AbstractTopical application of medications is difficult through intact skin due to physiological barrier of stratum corneum. Effective transdermal drug delivery system can offer distinct advantages over the topical application and oral administration of drugs. Laser systems have showed clinical benefits for patients in various types of scars for decades. In particular, the advent of fractional resurfacing advanced laser has enhanced the scar treatments dramatically. A fractional laser irradiates cells with high precision by controlling the area and degree of ablation through laser settings. In addition to local thermal destruction and stimulation, fractionated devices may also play an important role in drug delivery through the skin. Preclinical studies substantiate enhanced drug accumulation for a variety of topically applied drugs after ablative fractional laser therapy. Laser-assisted drug delivery is an evolving technology with potentially broad clinical applications. Multiple studies demonstrate that laser pretreatment of the skin can increase the permeability and depth of penetration of topically applied drug molecules. We discuss the mechanisms of laser-assisted drug delivery for scar treatment to enhance our understanding of this evolving technology and suggest optimal protocols of treatment.

In broad terms there are two sets of age-related risks to mental health. The first set are those arising directly from experiences and losses common to later life, including physical ill health and/or disability, being a carer, retirement, and bereavement. These are associated with impaired psychological wellbeing and heightened risk of depression, particularly amongst older people with few economic or social resources. The second set of risks arise from ageism and age discrimination, and their intersection with other types of discrimination such as sexism for older women. Direct and indirect discrimination is widespread; it is located in all areas of society including health and social care services. It is profoundly damaging to older peoples’ psychological wellbeing and is associated with fear, helplessness, low self-esteem, anxiety and depression. It is also linked to exclusion, marginalisation and abuse. In recent years there have been efforts to ensure that older people are overtly included in policies intended to improve the population’s physical and mental health; this includes access to treatments e.g. for depression. There has also been a focus on addressing age discrimination in specific arenas e.g. in employment and mental health services. These initiatives have had mixed success.

This chapter presents basic experimental techniques and various apparatus for measuring the complex index of refraction and related quantities. Generally, measurements of transmittance, reflectance, and emittance are made using spectrometers or lasers. Other important techniques, which measure directly the real refractive index, n, the absorption coefficient, βabs , and the scattering coefficient, βsca, such as interferometry, ellipsometers, calorimetry, and scatterometers, are also introduced. Ultimately, experimental procedures must be taught in the laboratory. Thus, devoting only one chapter to experimental technique and five to theory is not indicative of the importance of this fundamental topic. By discussing the measurement of basic optical parameters, it is intended that the concepts developed in the first five chapters will be reinforced. All of the theoretical models developed in the previous chapters contain measurable parameters. Basic theory often helps guide the design of a good experiment. Once data is available, it can be used to check the assumptions of the theory. This interplay between experiment and theory is an essential part of definitive work. The chapter has two main parts; the first covers measurements of the real and imaginary parts of the complex index of refraction and the second covers measurements of scattering. As established in Chapter 2, the characterization of bulk absorption mechanisms on optical propagation is accomplished by the complex index of refraction. Considerable effort was expended in Chapters 3, 4, and 5 to obtain models of the complex index. Thus, at this point, we wish to find ways to experimentally measure the complex index of refraction for various media. The broad-band spectral response of a medium is commonly measured by a spectrometer. There are two main types of spectrometers, dispersive and interferometric. Generally, spectrometers make broad-band transmission, emission, and reflection measurements, and therefore indirectly measure, n̄. Interferometric measurements, are the exception. Lasers, which feature narrow-band, high-intensity, highly directional light are often used to complement and calibrate broad-band spectrometer measurements.
