La amaurosi ipsilaterale e la emiplegia controlaterale alla stenosi costituiscono i sintomi patognomonici della malattia. Sono i deficit neurologici transitori emisferici, recenti e subentranti quelli che più frequentemente preludono all'ictus. L'esame Eco-color Doppler consente lo screening più affidabile e la tromboendoarterectomia carotidea costituisce la miglior profilassi dell'ictus nel paziente sintomatico.

Due to the high incidence carotid artery disease and cerebral ischaemia, accurate and timely diagnosis and treatment is required to reduce the significant morbidity and mortality associated with these conditions. This chapter discusses the epidemiology, pathophysiology, clinical presentation, investigation, and treatment of these conditions. Cerebral ischaemia and stroke can occur due to a heterogeneous number of pathological processes. These pathological processes can affect either large or small vessels and can be broadly classified into thrombotic, embolic, or occlusive causes. The most common cause of thrombotic ischaemic is atherosclerosis. The most common cause of embolic ischaemia is cardiogenic due to atrial fibrillation followed by extracranial atherosclerosis of the aorta or carotids. Both of these causes can affect both the large and small vessels. This chapter explores the current evidence for medical, interventional, and surgical treatment options.

Risk factors for carotid artery disease include typical atherosclerotic disease risk factors, such as smoking, hyperlipidemia, and diabetes mellitus. This chapter reviews the diagnostic approach to symptomatic carotid artery disease (including stroke and transient ischemic attacks), the overall management approach for symptomatic carotid artery disease (including endovascular and open carotid endartectomy), and medical management options for asymptomatic carotid artery disease. The vast majority of carotid artery lesions are of atherosclerotic origin. Other conditions such post-radiation vasculopathy, spontaneous or traumatic carotid artery dissection, Takayashu disease, fibromuscular dysplasia or Ehlers Danlos syndrome may affect the carotid artery but account for the minority of cases. Atherosclerosis of the extracranial carotid artery is common and is typically located at the origin of the internal carotid artery. It is estimated that 10-20% of ischemic strokes are related to carotid artery disease. The main mechanism of stroke related to carotid disease is embolic; rarely, it can be hymodynamic in case of critical carotid stenosis or occlusion in the presence of insufficient collateral circulation.

Abstract:
The morphology of adult carotid sinus and atherosclerosis are major risk factors for ischemic stroke. It has been reported that the function of carotid sinus is pressure sensing and the regulation of heart rate.
