Full textTo take into account the complexity and the global nature of the notion of development, various processes will be presented in detail. We will adopt sectoral points of view while limiting ourselves to elements which have always been central to the problematic of development, as well as those which became important following the various updates and extensions of the definition of development (environnement. , spatial mobility, urbanization, food safety. . . ). On the other hand, in order to dispose of a sufficient number of indicators and factors which allow the study of development, we cannot limit ourselves to the study of a region or sub-region, to a sector, or sub-sector. Therefore we have chosen to study tunisia's experience of development from independence to the present day. In both sections of this study, the emphasis has been placed on the means by which the various phenomena taken into consideration can make development last. We have adopted a geographical development approach which has necessarily included elements of demographics, economics and sociology. .

Full textThe main objective of EPIHAM study is to identify the main drugs associated with drug induced liver injury leading to hospital admission (DILIH) in France and the event rates associated with DILIH. Three methodological approaches were defined. Principal approach is case-population. The others are case-crossover and case control analyses. Cases were identified among adult patients present in SNIIRAM database having a first hospitalization between 01/01/2010-2010/31/12/1204, the main diagnosis of which is toxic liver disease (diagnostic codes according to the International Classification of Diseases (ICD-10) K71.1, K71.2, K71.6, K71.9) orK72.0. Reference population was defined from the EGB. Index date (ID) was considered as the date of hospital admission for DILI. The dispensations of all treatments preceding the ID were studied by considering an exposure windowvarying from 7 to 60 days before ID. The most frequently found are classical: analgesics and firstly paracetamol, followed by drugs acting on digestive system (proton pump inhibitors, prokinetics, antispasmodics). The following were amoxicillinalone or combined with clavulanic acid, ibuprofen, codeine combinations, and furosemide. These results can inform health authorities, practitioners and patients about the acute hepatitis risk leading to hospitalisation associated with each of these molecules, both individually (absolute risk, attributable risk) and within a therapeutic drug family (relative risk) as well as more generally for the population and the health system (absolute number of attributable cases)

Full textAn epidemiological transition is observed in tropical areas with the growing of cardiovascular atherosclerosis diseases. The objective of this thesis was to contribute to a better knowledge of the epidemiology of cardiovascular risk factors in tropical area, particularly in Benin. An analysis of prevalence of cardiovascular risk factors in general population was led from Benin national STEPS survey. A systematic review of cardiovascular prospective cohort studies in sub-Saharan Africa (SSA) was conducted. Methodology of TAHES (Tanvè health study) cohort was described; a pilot study was implemented. These data show high frequencies of several cardiovascular risk factors in Benin; differences between urban and rural areas were observed. They underline the rarity of cardiovascular cohort studies in ASS. Besides, they support the hypothesis of cardiovascular cohort feasibility in general population in Benin

Full textThis thesis is a study of sources of agricultural growth in a Chinese province that experienced a limited demographic transition and a rather great growth of population in comparison with the supply of foods. The essentiel aim of this thesis is to analyse the relations between the population growth, the agricultural progress and the institutionnel change. The analysis is based at first on Boserup's hypothesis according to which the demographic pressure can give a stimulus to agrarien evolution in causing a more intensive utilization of soil. It attempts to examine if this hypothesis can help us to explain the agrarien evolution in this Chinese province. After that, this thesis analyses the elements which were in relation with the agrarien changes as the productivity of labor, the agricultural policy and also the institutionnel changes. Full textAprès avoir conceptualisé le surendettement, on a estimé, à partir de l'analyse de 3 enquêtes INSEE et de dossiers de surendettement, le stock de surendettés (1 à 2 millions de ménages selon le degré de difficultés rencontrées), étudié les caractéristiques des individus et analysé la dynamique du processus, entrées et sorties fréquentes du surendettement (3 millions de français ont ainsi été surendettés plus de 3 années entre 1994 et 2001).
Au total, au moins 30% des jeunes pourraient connaître un jour des difficultés financières, c'est dire le coût social d'un tel phénomène.

The strategy concentrates on modern farming as the most rapid way of resolving the agrarian accumulation problem - food and investible surplus. At most, assuming that there is sufficient administrative capacity and skilled labour to implement the strategy, it will benefit the APP and the CAS, about half of agrarian production but only a quarter of the rural population. It would take place, moreover, basically on the irrigated and fertile Pacific plain where two-thirds of the population resides. Meanwhile, the land reform itself has effectively established a middle farmer and service co-operative group which occupies over half the rural population and provides the other half of production; this group has no specific part to play in the MEDA. Depending on the circumstances, this group might either develop into a 'kulak' class dependent on the market and opposed to further agrarian reform or else collapse and migrate to the towns in the long run. But in any case, it will be responsible for the bulk of the food production in the short term (which may well last for a decade) and thus requires specific attention. To overcome this contradiction, which is already apparent, the APP would have to restore part of the role of estate agriculture in the rural economic formation, acting as a point of articulation for small and medium farmers instead of acting as a competitor to them. This articulation essentially implies subordinating them to the state rather than marginalising them, but also means shifting input and investment resources towards them in exchange for guaranteed production sold to the state. It is too early to make a definitive characterisation of the Nicaraguan agrarian reform, not only because the Sandinist Revolution is still in an initial tran-sitional stage, but also because of the current war conditions which, depending upon external political factors, could change the direction of social change. None the less, the nature of the agrarian reform project, the experience of its implementation to date, and the long-term agricultural development proposal do provide elements for some tentative conclusions. First, that the three lessons learned from other agrarian reform experiences were applied to some effect. Namely: (a) that production must be maintained throug the reform period, in order to avoid collapse of exports or falling urban living standards; (b) that the process of nationalisation should not proceed faster than the capacity of the state to assimilate the private sector; (c) that while individual land distribution should be avoided, co-operativisation should be voluntary and gradual. In this way, the collapse of productive forces as production relations are transformed was avoided. Second, that it is possible to effectively remove the control of agrarian capital over the economy without land expropriation as such, if the 'sur-rounding' mechanisms such as commercialisation, credit and inputs are used effectively. Indeed it might even be suggested that social control of the relations of exchange in agriculture are as important as ownership of the means of production, especially land.
