Abstract:
The opioid system of the brain is the major target for opiate drugs such as morphine and heroin, and has been implicated in processes such as pain, stress and reward. Many of these effects take place at the mu opioid receptor (mOR), which is distributed throughout the brain. Significantly, genetic polymorphisms at the mOR may confer a greater dopamine response to the reinforcing effects of alcohol, and it has been suggested that addiction per se may be associated with alterations to the opioid system. There is evidence for the potential efficacy of mOR antagonists (e.g. naltrexone) in reducing drug and alcohol relapse, increasing treatment retention and attenuating the subjective effects of substances of abuse. Medications with partial agonist activity at the kappa opioid receptor (e.g. nalmefene) may also confer an additional clinical advantage by reducing binging following relapse.

Abstract:
This chapter reviews the clinical syndrome of chronic multiple organ dysfunction (MOD) following acute critical illness. Chronic MOD, also referred to as chronic critical illness, occurs in patients who have survived the acute phase of their illness or injury but remain dependent on life support for weeks or months. This condition presents unique physiologic and metabolic abnormalities distinct from those encountered in the acute illness. These include neuroendocrine and immune dysregulation, ICU-acquired weakness, persistent respiratory failure, and brain dysfunction. The symptom burden for these patients is high, and long-term survival is limited for elderly patients and those for whom MOD persists for weeks. Comprehensive and systematic programmes will need to be designed and implemented involving bundled best-practice interventions in order to reduce the incidence and treat the consequences of chronic MOD.

Abstract:
Surgical resection of supratentorial brain tumors presents a unique set of challenges to the anesthesiologist. While symptoms from these lesions vary in presentation, increased intracranial pressure (ICP) is frequently part of the constellation and an important piece of anesthetic management. Along with maintenance of ICP, the goals of induction, maintenance, and emergence from these resections are to maintain cerebral blood flow (CBF) and cerebral metabolic rate and optimize neuroprotection. There is no single technique that is agreed upon in literature to encompass these goals, however a combination of IV anesthetics with low-dose volatile agents (0.5 MAC) is common in practice. Management of ICP is the hallmark of these cases since increased ICP may lead to ischemia, poor surgical visualization, and catastrophic herniation.
