Abstract:

Abstract:
Puerto Rico, acquired by the United States from Spain in 1898, has a peculiar status among Latin American and Caribbean countries. As a Commonwealth, the island enjoys limited autonomy over local matters, but the U.S. has essentially dominated it militarily, politically, and economically for much of its history. Though they are citizens, Puerto Ricans do not have their own voting representatives in Congress and cannot vote for the president or VP. The island's status is a topic of perennial debate, evidenced by the 2012 referendum, in which a majority voted for statehood for the first time. More recently, the island's colossal public debt has sparked an economic crisis that is the focus of an upcoming Supreme Court case. The issue is intimately tied to the question of status, and consensus on the solution has proven elusive. Despite their ongoing colonial dilemma, Puerto Ricans display a strong national identity-after 118 years of occupation, the Island remains a Spanish-speaking, Afro-Hispanic-Caribbean nation. At the same time, the island's population is constantly in flux, with an estimated 60.7% of boricuas living stateside, while many others are also returning to the island. Despite the island's popularity as a tourist destination, few beyond its shores are familiar with its history. Puerto Rico: What Everyone Needs to Know provides a succinct, authoritative, and well-documented introduction to the Island's rich history, culture, politics, and economy. Jorge Duany, takes on the task of educating readers on the most important facets of the unique, troubled, but much beloved isla del encanto.

Full text
Abstract:
Breast cancer is the most frequently diagnosed cancer in women worldwide, with annual estimates of 1.7 million newly diagnosed cases and 522,000 deaths. Although more breast cancers are diagnosed in economically developed than in developing countries, the reverse is true for mortality, reflecting limited screening and less effective treatments in the latter. Breast cancer incidence has been on the rise in the United States for many years, but in recent years this is restricted to certain subgroups, while internationally there have been continued generalized increases, likely reflecting adoption of more Westernized lifestyles. Breast cancer is widely recognized as being hormonally influenced, with most of the established risk factors believed to reflect the influence of cumulative exposure of the breast to stimulatory effects of ovarian hormones—leading to increased cellular proliferation, which in turn can result in genetic errors during cell division.

Abstract:
The epidemiology of stomach cancer is characterized by its wide variation of incidence by geography and population. Despite a declining secular trend of incidence in most Western populations, it is still one of the most common cancers worldwide. Helicobacter pylori infection is the strongest and most important risk factor known today. Although eradication of H. pylori might be an efficient primary prevention strategy, a deeper understanding of effect-modifying factors, including bacterial genetics, human genetics, and environmental risk factors, may enable us to focus even more precisely on the relevant high-risk groups. Dietary factors, particularly the low intake of antioxidant-rich fruits and vegetables and high intake of salt, are likely to be important. The roles of microbes other than H. pylori in the oral cavity and stomach have recently gained increasing attention. A subtype of stomach cancer, cardia cancer, appears to have a different epidemiology and risk factor profile.

Full text
Abstract:
Testicular cancer is highly curable and, though relatively rare, it is the most common cancer among young men. Incidence of testicular cancer has been increasing constantly around the world since the beginning of the twentieth century, but factors responsible for the rise in incidence remain enigmatic. Only few risk factors for testicular cancer are established, including age, ethnic group, cryptorchidism and hypospadias, contralateral testicular cancer, family history, and height. While analytic epidemiologic research has provided numerous etiologic clues, many of them remain tentative. Overwhelming evidence indicates the fundamental importance of environmental factors in the etiology of this enigmatic cancer. Prenatal exposures seem to be instrumental in shaping the risk of testicular cancer, but postnatal exposures acting in childhood, adolescence, and very early adulthood are also important. Testicular cancer has also a strong genetic component that is studied through international collaborations and genome-wide association studies.

Silverman, Debra T., Stella Koutros, Jonine D. Figueroa, Ludmila Prokunina-Olsson, and Nathaniel Rothman. Bladder Cancer. Oxford University Press, 2017. http://dx.doi.org/10.1093/oso/9780190238667.003.0052.

Full text
Abstract:
In the United States, an estimated 74,000 new cases of bladder cancer and 16,000 deaths occur annually. Because of favorable survival, approximately 578,000 persons are alive after diagnosis. Recurrence after treatment is common, necessitating lifelong surveillance and care, making bladder cancer the most costly malignancy from diagnosis to death. The median age at diagnosis has increased from 67 years in 1980 to 73 years in 2008–2012. Incidence rates rise dramatically with age, suggesting that as the US population ages, the burden of bladder cancer in the United States will continue to grow. Worldwide over 1.3 million cases are estimated to occur annually. Cigarette smoking is the principal known cause of bladder cancer, along with various chemical exposures. Recent studies suggest that, over the past two decades, the risk for current smokers has increased from three times the risk of nonsmokers to about four to five times the nonsmoker risk.

Full text
Abstract:
Stomach cancer is the fifth most common incident cancer worldwide and the third leading cause of cancer death. Almost half of the world’s cases occur in Asia, with 42% in China alone. Although the incidence and mortality rates from stomach cancer are decreasing, the global disease burden remains high. Moreover, the absolute number of cases continues to rise because of population aging. Adenocarcinomas comprise over 90% of gastric malignancies. The adenocarcinomas are further classified according to anatomic location (cardia vs. non-cardia), histology (e.g., intestinal or diffuse, signet ring or non-signet ring) and most recently by molecular classification. Adenocarcinomas in the body and antrum of the stomach are usually caused by chronic infection with Helicobacter pylori (H. pylori); the incidence of these tumors is decreasing worldwide. Cardia tumors have epidemiological characteristics more similar to those of esophageal adenocarcinoma; the incidence of these tumors is increasing, particularly in high-income Western countries.
