Abstract:
Bohler angle and the crucial angle of Gissane are used on the evaluation of calcaneus fractures. However, few authors have described the variation of the angles when the calcaneus is growing. In this study, Bohler angle and the crucial angle of Gissane in paediatric population were measured using lateral foot radiographs of 429 patients, from 0 to 16 years of age. The control group was composed of 70 adult patients. The sample had a mean Bohler angle of 35.4° ± 5.9° and a mean crucial angle of Gissane of 110.5° ± 7.4°. The greater mean difference was identified for Bohler angle (8°) in the age group of 5 to 8 years (39.6° ± 5.7°) and for the crucial angle of Gissane (5°-6°) in the age group of 0 to 4 years (115.8° ± 7.3) ( P < .05). The influence of the ossification centres on the geometry of the calcaneus across age groups makes Bohler angle and the crucial angle of Gissane higher in young children. The increase in Bohler angle points out the relative development of the posterior facet in young children and the importance of the reconstruction of the posterior facet height in the intra-articular calcaneus fractures. Abstrak Penelitian ini bertujuan untuk mempelajari pengaruh proses perlakuan panas (heat treatment) terhadap kekerasan material special K (K 100). Material Special K (K100) dengan ukuran 25 mm x 20 mm x 50 mm dilakukan proses heat treatment sesuai dengan instruction for treatment BOHLER STEEL MANUAL, yaitu hardening dengan temperatur 950C dan quenching dengan media pendingin oli, kemudian dilanjutkan tempering dengan temperatur 200 C dan holding time 60 menit. Kekerasan sebelum dan sesudah proses heat treatment diperiksa dan dibandingkan. Hasilnya menunjukkan bahwa harga kekerasan rata-rata raw material Special K (K100) adalah 20 HRC, setelah proses hardening dan quenching 64 HRC, dan setelah tempering adalah 62 HRC. Abstract:
Objective: The aim of our study is to determine the normal ranges of the calcaneal parameters in the Indian population, and to compare the results with the data in the literature. Materials and Methods: The study was conducted at Coimbatore Medical College Hospital, Coimbatore on the feet (324 in number) of male and female Indian adults. Lateral view of the ankle was taken using a digital X-ray machine. Two parameters namely Bohler`s and Gissane angles were measured, independently by two radiologists to prevent inter-observer variation. Results: The Bohler`s and Gissane angles for the Indian population are statistically different from those seen in the published data for other population groups, as evidenced by the P value (P < 0.05). Conclusion: Calcaneal parameters specific to the Indian population have to be taken into consideration by the orthopedic surgeon to improve the standard of calcaneal fracture treatment in India.

Abstract:
Objective This study was performed to analyze the outcomes of calcaneal fractures using a minimally invasive internal fixation method with a thin plate and bone grafting. Methods This retrospective analysis included 21 patients treated using our minimally invasive approach. The outcome measures were the change in the Bohler and Gissane angles and the calcaneus width after fixation. The clinical outcomes were evaluated by the Maryland foot scoring system and assessment of soft tissue complications. Results The follow-up time was 12 to 18 months. The Bohler angle, Gissane angle, and width of the calcaneus were significantly different between the preoperative and postoperative periods. The rate of excellent and good outcomes was 85.7% according to the Maryland foot scoring system. The incidence of soft tissue complications was 14.3%. Conclusion Treatment of calcaneal fractures using a minimally invasive internal fixation method with a plate and bone grafting provides good to excellent clinical outcomes with few soft tissue complications.

Treatment of these fractures has evolved. Due to the concern of wound complications associated with extensile open treatments, smaller incision techniques, such as the sinus tarsi approach, are increasing in popularity. A number of fixation strategies are utilized with this approach, and it is unknown which most accurately restores radiographic alignment. The purpose of this study is to compare the postoperative radiographic outcomes of a plate and screw construct versus a cannulated screw construct when using the sinus tarsi approach for open reduction and internal fixation (ORIF) of calcaneus fractures. Methods: After IRB approval, records for all patients treated surgically at our institution for calcaneus fractures from 2012 to 2017 were reviewed. Inclusion criteria were intra-articular calcaneus fractures, patients aged 18 years or older, and use of the sinus tarsi approach. Exclusion criteria were open fractures and clinical follow up less than 6 weeks. A total of 51 fractures underwent ORIF using cannulated screws alone (Group 1), and 23 fractures underwent ORIF using a sinus tarsi plate (Group 2). The primary outcomes of interest included pre- and postoperative Bohler and Gissane angles, wound complications, and unplanned return to OR. Results: There was no statistically significant difference between preoperative Bohler angles for Group 1 (14.5 degrees) versus Group 2 (12.3 degrees) (p=0.35), nor was there a significant difference between postoperative Bohler angles between Group 1 (30.1 degrees) and Group 2 (27.0 degrees) (p=0.09). Similarly, preoperative Gissane angles for Group 1 (128.4 degrees) and Group 2 (134.5 degrees) (p=0.17) and postoperative Gissane angles for Group 1 (116.7 degrees) and Group 2 (118.8 degrees) (p=0.44) showed no statistically significant difference. There were a total of 3 wound complications in Group 1 versus 2 wound complications in Group 2 (p=0.76). There was no statistically significant difference in operative duration (p=0.97) or the number of unplanned returns to the OR between the two groups (p=0.77). Conclusion: When comparing the cannulated screw and plate and screw fixation techniques, there was no difference in restoration of the Bohler and Gissane angles. Both techniques had similar rates of postoperative complications and return to the OR. Our data suggests that fixation using cannulated screws alone versus sinus tarsi plate provide similar radiographic outcomes and risk of complications. The two techniques are also similar in terms of implant costs. Our results indicate that either technique effectively improves radiographic parameters.

Aerospace gas turbine disks operate in an environment of relatively high stresses caused by centrifugal forces and elevated temperatures. Because of the strong mechanical requirements and narrow specifications of such parts not only a correct, defect free final geometry is necessary, but also a defined microstructure. Even though the microstructure evolution during thermo-mechanical processing is well studied and understood for superalloys like IN718, the influences cannot easily be described analytically. Thus simulation tools are used to assure process stability and to optimize design parameters to meet the tough requirements in aerospace industries. Microstructure simulation of IN718 (and other materials) is well established at Bohler Schmiedetechnik GmbH & Co KG and appreciated by its customers. The advent of the newly developed nickel-base superalloy ATI Allvac® 718PlusTM led to extensive investigations and the development of an adapted microstructure model by Bohler Schmiedetechnik GmbH & Co KG and its research partners. Aim of this paper is a comparison of the microstructure evolution in IN718 and ATI Allvac® 718PlusTM during the thermo-mechanical treatment of turbine disks. Influences of process temperature, strain and strain rate on the final grain size are discussed by finite element simulations with a coupled grain structure model. Experimental results from trial forgings are compared with the outcome of the microstructure simulations.

The purpose of this study was to compare the postoperative radiographic outcomes of a plate and screw construct versus a cannulated screw construct when using the sinus tarsi approach for open reduction and internal fixation (ORIF) of calcaneus fractures. Methods: After institutional review board approval, records for all patients treated surgically at our institution for calcaneus fractures from 2012 to 2017 were reviewed. Inclusion criteria were intra-articular calcaneus fractures, patients aged 18 years or older, and use of the sinus tarsi approach. Exclusion criteria were open fractures and fractures with less than 6 weeks of postoperative weightbearing, which were excluded for radiographic outcomes. A total of 51 fractures underwent ORIF using cannulated screws alone (group 1), and 23 fractures underwent ORIF using a sinus tarsi plate (group 2). Sixty-one fractures (41 vs 20, respectively) met criteria for radiographic comparison. The primary outcomes of interest included pre- and postoperative Bohler and Gissane angles, wound complications, unplanned return to the operating room (OR), and cost comparison. Results: There was no statistically significant difference between preoperative Bohler angles for group 1 (14.4 degrees) versus group 2 (12.2 degrees) ( P = .44), nor was there a significant difference between postoperative Bohler angles for group 1 (30.1 degrees) versus group 2 (27.1 degrees) ( P = .14). Similarly, preoperative Gissane angles for group 1 (130.5 degrees) and group 2 (133.4 degrees) ( P = .54) and postoperative Gissane angles for group 1 (118.2 degrees) and group 2 (119.8 degrees) ( P = .44) showed no statistically significant difference. There were a total of 3 wound complications in group 1 versus 2 wound complications in group 2 ( P = .66). There was no statistically significant difference in operative duration ( P = .97) or the number of unplanned returns to the OR between the 2 groups ( P = .68). Based on the implants used at this institution, and depending on the number of screws used, the estimated cost range of a plate construct was $1070 to $1235, while the estimated cost range of a cannulated screw construct was $717 to $1264. Conclusion: When comparing the cannulated screw and plate and screw fixation techniques, there was no difference in restoration of the Bohler and Gissane angles. Furthermore, the amount of angular correction achieved by initial reduction showed no statistically significant difference between groups, and the amount of reduction lost between initial and final postoperative radiographs showed no statistically significant difference between groups. With regard to the 2 techniques, there was no statistically significant difference in rates of postoperative complications and return to the OR. Our data suggest that fixation using cannulated screws alone versus sinus tarsi plate provides similar radiographic outcomes and risk of complications. The 2 techniques were also similar in terms of implant costs. Our results indicate that either technique effectively improved radiographic parameters. Level of Evidence: Level III, retrospective comparative study.

Computed tomographic (CT) coronal images of the posterior talocalcaneal joint were compared with lateral radiographic views and intraoperative findings in 35 patients with 36 intra-articular calcaneal fractures. The severity of articular incongruity and rotational displacement of the posterior facet fragment were not well appreciated in coronal CT images of nine patients, although Bohler and Gissane angles were significantly decreased in lateral radiographs. This disparity between the radiographic and coronal CT scans can be explained by the intraoperative finding of rotation of the fractured central or lateral portion of the convex posterior calcaneal facet along a horizontal axis in the coronal plane.

ABSTRACT Introduction For the quantification of intra-articular fracture of the calcaneus, different types of angles have been mentioned in the literature. Of these, the Bohler's angle (BA) and the critical angle of Gissane (CAG) are used in common practice. However, there is a great paucity of literature conducted on the Indian populace to find out the normal range of BA and CAG. In this article, our objective is to homologate and confirm the normal range of BA and the CAG in the Indian population and compare their values to the published international reference values. Materials and methods It is an observational cross-sectional study done between 2017 and 2019. X-rays were anonymized by hiding the identification data. The BA and CAG were measured together and by two authors simultaneously. Each angle was measured two times. Two-sample T-test was chosen to compare the means of the values and the p value <0.05 was considered significant. Results Three hundred and eighty-four (n = 384) X-rays were studied. The average age of the patients was 41.73 years. The mean (°) of BA was 33.53 ± (SD) 6.78 (max = 46.35, min = 16.23) and the mean (°) of CAG was 115.90 ± (SD) 11.00 (max = 142.88, min = 90.22). Conclusion The distinct apprehension of the normal dispersion of Bohler's angle and the CAG is utmost needed for the clinical practice in a population (racial) group. So, it is praiseworthy to conduct a multicenter study over a large Indian population (considering racial variation) with inter and intra-rater reliability. A group of two electrodes produced by Bohler Company, Gridur 24 and B.M.N were taken as reference, in order to compare them in terms of the mechanical properties of the deposited metal, with the experimentally developed electrodes EICr12Fe and EIMn13SiFe. They were made determinations of the chemical composition of the deposited metal with the two types of electrodes, measurements regarding the hardness of the metal deposited, comparative determinations of the structure of the weld deposit, and of the welding penetration, determinations of the deposition yield and verification of the welding behavior.

Abstract:
Background: Open reduction and internal fixation is the standard surgical treatment of calcaneal fractures. However, it is associated with a high rate of wound problems. Objectives: In this study, we evaluated the clinical and radiologic outcomes, as well as the wound complication rates of sinus tarsi minimally-invasive approach in the treatment of intra-articular calcaneus fracture. Methods: In a retrospective study, 62 patients who were referred with an intra-articular calcaneus fracture and treated with a minimally-invasive sinus tarsi approach were included. The radiographic evaluations included the assessment of Bohler and Gissane angles before and after the surgery, as well as the height and length of the calcaneus. The clinical outcome was assessed with the American Orthopedic Foot and Ankle Society (AOFAS) questionnaire. Results: The Mean±SD age of the patients was 41.8±12.7 years. The Mean±SD follow-up of the patients was 21.3±10 ‎months. After 6 months, in 43 patients (69.3%) both Bohler’s and Gissane’s angles were significantly improved after the surgery (P=0.003 and P<0.001, respectively). The calcaneus height was significantly improved after the surgery (P=0.009), as well. The Mean±SD AOFAS score of the patients was found 79.6±7. Wound infection was seen in only 1 case (1.6%). Delayed wound healing occurred in 4 cases (6.4%). No other wound complication such as dehiscence and skin necrosis was recorded. Conclusion: Minimally-invasive sinus tarsi approach is an efficacious procedure for the treatment of intra-articular calcaneus fracture with a minimized rate of wound complications.

ABSTRACTBalanced mechanical properties are needed for TiAl low pressure turbine blades envisaged for use in new generation aircraft engines. However, thermomechanical processing of γ-TiAl based alloys is a challenging task due to a small “processing window”. Isothermal forging, as state of the art process for this class of material, results in high productions costs and lower productivity. Due to these facts Bohler Schmiedetechnik GmbH & Co KG has developed a higher efficient “near conventional” thermomechanical processing technology. Lower die temperature and processing at standard atmosphere as well as the use of standard hydraulic presses with higher ram speed result in a highly economical process. Subsequent heat treatment strategies can be used to tailor microstructure and, therefore, mechanical properties according to customer needs. The paper summarizes our effort to establish a near conventional forging route for the fabrication of TiAl components for aerospace industry.

Based on 2-dimensional CT scans, the fractures were categorised using the Sanders classification. There were 16 type-II, 20 type-III, and 12 type-IV fractures. Radiographs and Maryland foot scores were used for evaluation of the results at a mean of 38 (range, 26–66) months. Results. Anatomic reduction of the posterior calcaneal facet was achieved in 38 of 48 fractures. The Bohler and Gissane angles were restored to between 92 and 99% of normal, respectively. Despite this, the mean functional scores were 84 in type-II, 83 in type-III and 67 in type-IV fractures. Conclusion. Surgical results were superior in type-II and -III fractures. Type-IV fractures fared poorly, despite excellent restoration of calcaneal anatomy; subtalar arthrodesis should have been considered.

Cabbage (Brassica oleracea) is an important vegetable crop that is cultivated worldwide. Previously, we reported the identification of two dominant complementary hybrid lethality (HL) genes in cabbage that could result in the death of hybrids. To avoid such losses in the breeding process, we attempted to develop molecular markers to identify HL lines. Among 54 previous mapping markers closely linked to BoHL1 or BoHL2, only six markers for BoHL2 were available in eight cabbage lines (two BoHL1 lines; three BoHL2 lines; three lines without BoHL); however, they were neither universal nor user-friendly in more inbred lines. To develop more accurate markers, these cabbage lines were resequenced at an ~20× depth to obtain more nucleotide variations in the mapping regions. Then, an InDel in BoHL1 and a single-nucleotide polymorphism (SNP) in BoHL2 were identified, and the corresponding InDel marker MBoHL1 and the competitive allele-specific PCR (KASP) marker KBoHL2 were developed and showed 100% accuracy in eight inbred lines. Moreover, we identified 138 cabbage lines using the two markers, among which one inbred line carried BoHL1 and 11 inbred lines carried BoHL2. All of the lethal line genotypes obtained with the two markers matched the phenotype. Two markers were highly reliable for the rapid identification of HL genes in cabbage.

Paediatric calcaneal fractures are rare injuries usually managed conservatively or with open reduction and internal fixation (ORIF). Closed reduction was previously thought to be impossible, and very few cases are reported in the literature. We report a new technique for closed reduction using Ilizarov half-rings. We report successful closed reduction and screwless fixation of an extra-articular calcaneal fracture dislocation in a 7-year-old boy. Reduction was achieved using two Ilizarov half-ring frames arranged perpendicular to each other, enabling simultaneous application of longitudinal and rotational traction. Anatomical reduction was achieved with restored angles of Bohler and Gissane. Two K-wires were the definitive fixation. Bony union with good functional outcome and minimal pain was achieved at eight-weeks follow up. ORIF of calcaneal fractures provides good functional outcome but is associated with high rates of malunion and postoperative pain. Preservation of the unique soft tissue envelope surrounding the calcaneus reduces the risk of infection. Closed reduction prevents distortion of these tissues and may lead to faster healing and mobilisation. Closed reduction and screwless fixation of paediatric calcaneal fractures is an achievable management option. Our technique has preserved the soft tissue envelope surrounding the calcaneus, has avoided retained metalwork related complications, and has resulted in a good functional outcome.

Category: Trauma Introduction/Purpose: To compare clinical outcome of Sanders type IV intra-articular calcaneal fracture treated with open reduction and internal fixation (ORIF) versus ORIF and primary subtalar arthrodesis (PSTA). Methods: Between March 2003 and November 2013, 22 patients with 22 Sanders type IV intra-articular calcaneal fractures were included in this study. Of these, 11 feet were treated with ORIF (ORIF group), 11 feet were treated with ORIF and PSTA (PSTA group). The mean follow-up periods was 34.6 months (range, 18-72 months). Clinical outcomes were assessed along with the American Orthopaedic Foot and Ankle Society’s Ankle-Hindfoot Scale (AOFAS score), the Visual Analog Scale Pain score (VAS score) at 6-, 12 month, and last follow-up. The patient satisfaction, returning to previous occupation and postoperative complications were also investigated. Results: Complete bone union were achieved in all patients. No statistical difference was found between the results for ORIF compared with PSTA: the mean preoperative Bohler angle were, respectively, -3.2±9.2 and -6.6±12.7 degrees (p=0.475); the mean last follow-up AOFAS scores were 73.8±14.9 and 80.5±4.6 (p=0.795); the mean VAS last follow-up VAS scores were 29.7±20.6 and 23.4±10.0 (p=0.986). Secondary subtalar arthrodesis were conducted in 5 patients (45.5%) of ORIF group due to subtalar osteoarthritis symptom within 2 years, postoperatively. Conclusion: We were unable to demonstrate a significant difference of clinical outcomes between ORIF and PSTA however, the patient satisfaction was higher in the PSTA group. PSTA may be considerable choice for patients who need fast recovery to daily activity and to prevent the need for secondary subtalar arthrodesis.

The aim of the present study is to determine the outcome of the patients who underwent minimally invasive screw fixation for calcaneal fracture in our institute.</p><p class="abstract"><strong>Methods:</strong> The present study was performed in 31 patients who underwent closed reduction and fixations with screw in this institute for the duration of 2 years were included in this study. Present study was conducted to determine the functional outcome of patients with calcaneal fractures treated with minimally invasive fixation of fractures with cancellous screws after indirect reduction with JESS or manual reduction or with Steinmen pin.<strong></strong></p><p class="abstract"><strong>Results:</strong> The average age of the patients was 44.14 years, ranging from 16 to 74 years, 28 patients were males and 3 were female. The AOFAS score at last follow up was 78.05 this improvement was also significant statistically (p&lt;0.05). The mean Bohler angle at the last radiographic evaluation was 32.87°. The complications noted were proud implant with superficial wound infection in one, deep infection in 2 patients and delay in wound healing in 2 patients, persistent pedal edema in 2 patients.</p><p><strong>Conclusions:</strong> ORIF of calcaneal fractures is associated with high skin complications and comorbidities. Percutaneous screw fixation has fewer skin complications and can have satisfactory functional outcome. JESS or similar external fixator can be used intraoperatively for achieving reduction through ligamentotaxis.</p>

El CDT-ASTIN formuló este proyecto, incorporando una tecnología de punta y la única planta piloto del país para su transferencia al sector productivo y su aplicación en la Formación Profesional que imparte el SENA. Con el objetivo de incrementar la eficiencia y vida útil de las herramientas utilizadas en los diferentes procesos de manufactura de las empresas participantes del proyecto, se fabricaron, mediante un estudio experimental, recubrimientos de nitruro de titanio (TiN) sobre probetas de acero tipo herramienta. Para ello se seleccionaron aceros de las casas fabricantes más relevantes en el medio industrial colombiano, específicamente el acero Thyrodur T-2510 de Ia firma Thyssen, el acero K-460 de Bohler y el acero RUS 3 de la firma Rochling. Posteriormente se efectuaron recubrimientos del mismo material sobre los aceros AISI D3. H13. P20, 420 y M2. Las probetas recubiertas fueron sometidas a diferentes ensayos de caracterización, donde se determinaron propiedades importantes como la dureza, la adherencia, la composición, la morfología y el espesor. Para tal fin se utilizaron técnicas de caracterización tales como la prueba Calote, la prueba de Rayado, el método de Rockwell C, la Perfilometría, la Difracción de Rayos X, la Microscopia Electrónica de Barrido y la Microscópica de Fuerza Atómica. Estas pruebas se realizaron en alianza y colaboración con los laboratorios de Física del Plasma de la Universidad Nacional de Colombia sede Manizales, el Laboratorio de Recubrimientos Duros del Departamento de Física de la Universidad del Valle y el Laboratorio de Metalografía del CDT-ASTIN. Una vez optimizados los parámetros de deposición del TiN se recubrieron las herramientas aportados por las empresas (207 piezas) y se sometieron a las respectivas pruebas de campo, lográndose un incremento de su vida útil que osciló entre 50 y un 600% con una reducción de los costos de producción de más del 40%.

Bohses Neuerungen etwa in der Darstellung der Liebe sowie seine stilistischen Innovationen, so die These, konnten sich durch das Verharren in bewährten romanesken Traditionen nicht richtig entfalten.

Our aim is to evaluate the functional outcome of calcaneum Locking compression plate in patients with intraarticular calcaneal fractures with Bohler’s angle &lt;20 degree who are admitted in the department of Orthopaedics, Government Medical College, Kottayam.</p><p><strong>Methods:</strong> In this study, 41 patients with 42 intraarticular calcaneal fractures were operated on with locking compression plate through lateral approach during the period of July 2015 to December 2016. Radiological evaluation done with X-rays. Bohler’s angle was measured from lateral view and those patients with angle &lt;20<sup>0</sup> were selected for study. Patients were followed up clinically and radiologically for 24 weeks. Radiological assessment was done by Bohlers angle. Functional outcome was assessed using American Orthopaedic Foot and Ankle Society (AOFAS) scale.<strong></strong></p><p class="abstract"><strong>Results:</strong> At 24 weeks follow up, 90.5% of the study population had excellent to good functional outcome and 9.5% had fair and none had poor result. All patients had stable hind foot with all having good dorsiflexion and plantar flexion. But some patients had limited inversion and eversion. The mean postoperative Bohler’s angle was 30.02±3.97.</p><p class="abstract"><strong>Conclusions:</strong> Open reduction and internal fixation with locking compression plate gives sound functional outcome, if the surgery is well timed. Anatomical articular reduction especially of the posterior facet to be achieved and restoration of Bohler’s angle to normal range to be attempted.</p>

Out of 34 fractures in 30 patients, 22 fracture calcaneum were joint depression type and 12 fractures were tongue type</span>.<strong></strong></p><p class="abstract"><strong>Results:</strong> <span lang="EN-IN">In 17 cases normal Bohler angle (i. e. 20<sup>0</sup>-40<sup>0</sup>) was achieved. There were 3 cases of post operative delayed wound healing, 2 cases of superficial wound infection and 1 case of sural nerve complication in the form of loss of sensation on lateral aspect of foot. According to American Orthopaedic Foot And Ankle Society (AOFAS) score, 13.33% of the patients had excellent results, 63.33% had good results, 13.33% had fair result and 10% had poor results. </span></p><p class="abstract"><strong>Conclusions:</strong> <span lang="EN-IN">Operative fixation of displaced intra-articular fractures of the calcaneus by reconstruction locking plate, restores the calcaneal height, allows early mobilization and weight-bearing, and maximizes the chances for good joint function</span><span lang="EN-IN">.</span></p>

Category: Trauma; Hindfoot Introduction/Purpose: Fractures of the calcaneus are life-changing events with a major socioeconomic impact from lost productivity. Traditional operative treatment with the extensile lateral approach has shown relatively high rates of wound complications. Less invasive approaches can improve fracture alignment and decrease wound healing complications. The purpose of this study is to report our experience with the sinus tarsi approach in treating calcaneus fractures. Methods: We retrospectively identified patients at our institution treated with a limited sinus tarsi approach for calcaneus fractures from 2009-2018. Demographic and radiographic data were collected including: age, sex, mechanism of injury, occupation, presence of diabetes, smoking status, Sanders classification, Bohler and Gissane angles. Postoperatively, we recorded the presence of complications, return-to-work time, and radiographic measurements. Results: Our analysis included 105 fractures in 100 patients: 86% males, 42% smokers, 4% diabetics, with an average body mass index of 26.5. The fractures were: Sanders type 2 (32%), type 3 (48%), type 4 (18%), and 2% were a tongue-type variant. Preoperatively 38% of fractures displayed a negative angle, 50% had an angle 0-20 degrees, and 12% over 20 degrees; postoperatively 13% had an angle 0-20 degrees, and 87% had an angle over 20 degrees. Of patients working prior to the injury, 72% had returned to work by 6 months, and 89% by 12 months. The wound complication rate was 12% (12/100), with only 2% (2/100) requiring additional procedures. There was no significant difference in wound complication rates in smokers versus nonsmokers (11.9% vs 12.2%, p=0.55). Conclusion: The limited sinus tarsi approach for depressed calcaneus fractures allows radiographic restoration of calcaneal height with a low rate of wound complications, even amongst active smokers. To the authors’ best knowledge, this is the largest published case series of calcaneus fractures treated with the sinus tarsi approach. Further follow up is needed to determine the success of this approach in mitigating long term complications.

Our study aimed to contribute paucity of data available for these rare fractures and help to propose a preferred method of treatment.</span></p><p class="abstract"><strong>Methods:</strong> A hospital based study carried out on 20 patients over period of 2 year with closed displaced intra-articular tongue type of calcaneal fracture treated with percutaneous fixation and outcome was evaluated using clinical, radiological criteria and American Orthopaedic Foot &amp; Ankle Society (AOFAS) score. All patients were followed up for minimum 9 months with mean follow up of 13.5 month.<strong></strong></p><p class="abstract"><strong>Results:</strong> All patients had evidence of union within 2 month follow up. Mean correction in Bohler angle and Gissane angle was 23.2° and 25.4° respectively at final follow up. Mean change in heel height was 3.15 mm and width change was 2.75 mm compare to opposite side. Mean eversion inversion arch was 28° and near normal ankle range of motion. Functional outcome calculated on bases of AOFAS score revealed 18 patients (90%) had good results and 2 patients (10%) had fair results. Mean AOFAS score was 81.25. Complications were reported in three patients, one had signs of peroneal tendinitis and two had persistent pain and heel widening<span lang="EN-IN">. </span></p><p class="abstract"><strong>Conclusions: </strong>We believe percutaneous fixation should be considered as a preferred method for mild to moderately displaced tongue type of intraarticular calcaneus fractures. It potentially allows anatomical fracture reduction with lesser complications and good functional outcome<span lang="EN-IN">.</span></p>

<p class="abstract"><strong>Background:</strong> Tibial plateau fractures constitute about 1% of all fractures and complex bicondylar fractures constitute 30% of all Tibial plateau fractures. Minimally Invasive Percutaneous Plate Osteosynthesis [MIPPO] is a method of biological fixation in which a plate is percutaneously inserted and fixed at a distance proximally and distally from the fracture site. By minimal exposure, this helps in the preservation of the essential fracture hematoma, minimal soft tissue dissection, avoidance of periosteal stripping and providing an adequate fixation. The objective of the study was to analyze the functional outcome of proximal Tibial fractures treated with MIPPO technique. The method of fixation shall be evaluated for the time period required for the patients to return to active work following surgery.</p><p class="abstract"><strong>Methods:</strong> In our study, all 18 patients with proximal Tibial fractures underwent definitive fixation by MIPPO technique. All our cases underwent initial stabilization as per the ATLS guidelines. Patients with closed Tibial plateau fractures associated with a tense haemarthrosis underwent aspiration of the joint under aseptic precautions. The limb was immobilized either in an above knee slab or through skeletal traction using a distal Tibial or calcaneal pin traction on a Bohler Braun splint until definitive fixation was carried out. In cases complicated with excessive swelling and blistering, definitive fixation was delayed until the swelling/ blistering subsided.<strong></strong></p><p class="abstract"><strong>Results:</strong> The post-operative results were designated as excellent, good, fair and poor according to pain, walking capacity, the range of motion and stability of the knee using Rasmussen’s grading system. In our study, the average functional knee score was 22.89. Rasmussen’s score does not consider articular congruity while assessing the functional outcome of the knee.</p><p class="abstract"><strong>Conclusions:</strong> MIPPO technique gives good to excellent results even in high energy Tibial condyle fractures [72.22% cases in our study]. Our patients were able to achieve a good functional range of movement, averaging 120 degrees. [Krettek et al – 124 degrees]. Those who were treated with early fixation and early mobilization were found to have a better functional outcome irrespective of the fracture type. No secondary bone grafting was required. </p>
