Abstract:
Cardiovascular diseases (CVDs) are the number one cause of death globally: people die annually from CVDs than from other cause. In this way nursing education system has key role of bringing to student the knowledge, skills and experience of managing CVDs. The aim of this review is to indicate the importance of simulation based cardiovascular system education in nursing. We conducted a systematic literature review to evaluate the study results within the simulation based cardiovascular system education in nursing. Akdeniz University center electronic databases including MEDLINE, CINAHL and PUBMED e.g. were searched studies published in English within the last five years with “nursing education”, “simulation based cardiovascular system education” and “simulation based education in nursing” key words. In the recent literature review, there are six-research studies simulation based cardiovascular system educations in nursing. A study conducted by Tawalbeh and Tubaishat (2014) indicated that traditional training is an effective teaching strategy; however, simulation is significantly more effective than traditional training. Another study has shown that use of the deliberate practice model and a simulation-based curriculum to learn cardiovascular assessment and diagnostic reasoning skills was found to be very important. Pilot students' data of another study in a multi-center research model reflected that undergraduate nursing students who received the simulation-based training achieved statistically significant pre-to-posttest improvement in cardiopulmonary assessment skills and cognitive knowledge. The study results conducted by Heidarzadeh and colleagues (2014) indicated that both of the simulation methods increased the perception of self-efficacy in cardiopulmonary resuscitation. Another RCT results conducted by Aqel and Ahmad (2014) indicated that high-fidelity simulation (HFS) provides students with interactive learning experiences in a safe controlled environment. Simulation is an effective teaching strategy for cardiovascular system education includes ACLS, cardiovascular assessment and diagnostic reasoning. Keywords: Cardiovascular; nursing; CVD.

Abstract Introduction. Cardiovascular diseases still remain the chief life-threatening condition in Poland. They are one of the main causes of sickness absences at work, as well as reasons for hospitalization and disabilities. The professionals responsible for providing nursing services to people suffering from cardiovascular diseases should take into account the activities addressing all spheres of patient’s life. Most attention should be paid to preventing complications, as well as interventions enabling both the patients to perform selfmonitoring and self-care and their relatives to provide unprofessional care.Aim. The aim of this work was to formulate a nursing care plan for a patient suffering from cardiovascular diseases, using the International Classification for Nursing Practice.Material and methods. For the purpose of this article, an individual case study method and literature analysis were used. The research was conducted in November 2015 at the Clinic of Hypertension and Diabetes of the University Clinical Centre at the Medical University of Gdańsk. Written consents were obtained from every patient.Results. In the process of providing nursing care to the patients, phrases describing “ready” diagnoses and nursing interventions included in the International Classification for Nursing Practice (ICNP®) were used. The care plans included the following nursing diagnoses: impaired cardiovascular system, altered blood pressure, pain, functional dyspnea, peripheral edema, impaired sleep, risk of infection, obesity, lack of knowledge about the disease/ poor self-control.Conclusions. The plan of nursing care for patients with cardiovascular diseases was based on the ICNP® reference terminology that fully reflects the key problems of the patient and the scope of interventions made by nurses.

Abstract.Many studies have been done on the respiratory and the cardiovascular system. Among them, only a few are on the interaction of these two physiologic systems. To explore the mechanism of the integration of these two physiological systems, computer simulation has been done; we report the preliminary results obtained in our laboratory. In this study, a mathematical model of the cardiovascular system integrated with the respiratory mechanical system has been established. The model is based on our previous work on cardiovascular modeling. The previous lumped lungi model has been replaced by a multielement model with more detail. Inter- thoracic and abdominal pressures are modeled as external pressure sources on the related cardiovascular elements. Using this model, a sequence of simulation studies have been carried out. Different respiratory modes have been simulated and the different effects are observed in the simulation results. The results indicate that by following a certain respiratory pattern, the circulation status can be improved. OBJECTIVES: to identify women with cardiovascular risk, five years after a preeclampsic episode (PE), and identify the follow-up of these women within the Unified Health System (Sistema Único de Saúde - SUS), in the city of Natal/RN. METHODS: a quantitative and exploratory study conducted at the Januário Cicco University Maternity Ward/RN. The sample consisted of 130 women, 65 with a PE episode and 65 who were normotensive. RESULTS: we found statistical significance with regard to body mass index, weight, family history of cardiovascular disease (CVD) and cardiovascular complications when comparing women with previous PE to normotensive women. The groups were unaware of their cardiovascular risk factors and, in addition, they reported difficulties in accessing primary health care (PHC) services. CONCLUSIONS: women with a PE history are at increased risk of developing CVD, unaware of late PE complications, and lacked customized care when compared to normotensive patients.

Neurohormonal activation is a commonly cited array of phenomena in the body’s physiologic response to heart failure. Although various neurohormones and pharmacologic agents that moderate their pathophysiologic effects have been reviewed in the nursing literature, both the mechanisms of neurohormonal system activation and cellular and organ system effects have been described only in brief. Accordingly, this article reviews mechanisms of neurohormonal activation and describes cellular and cardiovascular effects of the (1) sympathetic nervous system, (2) renin-angiotensin-aldosterone system, (3) kallikrein-kininogen-kinin system, (4) vasopressinergic system, (5) natriuretic peptide systems, and (6) endothelin in the context of heart failure. This article implicitly details the physiologic basis for numerous current and potential future pharmacologic agents used in the management of heart failure. It is intended that this article be used as a reference for advanced clinical nursing practice, research, and education.

Gastric dilatation volvulus is an acute and life-threatening disease requiring immediate intervention and intensive nursing. The presence of a large gas-filled stomach and subsequent twisting of the stomach results in compression of surrounding organs, hypovolaemic and cardiogenic shock. Cardiovascular effects of gastric dilatation volvulus include cardiac arrhythmias, hypotension, electrolyte and acid–base imbalance, and ischaemia-reperfusion injury. Understanding the negative effects on the cardiovascular system will allow detection and early intervention should complications arise.

ABSTRACT Objective: To identify in the literature the cardiovascular adverse events resulting from oral antineoplastic therapy. Method: Integrative review of the literature through the SCOPUS, Scientific Electronic Library Online (SciELO), Cumulative Index to Nursing and Allied Health Literature (CINAHL), Medical Literature Analysis and Retrieval System Online (MEDLINE) databases. The antineoplastic, cardiotoxicity, cardiovascular system and adverse reaction descriptors were used in Portuguese, English and Spanish. We selected 23 articles published between 1985 and 2015. Results: Twenty studies were related to cardiac events and eleven to peripheral vascular events. The most frequent adverse cardiac events were reduced left ventricular ejection fraction, myocardial infarction, changes in the electrocardiogram, heart failure and angina, whereas peripheral vascular events were hypertension and thromboembolism. Conclusion: Oral antineoplastic therapy is associated with different adverse events, including cardiac and peripheral vascular events.

Abstract:Long-term regulation of beat-to-beat variability involves several different kinds of controls. A linear approach performed by parametric models enhances the short-term regulation of the autonomic nervous system. Some non-linear long-term regulation can be assessed by the chaotic deterministic approach applied to the beat-to-beat variability of the discrete RR-interval series, extracted from the ECG. For chaotic deterministic systems, trajectories of the state vector describe a strange attractor characterized by a fractal of dimension D. Signals are supposed to be generated by a deterministic and finite dimensional but non-linear dynamic system with trajectories in a multi-dimensional space-state. We estimated the fractal dimension through the Grassberger and Procaccia algorithm and Self-Similarity approaches of the 24-h heart-rate variability (HRV) signal in different physiological and pathological conditions such as severe heart failure, or after heart transplantation. State-space representations through Return Maps are also obtained. Differences between physiological and pathological cases have been assessed and generally a decrease in the system complexity is correlated to pathological conditions.

OBJECTIVE: To present a conceptual framework based on the PRECEDE model conceived to guide research and the clinical practice of nurses in the clinical follow-up of patients with cardiovascular diseases. METHOD: The conceptual bases as well as the study designs used in the framework are discussed. The contextualization of the proposed structure is presented in the clinical follow-up of hypertensive patients. Examples of the intervention planning steps according to the intervention mapping protocol are provided. RESULTS: This conceptual framework coherently and rationally guided the diagnostic steps related to excessive salt intake among hypertensive individuals, as well as the development and assessment of specific interventions designed to change this eating behavior. CONCLUSION: The use of this conceptual framework enables a greater understanding of health-related behaviors implied in the development and progression of cardiovascular risk factors and is useful in proposing nursing interventions with a greater chance of success. This model is a feasible strategy to improve the cardiovascular health of patients cared for by the Brazilian Unified Health System.

In Spain, there has been a progressive increase in aging. Specifically, Leon has been one of the Spanish provinces with the highest aging index. Polypharmacy is highly prevalent among the elderly, with significant consequences for safety. The aim of this study was to assess the consumption of drugs in a nursing home in the province of Leon and establish the relationship between polypharmacy and the most common comorbid diseases. A descriptive, observational, and cross-sectional study design was used. Residents’ information and treatments were collected by reviewing on medical charts, completed with clinical information obtained from the physician of the nursing home. The mean age of residents was 86.8 years, and 63.8% were female. Mean medical consumption amounted to 7.02 prescriptions. Polypharmacy was observed in 54.9% of residents, and excessive polypharmacy in 22.1%. The most commonly used medications (ATC classification) were those acting on the cardiovascular system (84.4%) and the nervous system (82.8%). A high-level of drug consumption was observed in the nursing home. Interventions should focus on those residents hospitalized the last year and with recent contact with a general practitioner. There is a need to develop a comprehensive monitoring system to assess the quality of prescriptions for nursing home residents.

Cardiovascular disease is the leading cause of mortality in the UK with ischaemic heart disease and heart failure being the main contributors. The renin-angiotensin aldosterone system has an essential role in cardiovascular pathophysiology by increasing preload, afterload and left ventricular workload. Angiotensin converting enzyme inhibitor therapy is a first-line therapy in this patient population except for those who are intolerant of angiotensin converting enzyme inhibitors. Angiotensin receptor blockers are deemed as a safe alternative while achieving the same clinical benefit. Common adverse effects are hypovolaemia, hyperkalemia and renal dysfunction. The nursing role involves cardiovascular assessment, monitoring for adverse effects and patient education.

BACKGROUND Acute kidney injury due to contrast material occurs in 3% to 15% of the 2 million cardiac catheterizations done in the United States each year. OBJECTIVE To reduce acute kidney injury due to contrast material after cardiovascular interventional procedures. METHODS Nurse leaders in the Northern New England Cardiovascular Disease Study Group, a 10-center quality improvement consortium in Maine, New Hampshire, and Vermont, formed a nursing task force to reduce acute kidney injury due to contrast material after cardiovascular interventional procedures. Data were prospectively collected January 1, 2007, through June 30, 2012, on consecutive nonemergent patients (n = 20 147) undergoing percutaneous coronary interventions. RESULTS Compared with baseline rates, adjusted rates of acute kidney injury among the 10 centers were significantly reduced by 21% and by 28% in patients with baseline estimated glomerular filtration rate less than 60 mL/min per 1.73 m2. Key qualitative system factors associated with improvement included use of multidisciplinary teams, standardized fluid orders, use of an intravenous fluid bolus, patient education about oral hydration, and limiting the volume of contrast material. CONCLUSIONS Standardization of evidence-based best practices in nursing care may reduce the incidence of acute kidney injury due to contrast material.

The secondary variables were: gender, age, educational level, family history of cardiovascular disease, Charlson score and its individual conditions, living alone, quality of life, smoking, blood pressure, physical activity and adherence to the Mediterranean diet. A Cox regression model was constructed to predict cardiovascular disease one year after admission. This was then adapted to a points system, externally validated by bootstrapping (discrimination and calibration) and implemented in a mobile application for Android. Results: A total of 93 patients developed cardiovascular disease (30.7%) over a mean period of 1.68 years. The predictors in the points system were: gender, age, myocardial infarction, heart failure, peripheral arterial disease and daily activity (quality of life). The internal validation by bootstrapping was satisfactory. Conclusion: A novel points system was developed to predict short-term cardiovascular disease in hypertensive patients after hospital admission. External validation studies are needed to corroborate the results obtained.

Older people (OP) have high risk for hypertension related with changing during older process in particularly system of cardiovascular. The purpose of this study is to identify prevalence of hypertension and related factors among OP in nursing home of Jember, East Java, Indonesia. A cross-sectional study was conducted among 42 of OP in nursing home. A self-administered questionnaire was used to identify characteristic of participants. Data of activity daily living, cognitive function, emotional status, depression status, and spiritual activity was measured using Katz Index of Independence in Activities of Daily Living (ADL), Short Portable Mental Status Questionnaire (SPMSQ), The Mini Mental State Examination (MMSE), Depression Anxiety Stress Scale 14 (DASS-14), and Daily Spiritual Experience Scale (DSES), respectively. Blood pressure was measured using a standard protocol employing a stethoscope and sphygmomanometer. A logistic regression analysis was used to determine factors related to hypertension among OP. Blood pressure of systolic and diastolic among OP were 131.90 ± 16.56 mmHg and 82.02 ± 10.18 mmHg, respectively. The prevalence of systolic and diastolic hypertension among OP in nursing home were 38.1% and 21.4%, respectively. The factors of systolic hypertension were age (p= 0.038), activity daily living (p= 0.043), depression status (p= 0.012), and spiritual activity (p= 0.015). Meanwhile, factors of diastolic hypertension were age (p= 0.041) and education (p= 0.049). Increasing age of OP related a risk factor of hypertension by supported by there are psychological problems like depression, although could be reduced by existence of spiritual activity of OP. Provision of health education on changes in the cardiovascular system and adaptation of lifestyle changes in the OP will be able to reduce hypertension factors in the OP.

Institutionalized elderly may be regarded as the paradigm of this complexity because of their multiple chronic diseases and decreased functional and cognitive functions.ObjectiveTo explore and characterize the prevalence of comorbidity and polypharmacy in a sample of institutionalized elderly.MethodsA cross-sectional study was conducted with an elderly sample recruited from three Portuguese nursing homes. Clinical information was obtained through interview and by review of residents’ medical records. The Anatomical Therapeutic Chemical/ATC classification was used to indicate the main group of medicines used, and polypharmacy was categorized into minor (2–4 medicines) or major (≥ 5). Comorbidities were coded using the individual body systems of Cumulative Illness Rating Scale for Geriatrics/CIRS-G.ResultsThe sample included 175 elderly with a mean age of 81 (sd = 10) years and institutionalized for an average of 7 (sd = 11) years. Residents presented a mean of 9 (sd = 4) co-morbid medical conditions, mostly psychiatric (80.8%), vascular (76.7%) and endocrine/metabolic (70.3%). Major polypharmacy was verified for 73.9% of residents. The mean number of medicines was 7 (sd = 3), most commonly for cardiovascular (86.0%) and nervous system (79.1%) and for blood and blood-forming organs (69.2%).ConclusionsAs in other studies in similar settings, polypharmacy was fairly common. Abstract:
ABSTRACTRecent government changes to Critical Care in the Health Service have resulted in an increase in the numbers of critical care patients being nursed in Post Anaesthetic Care Units (PACU). Many recovery nurses have had to increase their knowledge and skills accordingly. This article will analyse the high dependency nursing care given to a patient in the PACU necessitated by the haemodynamic compromise of the patient's cardiovascular system following surgery. The need for admission to a high dependency bed will be discussed along with the assessment of shock, tissue perfusion, fluid replacement, and any pharmacological intervention. Psychological, social and ethical issues relating to the patient and family will also be identified. Concentrating on the first 24 hours post-operatively, a careful discussion and analysis of the clinical management given will be made, with reference to the clinical and physiological measurements. After an evaluation and reflection of the medical and nursing care given to the patient, this article will conclude with a brief reiteration of the key issues.

Abstract:
As doenças cardiovasculares possui grande relevância nas taxas de mortalidade e aumento no número de hospitalizações, essas doenças acometidas por diversos fatores devem ser tratadas imediatamente após o aparecimento dos primeiros sintomas. Este estudo teve como objetivo analisar quais intervenções de enfermagem que são adotadas nas unidades de urgência e emergência em pacientes com suspeita de infarto agudo do miocárdio e descrever a importância das intervenções e levantar os principais diagnósticos de enfermagem segundo NANDA. Trata-se de uma pesquisa de revisão bibliográfica descritiva com análise qualitativa, os dados foram coletados nas Bases de Dados de Enfermagem (BDENF) da Biblioteca Virtual de Saúde (BVS) e livros publicados no período de 2009 a 2012. O infarto agudo do miocárdio pode ser definido como um processo pelo qual áreas de células do miocárdio são destruídas de forma permanente. Diagnosticar precocemente o infarto do miocárdio e iniciar os cuidados emergenciais aumenta a chance de sobrevida do paciente infartado. Uma atuação rápida e eficiente do enfermeiro é imprescindível para o sucesso do tratamento e a importância da implementação da sistematização da assistência de enfermagem, possibilitando um cuidado integral ao paciente de forma segura.Descritores: Infarto Agudo do Miocárdio, Intervenções de Enfermagem, Sistematização da Assistência de Enfermagem. It is a bibliographic, descriptive and qualitative study the data were collected on the Basis of Data of Nursing (BDENF) of the Virtual Health Library (BVS) and in books which was published in 2009 to 2012. Acute myocardial injury can be defined as a process which areas of myocardial cells are permanently destroyed. Early diagnosis and emergency care increases patients´ survival. Este estudio tuvo como objetivo analizar las intervenciones de enfermería se adoptan en las unidades de emergencia y los pacientes con sospecha de infarto agudo de miocardio y describir la importancia de las intervenciones y plantear los principales intervenciones segundo diagnósticos de enfermería de la NANDA. Se trata de un estudio de análisis cualitativo, descriptivo y bibliográfico, se recogieron datos sobre la Base de Datos de Enfermería (BDENF) de la Biblioteca Virtual en Salud (BVS) y los libros publicados entre 2009-2012. Infarto agudo de miocardio se puede definir como un proceso por el cual las áreas de las células miocárdicas se destruyen de forma permanente. El diagnóstico precoz de infarto de miocardio e iniciar la atención de emergencia aumenta las posibilidades de supervivencia de los pacientes con infarto de miocardio. Una enfermera actuación rápida y eficaz es esencial para el éxito del tratamiento y la importancia de la aplicación de la sistematización de la asistencia de enfermería, de atención integral a los pacientes de forma segura.Descriptores: Infarto Agudo del Miocardio, Intervenciones de Enfermería, Sistema de Atención de Enfermería.
