Abstract:

Habilitacijos procedūrai teikiamoje darbų apžvalgoje apibendrinti po daktaro disertacijos apgynimo (1999 m.) atliktų mokslinių tyrimų, skirtų intersticinėms plaučių ligoms, darbų rezultatai. Šioje apžvalgoje remiamasi autoriaus atliktų ir su bendraautoriais vykdytų intersticinių plaučių ligų tyrimų nuo 1999 m. iki 2008 m. Vilniaus universiteto Medicinos fakulteto Krūtinės ligų, alergologijos ir radiologijos klinikoje, Viešojoje įstaigoje Vilniaus universiteto ligoninės Santariškių klinikose, rezultatais. Svarbiausias darbo tikslas buvo nustatyti optimalią intersticinių plaučių ligų diagnostikos ir diferencinės diagnostikos seką kasdienėje klinikinėje praktikoje, pateikti praktines dažniausių intersticinių plaučių ligų diagnostikos rekomendacijas. Darbo uždaviniai buvo nustatyti klinikinių simptomų, radiologinių požymių, plaučių funkcijos rodiklių, bronchoalveolinio lavažo skysčio tyrimo ir bronchoskopinės plaučių biopsijos histologinio tyrimo diagnostinę vertę. Šioje apžvalgoje apibendrinamas 30 mokslinių straipsnių, paskelbtų 1999–2009 m. Dauguma straipsnių parengta kartu su kitais bendraautoriais (23 iš jų apžvalgos autorius buvo pirmasis autorius). Darbo rezultatų pagrindu perskaityta 14 pranešimų nacionalinėse ir 19 pranešimų tarptautinėse konferencijose (14 iš jų užsienio šalyse).
The results of scientific research published since acquisition of PhD degree in 1999 are reviewed in this survey for habilitation procedure. The interstitial lung diseases research was conducted and performed by author together with other researchers at Clinic of Chest Diseases, Allergology and Radiology of Vilnius Univesity and Vilnius University Hospital Santariškių klinikos in 1999–2008. The aim of this research was to determine optimal diagnostic pathway for interstitial lung diseases in daily clinical practice, to propose practical recommendations for diagnostic of the most common interstitial lung disorders. The tasks of the research were to evaluate diagnostic value of clinical symptoms, radiological signs, indices of respiratory function, examination of bronchoalveolar lavage fluid and bronchoscopic lung biopsy material. The review material covers 30 scientific papers published during 1999–2009. The most of papers were prepared together with co-authors, but in 23 of them the author of this survey was the first author. On the base of obtained scientific material 14 presentations at national conferences and 19 presentations at international conferences (14 of them in foreign countries) were done.

Abstract:

Darbo tikslas. Įvertinti endoskopinio tyrimo reikšmę neužkrečiamųjų ligų diagnostikai arkliams. Darbo uždaviniai. Susipažinti su užsienio autorių publikuojama literatūra, apie endoskopinių tyrimų pritaikymą veterinarinėje medicinoje; įvertinti, kokiems tikslams naudojami endoskopijos metodai; įvertinti, kuris endoskopinis tyrimas dažniausiai atliekamas; įvertinti, kokios patologijos dažniausiai aptinkamos; nustatyti, pagal kokius simptomus atliekamas endoskopinis tyrimas. Tyrimo metodika. Tyrimas buvo atliekamas 2011 – 2013 metais magistrantūros studijų metais LSMU VA Stambiųjų gyvūnų klinikose. Tyrimo metu buvo atliekami endoskopiniai tyrimai: gastroskopija, bronchoskopija, laringoskopija. Tyrimo įranga – videoendoskopo sistema VIDEO MED. Iš viso konsultuoti 169 pacientai, iš jų 35 pacientams buvo atliktas endoskopinis tyrimas. Tyrimas buvo atliekamas remiantis klinikiniais simptomais. Rezultatai. Endoskopiniu tyrimu tirti 35 pacientai, kuriems pasireiškė būdingi klinikiniai simptomai. Gastroskopija buvo atlikta 17 pacientų (49 proc.), bronchoskopija atlikta 12 arklių (34proc.), o laringoskopija – 6 arkliams (17 proc.). Tyrimo metu nustatytos šios patologijos: EGUS, LOPL, gerklų paralyžius. Arklių skrandžio opų sindromas buvo nustatytas visiems 17 pacientų, kuriems buvo atliekama gastroskopija, tai sudaro 100 proc. LOPL nustatyta 10 pacientų iš 18, kuriems buvo atliekamas kvėpavimo takų endoskopinis tyrimas, tai sudaro 56 proc. Gerklų paralyžius buvo nustatytas visiems 6... [toliau žr. visą tekstą]
Objectives of the research paper. To get acquainted with material, about the appliance of endoscopic diagnosis in veterinary medicine, published by foreign authors; to study what these are the aims to use endoscopy methods; to discover the most highly endoscopic diagnosis used; to research what most common cases of pathology are diagnosed; to identify which symptoms are clues to determine an endoscopic diagnosis. Methodology. The research was carried out in 2011 – 2013, during the years of obtaining Master’s Degree in Heavy Animals clinics at LHSU VA. The following diagnoses of endoscopy were made in the course of the research: gastroscopy, bronchoscopy, laryngoscopy. The research equipment used was a video endoscopy system VIDEO MED. In total 169 patients were consulted and 35 out of the total were examined using endoscopy. The study was carried out on the basis of clinical symptoms. Results. 35 patients with clinical symptoms were examined using the method of endoscopy. Gastroscopy was carried out on 17 patients which make 49 per cent, bronchoscopy – 12 horses ( 34 per cent), laryngoscopy – 6 horses (17 per cent). During the research, the following cases of pathology were diagnosed: EGUS, COPD, laryngeal paralysis. Gastric ulcer syndrome in horses was diagnosed in all 17 patients (100 per cent), which underwent the medical examination. Abstract:

Im Rahmen der vorliegenden Arbeit ist ein neues bronchoskopisches Verfahren entwickelt und getestet worden, mit dem eine Längs- und Querschnittsvermessung zentraler Atemwegesstenosen in Echtzeit ermöglicht wird und anschließend eine 3D-Rekonstruktion des untersuchten Abschnitts dargestellt werden kann. Für die Querschnittsflächenbestimmung wird durch den Arbeitskanal des Bronchoskops eine Lasersonde eingeführt und ein Laserlichtring auf die Trachealwand projiziert. Die Abbildung des Lichtrings wird im bronchoskopischen Bild noch während der Untersuchung mit Hilfe einer speziell im Rahmen dieser Arbeit entwickelten Software Endo3D segmentiert und vermessen. Durch Speichern aufeinanderfolgender Querschnittsflächen kann ein 3D-Datensatz erstellt, visualisiert und das Volumen berechnet werden. Experimentell wurde die Methode an Kunststoffmodellen mit bekannten Maßen und Präparaten aus Schweineluftröhren getestet. Die Referenzwerte für die Volumen der Schweineluftröhren wurden durch Wasservolumetrie bestimmt. Klinisch wurde die Methode in einer Pilotstudie getestet. Bei 10 Patienten wurden Querschnitte und Volumen unterschiedlich langer Trachealabschnitte vermessen. Als Vergleichsmethode wurde jeweils eine CT durchgeführt. Die Ergebnisse im experimentellen Teil zeigten bei sehr guter Reproduzierbarkeit eine gute Korrelation zwischen den bronchoskopisch gemessenen Werten und den realen bzw. Referenzwerten. Die Korrelation der klinischen Ergebnisse erwies sich als befriedigend ohne Hinweis auf einen systematischen Fehler. Ein kleiner systematischer Messfehler im experimentellen Teil zeigte sich als irrelevant für die Klinik. Die hier vorgestellte Methode ermöglicht untersucherunabhängige Verlaufsbeschreibungen von Stenosen und erlaubt die Schaffung einer einheitlichen Klassifikation. Darüber hinaus ist damit zukünftig eine individualisierten Stentimplantation sowie Tumorvolumenberechnungen denkbar.
In this paper a new bronchoscopic method was developed and tested, that performs measuring both cross-sectional areas and length of central airway stenoses in real-time. Furthemore this method enables to represent a three-dimensional reconstruction of the airway section that was analysed. To measure th cross-section area, a laser probe inserted into the operating channel of a bronchoscope projects a ring of light onto the trachal wall and marks the cross-sectional area. A new software especially developed for this method makes it possible to identify the projected ring of light and measures the cross-sectional area after applying lens distortion correction algorithms. By saving a succession of cross-sections 3D-data is provided for visualizing and volume calculation. The measuring accurracy was first tested employing plastic tubes with known diameters and 17 models of porcine trachea. The cilinical evaluation was realized in a pilot study. Sections of different length of tracheas of 10 patients were analysed by both the new method and by CT. The results of the experimental part showed good correlation between the reference methods and a very good reliability. The correlation between CT and bronchoscopic measuring results was slightly less good than the experimental correlation, but they showed no systematic measuring error. A little systematic error in the experimental evaluation prooved to be irrelevant for clinical results. The new method enabels objective description of stenoses and makes it possible to develop a standardized classification. In future indvidual stent construcions or calculation of tumor volumes are conceivable.

Abstract:

In der vorliegenden Arbeit wurden retrospektiv 66 Fälle stattgehabter Fremdkörperaspirationen aus den kinderchirurgischen Akten und Ambulanzakten der Jahre 1997 bis 2008 des Universitätsklinikums Leipzig rekonstruiert und im Hinblick auf die nachfolgend genannten Kriterien statistisch ausgewertet: Geschlecht und Alter der Kinder, Indikation zur Bronchoskopie, Einweisungsart, Dauer bis zur Diagnosestellung, endoskopische Sichtbarkeit des Fremdkörpers, Entfernungsschritte, erforderliche Nachbeatmung, Anhalt durch Röntgenbefund, auskultatorischer Anhalt, anamnestischer Anhalt, Eigen- oder Fremdanamnese, das Vorhandensein von Husten und die Dauer bis zur Entlassung aus dem Krankenhaus. Die Kriterien wurden standardisiert und mittels des Statistikprogramms SPSS 18 ausgewertet. Anhand der ausgewerteten Daten wurde ein Algorithmus zur innerklinischen Vorgehensweise bei Verdacht auf Fremdkörperaspiration erstellt, welcher eine interdisziplinäre Arbeitsrichtlinie (Standard Operation Procedure, SOP) für andere Kinderzentren liefern kann. Das geschärfte Bewusstsein für die Häufigkeit einer Fremdkörperaspiration vor allem bei Kindern und die damit verbundene problemorientierte Diagnostik und Therapie soll mit einer einschlägigen Arbeitsrichtlinie in Zukunft mehr Kindern als bisher zunutze gemacht werden.

Abstract:

The aim of this work has been the development and optimization of methods for early diagnosis of lung cancer, their utility and integration into daily practice. Firstly, we developed a device for measurement of endobronchial temperature (thermobronchoscopy) and found significant difference in endoluminal temperature above tumors and infiltrated lymph nodes compared to healthy regions. We further designed an appliance for near infrared spectroscopy of the bronchial mucosa and identified spectroscopic features useful for localization of solitary pulmonary nodule. The use of the appliance improved yield of endobronchial biopsy compared to endobronchial ultrasound. Abstract:

In der vorliegenden Arbeit wurden retrospektiv 66 Fälle stattgehabter Fremdkörperaspirationen aus den kinderchirurgischen Akten und Ambulanzakten der Jahre 1997 bis 2008 des Universitätsklinikums Leipzig rekonstruiert und im Hinblick auf die nachfolgend genannten Kriterien statistisch ausgewertet: Geschlecht und Alter der Kinder, Indikation zur Bronchoskopie, Einweisungsart, Dauer bis zur Diagnosestellung, endoskopische Sichtbarkeit des Fremdkörpers, Entfernungsschritte, erforderliche Nachbeatmung, Anhalt durch Röntgenbefund, auskultatorischer Anhalt, anamnestischer Anhalt, Eigen- oder Fremdanamnese, das Vorhandensein von Husten und die Dauer bis zur Entlassung aus dem Krankenhaus. Die Kriterien wurden standardisiert und mittels des Statistikprogramms SPSS 18 ausgewertet. Anhand der ausgewerteten Daten wurde ein Algorithmus zur innerklinischen Vorgehensweise bei Verdacht auf Fremdkörperaspiration erstellt, welcher eine interdisziplinäre Arbeitsrichtlinie (Standard Operation Procedure, SOP) für andere Kinderzentren liefern kann. Das geschärfte Bewusstsein für die Häufigkeit einer Fremdkörperaspiration vor allem bei Kindern und die damit verbundene problemorientierte Diagnostik und Therapie soll mit einer einschlägigen Arbeitsrichtlinie in Zukunft mehr Kindern als bisher zunutze gemacht werden.:1.
