Abstract:
Breast cancer is the most common cancer in women, with an estimated number of new cases not less than 1,050,346 per year. Meanwhile, in North Sumatra in 2019, breast cancer occupied the highest position or 856 cases/incidents of the total number of cancer patients. Efforts to control breast cancer have been carried out by the government of the Republic of Indonesia in particular through the early detection program or BSE. BSE is breast self-examination which aimed to determine the presence or absence of breast cancer in women. This examination performed using a mirror and carried out by women aged 20 years and over. The main indication of BSE was to detect the occurrence of breast cancer by observing the breast from the front, left and right side, whether there were lumps, changes in skin colour, noisy putting and discharge or pus and blood. This research method was descriptive qualitative with 7 informants (subject teachers). This study aimed to determine breast self-examination (BSE) conducted by the Methodist 8 Medan high school teacher. The results showed that all teachers had good knowledge and attitudes about BSE but had fewer actions about BSE. The conclusion of good knowledge and attitudes that teachers have is obtained by obtaining information about BSE through electronic media, namely the internet and fewer actions regarding BSE due to a lack of knowledge to carry out BSE appropriately according to the stages due to the absence of BSE counseling or seminars on BSE.

Breasts are an important organ for women. Breasts can experience various disorders or diseases, bothserious and mild disease. Among these are breast cancer which is one of the most dreaded breastabnormalities. Breast cancer is a cancer that comes from the gland, glandular, and breast supporttissue.Breast cancer has been known to attack only women. In fact, malignant tumors are also biased alsoagainst men. The histopathologic feature is a diagnostic feature of breast cancer. The purpose of this studywas to determine the histopathologic description of Breast Cancer at RSUP.H.Adam Malik. The sample inthis study is all breast cancer patients who have been diagnosed by doctors who visit the RSUP.H.AdamMalik (Population). The research method used is descriptive of taking secondary data in medical record atRSU.H.Adam Malik. The results of this study can be seen that of 60 breast cancer patients who do biopsy,the majority of patients have histopathology of breast cancer invasive Ductal Carcinoma as much as 31people (51.66%) and Invasive Breast Cancer Nos Type of 29 people (48.34%).

Abstract:
ABSTRACT An overview of my 40-year career will be provided, spanning both industry and academe, and two continents. During my industrial years at LANXESS (formerly the Rubber Division of Bayer), I solved long-standing (10-yr) major manufacturing problems related to Taktene-55 and developed on-line and off-line process control tools that are still in operation. I also developed new technologies (bimodal butyl, one-step halobutyl, branched butyl, liquid carbon dioxide process) that resulted in patents. After transferring to academe, I continued the development of new polyisobutylene-based materials. I have held the Bayer (LANXESS) Industrial Research Chair for 12 yr, working closely with the rubber industry. My most important accomplishments include developing advanced elastomers and thermoplastic elastomers for health care, enzyme-catalyzed polymer functionalization, a “green” synthesis of disulfide polymers and gels, and research into natural rubber biosynthesis. Poly(styrene-isobutylene-polystyrene) is used in a Food and Drug Administration–approved drug-eluting stent, implanted in more than six million patients, saving lives. The recently patented poly(alloocimene-isobutylene-alloocimene) is also a potential biomaterial and also a potential halogen-free halobutyl rubber. I will also discuss my adventure of a field experiment at a Brazilian Hevea plantation to verify our laboratory discovery that the rubber content of Hevea latex can be increased by 20–50% using a special method of tapping. My goal now is creating safer breast implants with cancer-fighting and healing properties. I am proud that the Rubber World trade journal listed me among the 125 inventors that influenced rubber technology in a profound way. I thank my family, Professor Joseph P. Kennedy, and Dr. Adel Halasa for their mentorship and support.

Background: Breast cancer has the highest prevalence in Indonesia. The objective of the research was to analyze the risk factors which influenced the incidence of breast cancer in women of reproductive age. Subjects and Method: This was a casecontrol study conducted at Dr. Pirngadi Hospital, North Sumatera. The population was all 15-49 year-old breast cancer women. Sample of 42 women of reproductive age with breast cancer and 42 women without breast cancer was selected for this study. The dependent variable was breast cancer. The independent variables were family history of breast cancer, menarche, history of breastfeeding, and history of fibrocystic disease. The data were collected by questionnaire and medical record. The data were analyzed by a multiple logistic regression. Results: History of family with breast cancer (OR= 4.93; 95% CI=1.33 to 18.28), age of menstruation (OR= 8.46; 95% CI=2.34 to 30.64), history of breastfeeding (OR= 8.46; 95% CI= 2.19 to 32.70), and history of fibrocystic disease (OR= 7.12; 95% CI=1.99 to 25.48) were associated with breast cancer. Conclusion: History of family with breast cancer, age of menstruation, history of breastfeeding, and history of fibrocystic disease are associated with breast cancer. Abstract:
Background: Breast cancer is a malignancy in breast tissue that can be originated from ductal epithelium and lobules. It is the main cause of death from cancer suffered by women in the world. Age and hormonal are important risk factors for breast cancer where the longer a person is exposed to estrogen, the higher is the risk for breast cancer. The estrogen receptor is one of the main prognostic and predictive factors examined in breast cancer and can be determinant for hormonal therapy. 
 Objectives: This study aimed to discover the relation between age and estrogen receptor in breast cancer patients of RSUD Dr.Pirngadi Medan in 2018.
 Methods: This research was an analytical study with a cross-sectional design. The research sample was 96 breast cancer patients by medical records of RSUD Dr.Pirngadi Medan in 2018 collected by a consecutive sampling method. Analysis of the data in this study was done by univariate to describe the characteristics of research subjects, while bivariate analysis used the chi-square test method to analyze the relationship between age and estrogen receptor.
 Results: The patients with breast cancer in RSUD Dr.Pirngadi Medan 2018 was mostly >40 years old (79.2%), with negative estrogen receptor hormonal status (53%), status was married (99%), and the highest level of education was high school (54.2%). The results of the study showed that there was a relationship between age and estrogen receptor in breast cancer patients with p-value =0.007 (p value<0,05).
 Conclusion: There was a significant relationship between age and estrogen receptors and estrogen receptors in breast cancer patients in RSUD Dr.Pirngadi Medan 2018.

Abstract:
The present study aimed to investigate the effect of feeding fermented mixture of cassava pulp and M. oleifera leaf meal (FCPMO) on the breast meat characteristics of broiler chickens. From day 8 onward, 400 Lohmann broiler chicks were allotted to four dietary groups included CONT (maize-soybean-based feed with no additive), BACI (maize-soybean-based feed supplemented with 0.1% zinc bacitracin), FERM (feed containing 20% of FCPMO) and FERB (feed containing 20% of FCPMO and 0.1% probiotic Bacillus subtilis). At day 35, one male chick from each replicate (10 chicks per treatment group) was taken and slaughtered. After being eviscerated and de-feathered, sample of breast meat was obtained for the assessment of meat characteristics. Results showed that feeding FCPMO had no effect (P>0.05) on pH and water holding capacity (WHC) of breast meats. Compared to other groups, FERB meat had higher (P<0.05) cooking loss. FERM and FERB meats had lower (P<0.05) moisture content than that of CONT and BACI meats. Feeding FCPMO with or without B. subtilis increased (P<0.05) crude protein content of breast meats. Feeding FCPMO resulted in lower (P<0.05) fat content in the breast meat. Compared to other meats, breast meats from BACI had higher (P<0.05) ash content. Cholesterol was lower (P=0.08) in FERB than in BACI and FERM meats. The 2,2-diphenylpicrylhydrazyl (DPPH) IC50 values were higher (P<0.05) in meat of CONT than that in other groups. Feeding FCPMO resulted in higher (P<0.05) polyunsaturated fatty acids (PUFA), lower (P<0.05) monounsaturated fatty acids (MUFA) and had no effect on saturated fatty acids (SFA) contents of broiler breast meats. In conclusion, feeding FCPMO produced healthy broiler meat as reflected by the higher contents of protein, PUFA and antioxidants, and lower fat in meats.

Abstract:
Breast milk (breast milk) is an emulsion of fat contained in a solution of protein, lactose, and organic salts which will be secreted by both breast glands as the main source of food for the baby. Increasing milk is very necessary for mothers by consuming foods such as: sweet potato leaves, the content contained in sweet potato leaves is low in fat and cholesterol etc. Sweet potato leaves can increase milk production because sweet potato leaves contain lactagogum. The purpose of this study was to determine whether or not there was a relationship between consuming sweet potato leaf extract and an increase in breast milk production at the SitiHajar Medan clinic, Marelan. Methods: pre = post test. This study measured the mother's milk before and after being given sweet potato leaves from January 2021 to February 2021. Samples were taken at the SitiHajar Medan clinic with a total of 50 samples, with the inclusive criteria being mothers who gave birth normally and healthy babies. Univariate analysis was carried out to describe the variables with the results of 46% as many as 23 (people) intervention of mother's milk production increased after consuming sweet potato leaves compared to before consuming with a result of 4% as many as 2 (people). Bivarate analyst to determine the relationship between independent and variables using the Wilcoxon test, based on the results of the Wilcoxon test, the p-value is 0.000 (<0.05). and In conclusion, there is a significant relationship between consuming sweet potato leaves and increasing breast milk production.

Abstract:
Patient with breast cancer is experienced a psychological pressure with cancer diagnosis. This condition leads to anxiety and hopeless feeling toward life. One of the therapies which found effective to minimize anxiety is progressive muscle relaxation. The objective of the reasearch was to find the influence of progressive muscle relaxation therapy as the nursing intervention in reducing the anxiety level of the patient with breast cancer. The research applied quasi experiment method with pre-test and post-test control group design. The samples involved are 26 patients with breast cancer, which consist of 13 patients with intervention and 13 patient without intervention. The research instrument used to assess the anxiety level was HRS-A (Hamilton Rating Scale-Anxiety) questionnaire. The research conducted at RSUP Haji Adam Malik Medan from May to June,2016. Progressive muscle relaxation theraphy was carried out in 3 days with the frequency of 6 times exercise. The result of the research showed that t-value was -4.275 and p-value=0.000 or p-value < 0.05 (0,000) which indicated that there was the influence of progressive muscle relaxation therapy on the decrease in the level of anxiety of breast cancer patients. Therefore, nurse can use progressive muscle relaxation as non-pharmacological in coping with breast cancer patients.

Abstract:
The increased rate of breast cancer incidences especially among postmenopausal women has been reported in recent decades. Despite the fact that women who inherited mutations in the BRCA1 and BRCA2 genes have a high risk of developing breast cancer, studies have also shown that significant exposure to certain metal compounds and organic solvents also increases the risks of mammary gland carcinogenesis. While physiological properties govern the uptake, intracellular distribution, and binding of metal compounds, their interaction with proteins seems to be the most relevant process for metal carcinogenicity than biding to DNA. The four most predominant mechanisms for metal carcinogenicity include (1) interference with cellular redox regulation and induction of oxidative stress, (2) inhibition of major DNA repair, (3) deregulation of cell proliferation, and (4) epigenetic inactivation of genes by DNA hypermethylation. On the other hand, most organic solvents are highly lipophilic and are biotransformed mainly in the liver and the kidney through a series of oxidative and reductive reactions, some of which result in bioactivation. The breast physiology, notably the parenchyma, is embedded in a fat depot capable of storing lipophilic xenobiotics. This paper reviews the role of metal compounds and organic solvents in breast cancer development.

PURPOSE: To explore the independent prognostic impact of medial hemisphere tumor location in early breast cancer. PATIENTS AND METHODS: A comprehensive database was used to review patients referred to the British Columbia Cancer Agency from 1989 to 1995 with early breast cancer. Patients were grouped according to relapse risk (high or nonhigh) and adjuvant systemic therapy received. Multiple regression analysis was used to determine whether the significance of primary tumor location (medial v lateral hemisphere) was independent of known prognostic factors and treatment. RESULTS: In the adjuvant systemic therapy groups, medial location was associated with a 50% excess risk of systemic relapse and breast cancer death compared with lateral location. Five-year systemic disease-free survival rates were 66.3% and 74.2% for high-risk medial and lateral lesions, respectively (P < .005). Corresponding 5-year disease-specific survival rates were 75.7% and 80.8%, respectively (P < .03). No significant differences were observed between medial and lateral location for low-risk disease regardless of adjuvant therapy or for high-risk disease with no adjuvant therapy. Local recurrence rates were similar for all risk and therapy groups. CONCLUSION: The two-fold risk of relapse and breast cancer death associated with high-risk medial breast tumors may be due to occult spread to internal mammary nodes (IMNs). Enhanced local control, such as with irradiation of the IMN chain, may be one way to reduce the excess risk. Ongoing randomized controlled trials may provide prospective answers to the question of the optimal volume of radiotherapy.

Introduction: Breast cancer is formed from breast cells, either lobule or duct epithelium, which undergoes uncontrolled growth and development. The therapy of breast cancer to reduce mortality and morbidity can be done by way of surgery, chemotherapy, and radiotherapy. Doxorubicin chemotherapy is a therapeutic option to kill cells directly or by stopping the cell division based on the tumor pathogenesis. Method: This research is a paired numerical comparative analytical study using a retrospective cohort approach. This study was conducted on 56 breast cancer patients to see the differences in myelosuppression before and after doxorubicin chemotherapy at H. Adam Malik General Hospital Medan from January-December 2019. The data were obtained through recording the routine blood laboratory results listed in the patient's medical record and were analyzed with dependent t-test by using SPSS. The difference is significant if p <0.05. Result: The results showed that the average age of the research subjects was 40.79 years, 28 patients (50%) graduated from high school, 38 patients (67.9%) did not work and 53 patients (94.6%) had marital status. In a significant myelosuppression difference, anemia occured with chemotherapy Hb levels of 8.48 ± 1.09, the Hb difference in Hb prior to and after doxorubicin chemotherapy obtained a mean of 1.28 with p value <0.001. Conclusion: Based on the demographic characteristics of patients with breast cancer, it was found that the mean age of patients with breast cancer in women was 46.79 years; most of the subject’s education level was highschool, unemployed and married status. There was a very significant difference between the subject's Hb level and breast cancer before and after doxorubicin chemotherapy. Keywords: myelosuppression, doxorubicin, breast cancer.

Breast cancer attacks a lot of women with the rate of the incidence increases each year and it is very dangerous since it causes death. It can be handled by carrying out chemotherapy treatment although it takes a long time with a lot of its side effects plus the side effect of the cancer itself so that patients will undergo psychological problem which causes them to stop the treatment. The objective of the research was to find out the description of the level of stress, anxiety, and depression in breast cancer patients who were under chemotherapy treatment. The research was descriptive. The population was 41 breast cancer patients who were under chemotherapy treatment in the inpatient wards of RSUP H. Adam Malik, Medan, and all of them were used as the samples (total sampling). The data were gathered from May 20 until June 20, 2016 by using DASS 42. The result of the research showed that 73.2% of the respondents did not have any stress, 58.5% of the respondents did not have any anxiety, and 80.5% of the respondents did not have any depression. It is recommended that the health service team involve patients’ families in providing correct intervention for the breast cancer patients who undergo stress, anxiety, and depression so that the length of treatment can be reduced, cost can be saved, and productivity can be increased.
 Incidence of Breast cancer in women is increasing every year. This cancer is very dangerous because it leads to death. Metals and metal compounds are part of our environment. Several metals are essential for physiological functions (e.g., zinc or magnesium); while the beneficial effects of others are uncertain (e.g., manganese), some metals are proven to be toxic (e.g., mercury, lead). Additionally there are organic metal compounds; some of them are extremely toxic (e.g., trimethyltin, methylmercury), but there is very little knowledge available how they are handled by organisms. Scientific evidence indicates that long-term exposure to (some) metallic compounds induces different forms of cancer, including breast cancer. On the other side, several metal compounds have clinical use in treating life-threatening diseases such as cancer. In this paper we discuss the recent literature that shows a correlation between metal exposure and breast cancer.

Objective -The aim of this study was to determining the survival rate of patient with metastatic bone disease in Haji Adam Malik Hospital Medan. Material and Methods-The aim of this retrospective study is to determine the survival rate of metastatic bone disease patients. Fifty-five patients who diagnosed of metastatic bone disease during January 2012 – December 2017 were entered the inclusion criteria were ask and demographic data were also recorded to evaluate further associated with primary lesion, and surviving status. Survival rate was assessed by Kapplan – Meier Curve. Results- From January 2012 to December 2017, a total 55 metastatic bone disease patients were aged 43-77 years. Breast cancer (40%) was the most common primary cancer, with the lowest survival rate in cervic (10 months). Overall, patients who treat surgically and chemotherapy have survival rate 17 months. Conclusion-This study revealed that female (33 patients) have 15 months of survival rate and mael (22 patients) have 18 months of survival rate. Breast was the most common primary location tumor (40%) and have highest survival rate (20 months). Cervix was the lowest survival rate (10 months) Overall patients who underwent surgical and chemotherapy have survival rate in 17 months.

Babies often do not get breast milk optimally because the mother's milk production is not smooth, which is thought to be due to the mother's lack of knowledge about nutritional consumption and the husband's lack of role in providing support.Purpose: The purpose of this study was to analyze the relationship between maternal knowledge about nutritional consumption and the role of husbands with the smooth production of breast milk.Methods : This research is a quantitative study with a cross sectional approach. The research was conducted in the working area of the South Area Medan Health Center. The study population was all mothers who breastfed their children aged 0–12 months and the sample was obtained 33 people. Accidental sampling. Data analysis used univariate analysis, bivariate analysis with chi-square test at the 95% confidence level ( = 0.05).Result : The results showed that the knowledge of postpartum mothers about nutritional consumption and the role of husbands were related to the smooth production of breast milk in the South Area Medan Health Center in 2020, p <0.05. Mothers with good knowledge and good husbands' roles tend to produce milk smoothly compared to mothers with less knowledge and their husbands who have less role to play during childbirth / breastfeedingConclusion: The role of the partner is related to the smooth production of breast milk in the South Area Medan Health Center in 2020, p = 0.001 <0.05. Bayi sering tidak mendapatkan ASI secara optimal dikarenakan produksi ASI ibu tidak lancar yang diduga karena kurangnya pengetahuan ibu tentang konsumsi nutrisi dan suami kurang berperan dalam memberikan dukungan.Tujuan : Tujuan penelitian ini untuk menganalisis hubungan pengetahuan ibu tentang konsumsi nutrisi dan peran suami dengan kelancaran produksi ASI.Metode : Penelitian ini adalah penelitian kuantitatif dengan pendekatan cross sectional. Penelitian dilakukan di wilayah kerja Puskesmas Medan Area Selatan. Populasi penelitian adalah seluruh ibu yang menyusui anaknya usia 0–12 bulan dan sampel diperoleh 33 orang. Penarikan sampel secara aksidental (accidental sampling). Analisis data menggunakan analisis univariat, analisis bivariat dengan uji chi-square pada tingkat kepercayaan 95% (=0,05).Hasil : Hasil penelitian bahwa pengetahuan ibu nifas tentang konsumsi nutrisi dan peran suami berhubungan dengan kelancaran produksi ASI di Wilayah Kerja Puskesmas Medan Area Selatan tahun 2020, p <0,05. Ibu yang berpengetahuan baik dan peran suaminya baik cenderung lancar produksi ASInya dibandingkan ibu yang berpengetahuan kurang dan suaminya kurang berperan pada masa nifas/menyusui.Kesimpulan: Peran suami berhubungan dengan kelancaran produksi ASI di Wilayah Kerja Puskesmas Medan Area Selatan tahun 2020, p = 0,001<0,05. Suami yang berperan baik selama masa ibu nifas/menyusui membuat produksi ASI ibu lancar dibandingkan dengan suami yang kurang berperan pada masa nifas/ menyusui.Saran: Ibu dapat meminta suami untuk meminta dukungan selama masa nifas/ menyusui dengan berperan aktif memberikan bantuan yang dapat membuat ibu nifas/menyusui menjadi lebih nyaman sehingga dapat memperlancar produksi ASI. Background: Breast cancer is influenced by various risk factors, including age and obesity. Older women who are overweighted and obese have a higher risk of developing breast cancer. This study aims to find the correlation between age and body mass index (BMI) with histopathological features of breast cancer patients in RSUP Haji Adam Malik Medan.Methods: This research is an analytical study using a cross-sectional design with 103 samples obtained from medical record data by random sampling. The data obtained were then adjusted to the research criteria and grouped based on predetermined variables.Results: The majority of the age group was 41-50 years (36.9%), obesity BMI (40.8%), histopathological subtype of invasive carcinoma of no special type (NST) (85.4%), and grade II (46.6%). Kruskal-Wallis test result between histopathological subtypes and breast cancer grading based on age, respectively, obtained p=0.503, r=.325; (α>0.05), and p=0.207, r=0.393; (α>0.05), while based on BMI obtained p=0.017, r=0.021; (α<0.05), and p=0.018, r=0.018; (α<0.05). The OR value (95% CI) obtained on overweight-obese BMI with invasive carcinoma NST subtype was 7.63 (7.27–14.90) and other subtypes were 2.40 (1.14–13.75), and for grades II and III, they were respectively 3.57 (1.32–8.09) and 3.27 (1.17–9.91).Conclusions: There was a correlation between BMI with histopathological subtypes and breast cancer grading, but the correlation tended to be weak. Whereas, with age, there was no correlation. BMI overweight-obese were more likely to have invasive carcinoma NST subtype and higher-grade of breast cancer.

This paper presents a new method for evaluating 3D breast motion relative to the thorax using a new breast coordinate system. A local virtual origin is defined by four breast boundary points, in a constant relative distance from four bone-fixed thoracic points. The validity and reliability of the method was tested with 11 subjects and confirmed (interclass correlation coefficient = 1.00). The feasibility of the new system was tested with two subjects with pert and ptotic breasts, respectively. Each subject participated in motion experiments whilst treadmill jogging, with and without a sports bra. In the braless motion experiments, the breast movement trajectories of six breast positions resembled a butterfly shape. In a jogging cycle, there was a single anterior and medial peak, and a double peak in the vertical breast displacement. The ptotic breast displacement during jogging was larger and wavy, with a more evident vertical and medial-lateral vibration. It tended to move more medially and anteriorly than the pert breast. The bra trajectories of the subjects wearing sports bras showed that the superior movements of the upper breast need further control. To achieve this, it was proposed that the neckline of the bra should better fit the upper breast boundary.

Abstract Background Various theories exist to explain the etiology of iatrogenic symmastia. Subglandular over-dissection of the medial breast pocket over the sternum, disruption of midline sternal fascia, oversized implant base diameter, and over-dissection of the medial pectoralis muscle attachments to the sternum are popular explanations. Objectives The authors hypothesized that the most common risk factor for iatrogenic symmastia is subpectoral breast augmentation. Methods A retrospective chart review was conducted including all symmastia patients who underwent surgery from January 2008 to April 2018 by a single surgeon (C.L.M.). ASAPS members were also surveyed regarding the etiology and incidence of symmastia in their practice. Results Twenty-three patients with symmastia were included in the retrospective chart review. All had previous subpectoral breast augmentation. In the ASAPS survey, 91 plastic surgeons reported seeing an average of 2.2 consults for acquired symmastia over the preceding year; 1.9 of the 2.2 (84.9%) acquired symmastia consults previously underwent subpectoral breast augmentation. Most surgeons attributed these patients’ symmastia to over-dissection of the medial pectoralis muscle attachments to the sternum. Conclusions Symmastia is most often caused by pectoralis major sternal dehiscence during subpectoral breast augmentation. In the senior author’s experience, all patients with iatrogenic symmastia previously had a submuscular breast augmentation. The ASAPS survey supports multiple causes for symmastia with retromuscular breast augmentation occurring in the majority of patients. Repair of symmastia should include securing Scarpa’s fascia to the sternum, reattaching the pectoralis major to the sternum, avoiding another subpectoral implant, and using postoperative modalities to protect the repair.
