's stress levels and determine whether they walk or drive, the quality of the air they breathe, and how free they are from crime. Chapter abstract This chapter spotlights four transversal principles that undergird and animate Bourdieu’s research practice, and can fruitfully guide inquiry on any empirical front: the Bachelardian imperative of epistemological rupture and vigilance; the Weberian command to effect the triple historicization of the agent (habitus), the world (social space, of which field is but a subtype), and the categories of the analyst (epistemic reflexivity); the Leibnizian-Durkheimian invitation to deploy the topological mode of reasoning to track the mutual correspondences between symbolic space, social space, and physical space; and the Cassirer moment urging us to recognize the constitutive efficacy of symbolic structures. The chapter also flags three traps that Bourdieusian explorers of the social world should exercise special care to avoid: the fetishization of concepts, the seductions of “speaking Bourdieuse” while failing to carry out the research operations Bourdieu’s notions stipulate, and the forced imposition of his theoretical framework en bloc.

Chapter 2 explores the present understanding of Pompeii’s evolution by disassembling the apparent patchwork of grids across the city and reconsiders the presumed awkwardness in their adhesion. To do this, the traditional tools of formal analysis—street alignments and block shapes—are employed with and critiqued by the stratigraphic evidence recovered in the last three decades of excavation below the 79 CE levels. The result is an outline of the development of Pompeii’s urban form as a series of street networks: from the archaic age, through the period of the “hiatus” of the fifth and fourth centuries BCE, to a reorganization of the city’s space so profound that it can genuinely be considered a refoundation, and finally to the adjustments of a refounded city in the Colonial, Augustan, and post-earthquake(s) periods.

This chapter examines masculine individualism’s push–pull relationship with sympathy, beginning in The Scarlet Letter’s Custom-House. There, Hawthorne’s narrator sympathetically presses Hester Prynne’s dusty, scarlet A against his heart, feeling a burning tingle as he places himself in her position. Sections read touch and untouchability in The Scarlet Letter, exploring what encouraged male readers to overcome what Henry Thoreau referred to as masculinity’s “gulf of feeling” to experience sentimental connection. Writing through alienation and writer’s block, Hawthorne’s tingling connection with Hester in the Custom-House propelled him to complete the novel in just five weeks, after being fired from his position as surveyor. His imagined intimacy with Prynne emerges against myths of the self-made man and the untouchable citizen-subject. Readings of tactility reveal alphabetic print and textual space as sites of flux and flow, intimacy and distance, as writers and readers sympathetically lean into and recoil from page and print.

Odontogenic infections often arise from dental caries (usually the mandibular teeth) or from dental extraction. Acute necrotizing ulcerative gingivitis (ANUG) is more common in immunocompromised patients. These infections may spread into the parapharyngeal and retropharyngeal spaces, involving the airway and mediastinum. Airway management is critical as odontogenic infections can compromise airways via mass effect. Complications include the following: abscess, facial or orbital cellulitis/abscess, intracranial invasion, Ludwig’s angina, Lemierre syndrome, carotid artery erosion, descending necrotizing mediastinitis, airway compromise, hematogenous dissemination to distant organs, intraoral or dentocutaneous fistula formation, and cardiovascular disease. Antibiotics are not a substitute for definitive airway management. In addition, many cases of odontogenic infection will require surgical drainage, either at the bedside in the emergency department or in the operating room. Prior to this, consider using a nerve block to obtain anesthesia to the affected area of the face. Patients with necrotizing infections need emergent surgery with wide local debridement.
