The modern concept of allogeneic hematopoietic stem cell transplantation (alloHSCT) implies the transfer of the donor’s hematopoietic system exclusively, the essence of which is the infusion of hematopoietic stem cells (HSCs) in the form of a single-cell suspension into the body of the recipient, who is previously subjected to intensive high-dose chemotherapy in order to eradicate tumor cells and destroy his own hematopoiesis, including immune cells (to exclude immunological conflict). This concept does not include transplantation of BM stroma, because the results of early works dating back to the 70–80s of the twentieth century spoke in favor of the fact that mesenchymal stem cells (MSCs) and their progeny are not capable of transplantation. Today, there are strong reasons to state that such an assumption is wrong. Despite the low engraftment efficiency of MSCs demonstrated in the early studies, alternative targeted strategies could lead to successful MSC transplantation. In this chapter, the question of the possibility of BM stroma transplantation is considered, arguments in favor of the expediency of co-transplantation of MSCs and HSCs in the therapy of blood system diseases are given, and a new concept of co-transplantation of BM stroma and the hematopoietic system is proposed.

The traditional herb Andrographis paniculata (A. paniculata) has a long history of use in eastern Asia and ancient China for the treatment of a number of illnesses, including laryngitis, rheumatoid arthritis, cancer, diarrhoea, and upper respiratory tract infections. Andrographolide is a compound derived from this herb. One or more of andrographolide's disease anticipation and/or healing strategies include anti-inflammation, anti-apoptosis, antioxidative stress and/or pro-apoptosis. Pharmacodynamic investigations have shown that andrographolide may cross the blood-brain barrier and distribute to numerous brain areas; as a result, its pharmacological effects on the central nervous system (CNS) have begun to be understood in recent years. For instance, andrographolide has been demonstrated in numerous models of cerebral ischemia to reduce the size of brain infarcts. These results suggest that andrographolide may be useful in the treatment of psychiatric disorders like anxiety and depression. Targeting treatments are part of the current AD treatment approach. The Food and Drug Administration has authorised the medication aducanumab, which is given intravenously and requires close patient monitoring because of a potentially harmful side effect known as amyloid-related imaging abnormalities (ARIA). There have been numerous scientific investigations on medicinal herbs. Due to their anti-inflammatory and antioxidant characteristics, herbs can be utilized to treat AD. Anti-inflammatory medications such as German chamomile, Ginseng, liquorice, turmeric, and white willow bark may decrease inflammation of the brain tissue in Alzheimer's. Researchers are working to create new medications with superior therapeutic efficacies. Andrographolide has been demonstrated to have therapeutic effects in a variety of AD models in several recent research. Inhibiting the neuroinflammatory response is thus a potential therapy option for AD. In this possible way, andrographolide and its derivative could offer a better choice to develop a novel therapeutic molecule to AD in near future.

Chronic liver disease and decompensated cirrhosis are the major causes of morbidity and mortality in the world. According to current data, deaths due to liver cirrhosis constitute 2.4% of the total deaths worldwide. Cirrhosis is characterized by hepatocellular damage that leads to fibrosis and regenerative nodules in the liver. The most common causes of cirrhosis include alcohol consumption, hepatitis C, hepatitis B, and non-alcoholic fatty liver disease. Dysbiosis and intestinal bacterial overgrowth play a role in the development of complications of cirrhosis through translocation. In liver cirrhosis, ascites, gastrointestinal variceal bleeding, spontaneous bacterial peritonitis infection, hepatic encephalopathy, hepatorenal syndrome, hepatocelluler carcinoma are the most common complications. In addition, there are refractory ascites, hyponatremia, acute on-chronic liver failure, relative adrenal insufficiency, cirrhotic cardiomyopathy, hepatopulmonary syndrome and portopulmonary hypertension. In the primary prophylaxis of variceal bleeding, non-selective beta blockers or endoscopic variceal ligation are recommended for medium and large variceal veins. In current medical treatment, vasoactive agents, antibiotics, blood transfusion, endoscopic band ligation are the standard approach in the treatment of acute variceal bleeding. Sodium-restricted diet, diuretics and large-volume paracentesis are recommended in the management of ascites. In the treatment of hepatic encephalopathy, lactulose, branched chain amino acids, rifaximin and L-ornithine L-aspartate can be used. New therapeutic approaches such as ornithine phenyl acetate spherical carbon and fecal microbiota transplantation have shown beneficial effects on hepatic encephalopathy symptoms. In addition to their antioxidative, anti-proliferative and anti-inflammatory properties, statins have been shown to reduce the risk of decompensation and death by reducing portal pressure in compensated cirrhosis. In the treatment of liver failure, some artificial liver devices such as molecular adsorbent recirculating system, the single albumin dialysis system, fractionated plasma separation and adsorption are used until transplantation or regeneration. The purpose of this chapter is to review the most up-to-date information on liver cirrhosis and to explain the complications assessment, current management and potential treatment strategies in decompensated cirrhosis.
