Abstract:
Background: Laparoscopic cholecystectomy has become a standard technique for gall bladder surgery of symptomatic cholelithiasis. However, conversion to open cholecystectomy is sometimes necessary. The aim of the present study was to assess the predictive factors that increase the possibility of conversion of laparoscopic cholecystectomy to open cholecystectomy.Methods: A total of 621 laparoscopic cholecystectomies were attempted at AL-Mawanee General Hospital and AL-Sader Teaching Hospital in Basrah, IRAQ from June 2012 till June 2016.Of these,43 had to be converted to open cholecystectomies. Patients assessed according to different factors, including age, sex, acute cholecystitis, adhesions of gallbladder and calot's triangle, obesity, previous abdominal surgery, anatomical variation of gallbladder and Calot's triangle and intraoperative complications (bleeding, bile duct injury, visceral injury).Results: Conversion to open cholecystectomy was performed in 43 patients (6.92%). The significant factors for conversions were adhesions of gallbladder and Calot's triangle(39.53%) followed by acute cholecystitis(34.88%). Rate of conversion in other factors are as the following i.e., isolated male gender (0%), age (0%), previous abdominal surgery (9.3%), obesity (2.33%), anatomical variations of gall bladder and calot's triangle (2.33%), intra operative complications including bleeding (4.65%), bile duct injury (4.65%), visceral injury (2.33%) were insignificant factors for conversion.Conclusions: Adhesions of gallbladder and calot's triangle is the most common predictive factor and cause for conversion from laparoscopic cholecystectomy to open cholecystectomy. Acute cholecystitis found to be the strongest factor for conversion despite its incidence is lower than adhesions of gall bladder and calots triangle. Male gender and age more than fifty years are not direct predictive factors for conversions.

Abstract:
Background: Major complications of laparoscopic cholecystectomy are bleeding and bile duct injury, and it is necessary to clearly identify structures endoscopically to keep bleeding and injury from occurring. The aim of this study was to depict the anatomic landmark in the Calots triangle, a vein (cystic vein), a constant feature which can help Laparoscopic surgeons to conduct a safe LC along with other precautions to be adopted. Methods: A total of 100 patients (58 male, 42 female) who underwent cholecystectomy were examined preoperatively by clinically. The origin and number of cystic veins and their relationship with the Calot triangle was evaluated. Results: The cystic veins were delineated intraoperatively in 80 of the 93 patients. The relationship between the cystic vein and the Calot triangle was identified in 80 (86.02%) of the 93 patients. One cystic vein was found in 53 (66.25%) patients, while multiple cystic veins were found in 27 (33.75%) patients. All these veins are above the cystic common bile duct junction. Conclusion: The configuration of the cystic veins and their relationship in the Calot triangle with cystic artery and cystic duct can be identified intraoperatively and used as a guideline for safe laparoscopic cholecystectomy.

Se ha publicado el fechado de Carbono 14 de un fragmento de cráneo estudiado por Florentino Ameghino a finales del siglo XIX. A Ameghino le sirvió para afirmar su teoría sobre el origen del hombre en las pampas al que le otorgaba un millón de años y el ser parte fundamental de la cadena del origen de la humanidad. Hubo polémicas, pero el tema quedó cerrado cuando Ales Hrdlicka y Bailey Willis en 1912 demostraron que estaba basado malas interpretaciones. El cráneo había llegado a un nivel geológico discutible y por causas no naturales.El hallazgo había sido hecho en un sitio que hasta poco antes estaba dentro del agua del Río de la Plata, cerca de 400 metros de la orilla antigua, lo que ayudaba al antiguo fechamiento. Al ubicarlo ahora en una fecha hacia 1720 dC resulta interesante construir hipótesis sobre su presencia allí (Politis y Bonomo 2011), en lo que hubo en el lugar y en las condiciones en que fue encontrado; es decir su contexto. Los datos de otros hallazgos frente a Buenos Aires ayudan a repensar el tema, y la revisión cartográfica y documental permite entender que afirmaciones hechas tanto por un bando como otro no tomaron en cuenta los eventos de alteración del sitio.

Abstract:
Introductions: With the advent of newer technology, the era of open surgery for gall bladder diseases has been preferably taken over by laparoscopic cholecystectomy. However, certain cases still require conversion to open surgery. In this review we aim to analyze the reason for conversion. Methods: This retrospective study was conducted at Patan Hospital, Patan Acdemy of Health Sciences, Nepal. All patients who underwent laparoscopic cholecystectomy from February 2009 to July 2012 were included in the study. File numbers of all the patients were obtained from operation room register. The patient files were analyzed for age, sex, duration of symptoms, liver function tests, ultrasound findings and the description in operation note for reason for conversion. Results: The age ranged from 12 to 81 years with mean age of patients 32.76 years and male to female ratio 1:2.9. The mean operating time was 65 minutes and average post operative hospital stay was 1.61 days. Out of 305 patients, 34 (11.14%) required open conversion. Factors responsible for open conversion were dense fibrosis at Calots in 11 (3.6%), adhesions due to previous abdominal surgery in 6 (1.9%), uncontrollable bleeding in 5 (1.6%), bile duct injury in 4 (1.3%) cholecystoenteric fistula in 3 (0.9%), Mirizzi’s syndrome 2(0.6%). Conclusions: Adhesions at the calot’s triangle was the common reason for conversion from laparoscopic to open cholecystectomy. Plain Language Summary: This study was conducted to determine the predictive factors for conversion of laparoscopic cholecystectomy. The study found that dense adhesion around calot’s triangle and adhesions pertaining to previous abdominal surgery were the main reasons for conversion to open surgery. So, before embarking on laparoscopic cholecystectomy, it is essential to take detail history and examination, to rule out the probable cause of conversion beforehand and minimize; morbidity, duration of surgery and cost. Avaliou-se a influência exercida pela aquisição de calorias de açúcar sobre a participação calórica dos demais macronutrientes da dieta. As estimativas deste estudo são baseadas em dados da Pesquisa de Orçamentos Familiares realizada no Brasil pelo Instituto Brasileiro de Geografia e Estatística entre julho de 2002 e junho de 2003. Modelos de regressão linear múltiplos foram utilizados para estudar a influência das calorias de açúcar sobre a participação calórica de cada um dos macronutrientes na aquisição domiciliar de alimentos com o controle do valor calórico total da aquisição de alimentos e variáveis sócio-demográficas. Cada caloria adquirida de açúcar eleva em 0,3 caloria a participação de gorduras na aquisição domiciliar de alimentos e diminui em 0,07 a participação de proteínas. Cada caloria de açúcar procedente de alimentos processados aumenta em 1,6 caloria a participação de gorduras e em 0,4 caloria de ácidos graxos saturados e diminui em 0,8 caloria a participação de outros carboidratos que não o açúcar. Os resultados encontrados trazem novas evidências sobre o papel prejudicial do açúcar à saúde humana.

The Australian taxa of Micropezidae (listed at end of paper) are keyed and described. Crus, Scela, Veru and Seva are new subgenera of Metopochetus Enderlein. In all, 21 new species are described in the genera Metopochetus, CothornobataCzerny, CrepidochetusEnderlein and Mimegralla Rondani. Cothornobata inermis (Malloch) is a new combination (from Grammicomyia). Lectotypes are designated for Calobata compressa Walker (now in Metopochetus), Calobata bivittata Macquart (now in Metopochetus),Mimegralla contingens australicaHennig (now M. australica), Calobata contraria Walker [synonym ofMimegralla sepsoides (Walker)]. Records of the following from Australia are found to be erroneous: Calobata albimana Macquart [synonym ofTaeniaptera trivittata (Macquart)],Mimegralla abana (Walker), Calobata brevicellulata Macquart (now in Mimegralla), Calobata coeruleifrons Macquart (now in Mimegralla) and Metopochetus tipuloides (Walker). The erroneous record of Cothornobata viriata (Enderlein) from Papua New Guinea is corrected. Distributions of species are recorded by means of grid references to a key map. Information on ecology and habits of Australian micropezids is summarised. The populations of two flightless species are vulnerable, because each is apparently dependent on an ecologically vulnerable host-plant species (families Brassicaceae and Cephalotaceae). A phylogeny of the recognised subfamilies and tribes of Micropezidae is given. Literature on fossil micropezids is reviewed.

Abstract:
Lizards of the genus Calotes are geographically restricted to South Asia, Indo-China and parts of Southeast Asia. The greatest diversity of the genus is from the biodiversity hotspots in South Asia: Western Ghats (Peninsular India), Sri Lanka and Indo-Burma. Here, we present a systematic revision of members of the genus Calotes from Peninsular India using a combination of molecular phylogeny, geographical distribution and morphological characters. We show that Calotes from the Western Ghats is paraphyletic and consists of three major clades, one of which is widely distributed in South and Southeast (SE) Asia, while the others are restricted to Peninsular India. The Peninsular Indian clade is composed of two sister clades: Psammophilus, with a wider distribution and a second clade, composed of two extant species, Calotes rouxii and Calotes ellioti and two new species, all restricted to the Western Ghats region. Based on morphological differences, we retain the generic status of Psammophilus and assign its sister clade to a new genus Monilesaurus gen. nov. and transfer the following species, C. rouxii and C. ellioti, to this new genus. We also provide diagnoses and descriptions for two new species recognized within Monilesaurus gen. nov. In addition, Calotes aurantolabium from the Western Ghats was observed to be deeply divergent and to share a sister-relationship with the clade composed of Calotes, Monilesaurus gen. nov., and Psammophilus. Based on its phylogenetic position and morphological attributes, we assign this species to a new genus Microauris gen. nov. These new discoveries highlight the evolutionary significance of the Western Ghats in housing novel lizard diversity.

Two new species of the leafhopper genus Calodia Nielson are described and illustrated: C. quadrimaculasp. nov. from Guizhou and Yunnan Provinces and C. zuoaesp. nov. from Yunnan Province, China. A checklist along with distribution and a key to species based on male genitalia of the genus Calodia from China are provided. Olidiana nigritibiana (Li), comb. nov. (earlier in the genus Calodia) is proposed. At present, this genus comprises 45 known species worldwide, of which 19 species are recorded from China.

BACKGROUND Cystic artery originates from right hepatic artery which is a branch of hepatic artery proper of coeliac trunk from abdominal aorta. Variations in the origin and course of cystic artery were observed in this study. Contents of Calot’s triangle were also studied. Knowledge of normal anatomy, variations of the biliary apparatus and the arterial supply to the gallbladder is important for surgeon. METHODS Descriptive cross-sectional study was done from April 2008 to January 2010 in 180 adult specimens and 50 foetal specimens in the Departments of Anatomy, Forensic Medicine and Pathology in Government Medical College, Thiruvananthapuram. The abdominal wall was incised and hepatobiliary region was identified and dissected. The gallbladder and coeliac trunk were identified. Cystic artery and Calot’s triangle were studied. RESULTS More than 90 percent of the cystic artery originates from the right hepatic artery in both sexes. Cystic artery originated from left hepatic artery in 4 cases, common hepatic artery in 5 cases and from middle hepatic artery in 1 case. The cystic artery within Calot’s triangle was observed in 41 %. Right hepatic artery was content in 29.9 %. Both the arteries were content in 19 %. Cystic artery and accessory hepatic duct were content of Calot’s triangle in 2.2 %. Double cystic artery was content of the triangle in 1 cadaver. Artery or duct was seen outside the Calot’s triangle in 16 cadavers. CONCLUSIONS It is important for surgeons to know the variations of cystic artery and Calot’s triangle during surgical interventions of the hepato biliary region. “Cystic artery syndrome” is a condition where cystic artery is seen winding around cystic duct although it originates from right hepatic artery. A new diagnosis is provided for the genus Paragrallomyia, and the common and widely distributed species Paragrallomyia albibasis (Enderlein) and Paragrallomyia thiemei (Enderlein) are redefined as part of a group of 17 superficially similar Neotropical species. This group, here treated as the ‘P. albibasis complex’, includes P. albibasis, P. inpai (Albuquerque), P. teresacristinae (Albuquerque), P. thiemei, P. vulpes (Cresson) and 12 new species (P. aequorea, P. bifurcata, P. brasiliensis, P. caliensis, P. citrina, P. colombiana, P. diminuta, P. ecuadoriensis, P. nodulosa, P. pseudoalbibasis, P. quadrifurca and P. quaternaria). Paragrallomyia parens (Cresson) is newly recognized as a junior synonym of Paragrallomyia albibasis. Taeniaptera nigriceps Hennig and Taeniaptera vulpes Cresson are transferred to Paragrallomyia and 13 other species previously treated as Paragrallomyia are transferred to Poecilotylus Hennig (Grallomyia aeripennis Enderlein, Taeniaptera aliceae Albuquerque, Grallomyia dilutimacula Enderlein, Calobata latifascia Wulp, Grallomyia lauta Cresson, Grallomyia longifurca Hendel, Calobata nigritarsis Macquart, Calobata rufifacies Macquart, Grallomyia seiuncta Czerny, Tanypoda simillima Hendel, Grallomyia strigata Enderlein, Calobata tibialis Macquart, Calobata vittipennis Coquillett). Abstract:
As fraturas com afundamento da calota craniana (FAC) são relativamente frequentes e seu tratamento neurocirúrgico está bem estabelecido, porém pouca atenção tem sido dada a esta patologia na faixa etária de 0 a 2 anos de idade. Este estudo baseia-se na análise retrospectiva de 43 pacientes com FAC e idade entre 0 e 2 anos. As principais causas da FAC foram as quedas, seguidas pelos atropelamentos. A maioria dos pacientes foi admitida em estado de alerta. O osso craniano mais frequentemente acometido foi o parietal (55,8%). A maioria apresentava FAC tipo 1, isto é, aquela na qual o osso deprimido permanece conectado à calota craniana. Tratamento cirúrgico foi instituído em 69,8% dos casos. A incidência de lesões encefálicas associadas foi mais baixa do que a relatada na literatura.

Abstract:
The intra-generic relationships of the Australian genus Calotis, with various chromosome base numbers from x = 8 to x = 4, were examined by the comparison of nucleotide sequences of the complete ITS region of nuclear rDNA and of the matK gene of chloroplast DNA. Within a monophyletic Calotis, four lineages were identified. Reconstruction of ancestral states suggests that the chromosome base number for Calotis is x = 8. Dysploidal reductions in chromosome base number from x = 8 to x = 7 and from x = 8 to x = 5 or 4 have occurred independently at least three times. Lower base numbers of x = 7, 5, and 4 are found predominantly in the arid and semi-arid zone species of Central and Western Australia. Total karyotypic length (genome size) is greater in perennials than in annuals within the genus Calotis. The elaborated pappus and surface structures of cypsela, and life form of species seem to be homoplasous with multiple origins in the evolutionary history of the lineage.

Abstract:
The montane agamid lizard genus Oriocalotes is currently considered monotypic, represented by the species, O. paulus. The systematic status of this taxon has remained questionable since its initial descriptions in the mid-1800s. A detailed molecular and morphological study was carried out to assess the validity of this genus, and its systematic position within the Asian agamid subfamily, Draconinae. Freshly collected and historical museum specimens from the type locality of O. paulus were examined morphologically, along with additional samples collected from localities in Mizoram state, Northeast India. Utilising newly generated molecular sequences (two mitochondrial and three nuclear genes), combined with those previously published for representative genera from the subfamilies Draconinae and Agaminae, Maximum Likelihood and Bayesian phylogenetic trees were constructed. Phylogenetic results suggest that Oriocalotes is part of the widespread South and Southeast Asian radiation of Calotes. Comparative morphological studies (including external morphology, hemipenis and osteology) between Oriocalotes and related genera further support this systematic placement. Oriocalotes is herein regarded as a junior subjective synonym of Calotes. Calotes paulus comb. nov. is also assigned a lectotype and given a detailed redescription based on the lectotype, paralectotypes and additional topotypic material. Furthermore, the specimens collected from Mizoram populations are found to be morphologically and genetically distinct from Calotes paulus comb. nov., and are described herein as a new species, Calotes zolaiking sp. nov.

Background: Laparoscopic cholecystectomy has become the gold standard treatment for symptomatic gallstones. However, a conversion to open surgery may be required to complete the procedure safely. The aim of this study is to identify the predictive factors of conversion from laparoscopic to open cholecystectomy in elective setting.Methods: A retrospective review of all patients underwent laparoscopic cholecystectomy electively for symptomatic gallstones from January 2016 to December 2017 was performed. Data considered for analysis were: demographic data, preoperative laboratory values of liver function tests, gall bladder wall thickness on ultrasound, preoperative ERCP, indication for surgery, history of acute cholecystitis, presence of intraoperative adhesions and frozen Calot's triangle. Conversion to open cholecystectomy was chosen as the dependent variable for both, univariate and multivariate analysis.Results: 546 patients underwent laparoscopic cholecystectomy. 333 were females (60.9%) and 213 (39.1%) males, with a mean age of 44.6 years. The most common indication for surgery was symptomatic cholelithiasis. Conversion to open cholecystectomy occurred in 48 cases (8.8%) and the most common reason for conversion was inability to define the Calot’s triangle anatomy due to inflammation/adhesions. Univariate and multivariate analyses of various variables demonstrated that male gender, gall bladder wall thickness >5 mm and presence of previous documented acute cholecystitis had statistically significant co-relation with higher rates of conversion (p<0.001).Conclusions: Presence of acute cholecystitis, gall bladder wall thickness >5mm on preoperative ultrasound and male gender were independent predictor factors for conversion from laparoscopic to open cholecystectomy. Such patients should be properly counselled about the increased risk for conversion and should be operated by surgeons experienced in laparoscopic procedures to reduce the rate of conversion and operative complications.

El linfoma de Burkitt que afecta a las amígdalas representa el 5% de los linfomas no Hodgkin primarios extranodales y ocurre usualmente en pacientes con VIH que presentan valores altos de CD4 y viremia crónica. Presentamos el caso de un varón de 37 años con diagnóstico de VIH menor de 1 año, irregular en la terapia antiretroviral combinada, que presentó una lesión ulcerada, necrótica en amígdala izquierda, y lesiones osteolíticas en calota craneana con compromiso de partes blandas a predominio occipital. En los exámenes auxiliares se halló un recuento de linfocitos CD4 de 50 cel/mm3 y carga viral de 1 250 000 copias/mL. La biopsia de amígdala fue compatible con Linfoma de Burkitt. El compromiso de calota craneana concomitante a este tumor es raro. Se inició quimioterapia con CODOX-M/IVAC (ciclofosfamida, vincristina, doxorrubicina, metotrexate + citarabina intratecal); sin embargo, la respuesta clínica fue desfavorable, a pesar de la supresión virológica posterior.

Methodology: This study consisted of one hundred & forty six patients with gallstone disease. All patients had full clinicalexamination and right hypochondrium was especially examined for assessment of murphy's sign, palpable mass and visceromegaly.Base line and specific investigations were done in all patients especially ultrasound of abdomen as diagnostic modality and forassessment of gallstone disease. Inclusion criteria was that all patients diagnosed as case of gallstone disease. LC procedure was notattempted in patients with history of abdominal surgery, pregnant ladies due to risk of foetal loss, carcinoma of gall bladder acutepancreatitis, obstructive jaundice and unfit patients for general anesthesia. Results: Out of 146 patients included in this study 133 werefemale (91%) and 13 male (9%); with female to male ratio of 10.2:1. The mean age was 39.21+6.20 years. Per Operative findings wereadhesions in calot's triangle 24(16.43%) cases, severe & tight adhesions around gallbladder and calot's triangle 21(14.38%) cases,obscured anatomy in calot's triangle 17(11.64%) cases, intrahepatic gallbladder 11(7.53%) cases, adhesions around gallbladder26(17.80%) cases, empyema 13(8.90%) cases, mucocele 9(6.16%) and anatomical variation 14(9.58%) cases. We observed out of 21patients who had Severe & tight adhesions around gallbladder and calot's triangle, lead to difficulty in performing laparoscopiccholecystectomy in 11(52.38%) cases followed by 17 cases of obscured anatomy in calot's triangle and 14 cases anatomical variationalso lead to performing difficult laparoscopic cholecystectomy in 6(35.29%) and 5(35.71%) cases respectively. Four (2.73%) cases outof 146 had to be converted to the open cholecystectomy procedure. Conclusions: We conclude our study revealed that variousperoperative factors which make the difficult laparoscopic procedure and lead to open cholecystectomy are severe adhesions in calot'striangle, severe & tight adhesions around gallbladder and obscured anatomy in calot's triangle.

No presente estudo de caso, apresentamos o relato de uma radioterapia hipofracionada realizada em paciente com câncer de pele não melanoma em couro cabeludo, destruindo sua calota craniana e ocasionando dor intensa. Paciente veio à primeira consulta em serviço de radioterapia referindo ter percebido “feridinha” em couro cabeludo há 6 meses, que por vergonha de expor à família tratou apenas com curativos. Paciente apresentou-se em bom estado geral, Escala de Performance Karnofsky (KPS): 90%, apresentando-se com lesão úlcero-infiltrativa destrutiva localizada em região parieto-occipital direita, com extensão > 25cm de diâmetro, destruindo couro cabeludo, calota craniana e infiltrando parênquima cerebral adjacente, com áreas necróticas e permeadas de pontos de sangramento, associado a odor fétido. O diagnóstico precoce desse tipo de câncer é recomendado para toda a população, necessitando que todos estejam atentos às suas principais formas de apresentação. Enfatiza-se a necessidade de sua prevenção com ações de educação em saúde direcionadas à população, com estímulo a proteção individual e atuação sobre elementos modificadores da doença.

O propósito do presente trabalho é descrever a técnica de expansão do couro cabe- ludo como etapa adjuvante à correção de contorno de calota craniana e de área de alopecia associada ao trauma, e/ou como sequela de tempo cirúrgico anterior. Foram empregados expansores de silicone de 480ml não personalizados (SILIMED - Comércio de Produtos Médico-Hospitalares, LTDA - Rio de Janeiro - Brasil), com válvula externa, no couro cabeludo para possibilitar a correção de contorno de calota craniana e de área de alopecia associada à sequela do trauma craniofacial. São descritos os procedimentos de expansão, as técnicas e recomendações de inserção e remoção do expansor. A expansão tecidual prévia do couro cabeludo constitui um recurso que permite o adequado recobrimento da região craniofacial reconstruída, evitando exposição do biomaterial e possibilitando a correção de áreas de alopecia.

Large patient series undergoing laparoscopic cholecystectomy fail to show anatomic variations which lead to intraoperative bleeding. Method: Cadaver material was used and corrosion casting and postmortem arteriography were employed. Results: Three types of cystic artery were devised according to the results. Type 1 normal anatomy. Type 2 more than one artery in Calots triangle and Type 3 no artery in Calots triangle. Discussion: only 40% of the second cystic artery is present in Calots triangle. The short second cystic artery is characteristic and its most often origin is from a segmental branch of the right hepatic artery. When there is no artery in Calots triangle its origin unusual, and the artery is either on the postero-lateral side of the cystic duct or it approaches the gallbladder through hepatic tissue. The specifics of MIS approach make changes in the way we understand the anatomic variations of the cystic artery. The classification is a result of practical experience and anatomical investigations.

The systematic status of many Asian Draconine agamids has long been in dispute. Herein, four such species, Mictopholis austeniana, Japalura kaulbacki, Calotes kingdonwardi and Salea kakhienensis are examined on the basis of external morphological characters. The monotypic genus Mictopholis was compared to other Asian Draconine genera and found to be indistinguishable from Pseudocalotes, with which it is here considered a synonym. Japalura kaulbacki is compared to other members of the genus Japalura as well as other similar species. Based on external morphology it is found to be separable from Japalura and conspecific with Calotes kingdonwardi, under which it is considered a junior subjective synonym. Salea kakhienensis is redefined morphologically based on nontype material and shown to represent a close member of this group. C. kingdonwardi is most similar to Salea kakhienensis both of which share a complement of characters with M. austeniana. The generic placement of S. kakhienensis relative to Salea, both with strongly overlapping synapomorphic characters is briefly discussed with respect to biogeographical implications that strongly support the exclusion of this species from the genus. Mictopholis austeniana, Calotes kingdonwardi and Salea kakhienensis are redescribed in detail and transferred to Pseudocalotes. The inclusion of these three species provides a generic range extension of approximately 800 km north-west of the previously considered range. The nomen Oriocalotes discolor is considered a primary objective synonym of Pseudocalotes kakhienensis new comb. and the systematic status of Calotes kingdonwardi bapoensis is briefly discussed.

Os seios venosos do crânio realizam a drenagem do cérebro e da medula espinhal, a fim de manter a homeostasia e o perfeito funcionamento do sistema nervoso central. Lesões na rede venosa cerebral podem causar déficits severos tais como hemiplegia, hemorragia, coma e morte. Os seios venosos são importantes pontos de referência para a realização de técnicas cirúrgicas de acesso ao cérebro. Este estudo visou analisar o trajeto do seio venoso sagital dorsal no crânio de cães braquicéfalos. Os animais braquicéfalos possuem crânios curtos e com características biométricas específicas. Foram utilizados 8 crânios de cães da raça Boxer, que foram submetidos à injeção de látex com pigmento corado e sulfato de bário. Após a perfusão, foram feitas radiografias contrastadas e imagens de tomografia computadorizada para relacionar o seio venoso com a estrutura óssea e dimensões relativas da calota craniana. Os crânios apresentaram índice cefálico (IC) médio de 91,24±8,34mm e índice crânio-facial (ICF) médio de 2,89±0,23mm. As mensurações do seio venoso sagital dorsal, relativas à calota craniana, apresentaram os seguintes valores médios: Área = 10,18±4,69mm²; D1 = 11,84±2,35mm; D2 = 19,57±2,61mm; D3 = 17,88±2,31mm; D4 = 25,32±5,68mm; e D5= 24,84±4,40mm.

Background: This study was conducted to evaluate results of earlycholecystectomy in acute cholecystitis in terms of procedural safety. Methods: In this study 50consecutive cases of acute cholecystitis who underwent early laparoscopic cholecystectomy(within 03 days of attack) were included. Patients with symptoms of more than 03 days durationor those with associated diseases were excluded. Evaluation of results was done by analyzingthe data in SPSS V-17. Results: Out of 50 patients operated 46 (92%) were female and 4 (8%)were male. Most of the patients were received within 24 hours after the onset of symptoms. Theage of patients ranged from 30-70 years with the median age of 45 years. Ultrasound revealedoedematous gall bladder with pericholecystic fluid in 38 (76%) patients. In 8 (16%) patients, gallbladder was small, shrunken and thick walled, there was empyema in 2 (4%) patients &mucocele with stone impacted at hartmann’s pouch in 2 (4%) patients. Average operation timewas 40 minutes. All patients were operated within 72 hours of onset of symptoms. Per operativelysevere inflammation was noted in 32 (64%) patients, adhesions with colon/stomach/omentum in10 (20%) patients, adhesions with CBD in 3 (6%) patients, distorted anatomy at Calot’s triangle in5 (10%) patients. 6 (12%) patients had bleeding from liver bed, but controlled with diathermy. In46 (92%) patients laparoscopic cholecystectomy was completed successfully. In 4 (8%) patients,laparoscopic procedure was converted to open cholecystectomy. Reasons of conversion wereacute cholecystitis with severe adhesions which caused bleeding in 2 (4%) patients, obscureanatomy of Calot's triangle in 2 (4%) patients. Post operatively, there was bile leak in 1 (2%)patient which was due to minor injury of CBD, which required re-exploration & suturing of defect.No patient developed post operative jaundice. There was no major bleed post operatively. 3 (6%)patients developed wound infection. Conclusions: Laparoscopic cholecystectomy is aneffective and safe technique of treating symptomatic gallstones even in cases of acutecholecystitis because of accelerated recovery couple with less postoperative pain and shorthospital stay.

RÉSUMÉ La géomorphologie et les dépôts quaternaires de la partie ouest de l'île d'Anticosti sont principalement associés à la glaciation du Wisconsinien supérieur et à la déglaciation au contact de la mer postglaciaire de Goldthwait. Le till de l'île a été mis en place au Wisconsinien supérieur au cours de la dernière avancée de l'inlandsis laurentidien. La dynamique de la glace à l'époque démontre une faible capacité d'érosion, le front du glacier se maintenant probablement près de la côte sud (actuelle) de l'île. Lors de la déglaciation, une calotte résiduelle s'est maintenue sur l'île. Une récurrence, survenue vers 12 500 ans BP, a permis l'édification de la moraine de Sainte-Marie. Cette moraine est formée de bourrelets morainiques sur la côte sud et d'épandages proglaciaires dans l'ouest et au nord de l'île. La calotte a existé pendant environ 1000 ans. Au cours de sa fonte, la mise en place du till d'Elsie s'est faite en association avec celles de nombreuses moraines de De Geer au nord des bourrelets de la moraine de Sainte-Marie, parallèlement à la côte. Une dernière récurrence venant du nord a permis la mise en place de la moraine de la rivière à l'Huile, probablement vers 9000 - 9500 ans BP, ce qui serait contradictoire avec les données provenant de la Côte-Nord du Saint-Laurent. Entre 11 000 et 9000 ans BP, le taux d'émersion a été de 2 m par siècle; il s'est abaissé par la suite à 25 cm par siècle.

Background: Surgery in chronic cholecystitis is very challenging because of inability to hold the gall bladder, dense adhesions, frozen Calot’s triangle and difficulty in applying clips. Precise and meticulous dissection is required to establish critical view of safety. There is no consensus among surgeons about appropriate intraoperative steps in difficult gall bladder (GB) surgery. The authors aim to present various intraoperative difficulties and strategies to overcome them. Methods: A prospective study of 81 patients of chronic cholecystitis was done in our institution. They were divided in two groups. Group A in which surgery could be done easily. Group B in which surgery was difficult and different intraoperative strategies were applied to overcome them. Results: Total 42 patients were included in group A and 39 patients in group B. Various difficulties encountered while performing laparoscopic cholecystectomy in group B were adhesions (53.8 %), inability to grasp the fundus of GB (15.3%), frozen Calot’s triangle (15.3%), inability to grasp the Hartmann’s pouch (12.8%) and cystic duct edema (2.5%). Conclusions: Intraoperative technique of identification of Rouviere′s sulcus first, followed by high peritoneal incision on the GB body. Subsequently blunt dissection of Calot’s triangle using gauze piece and hydro dissection by suction irrigation canula ventral to the sulcus. It created a retro gall bladder tunnel safely. It established the critical view of safety in all our cases.

La distribution des dépôts de marge glaciaire de la région de Rimouski a été analysée en fonction du cadre morphostructural préglaciaire, caractérisé par un relief appalachien typique dont les barres et les sillons, orientés NE-SO, sont subparallèles à la vallée du Saint-Laurent. Cette étude met en lumière l'influence déterminante du contexte morphostructural sur le mode de déglaciation, tant à l'échelle régionale que locale. L'invasion de la Mer de Goldthwait le long du Saint-Laurent, à partir de 14 000 BP, isole progressivement une calotte glaciaire régionale dans les Appalaches. Vers 13 400 BP, la marge septentrionale de la calotte appalachienne suit le tracé de la limite marine, fixée à 140 m d'altitude entre Rimouski et Luceville. Elle s'est stabilisée le long des premières grandes barres appalachiennes qui représentaient la première ligne d'ancrage de la marge glaciaire après l'invasion marine. Durant la phase de retrait qui suit, entre 13 360 et 12 700 ans BP, des masses de glace résiduelles sont abandonnées dans la vallée de ligne de faille de la Neigette et les sillons appalachiens de Saint-Fabien qui s'encombrent de formes de décrépitude (kames et kettles). Entre 12 400 BP et environ 12 000 BP, la marge glaciaire se stabilise sur le rebord du plateau de Sainte-Blandine, à moins de 13 km du littoral actuel. Cette période se signale par une récurrence glaciaire mise en évidence dans trois sites différents. Les modalités et la chronologie de la déglaciation au-delà de la limite marine ne sont pas connues.

SUMMARYPolyhedral scene analysis studies whether a 2D line drawing of a 3D polyhedron is realizable in space, and if so, it gives the results of parameterizing the space of all possible realizations. For generic 2D data, symbolic computation with Grassmann–Cayley algebra is needed in the analysis. In this paper, we propose a method called parametric calotte propagation to solve the realization and parameterization problems for general polyhedral scenes at the same time. In algebraic manipulation, parametric propagation is more efficient than elimination. In applications, it can lead to linear construction sequences for nonspherical polyhedra whose resolvable sequences do not exist.
