Full textCarabaya people from peru and kallawaya people from bolivia formed before the inka times and in the inkanato a single kurakazgo or chieftainship whose the two halves, hatun and huchuy, have been separated during the spanish colony. Carabaya kallawaya have played a political part in the tawantinsuyu and their territory has been the gateway to the chunchu's and anti's world. Moreover, they have been favoured by the inka. This is due to the fact that their activities have taken on, and still take on an eminent sacred and symbolic character : the hatun carabaya half was specialized in the gold production, coca and corn, whereas the huchuy carabaya half turned out itinerant healer-medicine men, holder from shamanic secrets. Ancient texts, but also the tradition lived today permit to rediscover the unity of the ancient kurakazgo cut in two by the hazards of history. Beyond the parting, carabaya from peru and kallawaya from bolivia perpetuated in a parallel way their common tradition and the complementarity from the two halves and have maintained alive their perception from the world by assimilation of new elements from modernity. This is recognizable through the representation system and in the realizations and technics, particularly in the medicine art and in the weawing whose the symbolic aspect is obvious. On both sides of the border, carabaya kallawaya have unfolded strategies of adaptation and resistance sometimes different, but they are still recognizable as an ethnical entity

Full textCardiac hypertrophy is an adaptative mechanism developed in response to hemodynamic overload. During chronic stress, it can lead to heart failure (HF), one of the most important causes of mortality in industrialized countries. Hypertrophic signaling pathway induced under α and β-adrenergic receptor involve small G protein Ras and calcium dependent proteins suchas the calcineurine (CaN) and the Ca2+/calmodulin-dependent protein kinases II (CaMKII), as well as their respective transcription factors NFAT and MEF2. In immune cells Carabin protein has been shown to inhibit CaN and Ras. The purpose of my thesis was to determine the role of Carabin in the cardiomyocyte. In vitro, we showed that Carabin is expressed in neonatal and adult rat cardiomyocyte and inhibits via its functional domains the Ras/ERK, CaN/NFAT and CaMKII/MEF2 signaling pathways. Under adrenergic chronic stimulation, Carabin overexpression blocks the development of hypertrophic markers (cell size increase, re-expression of embryonic genesand contractile protein reorganisation) preventing thus cardiac remodeling. Finally, in a rat model pressure overload cardiac hypertrophy, intra-cardiac injection of Carabin adenovirus inhibits cardiomyocyte growth. This study opens new insights into the treatment of HF

Full textThe training of specialised French physicians is studied through liminary behaviours belonging to cultural customs, with their own symbolic efficacy. During internship, some stages appear. First, dissections, which will forever differentiate doctors from all others. Next, after discussion of specifics about French internship, the "manners" students have to learn during their internship: a baptism celebrates their entrance into their new community; specific rules or manners characteristic to their staff waiting rooms; finaly, funerals which end their lives as interns. In addition, revues, satirical shows created by the youngest for the eldest, show the continuity and coherence of the career and the logic of the cultural customs studied here. Finaly, these apprenticeship years appear as a true initiation that changes those who go through it as such forever. The originality of this training lies first in the fact that official and traditional knowledges are built and learned together, in the middle of the hospital; and second in the subject of medical training itself, which leads to repeated transgression of the taboos of death, nudity and sexuality

Full textCardiac hypertrophy is an adaptive mechanism which occurs in response to hemodynamic stress. However, disease-related stresses can promote pathological cardiac hypertrophy and heart failure (HF). At present, our understanding of cardiac hypertrophy and pathological remodeling is still incomplete. In the first part of our study, we characterized Carabin, a protein expressed in cardiomyocytes that was downregulated in cardiac hypertrophy and HF. Four weeks after transverse aortic constriction (TAC), Carabin deficient mice developed exaggerated cardiac hypertrophy and accelerated the transition from hypertrophy to HF. Conversely, cardiac overexpression of Carabin through a gene transfer approach prevented TAC induced cardiac remodeling. Mechanistically, Carabin carries out a tripartite suppressive function. Indeed, Carabin through its Calcineurin (CaN) interacting site and Ras- GAP domain acts as an endogenous inhibitor of Ras and two calcium sensitive proteins, CaN and CaMKII. In the second part, we investigated the role of Epac1 (Exchange protein directly activated by cAMP 1) in béta -adrenergic receptors (béta -ARs)- induced cardiac hypertrophy. Our data showed that Epac1 deletion in mice protected against béta-ARs induced cardiac remodeling and improved cardiac function. Interestingly, béta-ARs-Epac1 activation promoted autophagic activity via Rap2B/CaMKKbéta/AMPK pathway to inhibit Epac1-induced cardiomyocyte hypertrophy. All of this work has identified the roles and mechanisms of action of Carabin and Epac1 in cardiac hypertrophy. Our study opens new therapeutic avenues for the treatment of HF
