Abstract:
Background: Given national calls for intentional career development during graduate and post-graduate scientific training, this study assessed career readiness development within the context of academic career courses. The current study evaluated the effects of academic career courses offered at two institutions that were specifically designed to increase career awareness, interest, and career-related confidence among doctoral students and postdoctoral fellows. Methods: Participants enrolled in a career course at trainees’ respective academic institutions and responded to pre- and post-course surveys (n=32, n=148). The paper offers a thematic analysis of each of the two courses using an individualized learning plan career development framework and describes the results of their respective pretest-posttest evaluations which indicated increases in career readiness. Results: Though the format and content provided in each course varied, participation was associated with increases in career readiness. Participants reported increased career-awareness including a greater familiarity with different types of careers overall. Furthermore, interest in tenure track faculty careers increased in both samples, which may assuage fears that exposure to diverse career pathways could reduce interest in academic careers. Transferrable skills, including career planning and awareness also significantly increased. Course participants reported an increase in the number and type of mentors they interacted with beyond their principal faculty mentor (other faculty, professional PhDs, peers, and administrative staff). Conclusions: Findings provide supporting evidence for the benefits of implementing structured career development efforts during PhD training; even with varying content, delivery methods, and instructor type, both academic career courses led to significant gains in career awareness and readiness. Successful development and delivery of academic career courses, with a focus on career planning skills, suggest that institutions can utilize these and are an effective way to prepare PhDs for their transition from training positions into careers.

Abstract:
A critical component of management graduates' future success and work happiness is the planning of their professional careers. In order to better understand how several elements, including as those related to family, personal interests, financial rewards, peer influences, and motivation to work, this study looked at how management graduates in Pokhara planned their professional careers. A 285 management graduates from various institutions and colleges located within Pokhara Metropolitan city were participated in questionnaire survey. This study's methodology is an analytical and casual comparative research design. There are positive correlation between career planning of management graduates and family related factors (.274), career planning and personal interest (.059), career planning and financial benefits (.092), career planning and peer influences (.499) and career planning and motivation to work (.514). It is concluded that those who are more motivated at work are more likely to engage in professional career planning. The multiple regression analysis showed that professional career planning was significantly predicted by personal interest (.438), family related factors (.237), motivation to work (.575), but not by financial benefits (.009) and peer influences (-.014). In terms of professional career planning, the model explained 48.6 percent of the variance, and the goodness of fit metrics (F-Statistic= 17.345) showed that the model suited the data well. According to the findings, financial benefits and peer influences may not have as big of an impact on management graduates' professional career planning as personal interest, family related factors and motivation to work. Hence, this study contributes to point out the most pressing impact on management graduates while planning their professional plans.

One of the factors that led to the Modernising Medical Careers (MMC)/Medical Training Application Service disaster of 2007 was the discrepancy between the numbers of applicants and the actual number of posts available for appointment. In its submission to the Tooke enquiry, the Workforce Review Team stated that 'workforce planning needs to be at the core of any changes to a medical career structure (and that) in general this should be demand, not supply, led'. In an attempt to predict opportunities for general surgery over the next five years we have estimated demand from numbers of consultant appointments advertised and supply from numbers of specialist registrars (national training numbers (NTNs)).

Abstract:
The paper reconstructs the ways of attributing meanings to IT specialists’ careers at BPO (Business Process Outsourcing) centers in Poland. The findings rely on empirical data, collected through autobiographical narrative interviews. The technique of analysis applied in the study was inspired by grounded theory methodology and allowed for the identification of a basic social process at the analytical stage, namely, “career planning.” On the one hand, the analysis also showed how IT specialists’ careers are given meanings concerning career planning; there seem to be three main ways of understanding one’s career in IT: (1) in terms of reaching economic and social stability, (2) as a transition period in a career as an IT specialist, (3) with regard to becoming an expert in IT. On the other hand, the second axis of analysis has been conceptualized, which is comprised of other subjective and objective elements that may shape career planning. These include the biographical experiences of work, the context of the Polish BPO industry, and career planning resources. Furthermore, a typology consisting of three ways of attributing meanings to IT specialists’ careers at BPO centers has been put forward and analyzed.

Abstract:
Everyone dreams of a brilliant career in life. But in reality, in planning and determining a career, there are many problems, especially for teenagers who will enter adulthood, namely high school students. Many students have difficulty deciding which major, study program or job to choose after graduation. To overcome this, the role of BK teachers as counselors in schools is needed to carry out career guidance. Career guidance is a counseling service needed by individuals or students to help develop their potential and find their identity as well as plan and determine their careers. Career guidance can be carried out using a trait and factor theory approach, which is a counseling method that focuses on identifying individual/student traits or characters so that students can find their identity and do not hesitate in planning and choosing their careers. From several studies that have been carried out, it is proven that career guidance with a trait and factor approach is effective in helping reduce student doubts about career termination as well as improving students' understanding and career planning.

<span>Individuals with substance abuse disorders (S-AD) are also human beings who need self-development and future careers. The career development of individuals with GPZ hasn’t been fully considered in planning drug rehabilitation programs, although overall, the rehabilitation program has been excellent. Measurement of career planning ability needs to be measured by the condition of individuals with GPZ. The purpose of this study was to analyse the career planning abilities of individuals with GPZ undergoing drug rehabilitation and the urgency of career counselling. Mix-method research with explanatory sequential design begins with quantitative data and then integrates qualitative data. The data on career planning ability variables were collected on research subjects in individuals with GPZ. Quantitative data were collected through a career planning ability questionnaire with 32 questions, then qualitative data through interviews with one individual, all taken by random sampling—the average career planning ability of individuals with low GPZ. Counsellors should be able to develop ten career planning skills on how to learn information, career discussions with adults, training related to education, knowledge of working conditions, discussion of career challenges, planning work after rehabilitation programs, managing leisure time, developing self-confidence during rehabilitation, as well as the state of oneself when working without illegal drugs. Although career planning skills are low, addiction counsellors have a role in their development, with ten abilities discovered through career counselling. Career counselling as an addiction counsellor attempts to help individuals with GPZ develop their potential.</span>

Abstract:
Incomplete understanding of attrition from the physician workforce has hampered policy-makers’ efforts to achieve optimal alignment of the supply of physicians with population demand for medical services. This longitudinal study of Ontario primary care physicians uses health administrative data to characterize changes in physicians’ practice patterns over time. We examined the workload and scopes of practice of 21,240 physicians between 1992 and 2013. We found that physicians reduce their workloads gradually as they age, retiring from clinical practice at an average age of 70.5. Furthermore, we found that 60% of family physicians who stop providing comprehensive primary care continue to provide clinical services for an average of three years, with reduced workloads, before retiring fully. Our findings clarify the process of physician attrition from the workforce and will help to improve estimates of attrition and make physician workforce planning more accurate and effective.

Hoare, Sarah. "Understanding end-of-life admissions : an interview study of patients admitted to a large English hospital shortly before death." Thesis, University of Cambridge, 2017. https://www.repository.cam.ac.uk/handle/1810/275055.

Full text
Abstract:
Hospital admissions for patients close to the end of life are considered ‘inappropriate’ in contemporary English health policy. Hospitals are supposedly unable to offer a ‘good’ death for patients, and dying there is thought to contradict patient choice, since patients are assumed to want to die at home. However, almost half of all deaths in England in 2015 occurred in hospital, and of these, nearly a third died within three days of admission. This thesis seeks to explore why these admissions are considered to be a problem and how they occur. Through a systematic review of UK literature I found that it cannot be stated that most patients want to die at home, because of the extent of missing data (preferences not asked, expressed, reported or absent). This finding challenges the justification that admissions are inappropriate because they contravene patient choice. Similarly inconclusive evidence about the undesirability, cost, and lack of need for patients to be in hospital were also found in a review of policy. Together with analysis of historical trends in hospital and hospice provision, it is apparent that attitudes towards end-of-life admissions reflect existing tensions about the role of hospital as an acute provider, and as a place of death. An analysis of interviews conducted with healthcare staff and next-of-kin involved in the admission of patients (case-patients) who died shortly after being admitted to Meadowbridge, a large English hospital explored these tensions further. I found that whilst hospital was not recognised as a place where ‘good’ deaths typically occurred, it was acknowledged as an emergency place of care. In this context, patients without obvious need for hospital care were nevertheless admitted to the hospital and the environment was subsequently recognised to offer distinct benefits. The need for emergency care reflected the difficulties of providing end-of-life care in the community. For dying to occur appropriately, home had to be adapted and care organised by healthcare staff. Both tasks were complicated by the unpredictability of dying, and family carers helped to absorb much of the uncertainty and support patients to die at home. Ambulance staff became involved when patients had care needs that exceeded care quickly and easily available in the community. When called to the case-patients, ambulance staff instituted familiar practices in transferring them to hospital. Hospital was recognised as a default place of care because ambulance staff struggled to facilitate alternative care and lacked sufficient professional authority to keep patients at home. The admissions of the case-patients represent the best attempts of staff to navigate the tangled practices of end-of-life care. These practices are the result of the actions of the staff, which in turn both constrained and enabled their action in providing care to patients. The term ‘inappropriate’ to describe admissions does not encompass these attempts, and moreover, devalued the significant care provided by healthcare staff in the community and hospital.

Abstract:
As training and development programs have become one of the most essential elements for organizations, the question rises of what influences training effectiveness. Recently training motivation has been considered as the key determinant. Thus, the objectives of this particular study were to examine if training motivation correlates well with its antecedents and its outcomes as well as to see if training motivation plays a mediator role in predicting the outcomes of training. Three pre-training variables were chosen for this study: general attitude toward training, training self-efficacy, and career planning. Immediate trainees’ reactions were chosen as a post-training variable. A total of 152 employees from a large Quebecois company participated in the present study by filling out one questionnaire before training and one after. The results of this study evidenced that training motivation can be predicted from general attitude toward training, training self-efficacy and career planning. Moreover, training motivation was shown to be the best predictor of trainees’ reactions. Our results also showed that training motivation fully mediates the relationship between training self-efficacy and trainees’ reactions and partially mediates the relationship between general attitude toward training and trainees’ reactions, but only partially. Mediation effect of training motivation was not found between career planning and trainees’ reactions. Additionally, this study discovered that new employees differ from permanent staff in their pre-training attitudes and post-training reactions. The outcomes of the present study are discussed, some theoretical implications are commented and practical implications for the companies are suggested.

Abstract:
This chapter guides the reader in understanding the integral aspect of work to one's existence by exploring different theories in vocational development as an investigative assessment of work options. It explores the idea of choice within the world of work and analyzes group behavior. The ramifications of the Covid-19 pandemic have had a profound effect on the world of work and on individual workers. The chapter presents practical applications to help scholar-practitioners understand the topic's broader external structures and means to adopt a more transdisciplinary approach to employee retention, engagement, and productivity. The National Career Development Guideline (NCDG) framework established the categories for career development into self-knowledge, educational and occupational lifelong learning. With an appreciation of this framework, this chapter explores different groups and the new world of work. This approach will help the reader move from general abstract principles to a socially situated knowledge base.

Globally, there is a high quest for graduate education with many prospective students aspiring to attain advanced qualifications to obtain a better career path and higher income. Besides, postgraduate education fosters skills development. However, in Uganda, many students enroll in different graduate programs but fail to complete them in the stipulated timeframe. Furthermore, most of the master's students tend to successfully finish their first year which basically has the coursework component yet fail to complete the second year that has the research component. Doctoral students make little progress when it comes to their research. The authors will examine general research preparation, writing, and methodology skills that are critical to graduate studies and research. With the necessary support, mentoring, and planning, graduate research can be made a better process for students and supervisors.

Abstract:
Divorced women in old age have long been considered a particularly vulnerable group, due foremostly to a lack of sufficient financial resources. Based on a qualitative analysis of ten semi-structured interviews, this chapter explores the experience of divorced women who have not re-partnered and are working past pension age in Germany and the United Kingdom. The chapter particularly focuses on the women’s subjective reasons for their continued employment. In their narratives, the long-term influence of the female homemaker model becomes evident. However, divorce in midlife is experienced as a major turning point. The women re-enter the labour market, individual career and pension planning becomes prominent and employment past pension age is portrayed as an important way to independently earn an adequate income for old age. From a more general angle, the interrelationship between life courses, gendered welfare regimes and later-life employment become clear.

Abstract:
In 1961, the government bodies responsible for film production (the Ministries of Culture of the USSR and RSFSR) forcibly imposed on a reluctant Lenfilm the complete reorganization of production planning. The old Scripts Department was shut down and three “creative units” set up. This change was pushed through by Lenfilm’s energetic and flamboyant new general director, Ilya Kiselev, who had begun his career as an actor. Of the creative units, the earliest to emerge was the Third Creative Unit, which soon had a role as the flagship of contemporary cinema, a genre heavily promoted during the Thaw. However, the Third Creative Unit ran into increasing trouble as political control tightened after Khrushchev was forced to resign, and in 1969, it was closed down altogether. Yet life was not always calmer in the other units, as witnessed in particular by the difficulties that gripped the Second Creative Unit’s efforts to produce movies commemorating the fiftieth anniversary of the October Revolution in 1967, and by the problems of the First Creative Unit in establishing its own character and repertoire. At the same time, the general political line at this period, while unpredictable, was not uniformly harsh, as manifested in the conclusion of Leningrad’s Party leader that audiences could “make up their own mind” about a film he disliked.

Abstract:
With respect to the changing profile of the engineer in a globalized world, the author believes that teaching specialized languages at technical universities completes the study of future engineers and opens them doors to an international career. Students should be made aware of their responsibility towards ensuring the effectiveness of knowledge transfer among laymen/endusers, who often have difficulty in understanding specialized texts. Therefore, an update of curriculum planning, made possible by the inclusion of a new course offer like Technical Documentation at the University ,,Politehnica" of Bucharest, is a timely requirement for technical universities in Romania, if we envisage a revised profile of the engineering qualification in the global labour market. The author proceeds first to an empirical analysis of texts chosen: German and Romanian documentation, user manuals for electrical appliances, which are instructional texts placed at the interface between technology, marketing and public. They are considered from the viewpoint of intelligibility of discourse and from an intercultural perspective as well. The results confirm the working assumption that documentation should correspond to certain quality standards. The author then describes various individual tasks students solve online in the German languge class, related on the one hand to websites of different companies and on the other hand to documentation of appliances these companies produce. Websites and user manuals are evaluated by students according to two checklists which enable them to differentiate between general vs. particular/intercultural features. Step by step they become aware of the way how knowledge transfer among endusers is performed and which are the means of improving it. In the long run both content and language skills are improved and they may constitute the premisses of a successful start as an engineer.

Abstract:
Abstract: The paper deals with methods and ways of managing career risks in quality jobs. It has been more than 30 years since the management systems according to ISO standards have been intensively implemented, and in parallel, numerous occupations have been profiled in which individuals see the development of their professional careers. A special impetus and direction for the development of certain professions in quality was given by EOQ, which, researching the needs of organizations for such knowledge and professions, developed schemes for training professional staff in quality. After a certain period of development, it has become very important to start studying the risks that accompany the career development of professional staff in quality and to become aware of what can go wrong in such a commitment. Research and written sources on this topic are very modest, so the authors relied on sources of career development in other jobs, and used indicators of analysis of education and interviews conducted with participants in quality education conducted in the last 30 years by OSKAR Ltd. The authors are aware that ready-made recipes cannot be written on this issue, but methods and ways to avoid risk traps in planning one's own career in quality can be made aware. The authors are also aware of the fact that in countries in transition (to which this paper refers) there is a state of general insecurity, and very often uncertain economic conditions are reflected in the status and career of professional staff in quality. In addition to all this, it is time to start seriously considering and researching career risk management in quality jobs.

Industry focused, full semester Courses A “Industry and Information Systems” and B “Overview of the Market and Introduction to Marketing” are lectured by the lead author at Tokyo Metropolitan Institute of Technology (TMIT). In these courses, overview of the global and Japanese economies and trends of each industry sector are introduced by the lecturer. Students are assigned to write a report on a specific industry sector, a specific market and other related topics of his or her interest, and opportunities for presentation and discussion on their draft reports with their classmates and the lecturer are given toward the end of the semester. Apart from these courses, a full semester Course C “Evolution of Industry, Business and Engineering Practice in the Global Economy,” utilizing collaborative learning approach, was proposed by the lead author in 1998, and was partially taught at American and Japanese universities, utilizing the Proceedings of the ASME Engineering & Technology Management Group Symposia at IMECE 2000–2002.1)–3) The surveys were conducted concerning the student’s interest of industry sectors, lecture topics, and international topics both at the beginning and at the end of the semester. In addition, surveys to sense students’ new discovery from lectures for all course modules and those to sense useful knowledge in their career planning and in general were conducted at the end of all classes. The results of surveys show that students found the discussion opportunities valuable to gain broader perspectives and insights into their own subject of study when writing their reports, by learning what their classmates are studying and from classmates’ comments. This paper also discusses what motivates students in learning, what influences students’ general, specific, and career interests, and how students’ interests may change from the beginning to the end of the semester for which these courses are given.

Abstract:
Life's changes and maturation will require continued growth, personally and professionally. In the last period of time this objective has became really hard to achieve, not only because of pandemic conditions around the world in terms of health protection and the volatility of work labor lately, but also because the social side of our personalities and the spiritual aspect of human identity have been severely tested. Nowadays, the majority of Romanian universities are coping to these challenges. In these trouble times, we all started looking for ways in which we could be more efficient or "work smarter" in order to maintain the feeling of normality in our lives. The need for counselling is constantly increasing for all participants to education (students and teachers) as well as for the employees, as general. Before any other reframing levels of analysis, the first step in someone's career counselling process begins from analyzing the self-perspective, in terms of personal qualities (strong points and weaknesses), personal needs, aspirations and professional interests. According to these issues, this paper is aimed to present the good experience of implementation a career counselling training program, included in the project "Scholarships for entrepreneurial education of doctoral students and post-doctoral researchers", financed through the European structural funds. The project has been implemented in University POLITEHNICA of Bucharest in partnership to University DUNAREA DE JOS from Galati. Despite the fact that the focus of project's activities has been on entrepreneurial competences' development, as the title of project emphasized, the career counselling activities represented one of the most important and appreciated project's activity. The target group's level of satisfaction regarding this project's activity has been analyzed in the research described in this paper, proving its authentic value in order to highly increase the awareness of participants for continuous need of reflection on their motivation and aspirations about career. The quantitative research-based is aimed to analyze the participants' perceptions and opinions referring to professional counselling program's implementation, in terms of topics approached, effect and impact on developing their key abilities, such as: communication, planning and evaluation, decision making, management of socio-emotional skills, information about labor market etc.

Abstract:
The qualitative aspects of the evolution of relations between globality and locality, society and the individual, and their links, are not without consequence for the multicontextual changes in the labour market, which present new challenges to employees. This vision implies completely new ways of interpreting the world in which “a career makes a career” – they are problems of „a borderless career”, „a varied career”, „a post-corporate career” as a novum, of sorts, in the planning of a broadly understood career, the rise in importance of having a career and the ability to plan, manage and monitor one’s own career in the perspective of one’s whole life. The consideration discusses the need for (meta)analytical career theory assumptions for the practice of cross-cultural career counseling. The author emphasizes that there is a need to generate new approaches, which will cross the boundaries defined by partial paradigms so that they will be relevant in the 21st century. Moreover, she raises the question of how to "manage" the rich heritage of career theories of the late 20th century, while at the same time increasing their theoretical-cognitive value. Another aspect that justifies the need for critical reflection on the reliability of theoretical perspectives is the study of methodological instrumentation that takes into account culturally shaded social and professional aspects. A special place is given to the constructivist school of thought and theory of career construction by Mark L. Savickas, which is said to focus on the nature of the social world and refer to the domain of life involving career and its proactive construction, by simultaneously presenting a new perspective on the participation in the global changes. Changes in the world of “borderless” careers shine a new light on the problem of individual proactivity – an individual’s proactive behaviours in their career.

Abstract:
Abstract Design and manufacturing integration at the early design stage in product development, that is, the integration of conceptual design and conceptual process planning, may make a big impact. Thus the development of systematic computer-based support for this integration is desirable. To select and evaluate manufacturing processes, some aspects of form information is necessary. Thus when only the functionally critical forms have been determined from major functional requirements, synthesis of the configuration shape in a generic form would enable early assessment of manufacturing, processes. As a stepping stone toward the development of the configuration shape synthesis and process selection, case studies have been conducted using real world industrial parts. In this paper, we present the case studies using a planet carrier and a gear box housing, and discuss the issues in development of the shape synthesis and process selection method to support the design and process planning integration.

<div class="section abstract"><div class="htmlview paragraph">Autonomous driving technology represents a significant direction for future transportation, encompassing four key aspects: perception, planning, decision-making, and control. Among these aspects, vehicle trajectory planning and control are crucial for achieving safe and efficient autonomous driving. This paper introduces a Combined Model Predictive Control algorithm aimed at ensuring collision-free and comfortable driving while adhering to appropriate lane trajectories. Due to the algorithm is divided into two layers, it is also called the Bi-Level Model Predictive Control algorithm (BLMPC). The BLMPC algorithm comprises two layers. The upper-level trajectory planner, to reduce planning time, employs a point mass model that neglects the vehicle's physical dimensions as the planning model. Additionally, obstacle avoidance cost functions are integrated into the planning process. In the upper trajectory planner, the fifth-order polynomial algorithm is also used to smooth the planned trajectory to meet the requirements of vehicle dynamics and passenger comfort. The lower-level trajectory tracker is responsible for real-time trajectory tracking and control, and the paper conducts experiments comparing the Model Predictive Control (MPC) algorithm with the Linear Quadratic Regulator (LQR) algorithm, under the premise of considering the feasibility, cost and safety of the experiment, the front wheel steering Ackerman experimental car is selected as the experimental carrier to verify the reliability of the MPC trajectory tracking control algorithm and ensure the stable driving of the vehicle along the planned trajectory. To address complex road environments, a dynamic obstacle avoidance algorithm is incorporated during the trajectory planning phase. This algorithm allows the vehicle to rapidly generate collision-free trajectories when encountering obstacles, utilizing techniques such as envelope polygonal distance and anti-roll constraints. Compared with other trajectory planning algorithms, the MPC algorithm used in this paper can better adapt to the uncertainty of the system and has better robustness in the face of external disturbances. Finally, the proposed approach is validated through simulation experiments using the Carsim-Simulink co-simulation tool at four different speeds: 10 m/s, 20 m/s, 30 m/s, and 40 m/s. The results demonstrate that the BLMPC algorithm not only ensures safe and comfortable driving but also exhibits high planning efficiency and obstacle avoidance performance in complex road environments. This research provides valuable guidance for advancing autonomous driving technology and its practical implementation</div></div>

Abstract:
Abstract This paper will study the LNG Transport opportunities from the Canadian Arctic to the Asian markets. Mackenzie Delta LNG (MDLNG) project is in the Canada's Northwest Territories (NWT) which contains publicly owned conventional natural gas reserves which could be developed for export that would provide immediate economic benefit to the Inuvialuit Settlement Region, NWT and Canada. This paper presents the results of a feasibility study undertaken to evaluate the shipping routes and ice conditions along the route from the Arctic to the Asian LNG markets. Arctic LNG carriers have been in use in the Russian Arctic for years already and we have been deeply involved in the design, development, and testing of those current LNG carriers. Russian rules are somewhat different and thus the operations would commence in Canada and US waters that would give some opportunities in the LNG Carrier design as well. In this paper we will go through the general differences in the LNG carriers design for MDLNG. Currently the plan is to build gravity-based structure (GBS) to the offshore MacKenzie Delta. The GBS would need additional ice management support and vessels. In this paper we would talk about ice management vessels needed to support the operations for loading the LNG carriers as well as talk about the recommended ice management operations. With modern technology, good design and planning, it can be shown that the LNG transportation by ships is a feasible solution compared to building pipelines across the Arctic.

Rost, Lucia, Amber Parkes, and Andrea Azevedo. Measuring and Understanding Unpaid Care and Domestic Work: Household Care Survey Toolkit. Oxfam, October 2020. http://dx.doi.org/10.21201/2020.6775.

Full text
Abstract:
This toolkit provides guidance on using Oxfam’s Household Care Survey (HCS) methodology, which was developed by Oxfam as part of the WE-Care initiative to transform the provision of unpaid care and domestic work (UCDW). UCDW underpins all our lives yet is overwhelmingly provided by women and girls. Recognising UCDW is essential for any initiative that aims to understand and address gender inequality. The HCS is a quantitative survey tool that generates context-specific evidence on how women, men and children spend their time, how care is provided, by whom, and the main factors that affect people’s responsibilities for UCDW, such as access to care services, infrastructure and social norms. The HCS can be used to generate a baseline, or to measure the impact of a specific policy or programme. The methodology can be integrated into different projects with different objectives and adjusted for use in various contexts. The HCS toolkit is designed to be used by development practitioners, policy makers, employers, academics and researchers. Part A provides guidance for planning, collecting, analysing and using HCS data. Part B provides guidance for understanding, adjusting and using the HCS questions. Both sections should be read before undertaking the survey.

Abstract:
This study aims to maximize the utilization of operations research (OR) findings in Egypt to strengthen research efforts contributing to the improvement of the family planning (FP) and reproductive health (RH) care program. To date, the field of OR in Egypt has not conducted a systematic review of the utilization of the OR studies completed over the last decade. The long-term objective of this study, as noted in this report, was to maximize the utilization of OR and strengthen future efforts contributing to the improvement of the FP and RH care program in Egypt. The study’s sample included selected studies completed by the principal agencies conducting OR in Egypt—Family Health International (FHI) (1989–1992) and the ANE OR/TA Project of the Population Council (1992–present). The analysis of the qualitative findings was based on a framework applied in a similar study in Indonesia by the ANE OR/TA Project and built on three elements: research product, scientific network, and researcher-audience relationship. Analysis of data indicated a general satisfaction with the contribution of OR to the FP and RH program in Egypt.

Abstract:
Background. Timely discharge of hospitalized patients can prevent patient harm, improve patient satisfaction and quality of life, and reduce costs. Numerous strategies have been tested to improve the efficiency and safety of patient recovery and discharge, but hospitals continue to face challenges. Purpose. This Technical Brief aimed to identify and synthesize current knowledge and emerging concepts regarding systematic strategies that hospitals and health systems can implement to reduce length of stay (LOS), with emphasis on medically complex or vulnerable patients at high risk for prolonged LOS due to clinical, social, or economic barriers to timely discharge. Methods. We conducted a structured search for published and unpublished studies and conducted interviews with Key Informants representing vulnerable patients, hospitals, health systems, and clinicians. The interviews provided guidance on our research protocol, search strategy, and analysis. Due to the large and diverse evidence base, we limited our evaluation to systematic reviews of interventions to decrease hospital LOS for patients at potentially higher risk for delayed discharge; primary research studies were not included, and searches were restricted to reviews published since 2010. We cataloged the characteristics of relevant interventions and assessed evidence of their effectiveness. Findings. Our searches yielded 4,364 potential studies. After screening, we included 19 systematic reviews reported in 20 articles. The reviews described eight strategies for reducing LOS: discharge planning; geriatric assessment or consultation; medication management; clinical pathways; inter- or multidisciplinary care; case management; hospitalist services; and telehealth. All reviews included adult patients, and two reviews also included children. Interventions were frequently designed for older (often frail) patients or patients with chronic illness. One review included pregnant women at high risk for premature delivery. No reviews focused on factors linking patient vulnerability with social determinants of health. The reviews reported few details about hospital setting, context, or resources associated with the interventions studied. Evidence for effectiveness of interventions was generally not robust and often inconsistent—for example, we identified six reviews of discharge planning; three found no effect on LOS, two found LOS decreased, and one reported an increase. Many reviews also reported patient readmission rates and mortality but with similarly inconsistent results. Conclusions. A broad range of strategies have been employed to reduce LOS, but rigorous systematic reviews have not consistently demonstrated effectiveness within medically complex, high-risk, and vulnerable populations. Health system leaders, researchers, and policymakers must collaborate to address these needs.

Kenya: Offer family planning on hospital wards. Population Council, 2000. http://dx.doi.org/10.31899/rh2000.1007.

Full text
Abstract:
In Kenya, more than one in three women hospitalized for gynecological problems has complications from miscarriage or unsafe abortion. These women generally receive no information or services for family planning (FP) or other reproductive health needs. During 1996–97, the Kenya Ministry of Health, the Population Council, and Ipas tested three models for providing postabortion care (PAC) and FP information and services in two areas of the hospital. Researchers compared model effectiveness by using surveys before and after the intervention. As concluded in this brief, the most effective way to ensure that women being treated for incomplete abortion obtain FP is to offer services in hospital gynecological wards. Having ward staff provide contraceptives on the ward is more convenient than having regular FP providers visit the ward or having patients go to a separate clinic. Findings have been key in informing expansion plans for PAC in Kenya.

Improving the quality of reproductive health care: How much does it cost? Population Council, 2003. http://dx.doi.org/10.31899/rh2003.1015.

Full text
Abstract:
Quality care for clients should be the focus of a family planning and reproductive health program, but can programs afford it? There is no simple answer. The multiple dimensions of quality of care make it more difficult to identify and measure affordable improvements in service delivery. Calculating program costs is challenging, and different methods can lead to widely varying estimates. Determining how much quality costs is a challenge, but it is both possible and important for programs’ sustainability. This brief focuses on various aspects of costs and examines information about the cost of improving quality (as opposed to the cost of quality of care in general), then outlines ways to improve quality while containing costs. Ideally, decisions about quality should be the result of a dialogue among policymakers, providers, and clients. Each program has to decide what standard of quality is appropriate to apply considering its situation, its resources, and the needs of the population it is meant to serve. This policy brief uses the framework developed by the U.S. Agency for International Development’s Maximizing Access and Quality initiative.

Integration of reproductive health service for men in health and family welfare centers in Bangladesh. Population Council, 2003. http://dx.doi.org/10.31899/rh2003.1002.

Abstract:
Over the last 30 years, Bangladesh’s national family planning (FP) program has mainly concentrated its promotional efforts on women’s adoption of FP. While the government policy has been effective in influencing women to accept contraceptive methods, men’s role in FP has been completely neglected. Similarly, male RH services are hardly available at Health and Family Welfare Centres (HFWCs). This report describes a project aimed at integrating male RH services within the existing government female-focused health-care-delivery system. The study was implemented by NIPORT in collaboration with the Directorate of Family Planning and the Population Council. Interventions resulted in a substantial rise of male clients in the clinics, however nearly all came for the treatment of general health problems. Only a small number of male RTI/STI clients sought services from the clinics. Although unexpected, there was a substantial rise in the number of female clients. The study concluded that RH services for men could be easily integrated into HFWCs without affecting the female and child focus of the clinics and that augmentation of the total number of patients at HFWCs will lead to more effective use of resources and decreased cost of treatment.

Abstract:
Since the mid-1970s, the Bangladesh national family planning program primarily focused on motivating women to use modern contraceptive methods and encouraging them to seek services from clinics. In addition, female field workers were recruited to deliver contraceptive methods at homes. The program design facilitated women’s access to information and medical care through clinics and home visits. In the process, however, the medical needs of males were marginalized. Men generally seek services from pharmacies, private practitioners, and district hospitals, and often ignore preventive steps and postpone seeking medical care for chronic health conditions. In cases of acute illness, they often resort to self-medication. As noted in this report, the study’s aim was to integrate male reproductive health services within the existing government female-focused health-care delivery system. The study concluded that reproductive health services for men could easily be integrated into the health and family welfare centers without affecting the clinics’ focus on serving women and children.

Using a rapid assessment approach to evaluate the quality of care in an integrated program: The experience of the Family Health Division, Ministry of Health, Botswana. Population Council, 1997. http://dx.doi.org/10.31899/rh1997.1001.

Abstract:
To strengthen existing efforts by the Government of Botswana in responding to the HIV/AIDS epidemic, USAID provided financial and technical assistance through the Botswana Population Assistance (BOTSPA) project. BOTSPA’s goal was to improve the quality and availability of family planning (FP) and sexually transmitted infection (STI) services through designing, implementing, and monitoring activities that would lead to the eventual institutionalization and sustainability of high-quality FP and STI/HIV/AIDS services. The Ministry of Health requested assistance from the Africa OR/TA Project II to develop and test an approach for routinely measuring the quality of service delivery and assessing skills retention by staff trained through in-service courses. A rapid assessment mechanism was designed and then used to determine the readiness of health facilities in Botswana to offer quality FP and STI management services. According to this report, the mechanism was found generally acceptable, easy to use by staff and managers, and provided valid information on the readiness of the health facilities to provide FP and STI services.
