The Northeast Modern Language Association will celebrate the new millennium by participating in a centenary reexamination of the Pan-American Exposition of 1901. The convention will be held 7–8 April 2000 at the Hyatt Regency Hotel in Buffalo, New York. Erie Community College will host the convention, and the local arrangements chair is Annette Magid. The keynote speech will be delivered by Michael Frisch, a historian at the State University of New York, Buffalo, who is orchestrating the scholarly reexamination of the 1901 Expo. The convention will feature readings by the poets Charles Bernstein, Robert Creeley, Carl Dennis, Irving Feldman, and Dennis Tedlock. Scholars are invited to respond to the call for papers by 1 September 1999. The call is available on the Web site or from the executive director.

Abstract:
A remarkable amount of award-winning contemporary American poetry incorporates nostalgia as a prominent idea discussed. This poetry appears to use nostalgia as means to a greater end. In other words, nostalgia, while a dominant theme within different works, is more a way to treat concepts such as representation and memory, more so than the work being an actual commentary on nostalgia itself. Given the poetry's predominant concept, it seems poets such as Carl Dennis, Natasha Trethewey and Ted Kooser could be representative of a literary historical moment. This moment is one which comments heavily on the past's presence within the present. While each poet's writing is heavily influenced by nostalgia, I posit the theory that these poets are speaking to a greater literary historical moment found in both the literature itself as well as current trends in literary theory. It is not that these poets are writing to a specific theory, rather, their Pulitzer-prize winning poetry is rooted in a trend of yearning for the past. As overt a connection between contemporary poetry's treatment of nostalgia and nostalgia theory itself, little, if any, literary criticism has connected these two. In his essay "Theorizing Nostalgia Isn't What It Used to Be," Paul Grainge contends, "Since the late 1980s, when memory became a topic of concerted critical interest, nostalgia has been taken up in critiques of reactionary conservatism, in accounts of retro phenomena, in relation to the growing memorial tendencies in Europe and America, and as central to particular theories of postmodernism" (20). Grainge continues on to describe two forms nostalgia takes: "mood" and "mode." Similarly, Svetlana Boym suggests nostalgia as either "reflective" or "restorative" (41). This type of current scholarship addressing nostalgia seems to set up a nostalgic reading of texts as more the end game of the literature—the literature is nostalgic. However, if literature then ends as only nostalgic, there seems to be a lack of nostalgic theory's breadth. Dennis, Trethewey and Kooser all address this gap through their poetry—expanding the notion of nostalgia as being more the vehicle leading one through the landscape of memory. Suggesting nostalgia as merely reflective or restorative, as Boym and Grainge have done, seems to create a sense of nostalgia as stagnant rather than as a dynamic movement within the literature, and even the act of recollection itself. The three poets addressed in my project all suggest at some level that this residue of the past can lead one to see that perhaps experience itself delights in memory. Furthermore, nostalgia's dependence upon present memory indicates not just a longing for the past, but rather the past's presence in the present. The act of remembering serves as a type of catalyst which transforms memories to manifestations in present circumstance.

Bakgrund: Mötet mellan sjuksköterska och patient har beskrivits som grundläggande isjuksköterskearbetet och är mer välstuderat i andra sjukvårdsmiljöer än hemsjukvården,som är basen för Sveriges sjukvård och engagerar ett stort antal behövande som vårdasav många anställda. Hemmet får allt större betydelse som vårdplats och antalet platserpå sjukhusen blir färre.Syftet var att beskriva sjuksköterskan i hemsjukvårdens upplevelse av mötet medpatienten och dennes närstående i deras eget hem (ordinärt boende).Metod: Studien gjordes som en allmän litteraturstudie där vetenskapliga artiklarsystematiskt söktes i olika databaser för att sedan analyseras. Sökningarna resulterade i11 artiklar.Resultatet: Presenteras i tre kategorier som beskriver sjuksköterskans upplevelser:Relationens betydelse i mötet med patient och närstående, förtroende, tillit och respekt imötet samt maktlöshet, frustration och en känsla att inte räcka till.Diskussion: Med kännedom om hur betydelsefull relationen till patienten upplevskänns det angeläget att som sjuksköterska i hemsjukvården arbeta personcentrerat ochatt hela tiden lyssna in patient och närstående och försöka anpassa sig till situationen.7Slutsats: Relationen mellan patient, närstående och sjuksköterska har stor betydelse föroch påverkar alla inblandade parter. Kunskap som är av betydelse för att möta de kravoch förväntningar hemsjukvården kommer ställas inför när allt fler och allt sjukarepersoner kommer behöva hjälp med allt mer avancerade insatser.
Background. The encounter between the nurse and the patient has been described asfundamental in nursing work and are more well-studied in other health care settingsthan home care, wich is the base for the Swedish health care and involves a largenumber of needy cared for by many people. The home is of increasing importance as aplace for care and the number of places in hospitals are becoming fewer.The aim was to describe the experiences nurses in home care have of the encounterwith the patient and his or her relatives in their own home (ordinary housing).Method: the study was conducted as a literature review and articles were searched forsystematically in different databases and then analyzed. The searches resulted in 11articles.The result is presented in three categories that describe nurses´experiences: Thesignificance of the relation in the encounter with patient and relatives, confidence, trustand respect in the encounter and powerlessness, frustration and a feeling of not beeingable to do enough.Discussion: With knowledge of how important the relation to the patient is percieved,it´s of importance to work person-centered and listen to the patient and the relatives andtry to adjust to the situation.Conclusion: The relationship between patient, relatives and nurse is of greatimportance to, and impact on all parties involved. Knowledge that is relevant toconfront the demands and expectations that home care will face when more andincreasingly ill people will need help with more advanced efforts.

O cuidado em saúde bucal configura-se como um importante foco das preocupações e das práticas atuais do cuidado de si. O entendimento de que as práticas direcionadas ao cuidado de si integram os processos implicados na constituição de subjetividades moveu-me na direção de me interrogar sobre a crescente busca por dentes brancos por meio de um cuidado bucal e de tecnologias que “garantam” o desejado sorriso modelar antes mesmo de preocupações com a saúde bucal. Enquanto efeito das práticas sociais, o corpo encontra-se implicado numa economia política de estratégias tecnocientíficas e verdades que o regulam conforme as normas de beleza, saúde, felicidade e consumo. Tal implicação direciona escolhas, por exemplo, de produtos a serem adquiridos, os quais passam a integrar tanto os hábitos quanto a constituição subjetiva e orgânica dos sujeitos. Se pensarmos nas instâncias cujas verdades nos interpelam cotidianamente, será possível ver que os modos como nos olhamos e cuidamos encontra-se amplamente difundido pela mídia, inclusive os cuidados de saúde bucal. Por meio de diversos meios e modalidades enunciativas, a mídia vem ocupando a posição privilegiada nos processos constitutivos dos sujeitos e nos modo de ver e de cuidar de si. Relativamente ao cuidado bucal contemporâneo, no Brasil, este se apresenta de forma ambígua, ao mesmo tempo, tem-se o adoecimento por cárie e a veiculação de verdades, em anúncios publicitários configurando um cuidado oral estético que articula saúde/beleza. Tais questões moveram-me na direção de conhecer práticas e verdades relacionadas ao cuidado bucal, enquanto cuidado consigo, em anúncios publicitários veiculados na televisão nos dias de hoje. Assim, tomei como objeto de análise anúncios publicitários postos em circulação na mídia televisiva, visto o papel de destaque que adquiriu na constituição das subjetividades contemporâneas. Para entender como foi se configurando o cuidado bucal, no presente, voltei meu olhar para outras épocas, desde 3500 a.C. até meados do século XX, procurando conhecer instrumentos, produtos, quem falava e o que se dizia sobre o cuidado bucal. Nessa direção, examinei anúncios publicitários da revista “A Cigarra”, importante revista feminina brasileira do século passado. Para tanto, foram estabelecidas conexões com o campo dos Estudos Culturais, em suas vertentes pós-estruturalistas, e os estudos foucaultianos. As análises mostraram-me que as preocupações e cuidados bucais foram transformando-se, mas trazendo desde uma época remota preocupações estéticas que foram sendo associadas à saúde. No século XX, tornou-se marcante a presença dos especialistas nos anúncios publicitários, relacionando os usos do dentifrício à beleza e à saúde femininas já associadas ao branqueamento, mas também à sedução. Nos dias de hoje, intensifica-se o discurso do branqueamento do corpo e boca com saúde/beleza e sedução, o que pode gerar a produção de desejo e busca pelo branqueamento dental e pelas tecnologias em nível molecular para alcançá-lo a qualquer preço. Isso aponta para a necessidade de olharmos criticamente as produções midiáticas e as “verdades” que circulam acerca do branqueamento dos dentes.
Oral health care is deemed as an important focus of concern and current practices associated with self-care. The understanding of practices of self-care integrates processes implied in the production of subjectivities. Therefore, I felt compelled to examine the rising pursuit for white teeth in our society. People are more concerned about obtaining the desired standard smile, achieved through oral care and dental technology, than about oral health. One of the focuses of contemporary propositions of self-care consists of dental care practices, which are widespread by the media. The body, thought as an effect of social practices, is part of a political economy of technoscientific strategies and truths that regulate it according to rules of beauty and health, happiness and consumption standards, which drive people to consume certain products, and thus become part of day by day habits and the subjective and organic constitution of individuals. The media plays a rising position in launching truth through different modalities of enunciation in media artifacts. This role made it a prime pedagogical instance, involved in the construction of the individuals’ views and self-care. However, the current oral care status presents itself ambiguously. On the one hand, we have the Brazilian population sickened by cavities; on the other hand, we have truths being promoted in advertisements, configuring an oral care that combines aesthetically health / beauty. These interrogations led me to examine practices and truths related to oral care which appear on advertisements published in women’s magazine of the last century. This way, it might be possible to analyze continuities and discontinuities in what is being said about oral care on televised advertisements in the present time. Therefore, thinking media as a relevant instance of productions of subjectivities, the object of analysis of this research is a set of televised advertisements currently available. In order to support this hypothesis, I established connections with Cultural Studies, in its connections with the post-structuralist perspective and Foucauldian studies. The analysis showed me that the worries and oral care have changed along the years, bringing along with it aesthetic concerns which have been associated to health since remote times. During the 20th century, the strong presence of specialists in ads has been noted, relating the usage of dentifrices to beauty and feminine health, which were already associated with not only whitening but also with seduction. Nowadays, the whitening speech for the body and the mouth and seduction has been intensified, which could generate the production of a desire and the search for dental whitening and technologies in a molecular level, to reach it at any price. Such finding leads to the need of looking critically at mediatic productions and the “truths” circulating around dental whitening.

Abstract While the car traditionally appears as male domain, the haptic aesthetic and poetic forms of Claire Denis’ film Vendredi soir (2002) disconnect cars from masculine power. Through a micro-analysis of the film’s forms, spaces, and bodies, the chapter shows how the film suspends the gendered relationships at play in public and private spaces. In this chapter, Ceuterick emphasises the affirmative aesthetic of the film, which transforms the car into a home, gendered bodies into lived bodies, and intimacy into a passage between real and imaginary.

AbstractImmediately after the Great Hanshin-Awaji Earthquake in 1995 and the Great East Japan Earthquake in 2011, pneumonia outbreak among the elderly increased the rate of fatalities. What caused this? To use lessons learned during the large-scale disaster, specialists concerned with the field of dentistry began to take action using new perspectives. Consequently, they noticed the importance of giving care to the entire oral cavity as well as the teeth. Based on reports from a dentist and a dental hygienist who tackled the oral care of the Kumamoto Earthquake victims in 2016, their methods of effective health care following a disaster is presented in this paper. We also propose a preventative medical activity that can be conducted sustainably for anyone, not just medical personnel.

Dealing with similar themes of care, hospitality, and feminism, it expands on an aspect that sat at the edges of Curator, the questioning of received ontological boundaries or defining categories. Denis covers both formerly and conceptually a taxonomy of borders, which are both physical and psychological. Her source material, Jean Luc Nancy’s essay about his heart transplant, is considered in relation to the way Denis produces a moving image work from a philosophical text, with particular concern for her treatment of narrative to produce bodily sensation. The ‘Other’ or figure of the stranger is pitted against the disintegrating power of patriarchy referenced in Denis’ casting of the actor, Michel Subor, who appears in L’Intrus and Beau Travail (1999) as well as Jean Luc Godard’s Petite Soldat (1955). Bioengineering is a technological miracle of health care and future health hope. From the entrepreneurial contributions of William Kolff with the original handmade dialysis machine to the application of the x-ray to medical diagnostics and treatment, biomedical pioneers brought technology to medical care, often at great personal risk. Few can conceive the magnitude of the impact of technology on our ability to return the sick and disabled to function. The “simple” steel and plastic hip implant is a technological wonder. The human is the most demanding of systems to be repaired by technology. The stress levels, cycle loading, chemical degradation and even biological rejection are without precedent in engineering application. Reliability is expected to be near 100%. Psychological and cosmetic compatibility are severe constraints. The current quality of life of many (if not most) of us is dependent upon technology, and forefront technology at that. The dentist no longer hurts and our teeth last longer. Numerous “replacements” are cosmetically acceptable. Medical diagnosties are everywhere, but have a long way to go. Emergency medicine is high-tech. The wonders of bioengineering are in our present and in our future. It is informative to review our bioengineering heritage from early orthopedics (splints, peg legs and crutches), through mobility facilitation (wheelchairs) and internal repair (aortic patches and arterial replacement) to modern diagnostics (MRI) and organ replacement (artificial hearts, kidneys, etc.). A recent renewed interest in biomedical devices paralleling the decoding of the genome and the proposed genetic future portends what Dr. Francis Fukuyama of Johns Hopkins called “Our Post Human Future.” We will explore our historical pathway to what we will call “our better human future through bioengineering.”

Total bacterial disinfection and elimination from the human root canal system are crucial in clinical endodontic procedures [Card et al., 2002; Kakoli et al., 2009]. The current approaches relying on mechanical instrumentation and root canal irrigation and medicaments have demonstrated that eradication of bacteria occurs when the bacteria are in direct contact with the medicaments. However, persistent infection following routine treatments has suggested that bacteria may harbor in the root canal anatomical irregularities and/or deep dentinal tubules, therefore, surface irrigation of medicaments may not be able to reach those regions. Heat treatment has been used for obturation of the root canal in endodontic practice. In this study we hypothesize that as an alternative, surface heating using a System B heating catheter through the root canal surface would be effective for bacterial elimination in the deep dentin. The heat-induced cytotoxic response kills bacteria in the root dentin via heat conduction from the thermal energy incident on the root canal wall. In principle, a high power setting and/or a long heating duration can always achieve sufficient temperature elevations in deep dentin. Yet, the detailed temperature distribution inside the dentin and possible thermal damage to the supporting periodontium are unknown. Therefore, it is of clinical importance to perform and investigate temperature elevations in dentin to provide clinicians with an optimized and effective treatment protocol to minimize unnecessary thermal damage to the surrounding structure.

Dentin and bone are formed when odontoblasts and osteoblasts synthesize and secrete collagen type I-rich extracellular matrix that mineralizes in a highly controlled manner. A wide spectrum of mouse and human disorders affecting tooth and bone biomineralization shows that dentin and bone formation are under strict genetic control. Although the controlling mechanisms of dentinogenesis and osteogenesis require further study, a large body of evidence points to the importance of the matrix metalloproteinases (MMPs) participate in a wide variety of extracellular matrix degradation. Detailed knowledge of MMPs may be important for understanding the pathogenesis of tooth development. Some researchers have pointed MMP-9 is an extracelluar proteinase that is highly expressed in osteoclasts and has been postulated to play an important role in their resorptive activity. Although MMP-9 has been reported to play a role in bone resorption, the association of this enzyme during deciduous tooth resorption has not yet been clarified. Based on accumulating evidence, we hypothesized that MMP-9 should play a role in teeth attrition. In this study, we have applied NMR relaxation technique to assess age-related MMP-9 KO tooth quality in vitro by quantifying changes in dentin and pulp simultaneously. The major hypothesis in this paper was that whether noninvasive NMR relaxation time measurements could be used to characterize MMP-9 KO changes in dentin and pulp, and to predict tooth quality. Specifically, we tested that age-related MMP-9 KO tooth changes result in an alteration of the NMR spin-spin (T2) relaxation time signal due to the structural changes in the tooth matrix. This signal can be further processed to produce a T2 relaxation distribution spectrum related to dentin and pulp, and their derived parameters can be used as descriptors of age-related MMP-9 KO tooth changes. In this study, the proton liquid-like NMR spin-spin (T2) relaxation decay signal was obtained from the Carr-Purcell-Meiboom-Gill (CPMG) NMR spin echo train method [1,2], then the relaxation decay signal was converted to T2 relaxation distribution spectra describing the size domain of dentin and pulp. Therefore, we can calibrate the intensities in NMR inversion T2 relaxation distribution spectra corresponding to the amount of dentin and pulp related to the structural changes. Here, we propose an NMR calibration method “NMR standard estimation” — the ratio of the amount of pulp to the amount of dentin obtained from NMR T2 distribution spectra that can be used to measure the age-related MMP-9 KO structural changes in teeth [3]. We are cognizant of the biological and physiological variability manifest in teeth size variations, but feel that this kind of NMR standard estimation — the ratio of amount of dentin to amount of pulp from the NMR T2 inversion spectrum can be used to determine age-related MMP-9 KO structural changes in teeth and eliminate any variations in size of teeth.

Integrity management of dent and dent associated with metal loss requires knowledge of in-line inspection (ILI) technologies, government regulations and industry codes, prescriptive requirements, and most importantly assessment models to estimate severity of the mechanical damage. The assessment models have greatly relied on the assumed capabilities of current ILI technologies to detect, discriminate and size the mechanical damage. Therefore, an investigation of the current ILI technologies and validation of their capabilities are practically important. In this paper, the current status of ILI technologies for dent and dent with metal loss is reviewed. Validation data provided by ILI inspection vendors and pipeline operators are analyzed in terms of probability of detection (POD), probability of identification (POI), probability of false call (POFC), and sizing accuracy using binomial probability distribution and confidence interval methods. Linear regression analysis is also performed to determine sizing error bands. High resolution pull test data validated with LaserScan 3-D mapping technology is used to demonstrate a better evaluation of ILI performance with minimized in-ditch measurement errors and the effect of change in dent geometry and dimension due to re-bounding and re-rounding. Issues associated with field measurement and improvement are discussed.

Design of haptic devices requires trade-off between many conflicting requirements, such as high stiffness, large workspace, small inertia, low actuator force/torque, and a small size of the device. With the traditional design and optimization process, it is difficult to effectively fulfill the system requirements by separately treating the different discipline domains. To solve this problem and to avoid sub-optimization, this work proposes a design methodology, based on Multidisciplinary Design Optimization (MDO) methods and tools, for design optimization of six degree-of-freedom (DOF) haptic devices for medical applications, e.g. simulators for surgeon and dentist training or for remote surgery. The proposed model-based and simulation-driven methodology aims to enable different disciplines and subsystems to be included in the haptic device optimization process by using a robust model architecture that integrates discipline-specific models in an optimization framework and thus enables automation of design activities in the concept and detail design phase. Because of the multi-criteria character of the performance requirements, multi-objective optimization is included as part of the proposed methodology. Because of the high-level requirements on haptic devices for medical applications in combination with a complex structure, models such as CAD (Computer Aided Design), CAE (Computer Aided Engineering), and kinematic models are considered to be integrated in the optimization process and presenting a systems view to the design engineers. An integration tool for MDO is used as framework to manage, integrate, and execute the optimization process. A case study of a 6-DOF haptic device based on a TAU structure is used to illustrate the proposed methodology. With this specific case, a Multi-objective Genetic Algorithm (MOGA) with an initial population based on a pseudo random SOBOL sequence and Monte Carlo samplings is used for the optimization.

Experiences dealing are seldom reported on the detection of sealed radioactive sources (SRS) or its shielding in waste by non-destructive assay (NDA) methods in literature. Further the knowledge and experience in this field of waste characterization will vary from specialist till basic. The main aim of this paper is to give some guidance with the aid of an overview or scheme with which the possibility can be assessed of the detection and of a shielded SRS in (historic) waste packages. This aim could be reached by simulating the gamma flux emitted by a SRS at the outline of a standard 220 litres drum. The simulations have been performed with the Monte Carlo Neutron Photon transport code (MCNP). The results, visualized by means of iso-plots, are then be used for assessing the detection probability of the SRS by available NDA techniques. The following conclusions could be made: • “Heavy and dens” objects (e.g. shielded sealed radioactive source) can be detected and an indication of its location can be obtained by a sudden significant decrease of the gamma photon flux (cold-spot) at the outline of the drum. This cold-spot is caused by the “heavy or dens” object itself. A cold-spot can be detected the best by analyzing and visualizing the gamma photon flux from the backscatter area (e.g. range 100 keV up to 200 keV) and second best by analyzing the total gamma flux. • With relative simple radiological equipment (e.g. dose rate meter) the possible presence and an indication of the location of a shielded SRS can be estimated. • The detection possibility of detecting a shielded SRS depends not only on the applied NDA technique, but also on the applied effort (e.g. number of measurements, measuring time, applied measurement grid), the properties of the waste packages (e.g. density of the waste matrix and uniformity of the inside dispersed radioactivity), the location and the properties of the shielding of the SRS (e.g. geometry). • For confirmation and the determining of the exact location advanced NDA techniques (e.g. transmission computer tomography or other NDA that techniques gives three dimensional results) have to be applied.

Chez l’enfant, les fractures alvéolo-dentaire sont plus fréquentes que les fractures basales car l’os est plus élastique que chez l’adulte. Cependant la présence de germes fragilise la mandibule (Cole 2009). Concernant les fractures squelettiques maxillo-faciales, 55% concernent les condyles mais seulement 8% concernent l’angle. La prise en charge entre l’adulte et l’enfant diffère du fait du risque d’accident d’évolution des germes et des troubles de croissance secondaires. Le cas présenté à été pris en charge au Centre Hospitalier de Pau. Il concerne un enfant de 8 ans ayant présenté une fracture comminutive de l’angle mandibulaire gauche avec plaie submandibulaire et sublinguale suite à un accident domestique. L’examen clinique dans ce contexte fut difficile. L’examen tomodensitométrique avait mis en évidence une fracture déplacée de l’angle mandibulaire gauche passant par le germe de 37. Il a bénéficié d’une exploration cervicale ( vasculaire, nerveuse et glandulaire) et d’une ostéosynthèse basilaire et sur la ligne oblique externe par voie d’abord mixte selon les plaies. Une plaque de 1,5mm sur la ligne oblique et sur le rebord basilaire ont été posées avec des vis mono corticale de 5 mm. Un contrôle clinique et radiologique a été réalisé a 15 jours et 2 mois sans complications post opératoire. L’ablation du matériel à été réalisé à 3 mois en reprenant les mêmes voie d’abord. Le contrôle a 1 mois et 6 mois après dépose du matériel n’a pas mis en évidence d’accident dévolution sur le germe de 37. La réduction de la fracture de l’angle déplacé chez l’adulte se fait par une plaque rigide sur la ligne oblique externe associée ou non à une ostéosynthèse basilaire. Chez l’enfant les auteurs recommandent une réduction basilaire pour ne pas atteindre les germes des dents définitives avec abord cervical, endobuccal ou mixte (Zimmermann 2006). Consolidation osseuse chez l’adulte 6 semaines, 3 semaines chez l’enfant car le potentiel ostéogénique est supérieur à celui de l’adulte. Si le condyle est reconnu comme centre du potentiel de croissance, la mandibule répond à une croissance adaptative et maintenir une ostéosynthèse rigide après le délai de consolidation pourrait entraîner des troubles de la croissance (Wheeler 2011). Chez l’adulte l’ablation du matériel d’ostéosynthèse n’est pas systématique, et est réalisé en moyenne à 6 mois a 1 an post opératoire. Chez l’enfant la moyenne est a 3 mois post opératoire (Bakathir 200). Pour l’instant les plaques résorbables ne montrent pas entière satisfaction, du fait de la réaction inflammatoire importante qu’elles induisent.

Aujourdhui, le succès de lostéointégration des implants n’est plus à démontrer et les systèmes implantaires ne cessent de saméliorer. Toutefois dans le secteur antérieur, lobtention d’un résultat esthétique stable dans le temps, reste encore un challenge difficile. L’objectif est de développer un profil démergence péri-implantaire esthétique en référence aux dents adjacentes. Dans cette perspective, la reconstruction des défauts osseux du secteur antérieur est essentielle et déterminante. C’est un véritable défi car elle obéit spécifiquement à une triple problématique : Elle doit permettre la mise en place de l’implant dans une position idéale dans tous les sens de lespace. Elle doit recréer des volumes esthétiques en harmonie avec les dents collatérales et assurer ainsi le soutien de larchitecture des tissus mous. Elle doit enfin garantir la meilleure stabilité possible dans le temps du volume reconstruit pour éviter la formation de récessions inesthétiques et difficiles à corriger. Plusieurs techniques ont été décrites et peuvent être utilisées selon les indications et lexpertise du chirurgien. L’Os autogène a longtemps été considéré comme le « gold standard ». Mais outre les inconvénients liés au prélèvement, il a aussi montré ses limites dans le secteur antérieur en particulier lorsquil est utilisé sous forme de bloc, par sa susceptibilité à se résorber. La transformation du bloc en lame d’os corticale pour réaliser un coffrage rempli de particules autour du défaut, permet de traiter des des défauts verticaux et le résultat semble plus stable dans le temps. Pour éviter le prélèvement autogène, il est possible dutiliser des blocs allogéniques qui permettent dobtenir des reconstructions horizontales importantes. Toutefois, la manipulation est délicate et la stabilité du volume régénéré est largement discutée dans la littérature. La régénération osseuse guidée reste sans doute la technique la plus utilisée. Elle a lavantage de pouvoir, le plus souvent, placer l’implant dans le même temps opératoire ce qui simplifie et raccourcit notablement le temps de traitement. Toutefois dans le secteur antérieur il est souhaitable de reconstruire l’os dans une position coronaire et vestibulée. Dans ces conditions les membranes non résorbables et les armatures titane présentent des avantages sur les membranes résorbables qui manquent de rigidité pour maintenir le volume lors de la cicatrisation. Lors de notre exposé, les avantages et les inconvénients de chacune de ces techniques seront discutées pour en cerner les meilleures indications. En particulier, nous verrons comment l’utilisation de l’os allogénique sous forme particulaire associéà des membranes résorbables rigides, permet de simplifier les procédures, diminuer les risques dexposition et traiter un grand nombre de défauts osseux y compris verticaux. Enfin, la présentation d un grand nombre de cas cliniques pour illustrer ces techniques nous permettront dinsister sur limportance de la gestion des tissus mous à tous les stades du traitement. Lapport de tissus mous sous forme de greffe avant l’augmentation osseuse permettra daugmenter la hauteur de gencive attachée pour permettre une bonne vascularisation du greffon et surtout un recouvrement complet sans tension indispensable pour limiter le risque dexposition. Différentes techniques de greffes gingivales permettront ensuite, sans nécessité dexposer l’os régénéré, de repositionner la ligne de jonction muco-gingivale et recréer l’épaisseur et la hauteur de gencive attachée nécessaires. Une bonne gestion des tissus mous est indissociable des protocoles de régénération osseuse et seul la combinaison des deux peut permettre de répondre à notre objectif de développement d’un profil démergence esthétique et stable.

Abstract:
The Federal Railroad Administration’s Office of Research and Development is conducting research into passenger locomotive fuel tank crashworthiness. A series of impact tests is being conducted to measure fuel tank deformation under two types of dynamic loading conditions — blunt and raking impacts. This program is intended to result in a better understanding of design features that improve the puncture resistance of passenger locomotive fuel tanks. One reason for performing this program is to aid in development of appropriate standards for puncture resistance to be applied to alternatively-designed fuel tanks, such as on diesel multiple unit (DMU) passenger rail equipment. This paper describes the results of the third blunt impact test of retired F-40 locomotive fuel tanks. The test setup was designed for the Transportation Technology Center (TTC) in Pueblo, Colorado, to impart blunt impacts to the bottom of each fuel tank specimen. The specimens tested to date are from FRA-owned retired F-40 passenger locomotives. To conduct the test, each tank was emptied of fluid and mounted on a crash wall with the bottom surface exposed. A rail cart modified with a “rigid” indenter measuring 12 inches by 12 inches, was released to impact the bottom of fuel tank at a target impact speed. The first two tests, conducted on October 8 and 9, 2013, were designed to impact the center of two different tank designs. Tests were conducted at impact speeds of 4.5 and 6.2 mph and caused maximum residual dents of 5 inches and 1.5 inches, respectively. On August 20, 2014 the test of fuel tank 234 was conducted to impact the tank off-center between two baffles. Force-deformation measurements were collected for each tank during the three tests. The series of tests provide information regarding the influence of tank design on puncture resistance. In the test of tank 234, the target impact speed was 12.5 mph, and the actual impact occurred at 11.2 mph. The test resulted in a residual dent depth of approximately 9 inches, and buckling of several internal baffles. The impact did not result in puncture of the tank. Following the test, the tank was cut open to permit examination of the baffles. This examination revealed a different baffle geometry than was modeled based on pre-test measurements. Finite element analysis (FEA) was used to predict the behavior of the tank during the test. The FE model of the tank required several material properties to be defined in order to capture puncture behavior. The combination of metal plasticity, ductile failure, and element removal would permit the model to simulate puncture for this tank. Following the test, the tank was cut open, revealing a different baffle arrangement than had been initially thought. The post-test FE model was then updated to include the actual baffle arrangement of tank 234. With the actual baffle arrangement included in the model, the FE results are in fairly good agreement with the test. Additional changes to the ductile failure criterion were also made in the post-test model. The objective of this research program is to establish the baseline puncture resistance of current passenger locomotive fuel tanks under dynamic impact conditions and to develop performance requirements to ensure an appropriate level of puncture resistance in alternative fuel tank designs, such as DMU fuel tanks.
