Abstract:
A remarkable amount of award-winning contemporary American poetry incorporates nostalgia as a prominent idea discussed. This poetry appears to use nostalgia as means to a greater end. In other words, nostalgia, while a dominant theme within different works, is more a way to treat concepts such as representation and memory, more so than the work being an actual commentary on nostalgia itself. Given the poetry's predominant concept, it seems poets such as Carl Dennis, Natasha Trethewey and Ted Kooser could be representative of a literary historical moment. This moment is one which comments heavily on the past's presence within the present. While each poet's writing is heavily influenced by nostalgia, I posit the theory that these poets are speaking to a greater literary historical moment found in both the literature itself as well as current trends in literary theory. It is not that these poets are writing to a specific theory, rather, their Pulitzer-prize winning poetry is rooted in a trend of yearning for the past. As overt a connection between contemporary poetry's treatment of nostalgia and nostalgia theory itself, little, if any, literary criticism has connected these two. In his essay "Theorizing Nostalgia Isn't What It Used to Be," Paul Grainge contends, "Since the late 1980s, when memory became a topic of concerted critical interest, nostalgia has been taken up in critiques of reactionary conservatism, in accounts of retro phenomena, in relation to the growing memorial tendencies in Europe and America, and as central to particular theories of postmodernism" (20). Grainge continues on to describe two forms nostalgia takes: "mood" and "mode." Similarly, Svetlana Boym suggests nostalgia as either "reflective" or "restorative" (41). This type of current scholarship addressing nostalgia seems to set up a nostalgic reading of texts as more the end game of the literature—the literature is nostalgic. However, if literature then ends as only nostalgic, there seems to be a lack of nostalgic theory's breadth. Dennis, Trethewey and Kooser all address this gap through their poetry—expanding the notion of nostalgia as being more the vehicle leading one through the landscape of memory. Suggesting nostalgia as merely reflective or restorative, as Boym and Grainge have done, seems to create a sense of nostalgia as stagnant rather than as a dynamic movement within the literature, and even the act of recollection itself. The three poets addressed in my project all suggest at some level that this residue of the past can lead one to see that perhaps experience itself delights in memory. Furthermore, nostalgia's dependence upon present memory indicates not just a longing for the past, but rather the past's presence in the present. The act of remembering serves as a type of catalyst which transforms memories to manifestations in present circumstance.

Bakgrund: Mötet mellan sjuksköterska och patient har beskrivits som grundläggande isjuksköterskearbetet och är mer välstuderat i andra sjukvårdsmiljöer än hemsjukvården,som är basen för Sveriges sjukvård och engagerar ett stort antal behövande som vårdasav många anställda. Hemmet får allt större betydelse som vårdplats och antalet platserpå sjukhusen blir färre.Syftet var att beskriva sjuksköterskan i hemsjukvårdens upplevelse av mötet medpatienten och dennes närstående i deras eget hem (ordinärt boende).Metod: Studien gjordes som en allmän litteraturstudie där vetenskapliga artiklarsystematiskt söktes i olika databaser för att sedan analyseras. Sökningarna resulterade i11 artiklar.Resultatet: Presenteras i tre kategorier som beskriver sjuksköterskans upplevelser:Relationens betydelse i mötet med patient och närstående, förtroende, tillit och respekt imötet samt maktlöshet, frustration och en känsla att inte räcka till.Diskussion: Med kännedom om hur betydelsefull relationen till patienten upplevskänns det angeläget att som sjuksköterska i hemsjukvården arbeta personcentrerat ochatt hela tiden lyssna in patient och närstående och försöka anpassa sig till situationen.7Slutsats: Relationen mellan patient, närstående och sjuksköterska har stor betydelse föroch påverkar alla inblandade parter. Kunskap som är av betydelse för att möta de kravoch förväntningar hemsjukvården kommer ställas inför när allt fler och allt sjukarepersoner kommer behöva hjälp med allt mer avancerade insatser.
Background. The encounter between the nurse and the patient has been described asfundamental in nursing work and are more well-studied in other health care settingsthan home care, wich is the base for the Swedish health care and involves a largenumber of needy cared for by many people. The home is of increasing importance as aplace for care and the number of places in hospitals are becoming fewer.The aim was to describe the experiences nurses in home care have of the encounterwith the patient and his or her relatives in their own home (ordinary housing).Method: the study was conducted as a literature review and articles were searched forsystematically in different databases and then analyzed. The searches resulted in 11articles.The result is presented in three categories that describe nurses´experiences: Thesignificance of the relation in the encounter with patient and relatives, confidence, trustand respect in the encounter and powerlessness, frustration and a feeling of not beeingable to do enough.Discussion: With knowledge of how important the relation to the patient is percieved,it´s of importance to work person-centered and listen to the patient and the relatives andtry to adjust to the situation.Conclusion: The relationship between patient, relatives and nurse is of greatimportance to, and impact on all parties involved. Knowledge that is relevant toconfront the demands and expectations that home care will face when more andincreasingly ill people will need help with more advanced efforts.

O cuidado em saúde bucal configura-se como um importante foco das preocupações e das práticas atuais do cuidado de si. O entendimento de que as práticas direcionadas ao cuidado de si integram os processos implicados na constituição de subjetividades moveu-me na direção de me interrogar sobre a crescente busca por dentes brancos por meio de um cuidado bucal e de tecnologias que “garantam” o desejado sorriso modelar antes mesmo de preocupações com a saúde bucal. Enquanto efeito das práticas sociais, o corpo encontra-se implicado numa economia política de estratégias tecnocientíficas e verdades que o regulam conforme as normas de beleza, saúde, felicidade e consumo. Tal implicação direciona escolhas, por exemplo, de produtos a serem adquiridos, os quais passam a integrar tanto os hábitos quanto a constituição subjetiva e orgânica dos sujeitos. Se pensarmos nas instâncias cujas verdades nos interpelam cotidianamente, será possível ver que os modos como nos olhamos e cuidamos encontra-se amplamente difundido pela mídia, inclusive os cuidados de saúde bucal. Por meio de diversos meios e modalidades enunciativas, a mídia vem ocupando a posição privilegiada nos processos constitutivos dos sujeitos e nos modo de ver e de cuidar de si. Relativamente ao cuidado bucal contemporâneo, no Brasil, este se apresenta de forma ambígua, ao mesmo tempo, tem-se o adoecimento por cárie e a veiculação de verdades, em anúncios publicitários configurando um cuidado oral estético que articula saúde/beleza. Tais questões moveram-me na direção de conhecer práticas e verdades relacionadas ao cuidado bucal, enquanto cuidado consigo, em anúncios publicitários veiculados na televisão nos dias de hoje. Assim, tomei como objeto de análise anúncios publicitários postos em circulação na mídia televisiva, visto o papel de destaque que adquiriu na constituição das subjetividades contemporâneas. Para entender como foi se configurando o cuidado bucal, no presente, voltei meu olhar para outras épocas, desde 3500 a.C. até meados do século XX, procurando conhecer instrumentos, produtos, quem falava e o que se dizia sobre o cuidado bucal. Nessa direção, examinei anúncios publicitários da revista “A Cigarra”, importante revista feminina brasileira do século passado. Para tanto, foram estabelecidas conexões com o campo dos Estudos Culturais, em suas vertentes pós-estruturalistas, e os estudos foucaultianos. As análises mostraram-me que as preocupações e cuidados bucais foram transformando-se, mas trazendo desde uma época remota preocupações estéticas que foram sendo associadas à saúde. No século XX, tornou-se marcante a presença dos especialistas nos anúncios publicitários, relacionando os usos do dentifrício à beleza e à saúde femininas já associadas ao branqueamento, mas também à sedução. Nos dias de hoje, intensifica-se o discurso do branqueamento do corpo e boca com saúde/beleza e sedução, o que pode gerar a produção de desejo e busca pelo branqueamento dental e pelas tecnologias em nível molecular para alcançá-lo a qualquer preço. Isso aponta para a necessidade de olharmos criticamente as produções midiáticas e as “verdades” que circulam acerca do branqueamento dos dentes.
Oral health care is deemed as an important focus of concern and current practices associated with self-care. The understanding of practices of self-care integrates processes implied in the production of subjectivities. Therefore, I felt compelled to examine the rising pursuit for white teeth in our society. People are more concerned about obtaining the desired standard smile, achieved through oral care and dental technology, than about oral health. One of the focuses of contemporary propositions of self-care consists of dental care practices, which are widespread by the media. The body, thought as an effect of social practices, is part of a political economy of technoscientific strategies and truths that regulate it according to rules of beauty and health, happiness and consumption standards, which drive people to consume certain products, and thus become part of day by day habits and the subjective and organic constitution of individuals. The media plays a rising position in launching truth through different modalities of enunciation in media artifacts. This role made it a prime pedagogical instance, involved in the construction of the individuals’ views and self-care. However, the current oral care status presents itself ambiguously. On the one hand, we have the Brazilian population sickened by cavities; on the other hand, we have truths being promoted in advertisements, configuring an oral care that combines aesthetically health / beauty. These interrogations led me to examine practices and truths related to oral care which appear on advertisements published in women’s magazine of the last century. This way, it might be possible to analyze continuities and discontinuities in what is being said about oral care on televised advertisements in the present time. Therefore, thinking media as a relevant instance of productions of subjectivities, the object of analysis of this research is a set of televised advertisements currently available. In order to support this hypothesis, I established connections with Cultural Studies, in its connections with the post-structuralist perspective and Foucauldian studies. The analysis showed me that the worries and oral care have changed along the years, bringing along with it aesthetic concerns which have been associated to health since remote times. During the 20th century, the strong presence of specialists in ads has been noted, relating the usage of dentifrices to beauty and feminine health, which were already associated with not only whitening but also with seduction. Nowadays, the whitening speech for the body and the mouth and seduction has been intensified, which could generate the production of a desire and the search for dental whitening and technologies in a molecular level, to reach it at any price. Such finding leads to the need of looking critically at mediatic productions and the “truths” circulating around dental whitening.

Abstract:
This study is aimed to better understand how and why people choose dental school faculty as their oral healthcare provider. Increasing financial constrains in the U.S. dental schools has led dental schools to find an alternative funding sources and revenues from dental school faculty practice can be one of them. To effectively promote faculty practice, it is necessary to understand how and why people choose dental school faculty as their oral care provider. In addition, it is important to differentiate characteristics of comprehensive care patients and limited care patients since dental school faculty practices have a higher proportion of specialists. A survey of 1150 dental school faculty practice patients who recently chose their dentist was conducted and 221 responded. Information sources highly used and rated included other dentist, friends, family, clinic website, the Internet, and the insurance directory. Dentist related attributes that were perceived important included quality of care, professional competence of dentist, and explanation of treatment/you participate in the treatment decision. Dental practice related attributes that were perceived important included ability to get appointments at convenient times, reasonable waiting time to get appointments, and attitude/helpfulness of staff. Among the respondents, 121 pursued a comprehensive care and 93 pursued a limited care. The two groups differed in terms of demographics, other characteristics, and dentist selection. The comprehensive care patients were younger, highly educated, related to healthcare related profession, and to have private dental insurance (p<0.001). The comprehensive care patients were more likely to use and highly rate information sources such as clinic website, the Internet, and the insurance directory (p<0.001). They put more value on attributes such as the dentist is in my insurance network and convenient physical location (p<0.001). This study has shown that traditionally popular (family, friends) and newly emerging information sources (the Internet, clinic website, and insurance directory) were used and perceived important by dental school faculty practice patients. A dental school or dentist can use this study's findings to promote their practice to select appropriate communication channels and focus on attributes that dental consumers value the most. It is also important to apply different strategies to different consumer groups.

Abstract:
Esta pesquisa teve por objetivo analisar os efeitos da cal hidratada e do ácido polifosfórico nas propriedades mecânicas e suscetibilidade à umidade de misturas asfálticas densas. O ligante utilizado foi o CAP 50/70, chamado de controle, modificado com 0,6 e 1,2% de PPA, e como aditivo a cal hidratada em concentrações de 1,5 e 3,0%, perfazendo nove combinações de misturas asfálticas. Foram moldados 108 corpos-de-prova, divididos em 4 e 7% de volume de vazios, sendo que os corpos-de-prova de 7% foram divididos em condicionados e não condicionados. Os ensaios realizados para análise das propriedades mecânicas foram o Módulo de Resiliência e a Resistência à Tração Indireta. Os resultados mostram que o aumento da concentração de ácido polifosfórico melhora a rigidez e a flexibilidade, e melhora a suscebilidade ao dano por umidade, assim como aumenta a resistência à tração; para a cal hidratada, com o aumento da sua concentração, ocorreu aumento da rigidez e da flexibilidade de maneira mais discreta, se comparado ao ácido polifosfórico, e proporciona a diminuição da resistência à tração: o teor que apresentou melhor suscetibilidade ao dano por umidade foi de 1,5% de cal hidratada.
This work aims to analyze the effects of hydrated lime and polyphosphoric acid on the mechanical properties and moisture susceptibility of dense asphalt mixtures. In nine combinations of asphalt mixtures, CAP 50/70, named control and modified with 0,6 and 1,2% of PPA, was employed as binder, and hydrated lime in concentrations of 1,5 and 3,0% was used as additive. 108 test specimens have been molded and divided into 4 and 7% of volume void, considering that 7% were separated into conditioned and non-conditioned. The Resilient Modulus and the Tensile Strength were the tests conducted for the analysis of mechanical properties. The results showed that the increase in polyphosphoric acid concentration improved stiffness and flexibility and increased the susceptibility to moisture damage, as well as the tensile strength. Also in the results, with the concentration increase of the hydrated lime, there was a smaller increase of stiffness and flexibility, and it provided a decrease of tensile strength, considering that the percentage of 1,5% of hydrated lime presented the best susceptibility to moisture damage.

Abstract:
Background: Within Primary Dental Care (PDC), there is variation in dentists’ views about who should be treated in general dental practices and who should be referred to community dental services (CDSs), creating confusion about where patients can access dental care. Aims: This research aimed to explore the meanings which general dental practitioners (GDPs) and community dentists in England ascribe to their roles. It focused on why they make, accept or decline patient referrals within PDC. Methods: I conducted a configurative systematic review of literature about referrals within PDC in the UK. Data were synthesised using Critical Interpretive Synthesis. I interviewed ten GDPs and twelve community dentists working in England, covering topics informed by the systematic review. Transcripts were analysed using thematic analysis. Findings: Synthesising the literature showed that referral decisions were influenced by non-clinical factors including policies, financial contracts and dentists’ perceptions and values. Authors rarely reported directly from the perspective of primary care dentists. The interview study findings showed that the business of dentistry defined GDPs’ roles. Obscure rules and complex care systems underpinned community dentists’ roles. Participants depicted vulnerable people within ‘no man’s land’, situated between GDPs and community dentists. Vulnerable people included frail, older people, anxious and socially excluded adults, and children with high levels of disease. I identified three typologies of dentists. ‘Entrepreneurs’ felt no allegiance to the NHS and no obligation to treat vulnerable patients. ‘Altruistic carers’ cared for complex, deserving patients, rather than vulnerable patients. ‘Pragmatic carers’ accommodated some vulnerable patients but felt constrained from doing so by structural barriers. Conclusions: This study adds to our understanding of why dentists make, accept or decline referrals within PDC in England. It suggests that failure to resolve structural barriers or to consider dentists’ values will hinder attempts to reduce inequalities in access to PDC in England.

Neste estudo verificou-se o nível de contaminação das escovas dentais de 54 estudantes dos 2º, 5º e 9º períodos do curso de graduação em Odontologia da Universidade Federal de Juiz de Fora, Juiz de Fora, Brasil, com relação à higienização e ao armazenamento, bem como a eficácia da solução aquosa do digluconato de clorexidina a 0,12% em spray, na descontaminação das escovas dentais. O estudo foi dividido em duas etapas, com duração de 15 dias cada: na etapa 1, os participantes foram divididos em dois grupos (G1 e G2), onde cada acadêmico recebeu escova e creme dentais. Na etapa 2, o G1 passou a se chamar experimental e o G2, controle: o experimental recebeu escova e creme dentais, um frasco borrifador com solução básica aquosa, composta de água destilada e edulcorante estévia, acrescido de digluconato de clorexidina a 0,12%. No grupo controle, cada participante recebeu o mesmo conjunto e o frasco borrifador continha apenas a solução básica. Receberam um protocolo de orientação para utilização, desinfecção e armazenamento da escova dental. Antes de iniciar a pesquisa e após as duas etapas, os acadêmicos responderam a questionários com dados sobre seus hábitos de higiene e armazenamento das escovas dentais. As escovas dentais foram devolvidas em envelopes lacrados ao pesquisador para a análise microbiológica, realizada no Laboratório de Microbiologia do Instituto de Ciências Biológicas da UFJF. As escovas dentais foram inseridas em tubos de ensaio estéreis identificados, contendo 7mL de solução fisiológica esterilizada a 0,9% e agitados no Vortex®, por 20s. Com o auxilio de uma alça calibrada foi coletado 1μL desta solução e semeado por esgotamento no meio de cultura CHROMagar Orientation®. As placas foram incubadas a 35,5°C, em aerobiose por 24h. Os dados foram submetidos a testes estatísticos. Os resultados deste estudo mostraram que ocorreu um maior índice de contaminação das escovas dentais dos alunos do 2º período em relação às dos alunos do 5º e do 9º períodos. Houve uma redução acentuada nos índices de contaminação na etapa 2, sendo estatisticamente significante para os 2º e 9º períodos. No 5º período ocorreu uma redução, porém sem significância estatistica, que pode ser explicado pelo relato da ocorrência de contato de suas escovas dentais com outras escovas no local de armazenamento. Entre os grupos, a redução da contaminação foi estatisticamente significante entre G1 e o experimental. This study evaluated the level of contamination of toothbrushes from 54 students of 2nd, 5nd and 9nd period of the undergraduate course in Dentistry, Federal University of Juiz de Fora, Juiz de Fora, Brazil, with respect to hygiene and storage, and the effectiveness of the aqueous solution of chlorhexidine gluconate 0.12% spray for decontamination of toothbrushes. The study was divided into two phases, with duration of 15 days each. In Step 1, participants were divided into two groups (G1 and G2), where each received a toothbrush and a dental cream. In step 2, the G1 has been called experimental and G2, control: in experimental received toothbrushes and dental cream, a spray bottle with basic aqueous solution, composed of distilled water and sweetener stevia added with chlorhexidine gluconate 0.12%. In the control, each participant received the same set and spray bottle containing basic solution. Received protocol guidelines for use, storage and disinfection of toothbrushes. Before starting the study and after two steps, the students answered questionnaires with data on their hygiene and storage of the toothbrushes. The toothbrushes were returned in sealed envelopes to the researcher for the microbiological test, performed at the Laboratory of Microbiology, Institute of Biological Sciences of UFJF. The toothbrushes were placed in sterile test tubes identified, containing 7mL of sterile saline 0.9% and agitated in the Vortex®, for 20s. With a calibrated loop was collected 1μL this solution and spreaded by exhaustion in the culture medium CHROMagar Orientation®. The plates were incubated at 35,5°C under aerobic conditions, for 24h. Data were subjected to statistics. The results of this study showed that an increased rate of contamination of toothbrushes of 2nd period students compared to the students of the 5th and the 9th periods. There was a marked reduction in contamination levels in step 2 and was statistically significant for 2nd and 9th periods. In the 5th period was a reduction, but without statistical significance, which can be explained by the reported occurrence of contact for their toothbrushes with other brushes in the storage location. Between groups, the reduction in contamination was statistically significant between G1 and experimental. However, a decrease was observed between G2 and the control, showing that hygiene and proper storage are sufficient to reduce the contamination of toothbrushes.

Esta pesquisa tem como objetivo estudar a relação entre parâmetros de compactação e de deformação permanente à luz dos ensaios de compactação giratória Superpave ® e de ensaios de compressão uniaxial estática e dinâmica. Foram estudadas 8 misturas asfálticas com a mesma granulometria (Faixa C) e tipo de agregado mineral (granito) porém variou-se o ligante asfáltico (CAP 50/70 e CAP 50/70+SBS), o volume de vazios (4% e 7%) e a adição de cal hidratada. A partir dos resultados do ensaio de compactação observou-se que a inclinação das curvas de compactação das misturas compactadas a 7% foram mais acentuadas do que as de 4% e os parâmetros incidentais deste ensaio foram sensíveis à presença de cal e à variação do asfalto das misturas testadas. Quanto aos resultados dos ensaios de creep estático e dinâmico, notou-se que as misturas com Vv de 4% e as com asfalto modificado apresentam menor suscetibilidade à deformação permanente, no entanto, essas vantagens do asfalto modificado foram observadas somente nos ensaios de creep dinâmico. No que se refere às relações entre os parâmetros de compactação e de deformação permanente, os ajuste não se mostraram promissores como verificado pela qualidade dos ajustes (R2) exceto para algumas relações com CDI e TDIm.
This research aims to study relations between compaction and permanent deformation by means of Superpave® gyratory compaction and static and dynamic creep tests. Thus, eight asphalt mixtures were evaluated which were composed by same type (granite) and gradation (dense) of mineral aggregate but two types of asphalt binder (Pen 50/70 AC and Pen 50/70+SBS AC), two different levels of air voids (4% and 7%) and the addition of hydrated lime. Compaction results indicated that curve slopes of 7% air void asphalt mixtures were stronger than the 4% ones, and compaction parameters were sensitive to lime addition and asphalt type change. Static and dynamic creep test results indicated that 4% air void and modified asphalt mixtures presented lower susceptibility to rutting, but, these modified asphalt advantages were observed only in dynamic creep tests. In regard to relation between compaction and rutting parameters, linear models did not fit accurately, excepting some CDI and TDIm ones.

As lesões traumáticas envolvem função e estética que resultam de mínimas perdas de superfície do esmalte a fraturas complexas envolvendo polpa e até perda da coroa. O conhecimento técnico e a experiência clínica são fundamentais para a obtenção de um diagnóstico correto e para realizar uma terapia racional. É objetivo deste trabalho analisar o conhecimento dos especialistas em Dentística, titulados pela Faculdade de Odontologia de Araçatuba - UNESP, sobre fraturas coronárias e corono-radiculares. Para tanto, a partir de um questionário descritivo, foram abordadas questões referentes ao perfil dos profissionais entrevistados e procedimentos empregados na conduta frente a fraturas coronárias e corono-radiculares. Cento e cinqüenta e quatro questionários foram devidamente preenchidos, e os dados obtidos foram submetidos à análise descritiva, enquanto o teste estatístico foi aplicado para demonstrar freqüência e o nível de significância entre variáveis (teste qui-quadrado). Muito embora os entrevistados tivessem formação de especialista em Dentística, grandes dificuldades foram encontradas nos planos de tratamento propostos. Dos entrevistados, 42,8% foram incapazes de tratar todos os casos de traumatismo dentário. As fraturas de esmalte, dentina e cemento, com e sem envolvimento pulpar, foram as que apresentaram maiores dificuldades de resolução, pois necessitam de conceitos e práticas multidisciplinares para favorecer o tratamento e o prognóstico dos casos.
Traumatic lesions involve function and aesthetics that result from a minimal loss of the surface of enamel up to complex fractures involving pulp and even the loss of a crown. Technical knowledge and clinical experience are fundamentally necessary to obtain the correct diagnostic and to perform a rational therapy. The goal of this study is to analyse the knowledge about crown and crown-root fractures obtained by specialists in Dentistry, titled from Dentistry School at UNESP - Araçatuba. For that, starting from a descriptive questionnaire, some questions regarding the profile of inquired professionals and procedures used in the performance of treatment of crown and crow-root fractures. 154 questionnaires were answered and the obtained data was submitted to a descriptive analysis, while a statistical test was applied to show the frequency and the level of the significance among variables (square test). Despite the inquired professionals having background in Dentistry, a lot of difficulties were found in the planning of the treatments. 42.8% of the interviewed professionals were unable to treat all cases of dental traumatism. Crown-root fractures, involving pulp or not, were the ones that presented major difficulties being necessary some concepts and multidisciplinary approach that favor planning and treatment.

Esta dissertação apresenta um estudo da deformação permanente de misturas asfálticas à luz dos resultados de ensaios de fluência por compressão uniaxial estática e dinâmica. Para isso, um programa experimental completo foi desenvolvido combinando fatores relacionados às misturas asfálticas e às condições de ensaio. Foram produzidos corpos de prova de misturas asfálticas densas tipo CAUQ com asfalto convencional (CAP 50/70) e com asfalto modificado com polímero SBS (CAP 50/70+SBS), variando o volume de vazios em dois níveis, 4 e 7%, e a presença de aditivo em dois níveis, sem e com cal hidratada. Os corpos de prova foram submetidos a ensaios de fluência com carregamentos estático e dinâmico em dois níveis de tensões, 0,1 MPa e 0,4 MPa, e em duas temperaturas diferentes, 40 e 50ºC. Essas condições do programa experimental permitiram observar o efeito do modificador SBS, da presença da cal e do volume de vazios nas misturas asfálticas, e entender o efeito da temperatura, do nível de tensão e do tipo de carregamento no comportamento de cada tipo de mistura asfáltica. Os resultados indicam que (i) a adição da cal promoveu redução de deformação acumulada somente nas misturas com ligante convencional (CAP 50/70); (ii) as misturas com volume de vazios de 7% apresentam maiores acúmulos de deformação permanente independente da presença de cal, do tipo de carregamento e da temperatura do ensaio, sendo maiores para a mistura asfáltica com CAP 50/70; (iii) as misturas com ligante modificado se mostraram menos sensíveis às variações da temperatura e do nível de tensão; (iv) e a melhora das características das misturas com asfalto modificado com polímero é detectada somente a partir de ensaios dinâmicos.
This paper presents a study of rutting on hot mixture asphalt (HMA) by means of static and dynamic creep tests. A full factorial experiment was developed considering factors related to HMA and to the test conditions. It was produced HMA (unmodified (CAP 50/70) and SBS modified binders) specimens at two different air voids (4 and 7%) and with or without hydrated lime. The specimens were submitted to static and dynamic creep tests at two stress levels, 0,1 MPa e 0,4 MPa, and two temperatures, 40 e 50ºC. These conditions of the experimental program allowed to observe the effects of the modifier SBS, the presence of the lime and the air voids in HMA, and to understand the effects of the temperature, stress and the type of loading on the behavior of each type of asphalt mix. The results indicate that (i) the addition of lime promoted reduction of accumulative deformation only in mixtures with conventional binder (CAP 50/70), (ii) mixtures with 7% air voids showed greater accumulation of permanent deformation regardless of presence of lime, the type of load and temperature of the test, mainly for the conventional HMA, (iii) the Polymer Modified Asphalt Hot Mix Asphalt (PMA-HMA) were less sensitive to temperature and the stress variation (iv) the improvement of the polymer modified asphalt mixture characteristics (PMA-HMA) were detected only on dynamic load conditions.

Este trabalho teve como objetivo avaliar o módulo de resiliência de misturas asfálticas densas sob duas condições. A primeira foi o estudo dos efeitos do enrijecimento de misturas asfálticas densas em condições ambientais diferentes (cinco condições) que são: AAAL (ao ar e à luz), AASL (ao ar e sem luz), AVAL (baixa pressão atmosférica e à luz), AVSL (baixa pressão atmosférica e sem luz) e CLIMA (ao clima e intempéries), tendo sido utilizados para este experimento CAP 20, centro da faixa C do DNER como distribuição granulométrica e preparados segundo a AASHTO PP2. Neste estudo concluiu-se que não houve efeito significativo da exposição da luz artificial nas condições AL e SL, assim como não houve, também, efeito significativo na variação do módulo de resiliência nas condições de exposição ou não ao ar (AV e AA); nas demais condições houve melhora significativa nos resultados. Na segunda parte foram avaliados os efeitos de alguns fatores na susceptibilidade térmica de misturas asfálticas densas: tipo de agregado (basalto, gabro e granito), tipo de asfalto (CAP 20 e CAP 40), teor de asfalto (5%, 5,5% e 6%), presença de aditivo (com e sem cal) e temperaturas de ensaio (10ºC, 25ºC e 40ºC). Todos os corpos-de-prova foram preparados segundo procedimento da AASHTO PP2. Na segunda parte do experimento, concluiu-se que o CAP 40 produziu valores médios de Vv superiores aos do CAP 20, as misturas com teor de 5,5% apresentaram maior resistência à tração a 25°C, o módulo de resiliência e a relação MR/RT diminuem com o aumento do teor de CAP.
This work intended to evaluate the resilient modulus of dense-graded hot-mix asphalt under two conditions. The first condition comprehends a study of the hardening effects of densegraded hot mix asphalt submitted to five environmental conditions: AAAL (air and artificial light), AASL (air and no artificial light), AVAL (low atmospheric pressure and artificial light), AVSL (low atmospheric pressure and no artificial light) and CLIMA (exposition to intemperism). In this phase, it was used an AC-20 asphalt binder and DNER middle band \"C\" as aggregate\'s particles size distribution. Specimens were prepared according to AASHTO PP2. Based on the results of the first part of the experiment, it was concluded that there was no significant effect of the artificial light exposition on AL and SL conditions, as well as on the variation of resilient modulus under air conditions (AV and AA). The other conditions presented better performance results. The second part of the experiment aimed to evaluating the effect of some factors in the thermal susceptibility of dense-graded HMA, that are: aggregate type (basalt, gabbro, granite), asphalt type (AC-20 and AC-40), binder content (5,0%, 5,5% and 6,0%), presence of additive (with or without lime) and test temperature (10ºC, 25ºC and 40ºC). As in the first part of the experiment, all specimens were prepared according to AASHTO PP2. Results of the second part of the experiment led to the following conclusions: mixtures using AC-40 showed average air voids higher than mixtures using AC-20, mixtures with a binder content of 5,5% showed the highest values of tensile strength, at 25°C, the resilient modulus and the MR/RT ratio reduce when binder content increases.

Le discours sur l'Autre s'ancre dans le discours médical. Mais de quel Autre s'agit-il ? De quelle identité parlons nous dès lors qu'il s'agit d'altérité, de reconnaissances et de vulnérabilités du sujet de soin ? Qui reconnaît-on ? Nous explorons ici les équilibres relationnels au sein de la relation soignant-soigné en Orthopédie dento-faciale hospitalière. A travers une l'approche structurale des représentations sociales, via l'utilisation de cartes conceptuelles, est interrogé le rapport des partenaires thérapeutiques à l'objet prendre soin, notion symbolique et indicible de l'activité soignante. Introduire le prendre soin revient à souligner ce que la relation contient de dimension à la fois éthique, technique et politique. Avec la notion d'accueillance du sujet, élément matriciel central révélé par l'analyse de la représentation, lors des questionnaires réalisés, se dessinent les enjeux identitaires de la reconfiguration contemporaine de la relation de soin, modèle hybride entre une forme atténuée de paternalisme médical et une libéralisation relative. Cette perspective novatrice nous autorise à interroger ces différents registres de valeurs, qui sont autant de témoignages et d'illustrations de la notion de personne. Cette dialectique du même et de l'Autre, de l'identité et de la reconnaissance des acteurs, ouvre ainsi un débat de nature à la fois théorique, éthique et politique autour de l'espace relationnel du soin
A discourse focused on the Other is firmly anchored in medical discourse. But what Other is being spoken about? What identity are we talking about when considering the otherness, recognition and vulnerability of the recipient of care? Whom are we recognising? Here, we will be exploring the relational balance within the patient-carer relationship in the field of dentofacial orthopedics. A structural approach to social representations, using concept maps, will be adopted in order to examine the relationship of the therapeutic partners to the concept of care provision - a symbolic component of the treatment process which is difficult to define explicitly. Introducing the notion of care provision means placing an emphasis on the ethical, technical and political content of the care relationship. The notion of the favourable reception of the patient, a central element of the relationship which is revealed by an analysis of representations based on questionnaires, highlights the role of identity in the contemporary reconfiguration of the care relationship - a hybrid model which combines a modulated form of medical paternalism and comparative emancipation. This original perspective enables us to examine these various value registers, which illustrate and testify to the notion of the individual. This dialectic of sameness and the Other, of the identity and the recognition of the participants, thus gives rise to a debate, simultaneously theoretical, ethical and political, focused on the relational space associated with care

Idén till arbetet föddes när vi var inne i en av Gina Tricots butiker, då en kund frågade personalen hur mycket ett par jeans som hon skulle prova kunde töja sig. Kunden kunde inte få något fullständigt svar på sin fråga, utan den anställde svarade ”lite”. Utöver detta har vi även fått uppfattningen av att personalen är mindre kunnig om bland annat de olika material som företaget använder sig av. Vi anser då att en materialhandbok kan vara lösningen på detta problem. Vi vill att Gina Tricots personal ska bli mer kunnig inom textil och därmed kunna hjälpa kunderna på ett korrekt och professionellt sätt.I nära samarbete med vår externa, samt interna handledare har vi kommit fram till de delar som materialhandboken består av. Dessa är väv, trikå och grovstickat, de olika naturfibrer, konstfibrer samt blandmaterial som företaget använder sig av, obehandlad och behandlad denim, samt färgning och tryckning. Utöver dessa punkter har vi även valt att ha med påmonterade detaljer, tvättsymboler, frågor och svar, såväl som en skötselöversikt. Under de olika fibrerna finns även underrubriker, såsom egenskaper och skötselråd, för att lättare kunna finna den information man söker.Det har varit viktigt att hitta en balans för att få med så mycket information som möjligt, utan att det blir för tungt för personalen. Likaså var det med språket, det vill säga att få fram en så innehållsrik text som möjligt, men utan för svåra ord, då alla i personalen ska förstå innebörden av texten utan några tveksamheter. Materialhandboken är även gjord på så sätt att man inte behöver läsa från pärm till pärm för att förstå innehållet. Allt för att den ska vara så användarvänlig som möjligt.

The idea was born when we were at Gina Tricot, and a customer asked one of the staff how much a pair of jeans will stretch that she wanted to try on. The customer could not get a complete answer to her question, but the employee said "a bit". In addition, we also had the perception that the staff are untaught of, inter alia, the various materials the company uses. We thought a handbook of textiles was the best idea as a solution for this problem. We want Gina Tricots staff to become more knowledgeable in the textiles and thus be able to help customers in an accurate and professional manner.Working closely with our external and internal trainers, we have figured out what the handbook of textiles consists of. These are fabric, knitted and rough knits, the different natural fibers, artificial fibers and mixed fibers that the company uses, untreated and treated denim, as well as dyeing and printing. Beyond these points, we have also chosen details, laundry symbols, questions and answers, as well as a maintenance overview. Below the different fibers are their characteristics and care, to more easily find the information you seek.It has been important to find a balance in order to get as much information as possible in the handbook of textiles, without making it too complicated for the staff. It was similarly with the language, that is, to produce a text as rich as possible, but without the difficult words, when all of the staff should understand the meaning of the text without any doubt. The handbook of textiles is also designed so that it is not necessary to read from cover to cover to understand the content, to make it as user friendly as possible.

The theoretical part covers methodical foundations based on available resources. In the practical part, I described specific steps of the feasibility study which are relevant to the social care project. The initial part is dedicated to the environment analysis. Based on the market analysis, I defined the target group of the business plan, analyzed the competitive factors and designed a marketing strategy including a marketing mix strategy. Then I defined the project schedule, project management and human resources management. The thesis contains financial analysis determining the need for securing long-term and short-term capital. The thesis is concluded with chapters on project effectiveness and sustainability assessment and on risk management.

Pour cela, un grand nombre de méthodes ab initio (MRCI, Full-CI, QMC. . . ) ont été employées conjointement avec de nouvelles bases d'orbitales. Une valeur théorique quasi exacte de la longueur de diffusion, très importante pour l'étude des collisions entre atomes, a ainsi pu être fournie. Parallèlement, un calcul de la position des niveaux vibrationnels, observés par photo-association au voisinage des asymptotes 2 ^{3}S+2 ^{3}P et 2 ^{3}S+3 ^{3}S, a été effectué puis confronté aux résultats expérimentaux. Une base de données complète du spectre du dimère d'hélium est finalement donnée ainsi qu'une dernière application de ces états à l'étude des plasmas denses d'hélium

This thesis examines the history of Commonwealth Government higher education policy in Australia between 1958 and 1997 and its impact on the development of two groups of academic librarians: the Association of Librarians in Colleges in Advanced Education (ALCAE) and the Committee of Australian University Librarians (CAUL). Although university librarians had met occasionally since the late 1920s, it was only in 1965 that a more formal organisation, known as CAUL, was established to facilitate the exchange of ideas and information. ALCAE was set up in 1969 and played an important role helping develop a special concept of library service peculiar to the newly formed College of Advanced Education (CAE) sector. As well as examining the impact of Commonwealth Government higher education policy on ALCAE and CAUL, the thesis also explores the influence of other factors on these two groups, including the range of personalities that comprised them, and their relationship with their parent institutions and with other professional groups and organisations. The study focuses on how higher education policy and these other external and internal factors shaped the functions, aspirations, and internal dynamics of these two groups and how this resulted in each group evolving differently. The author argues that, because of the greater attention given to the special educational role of libraries in the CAE curriculum, the group of college librarians had the opportunity to participate in, and have some influence on, Commonwealth Government statutory bodies responsible for the coordination of policy and the distribution of funding for the CAE sector. The link between ALCAE and formal policy-making processes resulted in a more dynamic group than CAUL, with the university librarians being discouraged by their Vice-Chancellors from having contact with university funding bodies because of the desire of the universities to maintain a greater level of control over their affairs and resist interference from government. The circumstances of each group underwent a reversal over time as ALCAE's effectiveness began to diminish as a result of changes to the CAE sector and as member interest was transferred to other groups and organisations. Conversely, CAUL gradually became a more active group during the 1980s and early 1990s as a result of changes to higher education, the efforts of some university librarians, and changes in membership. This study is based principally on primary source material, with the story of ALCAE and CAUL being told through the use of a combination of original documentation (including minutes of meetings and correspondence) and interviews with members of each group and other key figures.

5 Abstract This thesis is focused on the research of adult day care centers. Its aim is to answer the question how the the day care centers define the care from the point of view of different stakeholders; how are the centers specific to other social services and what are the possible problems and obstacles. Theoretical part describes the current situation of elderly care and deals with topics such as the demographic development of the Czech Republic, the media image of the elderly, new trends in the area of care, active aging, social services etc. Empirical research deals with two day care centers from the points of view of different stakeholders. The research, which results in two case studies and their comparisons, is carried out using qualitative methods - semi-structured interviews, analysis of documents and survey. Data analysis shows that the main function of day care centers is "relief service" for families who provide care to the frail seniors. The distinctive feature of day care centers is emphasizing differnces with other social services for elderly, especially the residential ones. When persons with mental and physical disabilities who have been bedridden for a long time develop dental health problems, it often takes an excessive amount of resources and manpower to get them the dental treatment they require, which in turn takes a huge toil on their family members. At the same time, dental health is often neglected by such patients, and poor dental health can often result in detrimental effects on other bodily functions and overall health. In order to elevate healthcare standards in Taiwan, the National Health Insurance (NHI) Administration has been working with the Taiwan Dental Association since 2011 to carry out the Outpatient Dental Services for Patients with Special Needs program. This program, which provides quality dental treatment services for patients with disabilities who experience difficulties getting about, first started from working with welfare organizations for people with disabilities, before moving on to expand their range of services to include home services. These services improve the quality of life for these patients, as well as lighten the burden of caregiving on the part of the family members. 21-year-old Chung was a student at a technological university in Yunlin County. He had been experiencing severe headaches for a long time, and was diagnosed with a malignant brain tumor in November 2016. Chung underwent a series of surgeries and radiation treatment, and was subsequently discharged as his condition stabilized. However, even though he had been allowed to go home, he was often on the verge of consciousness and was confined to his bed. When Yang Mei-Tien, a hospital staff and home service specialist, made a house call, he saw that Chung had broken teeth that hindered his consumption of food. Yang referred Chung to Dr Chien, a dentist in Taoyuan City, who went on to provide home dental services to Chung and gave him teeth fillings as well as a dental cleaning. By and by, Chung became more willing to consume food orally as he could now swallow food more normally. Chung's family was very grateful to and moved by Dr Chien's approachable yet professional service. Chung’s father, who drives taxis for a living, converted his taxi to a barrier-free access vehicle. The older Mr Chung keeps an eye out for other families that might benefit from this healthcare scheme and refers them to the program. By paying it forward, Mr Chung is helping many other patients who need the support. Dr Chien has been an active provider of dental home services for years and is well aware of the needs of his patients and their family members. Even though he has to carry portable dental equipment weighing more than 30kg through narrow alleys for every house call, and he would usually be drenched with sweat by the time he arrives at his patient's houses, Dr Chien has never complained about his duty. To him, his mission is to safeguard the oral health of less privileged fellow citizens! A cerebral vascular disease caused the 83-year-old Granny Lai to be paralyzed on the right side of her body. Her advancing age also resulted in some of her teeth becoming loose. Consequences could be dire if any of her teeth came loose and she accidentally swallowed it, or if the teeth fell into her trachea. Her family was worried that clinics in their neighborhood would hesitate to take Granny Lai as a patient due to her special needs, and traveling to major hospitals was also a problem, for it would take a lot for Granny Lai to make the journey. Therefore, the family is thankful that there are home dental services provided by the NHI Administration. These services are tremendously helpful for the elderly and the disabled when it comes to taking care of their dental health. The grateful look on the faces of patients and their family members who have benefitted from these services is the best motivator for NHI to push for the continual provision of home dental services. Considering that some of the patients under the home dental services scheme might have other non-dental conditions, and also the fact that affordances of portable dental equipment are limited, the NHI Administration is currently only providing home dental services to persons with mental and physical disabilities, elderly patients who are unable to care for themselves, and patients who have a limited and reduced ability to take care of themselves (50% mobility when conscious). The range of services include basic teeth filling, cleaning, extraction, application of fluoride varnish, and emergency periodontal treatment etc. If further treatment were deemed necessary, the dental health care professional would refer the patient to a hospital. Statistics reveal that, between January 2018 and June 2018, there were 47 dentists participating in this program, and 423 patients have benefitted from it. In order to enhance the service capacity of this program, the Service Unit of the NHI Administration (northern Taiwan) has teamed up with dental associations and long-term care facilities in Taoyuan, Hsinchu, and Miaoli Counties. The inter-agency collaboration brings about great benefits in terms of the assessment and referral of patients, as well as with regard to providing the best treatment options in the shortest amount of time. There are numerous patients who, like Chung and Granny Lai, face huge obstacles in seeking medical help beyond their homes. Thus, we hope that an increasing number of passionate dentists would join us in providing home dental services to patients in need, and that this program could continually improve and grow, and eventually even inspire other medical fields and specializations to join us on this meaningful quest. Our study employs an analytical methodology to assess the feasibility of dental home services. Below are the research findings derived from our analysis: One, home dental services have become a trend. Two, home dental services elevate the quality of life for patients and reduce the caregiving burden on their family members. Three, home dental services possess the advantage of a growing market. Four, home dental services can contribute to the growth of the market. Five, expand the program and create a platform for home services to include other medical fields.

OBJECTIVE: Within the U.S. population approximately 20 percent of individuals are diagnosed with some type of disability and another 12 percent are considered severely disabled. Many times special health care needs (SHCN) patients with one condition are burdened by more than one disability. A variety of conditions such as behavioral issues, developmental disorders, and cognitive, congenital or systemic diseases have been proven to increase the risk of oral heath disease in individuals with SHCN. These conditions may affect the ability to communicate and limit dexterity for proper oral health maintenance, which is why caregivers must become advocates for these patients to insure their oral health needs are met. By communicating effectively with caretakers, dentists will be able to establish a positive impact on patients to make dental visits easier and less traumatizing. Unfortunately studies have found that dental and hygiene students graduate without feeling adequately prepared to treat patients with SHCN. With the proper education and training, more dental students and future dentists will take on the challenges that are brought by these individuals to provide the best care possible. Establishing a strong support system could potentially decrease oral health disparity in SHCN patients and improve overall care. The goal of this study was to assess whether or not there was a positive impact on the oral health of SHCN individuals when caregivers and oral health professionals worked together in a team treatment effort to fight against oral disease. METHODS: Study used a systematic review approach of literature where a PICO question was formed and the article selection process included a review of title, abstract, and application of inclusions and exclusion criteria. Printed-text and online databases such as Pubmed/MEDLINE, Google Scholar, and Web of Science were used to conduct a review of literature where a total of 210 papers were collected. After selection process, only four papers were eligible for systematic review. Pre-determined questions were used to determine the how involved caregivers and oral health professionals were during each study followed by an assessment using a point system, which scored caregiver and oral health professionals involvement during intervention. RESULTS: Selected studies showed heterogeneity in study design and sample size but all used multidisciplinary approach, which included some degree of caregiver and oral hygiene provider (OHP) collaboration. Two out of the four studies totaled nearly perfect scores in both caregiver and oral health professional involvement, which resulted in a positive effect on oral health care of SHCN patients. Case reports that included an interdisciplinary team approach to oral care procedures on patients, showed the benefits of teamwork and how collaborative practice between dental and medical professionals increased successful patient outcomes. Results illustrated the significant role that caregiver’s play in the dental team and how their absence during intervention treatment significantly lowered study scores for the overall success of patient care. CONCLUSIONS: Results demonstrated that studies with a well-rounded dental team, which included caregivers, OHP, health care professionals accompanied by social support systems, saw the greatest success and improvements in oral health care of SHCN patients. Nevertheless, a dedicated dental team where most members were present did not guarantee intervention success if caregivers did not show a significant involvement. Caregivers alone could positively impact oral health care of a patient more than if OHP worked alone. In conclusion, we found that each member of a dental team had the ability to impact change to an almost equal degree. For this reason, it is important for caregivers, OHP, dental staff and health care professionals to work together, support one another and communicate effectively to decrease oral health disparities in SHCN patients.

Purpose: Patient-provider communication has been studied extensively in the last two decades, and many researchers have confirmed the importance of communication between patient and provider in medical contexts. In spite of increased research in patient-provider communication in dentistry, dental care providers still report that patients often do not accurately follow oral health recommendations. Thus, there is the need for additional study on how patients make sense of the oral health information they receive and how they translate that information into action. This study aimed to obtain insight into how dental care patients perceive and make sense of the information they receive from their dentist and how they translate that information into action. Methods: 16 patients and 8 dentists from Indiana School of Dentistry’s (IUSD) Graduate Prosthodontic Clinic in Indianapolis, Indiana were included. Two in-depth interviews, one immediately following the dental visit and one 7-10 days later, were conducted with the patients, and one short interview was conducted with each patient’s dental care provider. Interviews were audio taped and transcribed. Results: The results show both patients and providers perceived the interaction during consultation positively. The majority of patients were able to accurately recall information they received from their dentists and made sense of new information through the lens of their previous experiences. Four additional factors that explain patients’ adherence with health advice were also found in addition to the previous studies. Conclusions: Successful dentist-patient interaction could be thought of as a match between what dentists think patients need to know, what patients think they want/need to know, and what patients actually know. Thus, some barriers that can keep dentists and patients from reaching information equilibrium are discussed. The study concludes by offering practical and theoretical implications.

1 Annotation This thesis is focused on the issues of burnout syndrome and supervision as a form of prevention. The topic is focused on staff dedicated to the seniors with dementia in day-care centers. The thesis is divided into two parts. The theoretical part is devoted to the definition of basic terms related to burnout syndrome, dementia, daily care center and supervision. Results in the practical part come from the research methods used, specifically self-prepared questionnaire and questionnaire prepared by Ch. Maslachová and Ayala Pines "Tedium Measure". Furthermore I expanded the thesis with interview with selected expert. Using open questions the area of supervision and burnout syndrome is being discussed. The primary target of this thesis is to contribute to the prevention of burnout syndrome and also to map, whether supervision is being used in day-care facilities and whether it helps preventing the burnout syndrome.

The work deals with the lives of people with mental illness - schizophrenia and opportunities that contribute to improving or maintaining their current health status. The theoretical part includes the characteristics of psychotic illness, legal protection of patients, possible treatment and aftercare. The practical part describes and compares three selected services or facilities. This is the Horizon Zlín, therapeutic workshops Kotva Strážnice and day service center Uherské Hradiště - Jarošov. It also includes assessment of the importance of individual therapeutic activities with clients focusing on the reduction of recurrent symptoms of schizophrenia and pointing out the importance of aftercare services and improving the facilities that this follow-up care for people with schizophrenic disorders are provided. A major benefit would be the establishment of other centers, care centers and therapeutic workshops offering services already mentioned, which would benefit not only for patients, their families, but also for the general public.

Present situation The theoretical part of the diploma thesis is divided into four separate chapters. The first chapter deals with cerebrovascular accident (CVA). Another chapter describes the consequences of illness in various areas of human life. The third chapter defines the concept of follow-up care and the fourth chapter deals with diagnosed obesity. Aims of the work and research questions The aim of the diploma thesis is to map out the influence of obesity on follow-up care of patients after cerebrovascular accident. Research questions: 1. How does obesity affect patient mobility after CVA? 2. How does obesity affect the hygienic care of patients after CVA? 3. How does obesity affect normal daily activities of CVA patients? 4. What is the impact of obesity on developing self-sufficiency in CVA patients? Quality research was done through semi-structured interviews with CVA patients who were hospitalized in the aftercare department. Other interviews were conducted with general nurses providing nursing care to CVA patients. Research group The research group consisted of 11 general nurses working at different follow-up care hospital stations, and 16 patients who were hospitalized at these departments after the CVA. Results By analyzing the information obtained, it has been determined that obesity affects the patients mobility especially after the cerebrovascular event. At the same time, it represents a physical and psychological burden for nursing staff, but also for patients. The problem has been the fact, that most patients do not perceive obesity as a condition affecting their whole life. Conclusion The diploma thesis has provided insight into the problematic of the influence of obesity on the follow-up care in patients after cerebrovascular accident. The findings should lead to a reflection on the issue of prevention of obesity in terms of both CVA risk and follow-up care, but also the need for educational activities.

In my thesis we devote to problems of patients with the implanted system HeartMate II, which is now approved as a support for an indication to bridging the time to heart transplantation and for a destination therapy for patients with a refractory heart failure. We also devote to the access of nurses to these patients. The theoretical part of the thesis is generally devoted to mechanical cardiac supports and indications to their introduction. Next part describes HeartMate II, specifics of the nursing care for a patient who is after implantation mechanical cardiac support HeartMate II and limitations, which follow for a patient at home. The target of the work is to: 1. Find out, what the priorities of needs of the patient with a mechanical cardiac support are in a nursing care. 2. Find out, how the patient with a mechanical cardiac support perceives himself and his future. 3. Find out possibilities of nursing interventions to a saturation of needs by priorities of patients with mechanical cardiac supports. For the qualitative part of the empirical research the method of questioning with technique a semi-structured interview with opened questions was chosen. The research group consists of four patients after the implantation of left-hand mechanical cardiac supports HeartMate II. In this part we were finding out answers to four research questions. 1. Which damaged needs are predominant by patients with mechanical cardiac supports? 2. Do patients with mechanical cardiac supports have enough information about possible complications related to the life after the implantation of a mechanical support? 3. How does the patient with a mechanical cardiac support perceive his future? 4. How does the patient with the mechanical cardiac support HeartMate II perceive himself? Then results of research were analyzed and written into schemes. In the quantitative part of the research four hypotheses were determined. 1. Interventions of nurses during a nursing care which is oriented to monitoring of vital functions are predominant over the orientation of needs that are changed by the life with a mechanical cardiac support. 2. Nurses are available to answer to patients with mechanical cardiac supports questions which are related to a service of the device HeartMate II. 3. Nurses are well informed about possible complications after the implantation of the left-hand mechanical support. 4. Nursing interventions of nurses are predominant over interventions of the other members of the team during the care of patient after the implantation of the left-hand mechanical cardiac support. The research group of the research consists of nurses, who are working at the department with beds, the intensive care unit of clinic of a cardiovascular surgery and at the resuscitation department of an anaesthesiology and resuscitation in IKEM. 95 respondents were addressed. The returnability of questionnaires was 69% from the beginning number. On the basis of the result of the research we think that targets were fulfilled. Patients have enough information about complications related with the life with the mechanical cardiac support HeartMate II. Patients consider a satisfaction of psychical needs, especially the feeling of a safety and a security, as a priority, although they are highly specific problems. In this view a psychotherapeutic access and a therapeutic communication of a nurse with a patient are important. We are going to hand results of the research over to the management of IKEM. A thematic schedule of a tutorial was created. This tutorial engaged in psychic needs of a patient, a psychotherapeutic access of nurses and a therapeutic communication with a patient.

A presença do médico dentista na equipe multidisciplinar da Unidade de Terapia Intensiva é um assunto relativamente novo. O presente trabalho tem como objetivo pontuar os riscos que as infecções bucais podem trazer aos doentes imunocomprometidos internados na Unidade de Terapia Intensiva, e sua co-relação com doenças sistêmicas e pneumonia nosocomial. Tambem foram destacadas medidas preventivas e de manutenção da saúde bucal. Após a conclusão da revisão bibliografica verificou-se a necessidade da criação de um protocolo de atendimento e uma correta aplicação do mesmo por um profissional capacitado.
The presence of the dentist in the multidisciplinary team of the Intensive Care Unit is a relatively new subject. The present study aims to assess the risks that oral infections can bring to immunocompromised patients admitted to the Intensive Care Unit and their correlation with systemic diseases and nosocomial pneumonia. Preventive measures and maintenance of oral health were also highlighted. After the conclusion of the bibliographical review, it was verified the need to create a care protocol and a correct application of it by a trained professional.

This thesis deals with the issue of placing toddlers in day-care centers. It takes into account three main topics. The point of view of the psychological needs of children, the point of toddlers' inclusion into day-care centers from the perspective of the centers themselves and the aspect of the family status in the Czech Republic and economic needs of the state. The aim of this thesis is to describe the suitability of putting toddlers into day-care centers from a theoretical point of view and to assess the convenience of putting toddlers into day-care centers in the Czech Republic.

Abstract:
This master's thesis deals with the possibilities of employment of direct care workers in day care centers. The thesis is divided into two parts. The theoretical part provides a primary insight into the system of social services with the specification of day care centers as social care services according to Act No. 108/2006 Coll., On social services. It also discusses the range of professions that can be employed in day care centers based on the Czech Republic's legal system. The practical part presents quantitative research which had an aim to find out the possibilities of employment of direct care workers in day care centers in a selected region, the capital city of the Prague region. The research monitors the professional proportionality of multidisciplinary teams concerning the level and field of education and employee participation in activities carried out in day care centers. Based on the research results, the last part of the work proposes recommendations for practices. There is a demonstration of work using the staging method and applying the principles of experiential learning. The demonstration's intention is the development of functional communication and the practice of establishing social contacts for clients with an autism spectrum disorder. Abstract:
The aim of this diploma thesis was to map a current range of social services for users with multiple disabilities over 18 years from České Budějovice and the surrounding area and also to find an interest in individual types of social services from a perspective of their legal representatives. The theoretical part deals with a concept of the combined disability and its basic division. Further it devotes in detail to social services that are determined for clients with multiple disabilities over 18 years. A part of the theoretical there is also a list of organizations currently providing social services for this target group in České Budějovice and surroundings. A conclusion of the theoretical part devotes to a community planning, its history and present days in the South Bohemian region as well as directly in the town of České Budějovice. The practical part has been processed by a quantitative research method, specifically in a form of an anonymous questionnaire survey and it provides a comprehensive information on a fact, which social services are currently used by the legal representatives of the users with multiple disabilities over 18 years from České Budějovice, what services they are in future interested in and how they would imagine their functioning. This part also summarizes information how the legal representatives of users with combined disabilities over 18 years participate in an establishment of new social services and it maps their awareness of the community planning. The conclusions of the diploma thesis should serve as a feedback to organizations providing social services for users with multiple disabilities over 18 years. Furthermore, they could be used as a source for the community plan and to help for an extension of the offer of provided social services in České Budějovice.

Abstract:
Master's project identifies, describes, analyses and provides sugestions of solving ethical problems whom social worker is dealing with when working and looking after clients in terminal phase of their life. Master's project is divided into 4 chapters. First chapter is focusing on explanation of terms like dying, aging, nursing home and social worker. Second chapter is about explanation of chosen ethical terms with whom social worker is often dealing with. Third chapter is focusing on each of all areas where problems may occur for social worker during his work, for example moving clients to the hospital, keeping their privacy during stay in a double bed room and optimizing daily schedules of dying patient. Last area of interest is divided into providing tasks like feeding, hygiene and spiritual care. Last chapter is conclusion of those problems with ideas and examples how to solve them. Moving clients in terminal stadium of their life is within competences of a nurse, however, it would be better if nursing homes would have their own doctors, who could examine and analyse each patient individualy. Dying in a double bed room with presence of another patient is uncomfortable and displeasing moment, that could be solved with spare room, where this roommate could stay during last moments of that dying patient. Food is served in accordance with schedule, which could be good to adjust to clients individual needs. Same adjustments would be good in schedule of staff, to ensure 24 hour care. Spiritual care is insufficient, and social worker is responsible for this situation and should fix it along with coopreation of various churches.

The thesis named "Opinions of opinions of contemporary mothers on the upbringing and education of children up to three years " examines and describes the services of day care centres for children up to three years in the Czech Republic in context with the family policy of our country and the Barcelona aims. Contemporary education possibilities of children up to three years in the selected states of EU in connection with their respective family policy are outlined too. The thesis also includes a research investigation of problematic questions concerning the upbringing and education of children up to three years, whose aim was to determine the concrete opinions and attitudes of the examined group of 75 mothers with children up to three years towards the family and non-family care in the region of Mladá Boleslav and its surroundings. It also examines which options of the children early care are used by mothers most often and what are the reasons behind. In the practical part of the thesis the general awareness of mothers about the options of the day care for children up to three years, including the personal attitude towards the nursery, was established. The harmonization of working and family roles represents an intensively discussed issue not only in our country, but in the whole European scale....

vi Abstract Continental subspecies of the great cormorant (Phalacrocorax carbo sinensis) is the Czech Republic's most important fish-eating predator. It causes damage to production fish, as well as sport fishermen and even after its removal from the Czech list of endangered species in 2013, it did not disappear from the spotlight of both local fishermen and researchers. This thesis evaluates the qualitative and quantitative composition of the cormorant's diet during winter 2011/2012 at two selected locations: the Žehuň pond (Kolín district) and Berounka river in the natural reserve Zábělá near Pilsen. This work studies the main components of cormorant's diet in the selected locations and evaluates potential fluctuations and changes in its composition during winter. The study also conducted the analysis of the proportion of commercially harvested valuable fish in cormorant's diet and studied its diversity of the species spectrum. The resulting data for the two sites were compared with each other and with relevant literature. Regurgitated pellets analysis, as well as analysis of diagnostic bones and fish remains located at the sleeping roosts (night roosts) were used for the diet analysis.
