Abstract. From 2002 to 2005 a FerryBox system was installed aboard two different ferries travelling between Cuxhaven (Germany) and Harwich (UK) on a daily basis. The FerryBox system is an automated flow-through monitoring system for measuring oceanographic and biogeochemical parameters installed on ships of opportunity. The variables were recorded in a time interval of 10–20 s, corresponding to a spatial resolution of about 100 m. The data set provides the parameters water temperature, salinity, dissolved oxygen and chlorophyll a fluorescence. There is a longer data gap between November 2002 and August 2003 in the time series due to a change of the vessel in October 2002. Abstract:
Objectives The objective of this study was to develop a test for the rapid (within 25 minutes) intraoperative detection of bacteria from synovial fluid to diagnose periprosthetic joint infection (PJI). Methods The 16s rDNA test combines a polymerase chain reaction (PCR) for amplification of 16s rDNA with a lateral flow immunoassay in one fully automated system. The synovial fluid of 77 patients undergoing joint aspiration or primary or revision total hip or knee surgery was prospectively collected. The cohort was divided into a proof-of-principle cohort (n = 17) and a validation cohort (n = 60). Using the proof-of-principle cohort, an optimal cut-off for the discrimination between PJI and non-PJI samples was determined. PJI was defined as detection of the same bacterial species in a minimum of two microbiological samples, positive histology, and presence of a sinus tract or intra-articular pus. Results The 16s rDNA test proved to be very robust and was able to provide a result in 97% of all samples within 25 minutes. The 16s rDNA test was able to diagnose PJI with a sensitivity of 87.5% and 82%, and a specificity of 100% and 89%, in the proof-of-principle and validation cohorts, respectively. The microbiological culture of synovial fluid achieved a sensitivity of 80% and a specificity of 93% in the validation cohort. Conclusion The 16s rDNA test offers reliable intraoperative detection of all bacterial species within 25 minutes with a sensitivity and specificity comparable with those of conventional microbiological culture of synovial fluid for the detection of PJI. The rise of Donald Trump has widely been seen as concurrent to the emergence of the “Alt-Right” that coalesces around intersecting themes of conservativism: White ethno-nationalist “race realism,” populism, misogyny, evangelical theocracy, border protectionism, and anti-liberalism. Media has been a key site of struggle in these developments, with attacks on mainstream media bringing into focus wider questions of truth and legitimacy in journalism. In particular, Trump’s rise has been synonymous with the heightened profile of the Breitbart News website, a purveyor of hyperpartisan, conservative political ideologies. In this article, we consider the place of Breitbart Sports within this dynamic political and media order. Our analysis of the lead-up to the 2016 Presidential election reveals the extent to which Breitbart Sports conveyed a vision of U.S. sport that promoted hard-right agendas in relation to U.S. global stewardship, veiled “race” reclamation discourses, media, immigration, social criticism, policing, sexual politics, and party politics. Breitbart Sports framing casts sport as a liberally infested cultural battleground, where conservativism is under threat. We conclude with a brief discussion about the role of new media in framing political exigencies and the role of sport in contemporary American society.

Growing up on New York City's Lower East Side of the 1950s there were no other Breitbarts to be found, so it was not surprising that I would be drawn to the heroic legend of my famous ancestor and namesake, Siegmund (Zishe) Breitbart. My parents were survivors of the Holocaust, born in Turka, a small shtetl in Galicia, Poland, not far from the Lodz birthplace of the “greatest Jewish strongman—the modern Samson.” Being related to the famous Jewish “Iron King” was, for me, a proud link to a legacy of strength, courage, and defiance in an environment scarred by the profound losses and overwhelming grief of my generation of “children of survivors.” In 1994, I attended the International AIDS Congress in Berlin, and was the first Breitbart, to my knowledge, to visit Zishe's grave site in a small Jewish cemetery in the former East Berlin. Zishe Breitbart was a looming figure of strength in my subconscious.

Ideations of corporeality are situated at the crux of "muscular Judaism" in early twentieth- century Europe. The sporting event was viewed as a battlefield for equalization. In the ideological context of Muskeljudentum, the apathy of Talmudjudentum (Talmudic Judaism) was replaced by exercise, in which the strengthening of the corporeal would rejuvenate the psychical. Jewish strongman Siegmund Breitbart capitalized on his masculine feats of strength and aesthetic appeal by creating public performances, which displayed not only militarized corporeality, but also provided a stage for the promotion of "muscular Judaism," through both symbolic and literal representations of Zionist ideology. Breitbart reappropriated masculine Jewish corporeality, embodied corporeal notions of reciprocity at the core of Muskeljudentum, and found individual agency through the militarized aesthetic and motion of his body.

Frankl's existential therapeutic approach, called logotherapy, serves as the underlying theoretical framework to this intervention.Development: Following the presentation of the context and the development of the intervention, its content is described.Conclusion: A brief description of the ongoing randomized controlled trial testing the intervention is then provided. This study examines the international activist movement known as Sleeping Giants, a social-media “campaign to make bigotry and sexism less profitable” (Sleeping Giants, n.d.). The campaign originated in the US with an anonymous Twitter account that enlisted followers in encouraging brands to pull their online advertising from &lt;em&gt;Breitbart News&lt;/em&gt;. The campaign achieved dramatic success and rapidly spread to regions outside the US, with other anonymously run and loosely allied chapters emerging in 15 different nations (as well as a regional chapter for the EU). Many of these were initially created to take on &lt;em&gt;Breitbart&lt;/em&gt; advertisers in their home countries, but in a number of cases they subsequently turned their attention to disrupting financial support for other far-right news media in—or impacting—their home countries. Based on interviews with leaders of eight Sleeping Giants chapters, as well as the related UK-based Stop Funding Hate campaign, this study examines the Sleeping Giants campaign with respect to its continuity with media activism of previous eras, while also seeking to understand its potential as one of the first high-profile activist campaigns to grapple with the impacts of programmatic advertising on the news ecosystem. In particular, we consider how the campaign’s interventions speak to the larger debate around the normative relationship between advertising and the performance of the news ecosystem.

The research examines the news on climate change in different media, through the analysis of agenda setting and framing, in the context of a construction of media discourse. The role of the media has been relevant in the symbolic struggle of climate change images. The polarized public opinion on climate change in the USA, which has led the Trump government to withdraw from the Paris Agreement, as well as the revocation of environmental policies, is analyzed by the coverage that media with Republican and Democratic political tendencies gave to the climate crisis during the 12 days of the 2019 Climate Summit. The 189 news articles broadcast by Fox News, Breitbart, CNN, and the New York Times were identified, analyzed, and contrasted. The results reveal that media with a Republican political tendency were the only ones that broadcast denial news of climate change. Breitbart reported the largest number of news items throughout the sample, mostly denialists, at 71%, using tactics related to the spectacularization of the climate phenomenon, ad hominem attacks on ecologists and politicians, the connection between environmental initiatives and “eco-fascism” or the “radical left”, as well as use of the half-truth fallacy and questionable sources associated with the fossil fuel industry. Fox News practically did not address the issue during the summit. The Democratic political tendency media did not report any kind of denial news; their information and opinions communicated environmental initiatives and climate change consequences.

Da die respiratorischen Infektionen schwerwiegende gesundheitliche Folgen haben können, aber häufig zu Beginn nur geringe Symptome verursachen, ist eine Frühdiagnostik und Frühbehandlung enorm wichtig. Neben der Diagnostik mit Nasen- oder Rachenabstrichen für virologische und bakteriologische Untersuchungen, ist die zeitnahe Behandlung mit Virostatika, Breitband-Antibiotika und oft auch Immunglobulinen empfohlen. Die Behandlungsdauer ist oftmals länger als bei Immunkompetenten. Als vorbeugende Massnahmen sind die Grippeimpfung und präventivhygienische Massnahmen im Alltag empfohlen.

Die Kontamination und Infektion mit Methicillin resistentem Staphylococcus aureus (MRSA) stellt ein zunehmendes interdisziplinär relevantes medizinisches Problem insbesondere bei der Behandlung chronischer Wunden dar. Es ist daher zu fordern, dass bei Patienten mit chronischen Wunden regelmäßig bakteriologische Abstriche erfolgen, damit MRSA-kolonisierte und -infizierte Patienten identifiziert, konsequente Hygienemaßnahmen und Therapien eingeleitet werden können und sich medizinische Einrichtungen nicht zu Vektoren weiterer Epidemien entwickeln. Insbesondere bei Infektionen sollte die Einleitung einer adäquaten Therapie gezielt nach mikrobiologischer Diagnostik durchgeführt werden und bei dem systemischen rationalen Einsatz von Antibiotika die spezifischen Resistenzen berücksichtigen. Der unkritische Einsatz von Breitband- und Reserveantibiotika fördert lediglich die bereits kritische Entwicklung weiterer Resistenzen.

After years of neglect, care at the end of life is receiving increasing attention and concern. It is then that the body is consumed by a progressive and mortal illness, and the person must cope not only with the bodily symptoms, but also with the existential crisis of the end of life and approaching death. As the body suffers, the mind is indeed “commanded … to suffer with the body,” as Shakespeare so well described. Thus, suffering near the end of life encompasses both the mind and the body. Providing optimal symptom relief and alleviation of suffering is the highest priority. However, evidence suggests that we continue to fall far short of this ideal (American Society of Clinical Oncology, 1996; Cassem, 1997; Cassel &amp; Foley, 1999; Carver &amp; Foley, 2000). Although pain management guidelines have been the most widely disseminated, we know that many patients continue to suffer not only from pain, but other troubling physical symptoms in their final days (American Nursing Association, 1991; Carr et al., 1994; American Pain Society, 1995; American Academy of Neurology, 1996; American Board of Internal Medicine, 1996; Ahmedzai, 1998). Despite clear advances in the identification and treatment of psychiatric disorders, we continue to underdiagnose and undertreat the debilitating symptoms of depression, anxiety, and delirium in the final stages of life (Carroll et al., 1993; Hirschfeld et al., 1997; Holland, 1997, 1998, 1999; Breitbart et al., 2000; Chochinov &amp; Breitbart, 2000). And, beyond these physical and psychological symptoms, we fall even shorter of our goals of alleviating the spiritual, psychosocial, and existential suffering of the dying patient and family (Cherny &amp; Portenoy, 1994; Cherny et al., 1996; Fitchett &amp; Handzo, 1998; Karasu, 2000). And this is in spite of the ethical imperative “to comfort always” (Pellegrino, 2000).

Untersucht wurden die Retest-Reliabilität (N = 103) und die Innere Konsistenz (N = 985) an jeweils unausgelesenen Schülerstichproben (6-17 Jahre) sowie die Diskriminationsfähigkeit des Gesamtscore und der Problem-Skalen zwischen kinder- und jugendpsychiatrischen Patienten und parallelisierten unbehandelten Schülern (N jeweils 310). Befriedigende Gütekriterien (Reliabilität und diskriminative Validität) wiesen der Gesamtscore und die Breitband-Skalen Externalisierung (Ausnahme: 12-17jährige Jungen) und Internalisierung auf. Von den Subskalen traf dies nur auf die Skalen Aggressivität (mit Einschränkung) und Hyperaktivität zu. There are many words of praise for people hailed as “great conservatives, writers, and pundits” such as Rush Limbaugh, Ann Coulter, and Andrew Breitbart (to whom the book is dedicated). On the other hand, the authors believe that even the people they describe as good, honest, unfailingly truth-seeking conservative talk radio hosts are just too timid to withstand the oppressive, unscrupulous importunities of the liberal left. Welcome, again, to the culture wars.

Threat and uncertainty were the two most frequent emergent issues for our Spanish patients.Significance of results:The implementation of MCGP in Spanish patients validated the themes proposed by Breitbart and colleagues' foundational work on MCGP and also suggested new issues relevant to patient well-being (classified as “emergent themes”). Taking our findings into account, we propose that these new themes be considered in the Spanish adaptation of MCGP as well as in future adaptations of this form of psychotherapy in treating Latin American patients.

One of the main themes of alternative right-wing media is a strong anti-immigrant approach, which has allegedly intensified a radical and polarized world-view throughout Europe and the United States. In this article, by comparing two right-wing news sites, I examine whether commonalities in their reporting can be discerned at a transnational level. The focus is on the US-based Breitbart London and the Finnish-language MV-lehti, both founded in 2014. The comparative study approaches the research data by utilizing the method of discourse narratology to examine the similarities and differences between the two in terms of their ideological parlances. The results show that there are indisputable commonalities, with parlances that seek to undermine liberal democracy as an outspoken opponent to strengthen the homogeneous battlefront. Accordingly, right-wing news sites in Europe adhere to the shared ideology, leaning on a strong confrontation between western and Islamic countries.

Patients with oncological diseases, especially palliative care patients, suffer from physical and psychological difficulties. The quality of life of such patients is bad, they do not have purpose to live and they feel anxiety and distress. In 1959 Victor Frankl wrote the book Man’s Search for Meaning in which he stated that the driving force of human life lay in the ability to discern the meaning of faith and spirituality. Inspired by Frankl’s ideas, the American psychiatrist William Breitbart with colleagues have developed both an individual and group model of Meaning-Centered Psychotherapy. Studies show that this therapy helps patients to cope with distress, to discover the meaning of life in palliative care patients, and to find the strength to look at life positively; also, it relieves the symptoms of illness. The Meaning-Centered Psychotherapy is integrated in various countries and has recently been initiated for palliative patients in Lithuania. The individual Meaning-Centered Psychotherapy was used in the case reviewed in this paper.

Well, it's finally happened. I've been attacked in the scientific literature (Salander, 2006). Well, just to clarify, it wasn't a personal attack, but rather an attack of the highest and most noble order: an attack on the scientific rigor of the conceptual underpinnings of my recent work on meaning and spirituality in advanced cancer patients (e.g., Breitbart, 2002). To be honest, and a bit less grandiose, it was not an attack against me alone, but rather a set of critical comments aimed at a growing number of investigators who have been publishing papers (a 600% increase over the past 10 years) dealing with religious or spiritual aspects of life-threatening illness (Stefanek et al., 2005). The critical comments by Dr. Par Salander of the Department of Social Welfare, Umea University, Sweden, in a recent issue of the journal Psycho-oncology (Salander, 2006) challenge the need for and the validity of the concept of spirituality.

The media play an important role in shaping the collective memory of their users. Popular movies, TV shows or commemorative newspaper texts influence the ways in which people remember and forget. Many scholars have attempted to describe this connection; however, little attention has so far been paid to alternative media. This article aims to analyse the features of the collective memory constructed by the media associated with the so-called alt-right (alternative right) movement in the United States. I argue that far-right media produce an ethnically exclusive collective memory, which consequently aims to counter the mainstream collective memory. The findings of this study come from the critical analysis of how the New York Times and Breitbart News engaged in a nationwide discussion on the Confederacys legacy that ensued in August 2017 after the decision to remove the Robert E. Lee monument in Charlottesville, VA and the mass protests that soon followed.

Abstract The alt-right movement dates from 2008 when white supremacist Richard Spencer invented the term to identify contemporary right-wing and far-right socio/political movements. The movement relies on mass media, communicating graphically and symbolically through “trolls,” “tropes,” and “memes.” The “Sadomasochist trope” valorizes aggressive actors like Donald Trump and Vladimir Putin, while demonizing “passive” individuals like Hillary Clinton and Barak Obama. Trolls communicate through memes, single-frame or short video phrases matched with photos and cartoons, to attract online audiences. When not attacking liberals and progressives, alt-right memes turn on traditional conservatives. The alt-right community maintains it is “under assault” in today’s politically correct, overly secularized, culturally diverse society. However, Donald Trump elevated alt-right icon and former Breitbart ceo Stephen Bannon to chief advisor, providing the alt-right movement access to the highest government levels. Will alt-right organizations continue their recent expansion, or will the public lose interest in the movement?

This article explores how successful digitally native activism generates social change. Digitally native movements are initiated, organized, and coordinated online without any physical presence or pre-existing offline campaign. To do so, we explore the revelatory case of Sleeping Giants (SG)—an online movement that led more than 4,000 organizations to withdraw their programmatic advertising spend from Breitbart, a far-right publisher. Analyzing 3.5 million tweets related to the movement along with qualitative secondary data, we used a mixed method approach to investigate the conditions that favored SG emergence, the organizing and coordinating practices of the movement, and the strategic framing practices involved in the tuning of the movement’s language and rhetoric toward its targets. Overall, we contribute to research on online movements and shed light on the pivotal role of peer production work and of language in leading an impactful online movement that aimed to counter online disinformation and hate speech.

Unbestritten ist, dass von der Digitalisierung der Kabelfernsehnetze in Deutschland wichtige Impulse für die Medienindustrie und angrenzende Bereiche ausgehen können. Andere Länder haben es vorgemacht; die Umstellung von analoger auf digitale Übertragung im Kabel hat ein vielfältiges neues Angebots- und Dienstespektrum hervorgebracht, das neben neuen Sparten- und Premiumfernsehprogrammen auch interaktive TV-Dienste und Internetbasierte Angebote wie Breitband-Internet und Voice over IP (Telefonie) umfasst. Warum aber kommt die Digitalisierung des Kabels in Deutschland seit Jahren nicht wesentlich voran? Woran liegt es, dass das deutsche Kabelfernsehnetz noch immer hinter seinen technischen Möglichkeiten zurückbleibt? Warum sind die bisherigen Digitalisierungsanläufe gescheitert und welche Aussichten haben die aktuellen Digitalisierungsbestrebungen? In diesem Aufsatz werden die aktuellen Konfliktlinien im deutschen Kabelmarkt dargestellt und die Schritte aufgezeigt, die die Akteure gemeinsam gehen müssen, um das gemeinsame Ziel der vollständigen Digitalisierung zu erreichen. AbstractIn this article, I explore how, from 1977 through 2009, the conservative Christian media empire, Focus on the Family, acted as a model for and a creator of alternative news long before the 2016 election. In particular, since 1977, Focus linked proper Christianity with recognition of a world of hazards by defining danger as those people and institutions who refused to submit to God, especially feminists, secular universities, and the welfare state. Through the creation of a closed-media network, Focus taught Christian conservatives to see the mainstream news as undermining biblical Truth by espousing stories that supported postmodern relativism over God's singular truth. Simultaneously, Focus generated its own news sources to fill the vacuum left by the mainstream with stories highlighting the political and social structures needed to support the Focus-defined traditional family. Soon, other conservative media outlets began using these frameworks to attract listeners and to add veracity to their stories. Although mainstream media portrayed Focus as passé by 2009, I argue that the model that Focus developed led seamlessly to the creation of Fox News and, later, to the formation of internet communities around outlets such as Breitbart and to the believability of Russian bots.

This past fall (October 2005), my good friend and colleague Lewis Cohen, M.D., of Baystate Medical Center, helped organize a unique conference (cosponsored by the Smith College of Social Work) that represented an early attempt by psychosocial palliative care clinicians, ethicists, and legal experts to grapple with the issues raised by the Terri Schiavo case, which had so captured the attention of the nation and raised tremendous disagreement and debate. As editor-in-chief of Palliative &amp; Supportive Care, I was, of course, quite thrilled when Dr. Cohen proposed the possibility of our journal publishing the main papers from this unique and timely conference. As the readers of Palliative &amp; Supportive Care will recall, this journal has expressed the opinion that the case of Terri Schiavo represented a watershed event in palliative care practice and research (Breitbart, 2005), and we called for more research into the experience of patients who die of dehydration. Of course, the issues raised by the Schiavo case are so much broader and do, in fact, range from palliative care practice and research to ethics and spiritual issues to legal issues regarding decision making at the end of life.

ABSTRACTObjective:Delirium is the most common neuropsychiatric complication of medical illness, a medical emergency that needs to be identified and treated vigorously. Delirium is too frequently underdiagnosed and untreated in the medical setting, which leads to increased morbidity and mortality, interference in the management of symptoms such as pain, an increased length of hospitalization, increased health care costs, and distress for patients and their caregivers (Inouye, 2006; Breitbart et al., 2002a, 2002b). In this article, we present an update of the use of antipsychotics in management of delirium based on the available literature and our own clinical experience.Methods:We reviewed the current literature on the role of antipsychotics in the management of delirium using standard computer-based search methods (e.g., PubMed).Results:Antipsychotic medications, including the new atypical antipsychotics, have been demonstrated to effectively manage a wide spectrum of the symptoms of delirium and are an essential component in the multimodal approach to managing delirium.Significance of results:The standard approach to managing delirium includes identification and elimination of factors contributing to the delirium in addition to pharmacological and nonpharmacological treatment interventions (Trzepacz et al., 1999). Newer atypical antipsychotics can play an important role in the management of the symptoms of delirium.

Abstract The organizers of the Anti-Milo Toolkit aimed to contribute to a broader counter-movement that would make it easier for university campuses to challenge and de-platform white supremacist and fascist speakers sheltered under the auspices of “free speech.” This toolkit gathers info-tracts, syllabi, flyer templates, and other activist materials collected and widely distributed across campuses in preparation to protest Breitbart journalist Milo Yiannopoulos's 2016 “Dangerous Faggot Tour.” Yiannopoulos was slated to speak at thirteen college campuses in support of an alt-right platform founded upon the weaponization of “free speech,” xenophobia, and transphobia. In addition to providing materials for protest, the authors of the “Anti-Milo Toolkit” take critical aim at appropriation of liberal-academic vocabularies by right-wing groups and Yiannopoulos's history of outing trans and undocumented students at his events, and call for widespread action against the spread of violent rhetoric targeting marginalized communities in order to maintain the university as a space of sanctuary. Because of its accessibility via digital channels and its wide range of short and readable pieces written in a variety of styles, and because of the collective's wide network of organizing connections, the kit circulated very broadly.
