Abstract:
Background: A carbuncle can be described as a cluster of boils, uncomfortable, and pus filled bumps - which develop a linked part of infection below the skin. A boil is known as a hurtful, pus-filled bump which created beneath your skin when harmful bacteria invade and irritate a number of the hair roots. Deroofing is regarded as a tissue saving method where the roof of a carbuncle, sinus, or cyst system is electro-operatively eradicated. The management of carbuncles is early usage of anti-biotic and surgical treatment. The surgical method used in the research is the deroofing and cruciate incision. Aim and Objective: In this research, the author desired to examine the effectiveness, patient satisfaction, and comparison of the deroofing and cruciate incision method of carbuncles. Methods: Open research included deroofed lesions in 88 sufferers with carbuncle as well as 122 with cruciate incision, managed through the clinicians of Jinnah Hospital Karachi Surgical Ward with a follow-up duration of around one year. Results: In deroofing, 30 of 88 (17 Percent) treated carbuncles revealed a repeat after a mean of 4.6 months. In most, Seventy-three treated carbuncles (83 %) didn't show a repeat after the median follow-up of 12 months. Conclusion: The cruciate incision is probably the most popular method of surgical treatment of cruciate incision. The deroofing strategy is also an effective, simple, non-invasive, tissue saving medical therapy to treat mild-to-moderate carbuncles at set locations, and it's appropriate as an office process. Abstract:
Analyzed the features of the surgical treatment of carbuncles. The severity of the purulent-necrotic process was assessed, the significance of the timely diagnosis of the inflammatory stage and the choice of the timing of surgical treatment is shown. The features of the options of surgical tactics - from gentle to radical surgical interventions, methods of intraoperative assessment of tissue viability in the inflammatory focus are described. The necessity of a differentiated approach to the treatment of carbuncles depending on the stage of the disease, the prevalence of the pathological process and its localization is substantiated. The significance of restorative skin-plastic surgery in the replacement of postnecrectomy defects of epithelial tissues in the treatment of extensive carbuncles is shown. Ways of improving the results of treatment of patients with carbuncles are outlined, including timely diagnosis, a differentiated approach to surgical treatment, rational antibacterial therapy, and adequate general treatment.

AbstractObjective:This paper reports on a non-conventional method for the management of facial carbuncles, highlighting its superiority over conventional surgical treatment in terms of cosmetic outcome and shorter duration of wound healing.Background:The mainstay of treatment for carbuncles involves the early administration of antibiotics in combination with surgical intervention. The conventional saucerisation, or incision and drainage, under normal circumstances results in moderate to large wounds, which may need secondary surgery such as skin grafting, resulting in a longer duration of wound healing and jeopardising cosmetic outcome.Case reports:The reported three cases presented with extensive carbuncles over the chin, face and lips region. In addition to early commencement of intravenous antibiotics, the pus was drained, with minimal incision and conservative wound debridement, with the aim of maximal skin conservation. This was followed by thrice-daily irrigation with antibiotic-containing solution for a minimum of 2 consecutive days. The wounds healed within two to four weeks, without major cosmetic compromise.Conclusion:The new method showed superior cosmetic outcomes, with a shorter duration of wound healing. Conservative surgical management can be performed under regional anaesthesia, which may reduce morbidity and mortality; patients with facial carbuncles often have higher risks with general anaesthesia.

Abstract:
A 26-year-old female presented recurrent painful, carbuncles at both axillae for 10 years. It caused offensive odor and scar. Tissue diagnosis was chronic hidradenitis suppurativa. She was treated using antibiotics followed by multiple excisions with drainage but showed no improvement. Isotretinoin provided no benefit. She was socially isolated and experienced lower quality of life. Consequently, she was sent for radiation therapy. Computed tomography simulation was performed revealing an ulcer with deep chronic pus tracts at her axilla. Three-dimensional conformal radiation therapy was provided with 6 MV photon (7.5 Gy in 3 fractions) covering all ulcers and pus tracts. On the last day of radiation therapy, carbuncles and wounds at the left axilla exhibited much improvement without pus. Three months follow-up showed much improvement of the lesions. The skin was smoother without pus or odor. Radiation therapy was confirmed one treatment option for chronic hidradenitis suppurativa.

The present study has been taken up to review one of the ethno medicinal significant of Ipomoea pes-tigridis L plant belonging to the family convolvulaceae. The ethno medical information of the plant reveals that the herb Ipomoea pes-tigridis Linn was used as an antidote to dogbite, boils and carbuncles, the leaves were applied as a poultice for boils, carbuncles and sores. The root of this plant was found to possess purgative action. The phytochemical studies on the leaves have been reported for the presence of carbohydrates, proteins and resins.

Abstract:
Selection of medications to apply locally for treatment of abscessive forms of boils and carbuncles of maxillofacial area still remains a debatable issue, despite their wide range. Alternative means for application use in surgical interventions in maxillofacial area are medications of natural origin. Drugs of natural origin that have anesthetic and anti-inflammatory properties include cayenne pepper fruits (Capsicum annuum). Therapeutic properties are caused by the presence in their structure of the main active ingredient – capsaicin alkaloid and other biologically active compounds (vitamins, flavonoids, minerals, etc.) which influence the main pathogenetic inflammation bonds. Capsaicin has a distinct keratoplastic effect and normalizes microcirculation in the tissues. Phytogel with anti-inflammatory and anesthetic action (patent for utility model No. 30032 “Phytogel with anti-inflammatory and anesthetic action”) was developed for clinical use based on phytosubstance of pepper cayenne fruits (patent for utility model No. 19547 “Method of obtaining phytosubstance with anti-inflammatory action”). The purpose of the study was to increase treatment efficiency of boils and carbuncles of maxillofacial area by using a medication based on the capsicum annuum fruits. Materials and methods. The analysis of inspection and treatment results of 45 patients having boils and carbuncles of maxillofacial area at abscessing stage is carried out. On the first, third, seventh and tenth day of treatment, a number of clinical indicators (presence and intensity of edema and infiltrate, condition of regional lymph nodes, presence and intensity of pain in the inflammation site, time of wound exudate clearance, intensity of regeneration, length of hospital stay treatment) and indicators obtained by laboratory methods (total number of leukocytes, leukocyte intoxication index, erythrocyte sedimentation rate) were studied and compared with the same parameters in the control group, where traditional treatment was carried out; and when using the comparison medication - 3% propolis ointment. Results and discussion. Analysis of the results showed that despite the similarity of the clinical picture and laboratory parameters on admission to the hospital, the healing of purulent wounds under the influence of capsicum annuum phytogel was more effective comparing with traditional methods of surgical treatment. Thus, wound epithelialization of patients in the experimental group No. 1 was observed on average on the seventh day, which was significantly faster than in other experimental groups. Sanation of the inflammatory focus also decreased body intoxication, which can be traced by the dynamics of changes in the degree of leukocytosis, leukocyte intoxication index, erythrocyte sedimentation rate. On the tenth day, the clinical blood parameters of patients of the experimental group No. 1 were close to the average norm. As a result, the length of stay in hospital was significantly reduced. The average bed-day for the experimental group No. 1 was 7.5 days, which is authentically less than for patients of the control and experimental group No. 2. Conclusion. According to the results of clinical and laboratory studies, the efficiency of the proposed method has been proved in comparison with traditional treatment of boils and carbuncles of maxillofacial area

Background: Hidradenitis suppurativa clinical appearance is painful, deep-seated inflamed lesions, most commonly found on axillary, inguinal, anogenital regions. It should be differentiated with primary cutaneous bacterial infection (abscesses, carbuncles, or furunculosis) and rare conditions such as lymphogranuloma venereum.Case: we report a 50-year-old male presenting with an unusual clinical presentation of hidradenitis suppurativa located in suprapubic such as bilateral fluctuant and suppurative nodules resembling lymphadenitis, led to an initial diagnosis of lymphogranuloma. We excluded lymphogranuloma diagnosis based on the serology test of Chlamydia trachomatis, bacterial cultures, and sensitivity tests. The results were non-reactive for the Chlamydia trachomatis serology test, and Staphylococcus aureus was found in bacterial culture.Conclusion: clinicians must be aware of diagnosis precisely, prevent the recurrent, and improve the prognosis.

Abstract:
Ruxiang, one of the most commonly imported herbs, has been mentioned in Chinese literature since the Wei and Jin periods. With the growth of overseas trade during the Tang and Song periods, the massive importation of herbs, including Ruxiang, brought about significant changes in the understanding and utilization of Ruxiang. This study examines historical changes in the perception and utilization of Ruxiang in China. In the literature of the third century, Ruxiang was mentioned as a “strange item from the south” and understood as a “medicinal herb.” Particularly during the sixth century, TaoHongjing(陶弘景)’s Bencaojingjizhu本草經集注 focused on the therapeutic effects of Ruxiang for treating carbuncles and abscesses, and specific prescriptions that used Ruxiang were related to these ailments. This understanding continued until the early Tang Dynasty, during which Ruxiang was mainly used for treating carbuncles and abscesses or intestinal gas and bad odor. The perception of the efficacy of Ruxiang changed in the eighth century. Notably, ChenCangqi(陳藏器) emphasized its efficacy in treating stroke, female infertility, blood circulation, deafness, and diarrhea in Bencaoshiyi本草拾遺. However, there are no records of Ruxiang being used to treat these conditions in formulary books(醫方書) compiled during that time as well as in Waitaimiyao外台祕要. By the end of the late Tang and Five Dynasties periods, with the establishment of local governments and the development of overseas trade, the importation of Ruxiang increased considerably, resulting in significant changes in how it was perceived and used. With Rihuazibencao日華子本草 compiled in the southern region in the tenth century, a more advanced understanding of the efficacy of Ruxiang emerged, which included protecting the regularity of Qi(氣), protecting the waist and knees, stopping pain, and promoting skin regeneration. Additionally, Taipingshenghuifang太平聖惠方 compiled during the Song Dynasty included various prescriptions that reflected the changing perception of Ruxiang’s efficacy since the eighth century, including treatments for stroke, female infertility, and diarrhea. Bencaotujing本草圖經 from the mid-Northern Song Dynasty emphasized the efficacy of Ruxiang in treating blood and promoting circulation, a fact that was further confirmed in Taipinghuiminhejijufang太平惠民和劑局方 prescriptions added during the Southern Song period. Ruxiang’s effectiveness in ‘boosting the meridian circulation,’ which was particularly mentioned in the added prescriptions during the Southern Song period came to be highlighted by Yishuixuepai易水學派 and medical scholars such as Zhangyuansu張 元素 and Wanghaogu王好古 during the thirteenth century. In conclusion, in the sixth century, Ruxiang was understood to be effective at treating carbuncles and abscesses and dispelling negative energy. Such an understanding of Ruxiang’s efficacy shifted toward an emphasis on its ability to invigorate blood circulation and alleviate pain in the eleventh to thirteenth centuries. This evolution reflected an expanded understanding of Ruxiang’s efficacy and the gradual establishment of an increasingly theoretical framework to explain it. Additionally, the expanded understanding of Ruxiang’s efficacy can be observed in the many usages to which it was put in medical books, which expanded significantly from the tenth and eleventh centuries onward, during the time of large-scale Ruxiang importation. The appearance of medical prescriptions that used Ruxiang in the Song Dynasty’s Taipingshenghuifang太平聖惠方 likely resulted from the expanded understanding of Ruxiang’s efficacy and the increased importation of Ruxiang from the late Tang Dynasty to the Five Dynasties periods.

Abstract:
Plants are an important source of natural products and they play a vital role in the field of medicinal chemistry and pharmaceutical science. Traditional medicines have been practiced and used for thousands of years, mostly in Asian countries, where plants are the main sources of medicine. Houttuynia cordata, a herb that belongs to the family Saururaceae, has a wide range of pharmacological activities and is used traditionally in conditions like anisolobis sores, heatstroke, lung carbuncles, malaria, scrotal abscess, tonsillitis, salammoniac poison and has also been widely accepted to possess anti-cancer, anti-oxidant, anti-hypertension, anti-inflammatory, anti-mutagenic, antibacterial, anti-viral and anti-purulent activity. Moreover, it is one of the herbs that was recognized during pandemic outbreaks, such as Severe Acute Respiratory Syndrome Coronavirus (SARS CoV) in China, virulent Newcastle Disease Virus (VNDV) in Java (Indonesia) and Newcastle (England). In this review, we briefly discuss the role of H. cordata as an anti-viral agent and the possibility of developing a dosage form against Coronavirus disease-19 (COVID-19).

Abstract:
Did Jan Žižka die of the pestilence? The author raises the question of the causes of the death of Jan Žižka (1360–1424), Czech national hero and one of the most able leaders of the Hussite revolution between 1419 and 1424. Žižka died suddenly, after a brief illness during the Moravian campaign in October 1424. The illness had symptoms and a course indicating bubonic plague, which from the mid XIVth century assailed the Czech lands, Moravia, and Silesia, as is confirmed by XVth-century and XVIth-century sources, including the Staré letopisy české and the chronicle of Eneasz Sylwiusz Piccolomini. In the XIXth century, this diagnosis was questioned in favour of a severe skin complaint, which produced cluster boils or carbuncles. Because no mortal remains of Žižka have survived, a genetic analysis of bone material is impossible that would establish the presence of plague-causing Yersinia pestis bacteria. Thus, the problem remains to the present in the sphere of hypotheses and discussion. This article is a contribution to the debate.

The high prevalence of skin and soft tissue infections (SSTI) urges healthcare professionals to improve the traditional (operative and pharmacological) methods of their treatment, and to find new methods that are applicable in outpatient practice, which can reduce the period of disability and reduce the number of complications. The choice of the right drug for such infections to influence the local infectious process is one of the most important objectives of the local wound therapy. The paper presents data on the effectiveness of Povidone-Iodine (Betadine) for external use with a broad spectrum of action, intended for the treatment of SSTI, including abscesses, furuncles, carbuncles, infected traumatic lesions of the skin and trophic ulcers in Phases 1 to 2 of the wound process. The drug has a pronounced combined effect: antimicrobial (bactericidal, bacteriostatic), wound healing, regenerating, local anesthetic and protective. This brings us to the conclusion about the advantages of Betadine and provide a rationale for its use to treat surgical infections of the skin and soft tissues in the outpatient setting.

The diabetic complications have become a world health problem. They prevail throughout the world but their percentages differ in different areas due to cultural influences. Therefore, we have determined prevalence of complications of diabetes in the rural population of district Jhang (Pakistan). For this purpose, a performa was developed and information was collected from two hundred and ten (210) diabetic patients about their age, sex, height, socioeconomic status, educational status, type of diabetes, duration of diabetes, age at diagnosis, blood pressure and blood sugar levels ( fasting and random) and the symptoms of diabetes. Mainly the complications were compared with different variables or risk factors like diet, type of diabetes, obesity, sugar levels and uncontrolled diet. Retinopathy, carbuncles, pregnancy and hypertension complications in both sexes had non-significant (P>0.05) relationship with controlled diet but significant (P<0.05) with uncontrolled diet. Nephropathy and neuropathy showed significant relationship with controlled diet and non-significant with uncontrolled diet. Retinopathy, neuropathy and gangrene in both sexes were highly significantly (P<0.001) related with both types of diabetes.

The plants of the genus Paris L. in the family Melanthiaceae have high medicinal value. Paris xuefengshanensis H.Li, Z.Wang & G.W.Hu, sp. nov. ined., found in Ma'lin Yao Township, Xinning County, Hunan Province, China, is a new species with traditional uses and cultivation in the local area. The main goal of this study is to understand the traditional knowledge of the Yao ethnic villagers in Malin Township, Xinning County, regarding the use of Paris xuefengshanensis through ethnobotanical field surveys. This includes gathering information on local names, uses, and methods of utilization. Additionally, the research aims to review the current status of pharmacological studies on plants belonging to the Paris genus through literature research. Research on the pharmacological activities of the genus Paris mainly focuses on its anti-tumor, anti-inflammatory, and hemostatic activities. Additionally, it possesses sedative and analgesic properties, as well as antibacterial, anti-myocardial ischemia, antioxidant, and immune-regulating effects. The survey results found that Paris xuefengshanensis is effective in treating unidentified swellings, snake bites, contusions, and carbuncles. It also has certain effects in pain relief and tumor treatment.

Arnebiae Radix (dried root of Arnebia euchroma (Royle) Johnst.) has been used in traditional Chinese medicine (TCM) to treat macular eruptions, measles, sore throat, carbuncles, burns, skin ulcers, and inflammation. Previous studies have shown that shikonins and shikonofurans are two of their main bioactive ingredients. However, systematic investigations of their constituents have rarely been conducted. It is necessary to establish a rapid and effective method to identify the chemical constituents of Arnebiae Radix. This will help to further improve the effective resource utilization rate of this plant. In this study, a rapid and effective UHPLC-Q-Exactive Orbitrap mass spectrometry method was established to simultaneously analyze chemical ingredients in Arnebiae Radix within a short period of time. Based on the results of a full scan MS, the MS2 database (mzVault and mzCloud), the diagnostic fragment ions, the retention time, and the bibliography, a total of 188 compounds were identified, with 114 of those being reported from Arnebiae Radix for the first time. The results of this study lay the foundation for obtaining a thorough understanding of the active ingredients in Arnebiae Radix and its quality control. This method may be widely used for the chemical characterization of different samples.

The incidence of inflammatory bowel disease (IBD), which predominantly comprises Crohn’s disease and ulcerative colitis, is increasing worldwide. However, the treatment of IBD still faces great challenges. The involved NF-κB is the main signaling pathway in human IBD and thus is a prime target. There is abundant evidence that Tou Nong San (TNS), which is a traditional Chinese medicinal decoction used for treating sores and carbuncles, has a positive effect on the inflammation. This study investigated the effects of oral administration of TNS on colitis induced by 2,4,6-trinitrobenzenesulfonic acid (TNBS) and the underlying mechanism(s). Quality control of the major compounds in TNS was performed by high-performance liquid chromatography, and six chemical constituents were identified in aqueous extracts. TNS led to improvements in weight loss and water and food intake in rats. The macroscopic and microscopic scores of rat tissues greatly decreased. Protein and mRNA levels of proinflammatory cytokines, including interleukin-17 (IL-17), tumour necrosis factor-α, IL-1β, and IL6, involved in the NF-κB signaling pathway were greatly reduced. The results suggest that the anti-inflammatory effect of TNS is associated with the regulation of the NF-κB signaling pathway, which contributes to the network pharmacological effect of TNS on human IBD in clinical practice.

Paris polyphylla is often used in Chinese medicine to treat conditions such as carbuncles, trauma, snake bites, and mosquito bites. In the present study, we investigated the effect and mechanism of the morphological transition and extracellular phospholipase activity of Candida albicans treated with polyphyllin I (PPI). First, the minimum inhibitory concentration and antifungal activity of PPI were evaluated using the multiple microdilution method and time-killing assays. Then, the effect of PPI on the morphological transition of Candida albicans in Spider liquid medium and Sabouraud-dextrose liquid medium containing 10% fetal bovine serum was observed under an inverted microscope and by scanning electron microscopy. Finally, egg yolk agar plates were used to evaluate extracellular phospholipase activity. Gene expression was detected by real-time quantitative polymerase chain reaction analysis. Our results suggest that PPI inhibited the transition from the yeast to the hyphal stage and decreased secreted aspartyl proteinase activity. We further confirmed that PPI significantly downregulated the expression of extracellular phospholipase genes and cAMP-PKA signaling pathway-related genes. Taken together, our results suggest that PPI exerts anti-Candida albicans activity by inhibiting virulence characteristics, including the yeast-to-hyphal transition and the secretion of aspartyl proteases and phospholipases. The study results also indicated that PPI could be a promising therapeutic strategy for Candida albicans.

Tou Nong San (TNS) is a traditional Chinese medicinal decoction used to treat sores and carbuncles. It contains four herbal drugs and one animal medicine: Radix Astragaliseu Seu Hedysari,Angelica sinensis, Ligustici Chuanxiong, Spina Gleditsiae, and stir-baked Squama Manis. Previous studies have shown that it has anticancer effects. This report validatesin vivoantitumor properties of TNS. The compounds contained in TNSE were confirmed by liquid chromatographmass spectrometer (LC-MS) analysis. Thein vivoantitumor activity of TNS extract (TNSE) was tested by feeding it to athymic mice harboring a human colonic tumor subcutaneous xenograft. Toxicity was monitored by recording behavior and weight parameters. Seven compounds were detected in TNSE by LC-MS. TNSE was fed to athymic mice for 2 weeks. No adverse reactions were reported. Compared to the control group, administration of TNSE to tumor bearing mice significantly reduced both tumor weight and volume. The expressions of p-PI3K, p-AKT, p-mTOR, p-p70s6k1, VEGF, and CD31 were significantly reduced, the expression levels of cleaved Caspase-9 and cleaved Caspase-3 were significantly increased in the TNSE groups compared to the control group as determined by western blot and immunohistochemistry. TNSE produced anticolonic cancer effects and the underlying mechanisms involved inhibition of the PI3K/AKT signal transduction pathway, inhibition of angiogenesis, and promotion of apoptotic proteins.

SUMMARYIn England, hospital admissions for severe staphylococcal boils and abscesses trebled between 1989 and 2004. We investigated this trend using routine data from primary and secondary care. We used The Health Improvement Network (THIN), a large primary-care database and national data on hospital admissions from Hospital Episode Statistics (HES). Time trends in the incidence of primary-care consultations for boils and abscesses were estimated for 1995–2010. HES data were used to calculate age-standardized hospital admission rates for boils, abscesses and cellulitis. The incidence of boil or abscess was 450 [95% confidence interval (CI) 447–452] per 100 000 person-years and increased slightly over the study period (incidence rate ratio 1·005, 95% CI 1·004–1·007). The rate of repeat consultation for a boil or abscess increased from 66 (95% CI 59–73) per 100 000 person-years in 1995 to peak at 97 (95% CI 94–101) per 100 000 person-years in 2006, remaining stable thereafter. Hospital admissions for abscesses, carbuncles, furuncles and cellulitis almost doubled, from 123 admissions per 100 000 in 1998/1999 to 236 admissions per 100 000 in 2010/2011. Rising hospitalization and recurrence rates set against a background of stable community incidence suggests increased disease severity. Patients may be experiencing more severe and recurrent staphylococcal skin disease with limited treatment options.

Crude herbs of Daphne genkwa (CHDG) are often used in traditional Chinese medicine to treat scabies baldness, carbuncles, and chilblain owing to their significant purgation and curative effects. The most common technique for processing DG involves the use of vinegar to reduce the toxicity of CHDG and enhance its clinical efficacy. Vinegar-processed DG (VPDG) is used as an internal medicine to treat chest and abdominal water accumulation, phlegm accumulation, asthma, and constipation, among other diseases. In this study, the changes in the chemical composition of CHDG after vinegar processing and the inner components of the changed curative effects were elucidated using optimized ultrahigh-performance liquid chromatography coupled with quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF-MS). Untargeted metabolomics, based on multivariate statistical analyses, was also used to profile differences between CHDG and VPDG. Eight marker compounds were identified using orthogonal partial least-squares discrimination analysis, which indicated significant differences between CHDG and VPDG. The concentrations of apigenin-7-O-β-d-methylglucuronate and hydroxygenkwanin were considerably higher in VPDG than those in CHDG, whereas the amounts of caffeic acid, quercetin, tiliroside, naringenin, genkwanines O, and orthobenzoate 2 were significantly lower. The obtained results can indicate the transformation mechanisms of certain changed compounds. To the best of our knowledge, this study is the first to employ mass spectrometry to detect the marker components of CHDG and VPDG.

Cirsium japonicum Fisch. ex DC. (CF) and Cirsium setosum (Willd.) MB (CS) are commonly used clinically to stop bleeding and eliminate carbuncles. Still, CF is mainly used for treating inflammation, while CS favors hemostasis. Therefore, the present study used UHPLC-MS to analyze the main chemical constituents in CF-CS extract. We optimized the extraction process using single-factor experiments and response surface methodology. Afterward, the hemostatic and anti-inflammatory effects of CF-CS extract were investigated by determining the clotting time in vitro, the bleeding time of rabbit trauma, and the induction of rabbit inflammation using xylene and lipopolysaccharide. The study of hemostatic and anti-inflammatory effects showed that the CF-CS, CF, and CS extract groups could significantly shorten the coagulation time and bleeding time of rabbits compared with the blank group (p < 0.01); compared with the model group, it could dramatically inhibit xylene-induced ear swelling in rabbits and the content of TNF-α, IL-6, and IL-1β in the serum of rabbits (p < 0.01). The results showed that combined CF and CS synergistically increased efficacy. CF-CS solved the problem of the single hemostatic and anti-inflammatory efficacy of a single drug, which provided a new idea for the research and development of natural hemostatic and anti-inflammatory medicines.

Purpose: To describe the profile of pyoderma patients in Dermatology and Venereology Outpatient Unit of Dr. Soetomo General Acdemic Hospital Surabaya in January 2016 to December 2018. Methods: This study is a retrospective descriptive analysis using secondary data derived from medical records at Dermatology and Venereology Outpatient Unit of Dr. Soetomo General Academic Hospital Surabaya. Result: The analysis of pyoderma patient at Dermatology and Venereology Outpatient Unit of Dr. Soetomo General Academic Hospital Surabaya in January 2016 to December 2018 reveals a total of 407 patients, predominantly male and the most age category were toddlers. Most clinical diagnosis were furuncles and carbuncles. Among the clinical manifestations, macula was the most frequent primary skin lesion, while erosion predominated among secondary lesions. The most commonly administered therapies were systemic and topical antibiotics. Background. One of the life-threatening diseases among patients with a single kidney is acute purulent pyelonephritis (APP). The disease is accompanied by oliguria or anuria and in many cases comes with the development of multiple organ dysfunction. Objectives. To present the features of the clinical manifestations of APP of a solitary kidney, the results of clinical and laboratory examination, the role of kidney imaging methods (USG, MRI and contrast-enhanced CT) in identifying foci of purulent destruction in the parenchyma, and the immediate results of treatment of patients with APP of a solitary kidney. Material and methods. 10 patients with APP of a solitary kidney were observed. The role of imaging methods (USG, CT and MRI) of the kidney in the detection of purulent destruction in parenchyma is shown. Methods of treating patients are considered: open organ-preserving surgery, nephrectomy; installation of an internal ureteral stent followed by antibiotic therapy and puncture nephrostomy followed by antibiotic therapy. Results. The best immediate results were noted in the group of patients who underwent open organ-preserving surgery in the volume of decapsulation of the kidney, excision of carbuncles, opening of abscesses and drainage of the calices-pelvis system of the kidney with nephrostomy. 3 out of 10 patients needed hemodialysis. Conclusion. Open organ-preserving surgery remains a reliable adequate method of surgical treatment of APP of a solitary kidney. In case of a single focus of destruction in the kidney parenchyma up to 3 cm in diameter or with signs of apostematous pyelonephritis, an effective method of treatment is the installation of an internal ureteral stent followed by antibiotic therapy. 30% of patients with APP need hemodialysis treatment.

ABSTRACT Few data exist on the clinical utility of the expanded-spectrum tetracyclines doxycycline and minocycline for the treatment of community-associated methicillin-resistant Staphylococcus aureus (MRSA) skin and soft tissue infections (SSTI). We performed a retrospective cohort study of 276 patients who presented with 282 episodes of MRSA SSTI to the emergency room or outpatient clinic at two tertiary medical centers between October 2002 and February 2007. The median percentage of patients infected with MRSA strains that were susceptible to tetracycline was 95%. Time zero was defined as the time of the first incision and drainage procedure or, if none was performed, the time of the first positive wound culture. The median patient age was 48 years. Abscesses constituted the majority of clinical presentations (75%), followed by furuncles or carbuncles (13%) and cellulitis originating from a purulent focus of infection (12%). A total of 225 patients (80%) underwent incision and drainage. Doxycycline or minocycline was administered in 90 episodes (32%); the other 192 SSTI were treated with β-lactams. Treatment failure, defined as the need for a second incision and drainage procedure and/or admission to the hospital within at least 2 days after time zero, was diagnosed in 28 episodes (10%) at a median of 3 days after time zero. On logistic regression analysis, receipt of a β-lactam agent was the only clinical characteristic associated with treatment failure (adjusted odds ratio, 3.94; 95% confidence interval, 1.28 to 12.15; P = 0.02). The expanded-spectrum tetracyclines appear to be a reasonable oral treatment option for patients with community onset MRSA SSTI in areas where MRSA strains are susceptible to the tetracyclines.

Purpose: to study the segmentation of organometric and immuno-biochemical digital indicators in ewes with ESO at the end of gestation.Materials and methods. The experiment involved three flocks of Tsygai, Volgograd and Stavropol breeds, each with 700 suyag sheep. Placental tissue samples were taken from clinically healthy and sick pregnant ewes, which were placed in 10% neutral formalin. Using a Cytadel 2000 histoprocessor (Shendon), the obtained samples were clarified in chloroform and enclosed in a Histomix paraffin medium (Biovitrum). Histosections were prepared on a rotary microtome (MICROM HM340E). Using the AxioScope.A1 (ZEISS) microscope, histopreparations were photographed and processed using the ZENpro 2012 (ZEISS) program. According to Schmidt and Thannhauser, the RNA content was determined using a two-wave spectrophotometer in UV. The activity of the G-6-Phase was studied by Swanson. In vacuum tubes Vacuette ™ (Austria), blood samples were taken in the morning before feeding, from the jugular vein. Results. In sheep with eclampsia, a decrease in the total mass of the placenta, its cotiledons, as well as an increase in the length of the umbilical cord, in which stroma sclerosis is noted in the vascular wall, was observed. Small calcification foci were found in the tissues of the afterbirth, and small terminal villi are located around the stem villi near the vascular membrane, tightly adjacent to them. The proportion of syncytiotrophoblast of crypts of carbuncles and villi of the chorion of cotyledons of mature syncytium decreases in comparison with physiologically occurring severity. The content of RNA in its subcellular fractions is mainly concentrated in the microsomal fraction, 18% - in mitochondria and 7% - in the nuclear fraction. Albumins in pregnant ewes with eclampsia were reduced by 1.35 times. BASC and phagocytic activity of leukocytes in queens with eclampsia of pregnant sheep is reduced 15 days before lambing, which poses a threat to the reproductive health of the mother and the viability of newborn lambs.

A carbuncle is a coalescence of several inflamed follicles into a single inflammatory mass with purulent drainage from multiple sinuses. They can develop in healthy individuals with no predisposing conditions or in immune-compromised ones. Author are presenting a case report of 57 years old Saudi presented with neglected huge carbuncle of the neck that was found complicated with intracranial extension in the posterior cranial fossa. He underwent successful simultaneous excision of the carbuncle and debridement of gross necrotic tissues; together with evacuation of the intracranial abscess. Both general and neurosurgical teams were involved. Patient made uneventful recovery and discharged home in good condition.

Introduction. The prevalence of acute pyelonephritis (AP) in pregnant women varies from 3% to 18%. There is an increase in the AP incidence in pregnant women over the past two decades with the growth of purulent-destructive forms incidence. Currently, there is no unity of views on the issues of therapeutic tactics in pregnant women according to the literature data. Besides, there are no clear criteria for the timing and choice of the method of upper urinary tract (UUT) drainage. This circumstance determines the relevance of the study of the effectiveness of minimally invasive methods of AP treatment and UUT obstructive lesions in pregnant women.Purpose of the study. To study the effectiveness of minimally invasive methods of AP treatment in pregnant women.Materials and methods. The results of AP treatment and various UUT obstructive lesions in 293 pregnant women from 2013 to 2018 years were analyzed. Patients` age: from 15 to 42 years (mean is 25 years). At the same time, 237 (81%) patients were in the second and third trimesters of pregnancy. Urodynamic disorders were resolved in 123 (42%) patients with stenting, ureter catheterization, and percutaneous nephrostomy (PNS). In 28 (9.5%) cases, UUT obstruction was caused by kidney stones, in 7 (2.4%) cases contact ureterolithotripsy (CULT) was performed on this occasion.Results. In 16 (5.4%) patients developed complications, such as sepsis and septic shock. Purulent-destructive AP forms were observed in 8 (2.7%) pregnant women. AP was stopped in all patients against the background of the restored urine passage and ongoing conservative therapy. The need for lumbotomy, kidney`s decapsulation, carbuncles` excision, nephrectomy did not arise in any case. Eventually, 221 (75%) pregnant women had timely deliveries. Preterm birth was observed in 52 (17%) pregnant women. Maternal mortality has not been reported. Antenatal fetal death was detected in 2 (0.6%) cases within 1-2 months after discharge from the urological division.Conclusion. Timely conservative managment of AP in pregnant women with adequately restored of urine passage allows to minimize severe septic complications and avoid traumatic open surgical interventions.The study did not have sponsorship. Infection of the lower lip can manifest differently. The likelihood of such prompt life-threatening septic complications, as thrombophlebitis, pneumonia, pleurisy, etc, may be underestimated due to the small size of the local inflammation of the lip. We present a 21-year-old Caucasian female with a Staphylococcus epidermidis infection of the lower lip (furuncle) complicated by lip abscess, suppurative cheilitis glandularis, lip necrosis, septic bilateral multilobar necrotizing (destructive) pneumonia, pleurisy (pleuritis), and pulmonary insufficiency of the first degree (type I). Clinical photos of the report demonstrate all stages of the course of inflammation and treatment of the severe lower lip infection from the moment of admission. Preoperative condition, the period of formation and demarcation of necrosis, cleaning of the wound from purulent content, demonstration of fibrinous exudate (also known as fibrinous plaques), the appearance of granulations, and visualization of lip scars as of the day of discharge from the hospital (day 26) are presented. The article also includes a chest X-ray at the time of admission, on the second day, and on the seventh day of hospital stay. The report is enriched as well by three-dimensional multi-slice computed tomography (3D MSCT) images on the eighth, eighteenth, and fortieth days from the moment of admission to our hospital. 3D MSCT of the lungs is showing the areas of pulmonary tissue necrosis and the healing phase. The article contains two videos (cine images) with a duration of 21 seconds and 2 minutes 11 seconds, which demonstrate chest MSCT on the hospital day 8 and 18. The patient was discharged on the 27th day of admission. According to our English-language literature search, it's a first ever reported case of the lower lip furuncle which led such a unique combination of severe local and septic complications documented at all stages of treatment from both aspects clinical and radiological. An update of the classification of uncomplicated and complicated forms of development of furuncles (i.e., boils) and carbuncles is done and presented. More than 37 literary sources were analyzed, and their key aspects complemented our report. Articles with necrotizing fasciitis of the lips were also analyzed.

In her anamnesis, she was infected with SARS-CoV-2 infection prior to the admission. Based on the complaints, anamnesis and gynecological examination data preliminary diagnosis was made as urinary system infection at pregnancy 26 weeks. Further examination revealed that she had carbuncle of the kidney and perirenal tissue, carbuncle of the left kidney, paranephritis on the left, acute obstructive pyelonephritis on the right. Lab results showed elevated leukocytes, anemia, and lymphopenia.<br /> <b>Conclusions:</b> This case shows us how woman’s weakened immune system from COVID-19 along with pregnancy, which can further weaken the immune system made her susceptible to a kidney infection, which progressed to a kidney carbuncle and perirenal tissue involvement.

One outbreak of food poisoning associated with ingestion of the liver of a large lutjanid fish was investigated in this study. The symptoms in three patients primarily included headache, nausea, vomiting, fever, vertigo, and visual disorientation and later included peeling of the skin. The species of fish implicated in this incident was Etelis carbunculus (family Lutjanidae) as determined by direct sequence analysis and PCR plus restriction fragment length polymorphism analysis for detection of the cytochrome b gene. Subsequently, several specimens of E. carbunculus of different body weights were collected, and the level of vitamin A in the muscle and liver was determined by high-performance liquid chromatography. The average level of vitamin A in E. carbunculus muscle was 12 ± 2 IU/g and that in the liver was 9,844 ± 7,812 IU/g. Regression models indicate that E. carbunculus with higher body weight and liver weight will have higher levels of vitamin A levels in the liver.

Abstract:
During the analysis of morbidity by nosological forms of diseases of the class «Diseases of the skin and subcutaneous» cellulose in cadets of two higher military educational institutions for 2012–2017, studying in the Kaliningrad enclave, the features were revealed. Diseases included in only three blocks were registered: infections of the skin and subcutaneous tissue; dermatitis and eczema; other diseases of the skin and subcutaneous tissue. The share of the first block reached 80.4–73.4 %; the second — 18.2 – 20 %, the third 1.4 – 5.8 %. The level and duration of the period of prevalence of the first block of diseases over the second was 4 among cadets at the university № 1, in the university № 2–3 years. The leading diseases were pyoderma (at university No. 1 – 120.9 ± 5.6‰, No. 2 – 42.5 ± 4.3‰), skin abscess, furuncle and carbuncle (No. 1 – 40.2 ± 3.4‰, No. 2 – 15.2 ± 2.6‰). The incidence of pyoderma in each university prevailed among cadets of the first three years of study over abscess, boil and carbuncle. Pyoderma in the university № 1 in each year of study was recorded reliably less often from 1 to 4 courses. Abscess, furuncle and carbuncle were registered significantly more often in the first course than in the second, in the second or fourth courses — the same, and only in the fifth course significantly less often than in previous courses. Pyoderma among cadets of 1 – 2 years of study at the university № 2 was registered in the same way, in the third-fifth courses — 2.8 – 4.4 times less often. Abscess, furuncle and carbuncle were registered by years of study without significant differences. Differences in the incidence of skin and subcutaneous tissue infections among cadets of the two universities indicated the presence of specific health risk factors. This necessitates further retrospective studies and full-scale (microbiological, immunological, etc.) tests.
