Abstract:
21062 Background: EGFR is a protooncogen that facilitates metastasis in cancer. It is overexpressed in colorectal carcinoma in 40% to 80%, and correlates a worse prognosis. Aim: To find patterns of EGFR overexpression in NMCC and to correlate these patterns with the follow-up. Methods: We analized NMCC with a selection criteria: 1. primary adenocarcinoma of the colon, without metastasis and with curative surgery, 2. pT3 pN0–2 pM0, progression-free in the first 6 months postsurgery, 3. initial follow-up began 6 months postsurgery, 4. minimum follow-up over 5 years. Dako PharmaDx kit (Glostrup, Denmark) was used for immunohistochemical study for EGFR. Dako staining and A431-AAM cells were used as positive control. Immunoreactivity for membrane (1(+), 2(+) and 3(+) intensity) and for cytoplasma was evaluated. Proportions of immunostained cells were also assessed. Results: 195 patients (p) were eligible. Year of diagnosis: 1994: 1p; 1995: 4p; 1996: 20p; 1997: 29p; 1998: 33p; 1999: 36p; 2000: 34p; 2001: 38p. Male/female: 93/102; Mean age: 71 years (28- 94). Location: 116/79 left/right. Stage II/III: 125/70. EGFR membrane positivity: 84/195 cases (43%), 49/125 (39%) in Stage II and 35/70 (50%) in Stage III. With a median follow-up of 7 years: Stage II: 29/125 relapsed (EGFR±: 12(24%)/17(22%)). Stage III: 33/70 relapsed (EGFR± : 21(60%)/12(34%)). EGFR membrane positivity was a poor prognostic factor with a significant progression-free survival (PFS) (p=0.042) and a non-significant overall survival (OS) (p=0.066). When follow-up period was increased over 10 years (patients from 1994 to 1998), EGFR membrane positivity maintained its poor prognostic factor in PFS (p=0.009) and presented a significant OS (p=0.02). Non-significant differences were found in immunostaining ratios. Conclusions: In patients with non-metastatic colon cancer, EGFR membrane positivity was a tumor progression prognostic factor, and this poor prognosis was maintained over time. AbstractAny singular level of a completely integrable system (c.i.s.) with non-degenerate singularities has a singular affine structure. We shall show how to construct a simple c.i.s. around the level, having the above affine structure. The cotangent bundle of the desingularized level is used to perform the construction, and the c.i.s. obtained looks like the simplest one associated to the affine structure. This method of construction is used to provide several examples of c.i.s. with different kinds of non-degenerate singularities.

Abstract:
Two new competitive immunoenzymatic assays (“C.I.C.-conglutinin” and “C.I.C.-C1q“) were employed to evaluate circulating immunocomplexes in 90 patients with a variety of autoimmune and neoplastic disorders. In autoimmune diseases the assays appeared very sensitive and relatively concordant, while in malignancies sensitivity was high with the C.I.C.-conglutinin method but much lower with the C.I.C.-C1q method. The clinical significance of these results is discussed with particular regard to the possibilities of application of the two assays in the diagnostic routine of a general hospital laboratory.

Abstract:
AbstractThis review-essay examines two recent works of scholarship on the life and work of the late Trinidadian intellectual and activist C.L.R. James (1901–89). While recognising the respective merits of Frank Rosengarten's Urbane Revolutionary; C.L.R. James and the Struggle for a New Society (2008) and Brett St Louis's Rethinking Race, Politics, and Poetics: C.L.R. James' Critique of Modernity (2007) the essay argues that a critical weakness of both works is their problematic discussion of James's Marxism. This review will aim not simply to defend the central importance of Marxism for James, but will also suggest that, as much as anything, it is precisely this that gives so much of his work a rare urgency and critical relevance in the twenty-first century.

Abstract:
This research was performed so as to appraise strength and properties of lateritic soils enriched with hydrated lime and bitumen as construction materials. Soil samplings S1, S2, S3, S4, and S5 was collected from Dualization of Sheda - Abaji, Abuja, F.C.T, Ni-geria and augment with 0, 3, 6, 9, and 12% of hydrated lime and bitumen. Laboratory tests like Consistency (Atterberg limit), California Bearing Ratio (C.B.R), compaction and sieve scrutiny were done on the soil sampling. The study exposed that construc-tive effects were gotten via the addition of hydrated lime and bitumen in order to augment the strength of weak soils. All the 5 samplings have C.B.R values of 48.0% with hydrated lime as additives, whereas bitumen additive is 40.60%. Introduction of addi-tives enriched the soil samplings from weak materials to an excellent base material with C.B.R values above 40%. Hence, aug-mentation of lateritic soil samplings from the studied locality could be perform efficiently by using lime.

Abstract:
Background: In this study, we analyzed epidemiologic characteristics and morphologic differences between Asian and European population in patients with abdominal aorta aneurysm (AAA). Method: We conducted concomitant study in Japan and Europe , in 31 Asian patients (AP) with AAA and 130 control subjects of the same population, and in 30 European patients with AAA and 126 control subjects (EP). We observed various demographic and anthropologic parameters. Aortography was performed in all patients at the same type of CT-scanners using the same examination protocol and postprocessing. For data analysis, various statistical models were used. Results: There are statistically significant differences in multiple epidemiologic and morphologic findings in AAA patients, as well as in comparison with controls in both groups. This is most distinct in terms of anthropologic characteristics and number of risk-factors, and in terms of morphologic findings, in the length of neck of the aneurysm, transverse diameter of abdominal aorta (AA) and common iliac artery (c.i.a), and volumes of c.i.a. We obtained paradoxical results considering diabetes mellitus, which presented itself as a protective factor for AAA. Conclusion: Demographic-epidemiologic analysis accompanied with morphologic measurements using modern imaging modalities enables obtaining new information on pathology of AAA in different races. There are differences in number of risk-factors, and in terms of morphologic findings, in the length neck, angle of the aneurysm, transverse diameters of abdominal aorta (AA) and in the length of common iliac artery (c.i.a), and volume of c.i.a.

Abstract:
Abstract: In engineering practice, the earth construction requires the compaction of the existing subgrade by improving the density and strength of the data. All types of earth structures, i.e., highways, pavements, etc., rest directly on the soil beneath them. The safety of these entities depends upon the strength/bearing capacity of the soil over which these are constructed. Therefore, a proper analysis of the soil properties and the design of their compression parameter become necessary to ensure that these structures remain stable and are safe against unequal settlements. To determine the suitability of any soil type for use as subgrade, subbase, or base material, one of the parameters generally used is the California bearing ratio (C.B.R.). The coarse aggregates available as a reinforcing material can enhance soil properties and increase its C.B.R. value. This paper will study the effects of coarse aggregates on the C.B.R. value, determine suitable size range of aggregates with their percentage, and their application for the earth structures. Abstract:
C.I.M is on-line. This issue marks the beginning of a new phase of publication for the journal: CIM is now available only on-line. Over its 30 year existence, CIM has been published in several formats and by different agencies. In recent years, CIM was owned and published by CMA as one of its associate journals. In 2004, CSCI purchased CIM and ownership reverted to the society. Since then, CIM has been type-set and formatted in-house and printed and distributed by University of Toronto Press (UTP) and, as in interim measure, the journal was available on the CSCI web-site. This process was threatened by the sale of UTP's off-set Printing Division. Links have now been established between CSCI, CIM and the University of Toronto Library to make use of the U of T Open Journal Access initiative to develop sophisticated on-line distribution. The Library supports journals via the Open Journal System (OJS) management software as well as providing archiving facilities. CIM will remain a subscription journal but, in the spirit of open access, all content will be freely available after a six month publication delay. Access will also be made available to all Canadian Universities via their library websites and authors will have immediate access for submitted articles.

Abstract:
Abstract Despite impressive single-agent activity of monoclonal antibodies (moabs) in lymphoproliferative disorders, activity can be enhanced by combination with other active agents. We have previously shown that alemtuzumab can be combined safely with rituximab and that the combination is active in pts with relapsed/refractory chronic lymphoproliferative disorders (Blood101: 3413, 2003). As the optimal dose, schedule and route of administration have not been established, and in the light of data on soluble CD52 levels in the serum of patients with chronic lymphocytic leukemia (CLL) and their association with prognosis, we are now investigating a continuous i.v. infusion (c.i.v.) followed by subcutaneous (s.c.) injection of alemtuzumab plus rituximab in pts with relapsed/refractory CLL with co-expression of CD52 and CD20. Rituximab is given at 375 mg/m2 i.v. on day 1 followed by 500 mg/m2 on days 8, 15, and 22. Alemtuzumab is administered at 15mg by c.i.v. daily for 6 days (days 2–7) followed by 30 mg s.c. twice weekly on days 3 and 5 of weeks 2 to 4. Up to 12 weeks of therapy can be given. Should pts continue beyond 4 weeks, no further c.i.v. of alemtuzumab is administered. All pts were premedicated with acetaminophen and an antihistamine, and received antiinfective prophylaxis with TMP/SMX and valacyclovir (or equivalent). CMV antigenemia and/or PCR testing was done prior to and at the completion of each course of therapy. Twenty-eight pts have been enrolled (26 CLL, 1 CLL/SLL, 1 MZL). Twenty pts are evaluable for response. Median age: 57 yrs (range 39–78), median number of prior therapies: 3 (1–5), median b2M 3.5 mg/dL (2.1–13.6). Ten pts (50%) had Rai stage ≥ 3. Six pts. (30%) were refractory to both fludarabine and alkylators. All evaluable pts received prior rituximab, but only 2 (10%) have been exposed to alemtuzumab. No pts received the combination of alemtuzumab plus rituximab before. Based on NCI-Working Group criteria for response, 6 pts (30%) achieved CR, 1 (5%) PRn, and 4 (20%) PR for an overall response rate of 55%. All responders achieved their maximum response after 4 weeks of therapy (one course) only. Response by site: liver/spleen 6 of 6 pts (100%), peripheral blood 13/15 (86%), marrow 14/20 (70%), and lymph nodes 8/15 (53%). No unexpected adverse events (AEs) occurred. Most non-hematologic AEs were infusion-related and ≤ grade 2 by NCI toxicity criteria: fevers (93%), chills (75%), fatigue (55%), skin rashes or injection site reactions (35%), nausea (25%), myalgias (20%), and diarrhea (10%). Although AEs occurred more frequently during c.i.v. alemtuzumab than during the s.c. injections, c.i.v. of alemtuzumab has been well tolerated compared to bolus infusions. No dose escalation was given prior to the start of the c.i.v. Infections occurred in 10 pts (50%) including CMV reactivation in 6 (21%). In summary, out data indicate a promising response rate in a poor prognosis group of pts after only 4 weeks of therapy. The combination is well tolerated. Side effects are predictable and manageable.

Abstract:
In three cores from an acidic lake in the Adirondack State Park of New York, iron cycling accounted for diagenetic enrichment of up to 14% of sediment dry mass. The accuracy of the sediment accumulation rates calculated from the constant rate of supply (c.r.s.) model and the dating and sediment accumulation rates calculated from the constant initial concentration (c.i.c.) 210Pb models were affected by post-depositional movement of iron about the sediment–water interface and through the sediment. Dating biases from iron diagenesis reached as high as 57% and biases to calculated sediment accumulation rates ranged up to approximately 15%. In general, however, the difference between the iron-corrected and uncorrected dating was not much greater than the error expected from routine analytical precision. However, under circumstances of low sediment accumulation rates and high iron enrichment, significant deviations in dating and sediment accumulation calculations for both c.r.s. and c.i.c. 210Pb models may become noteworthy.

Abstract:

Kuehn, Mary Beth, Devyn Hotho, and Maggie Prunty. "Parental Consent for Adolescent Research." Creative Nursing 22, no. 1 (2016): 51–55. http://dx.doi.org/10.1891/1078-4535.22.1.51.

Abstract:
The purpose of this article is to increase understanding about how to obtain institutional review board approval to waive parental consent for adolescent research that involves minimal risk to subjects and/or that cannot be carried out practicably without the waiver. Because of IRB guidelines, the researchers were unable to use passive parental consent for a study of adolescent nonmedical prescription drug use (NMPDU). Without knowledge of waiver 45 C.F.R. §46.116 of the U.S. Department of Health and Human Services Code of Federal Regulations regarding the protection of human research subjects, the study design required active parental consent for survey administration. This requirement limited study participation and increased costs. Using the waiver, the study design could have waived parental consent with adolescent assent. By increasing awareness of the 45 C.F.R. §46.116 waiver and its subparts, the researchers hope to inform future researchers about reducing barriers to future adolescent research. Lessons learned include how using the waiver may decrease research costs, reduce participant selection bias, increase participant response rate, and expand data available to contribute to critical, emerging adolescent health issues such as NMPDU.

Abstract:
AbstractThis essay explores the work of C.L.R. James in an effort to rethink the democratic import of dialectical thought. James is best known as a West Indian Marxist and the author of The Black Jacobins, but he is also a deeply democratic thinker and a creative reader of Hegel. Alongside the more orthodox Marxist strand of his thought, James develops a mode of dialectical reflection that is attentive to a sense of democratic uncertainty and that aims to enrich political participation among ordinary citizens. By thinking both with and against James, and by exploiting the ambiguities in his postwar writings on dialectics and American democratic culture, I argue that James inspires a kind of participatory democratic ethos grounded in the language and conceptual resources of Hegelian humanist thought and modern dialectical reflection.
