Full text
Abstract:
Standards for the Management of Open Fractures provides an evidence-based approach for the management of open fractures, focusing on lower limb injuries. It builds on and expands the National Institute for Health and Care Excellence (NICE) Guidelines to provide a practical approach with supporting evidence. The new edition has been extensively updated and expanded to include key aspects of management, ranging from setting up an orthoplastic service, through to dealing with bone and soft tissue injuries, including in young and older people, patient rehabilitation and psychological care, blast injuries, as well as complications such as infection. The book provides an important resource for orthopaedic and plastic surgeons managing patients with these challenging injuries.

Full text
Abstract:
♦ After forearm and digital injuries, tibial and ankle fractures are the commonest fractures in the immature skeleton and the majority of these involve the diaphysis or ankle♦ Compared to the morbidity seen in adults these are relatively forgiving injuries in children as the healing rate of bone and soft tissues is rapid and remodelling will occur♦ It is wise, however, to guard against overconfidence in the remodelling potential of certain injuries; for example, angulated mid-diaphyseal fractures, rotational malalignment, and metaphyseal fractures within 2 years of skeletal maturity♦ Children will tolerate manipulative/cast treatment better than adults as the duration of treatment is usually shorter and rapid rehabilitation is almost the norm with or without physiotherapy♦ Postfracture overgrowth does occur but is less than that following femoral fractures and seldom clinically significant (over 10mm)♦ Isolated fibular fractures are of minor importance but need to be taken into account in managing complex injuries involving the distal tibia♦ It is convenient to discuss injuries according to three anatomical sections: proximal, diaphyseal, and distal.

Abstract:
The entry of the USA into the war changed the dynamics of life at the American embassy in Grosvenor Square. Edith and E. F. Penrose became directly involved in planning the international conferences which would shape the postwar world, including Hot Springs, and dealing with the immediate problems of the end of the war such as the Morgenthau Plan. Edith and E. F. Penrose married in 1944 and Edith bore a son, Trevan, in July 1945. Edith’s brothers Jack and Harvey both had distinguished careers in the US Air Force in North Africa and Italy before Jack was shot down in early 1945. Both Edith and Penrose were directly involved in the creation of the United Nations Relief and Rehabilitation Administration which dealt with refugee issues, and in supporting Winant and Eleanor Roosevelt during the formation of the United Nations and the first General Assembly in London in 1946.

Narouze, Samer N. Cervical Sympathetic Block: Fluoroscopy. Oxford University Press, 2016. http://dx.doi.org/10.1093/med/9780199908004.003.0027.

Full text
Abstract:
In those patients with significant sympathetically maintained pain, repeated blocks may provide a therapeutic value and help facilitate physical therapy and rehabilitation. Cervical sympathetic blocks have been traditionally performed by using surface landmarks, however imaging-guided blocks are strongly recommended to avoid potential serious complications. Most preganglionic sympathetic efferents innervating the head, neck, and upper extremity either pass through or synapse at the stellate ganglion. This provides an ideal target for blockade of sympathetic innervation to the head, neck, and upper limbs. The stellate ganglion block can be performed at the C6 and C7 transverse processes. Fluoroscopy is a reliable method for identifying bony surfaces, which facilitates identifying the C6 and C7 transverse processes; however, this is only a surrogate marker, because the location of the cervical sympathetic trunk is defined by the fascial plane of the prevertebral fascia, which cannot be visualized with fluoroscopy.
