Abstract:
Los siliconomas, o granulomas por silicona, representan una reacción inflamatoria crónica y natural del huésped para frenar una sustancia extraña, en este caso, la silicona. Corresponden a una de las posibles complicaciones después de la rotura o fuga de gel de silicona desde implantes mamarios. Es de creciente importancia después de la problemática desencadenada con los implantes mamarios Poli Implant Prothése® (PIP), ya que la frecuencia de rotura es mayor que con las prótesis de otras fábricas, y llegaron a afectar más de 300.000 mujeres en 65 países diferentes. Se presenta el caso de una mujer de 50 años con historia de mamoplastia de aumento con implantes de gel de silicona de marca PIP, que cursó con ptosis mamaria, lipodistrofias localizadas y una masa axilar. Los hallazgos histopatológicos junto con la información clínica permitieron hacer un diagnóstico de siliconoma, resultado de la migración de silicona a partir de un implante de marca PIP con rotura extracapsular.

Abstract:
Este artículo presenta los resultados de un estudio que tuvo como objetivo identificar cómo se desarrolla la articulación intergubernamental a propósito de la experiencia de un proyecto de inversión pública (PIP), que buscaba mejorar los aprendizajes de educación básica del distrito de Julcán en el marco de la gestión educativa descentralizada. Se seleccionó el distrito de Julcán como caso, dado que cuenta con un PIP de tres años que fue diseñado como innovación para atender la problemática educativa del distrito articulando las instancias de gestión y niveles de gobierno. Los hallazgos de la investigación muestran que si bien se ha impulsado la articulación intergubernamental -específicamente, a nivel horizontal (dentro del mismo nivel de gobierno y sector)- existen muchas limitaciones para crear mecanismos y canales que posibiliten el desarrollo e institucionalización de esta articulación, principalmente entre el nivel local y regional. Localmente, la poca articulación que se ha observado se refiere básicamente al tema administrativo o al financiamiento. No obstante, el estudio realizado muestra algunos indicios que ayudan a definir cuáles son estas posibilidades de articulación a partir de la identificación de los nudos críticos.

Abstract:
So far, diagnosing on candidiasis has still been limited in standard blood culture. The traditional method of this microbiology culture is less sensitive, many patients with candidasis infection have negative blood culture, and need a long time. This case encourage many researchers to use another alternative methode to diagnose. The purpose of this research is to examine the potency of Candida albicans protein extract as bioreseptor on immunocencor to detect Candida albicans and its biofilm in the blood of candidiasis patients. The research method which is used is descriptive with the following steps : (1) isolating enzim from the liquid of digestive gland of snail (Achatina fulica); (2) extraction of protein candida albicans through enzimatis and mechanic methods and (3) Analyzing the protein extract as bioreseptor through immunodot assay.The research result shows that the snail enzyme has the protein content 1.35 mg/ml and the specific activity 1.96 unit/mg. This snail enzyme can hydrolyze the cell wall of Candidia albicans and, with the help oh sonication, produce extract of planktonic extracel protein (PEP) and biofilm (PEB), extract of planktonic intracell protein (PIP), and biofilm (PIB), with each protein content 1.44; 1.29; 1.29 and 1.21 mg/ml. The characeristic of biofilm intracell protein (PIB) is antigenic on antibody anti-candida (positive control) with giving red spot on immunodot assay. Immunodot assay can distinguish negative control serum (health man) and positive Candidiasis control by using antigen 1 ng/nl and 50nl serum.

Abstract:
ABSTRACT The in vitro activities of piperacillin (PIP) against β-lactamase-negative ampicillin (AMP)-resistant (BLNAR) Haemophilus influenzae were compared with those of cefotaxime (CTX) and ceftriaxone (CRO), and the potency of PIP as therapy for meningitis caused by BLNAR is also discussed. PIP showed good activity (MIC at which 90% of strains are inhibited, 0.25 μg/ml) against 69 BLNAR strains, and its activity was comparable to that of CRO and superior to that of CTX. No significant correlation was observed between the MICs of PIP and CTX or CRO or AMP, whereas a high correlation was observed between the MICs of CTX and CRO. In the killing study, PIP showed potent bactericidal activity compared with those of CTX and CRO. By microscopic examination, PIP caused the formation of a spindle and short filamentous cells with bulges and induced cell lysis in BLNAR strains, while treatment with CTX and CRO resulted in the formation of large, spherical cells without any obvious lysis. The affinity of Bocillin FL, a fluorescent penicillin used for determination of the 50% inhibitory concentration (IC50s) for penicillin-binding proteins (PBPs), to PBPs 3a and 3b of BLNAR strains was drastically decreased compared with that to an AMP-susceptible strain (ATCC 33391). In the case of the BLNAR strains, the IC50s for PBPs 1a, 1b, and 2 were similar to those for the PBPs of ATCC 33391. Since the affinity of binding to PBPs 3a and 3b of the BLNAR strains decreased drastically, the second targets among the PBPs were PBP 2 for PIP, PBP1 (1a and 1b) for CTX and CRO. In conclusion, PIP showed excellent activities against BLNAR strains in a manner different from those of cephem antibiotics, suggesting that it could be a candidate therapeutic agent for the treatment of meningitis caused by BLNAR strains.

Abstract:
Protein folding problem (PFP) consists of determining the functional three-dimensional structure of a target protein. PFP is an optimization problem where the objective is to find the structure with the lowest Gibbs free energy. It is significant to solve PFP for use in medical and pharmaceutical applications. Hybrid simulated annealing algorithms (HSA) use a kind of simulated annealing or Monte Carlo method, and they are among the most efficient for PFP. The instances of PFP can be classified as follows: (a) Proteins with a large number of amino acids and (b) peptides with a small number of amino acids. Several HSA have been positively applied for the first case, where I-Tasser has been one of the most successful in the CASP competition. PEP-FOLD3 and golden ratio simulated annealing (GRSA) are also two of these algorithms successfully applied to peptides. This paper presents an enhanced golden simulated annealing (GRSA2) where soft perturbations (collision operators), named “on-wall ineffective collision” and “intermolecular ineffective collision”, are applied to generate new solutions in the metropolis cycle. GRSA2 is tested with a dataset for peptides previously proposed, and a comparison with PEP-FOLD3 and I-Tasser is presented. According to the experimentation, GRSA2 has an equivalent performance to those algorithms.

A paralisia facial periférica (PFP) se caracteriza pela interrupção do estímulo neuronal dos ramos do VII par craniano devido a diversos fatores etiológicos que acometem de forma aguda a informação motora total ou parcial da musculatura facial. A PFP pode estar relacionada a processos infecciosos, traumáticos, neoplásicos, autoimunes, metabólicos e iatrogênicos. O objetivo desse artigo é relatar o caso clínico de um paciente de 41 anos de idade, leucoderma, sexo masculino, portador de PFP, compatível com Síndrome de Ramsay Hunt tipo II. O tratamento foi o farmacológico associado à tera- pia LED de baixa intensidade (640±20nm), aplicada em todo trajeto do nervo facial e seus ramos, por 30 segundos, com densidade de energia de 1,7 J/cm2, energia de 3 J e potência de 100 mW. Após a primeira sessão, houve melhora expressiva no quadro clínico, promovendo aumento do tônus muscular e maior contratura da mímica fa- cial, como franzir a testa, fazer bico, sorrir e fechar completamente o olho. Durante 15 dias, foram realizadas cinco aplicações, resultando na ausência de qualquer sinal ou sintoma referente à paralisia. Concluiu-se que a terapia LED de baixa intensidade foi eficaz no tratamento de PFP, com o protocolo testado.

Abstract:
This study used the Source Loading and Management Model for Windows (WinSLAMM) to develop a set of calibrated hydrologic models for three types of regional permeable pavements—porous concrete pavement (PCP), permeable interlocking concrete pavement (PICP), and interlocking block pavement with gravel (IBPG). The objective was to assess the hydrologic performance of permeable pavements, including the runoff depth, peak discharge, percentage increment in runoff reduction of pavements as a function of rainfall depth, development area, and base aggregate porosity, respectively. The permeable pavements were monitored over a wide range of rainfall events in the semi-arid Lower Rio Grande Valley of South Texas. Data regarding rainfall intensities, source characterizations, runoff coefficients, and pavement design were initialized as WinSLAMM input. Validation results showed that the calibrated models could over or under-predict runoff reduction within a 30% error range. PCP and IBPG were very effective and could be capable of handling storms as large as 50-year frequency over a 24-h time period. The modeling results showed that PCP might require a 50–60% lesser footprint area as compared to PICP and IBPG, respectively. Additionally, PCP might be able to store 30% additional runoff if the porosity of base aggregates was increased by 40%.

El presente trabajo consiste en la aplicación de la técnica de análisis adimensional utilizando el teorema p de Buckingham al estudio del desgaste de herramientas de acero rápido sinterizado, acero rápido, de metal duro widia e inserto de carburo de tungsteno, realizando operaciones de cilindrado. Las pruebas se llevaron a cabo en talleres industriales de Bogotá y Barrancabermeja, utilizando diferentes materiales para la pieza de trabajo y variadas condiciones de corte para la herramienta hasta llegar a desgastarla. La información recopilada permitió graficar la relación log-log entre dos parámetros adimensionales (PI1, PI2). Los datos de corte mostraron convergencia en torno a zonas particulares que facilitaron hacer las comparaciones entre herramientas. La comparación experimental, basada en los parámetros adimensionales PI1 y PI2, demostró que es posible elaborar una norma Colombiana para evaluar herramientas en variadas condiciones de operación.

Symphalangism is a rare condition which manifests in either PIP or DIP joint congenital fusion. Symphalangism may be with some other skeletal deformities. In our case, all PIP joints of both hands were fused with bilateral hypoplasia of carpal bones and Minaar type III lunatotriquetral coalition congenitally. No motion was detected in PIP joints with absence of cutaneous creases over all PIP joints. Radiologically, carpal hypoplasia and lunatotriquetral coalition were seen in all fingers with the absence of PIP joints. The patient's skeletal survey revealed no other pathology. He had no complaints related to his hands. So, regular follow-up was recommended. This pathology was presented as rarely observed although clinical problem is usually not so much.

Introduction: In recent years, practice friendly models of suicidal behavior have emerged in the form of “ideation-to-action” frameworks. These frameworks focus on processes influencing both the formation of suicidal ideation and the translation of that ideation to suicide attempt and death. In this paper, we proffer an emerging model of suicide, Preferences in Information Processing (PIP), to augment contemporary suicide theories. First, we provide a primer on dual-process models of information processing theory and research as the foundation for the PIP. Next, drawing on a number of initial cross-sectional studies, we outline a rationale and examples of how preferences in motivated affect (i.e., Need for Affect) and cognition (i.e., Need for Cognition) may be integrated into existing ideation-to-action frameworks. Methods: We conducted secondary analysis of our pooled community sample PIP data. Results: We present new findings suggesting Need for Affect avoidance and Need for Cognition may be clinically relevant for persons at escalated risk for suicide. Discussion: The PIP offers new testable propositions within ideation-to-action suicide frameworks. We end with recommendations for a research agenda to further investigate the potential validation and utility of a PIP approach to suicidology.

The result of this reaearch found that the implementation of Program Indonesia Pintar (PIP) in Rembang Regency has been done well with proof that it has been distributed to all students whose parents can pay the cost of education so that their children can go to school up to high school. All the steps of Program Indonesia Pintar (PIP) has been done starts from socialization, opinion, verification, and the distribution of the cash money which person requires or not, it's because of finance constraint. The obstacle is the distribution of this program to people it's because people who can pay for the cost of education of their children so they can go to up to high school, that means people can not use the cash money for anything but educational stuff, or for families who do not actually need and deserve the money from Program Indonesia Pintar (PIP) do not have the chance to apply for thisaid. Program yang sedang dijalankan oleh pemerintah saat ini yaitu salah satunya Progam Indonesia Pintar (PIP) merupakan program yang diharapkan dapat mengurai permasalahan perekonomian terkait dengan bidang pendidikan. Semua tahapan Progam Indonesia Pintar (PIP)sudah dilaksanakan mulai dari sosialisasi, usulan, verifikasi hingga penyaluran danaProgam Indonesia Pintar (PIP). Kendala yang ditemukan seperti belum dapat tersalurkannya dana ke masyarakat yang memang benar-benar membutuhkan atau salah dalam penyaluran kepada masyarakat yang sebenarnya tidak berhak, hal ini disebabkan karena kendala administrasi. Adapun saran dari penelitian ini diantaranya bahwa masyarakat hendaknya memaknai program ini sebagai salah satu upaya pemerintah dalam memberikan bantuan pendidikan kepada siswa hingga jenjang menengah, sehingga tidak mengambil keuntungan dari program ini untuk kepentingan-kepentingan yang diluar keperluan sekolah, atau bahkan bagi keluarga yang tidak seharunya menerima bantuan dari Progam Indonesia Pintar (PIP) tidak berupaya untuk mengajukan bantuan.

Abstract:
IntroductionReducible open dorsal dislocation of the single finger's PIP joint is an infrequent case of orthopedic emergencies. The severity of this injury may be underestimated. These injuries are associated with long-term complications such as synovitis, stiffness, degenerative arthrosis, septic arthritis, and loss of the digit if suboptimally treated.CaseA 90-year-old male came to the emergency department with an open dorsal PIP dislocation due to a fall on his right hand. There was a transverse skin laceration just proximal to the PIP flexion crease of his small and ring fingers. The condyles of his small finger's proximal phalanx protruded through the wound. X-rays showed a dorsal dislocation of the PIP joint without fracture. There was no neurovascular injury determined. The proximal phalanx was hyperextended slightly with gentle axial traction. After irrigation, the skin wound was closed primarily without repair of damaged structures, and systemic antibiotherapy was performed for a week. The PIP joint was immobilized for three weeks by applying the splint dorsally with the joint in 20 degrees of flexion. Active range of motion exercises were then implemented, and the patient regained full digital flexion with only a 10 degree loss of extension within eight weeks.DiscussionForced hyperextension with axial compression causes a dorsal dislocation of the PIP joint. Dorsal PIP dislocations are more common than volar IP dislocation. Open dorsal dislocation of the PIP joint is an infrequent case of orthopedic emergencies. Such injuries can be treated safely in the emergency department by closed reduction and extensor splinting until a definite treatment plan is made by an orthopedic hand surgeon. The complications must be taken into consideration. Early active motion leads to significantly superior results in the range of motion than static splinting, because prolonged immobilization may result in flexion contracture.

<p>Although the government has allocated cash transfer programs to reduce poverty and improve the education sector, the participation of senior high schools in rural areas in Indonesia is still meager. This research investigates the relationship between conditional cash transfers in Indonesia (Program Indonesia Pintar–PIP and Program Keluarga Harapan–PKH) with high school participation in children in Rural Indonesia. Using the 2018 socioeconomic survey data released by BPS, we found that children benefited from PIP children had a greater opportunity to participate in high school compared to those who did not. On the other hand, children from PKH beneficiary have a lower chance of attending high school, compared to those who come from families who do not receive PKH. Although conditional cash transfers generally have a positive effect on children's school participation, the success of conditional cash transfers relies on the knowledge of the people about the importance of children's education investments, simple program schemes, secure payment schemes, and efficient and effective technology-based controls.</p>

We present a case of a 21-year-old male patient who developed an osteochondroma at the neck of the proximal phalanx of the left little finger, which interfered with reduction of a dorsally dislocated PIP joint. This is the first such case report in the literature. At the time of surgery, we excised the osteochondroma and reconstructed the collateral ligament that produced locking of the PIP joint. This treatment brought a quick and essentially complete recovery of the PIP joint function. At 3 years follow-up in the affected digit, the patient has no limitations in daily activities.

Based on the significant findings reported in the previous paper on academic progress and behavioral changes documented in annual evaluations, further study of the Partners in Prevention (PIP) Program was conducted. The PIP Program was compared to a school counseling approach through a quasi-experiment, which looked at how each approach influences change in academic achievement, the behaviors that present as barriers to academic success and the students', parents', teachers', and practitioners' perceptions of progress on intervention goals. While the PIP Program was found to have greater influence than the school counseling approach on overall grades and reading averages and on students' perception of their improvement on targeted goals, there were no differences between the two approaches in terms of improvements in behavior.

Pain or dysfunction of the finger joints due to arthritis or traumatic injuries that fail medical management may necessitate arthroplasty or joint replacement. The goals of the finger joint implant arthroplasty are to relieve pain, to correct deformity, and to improve the function and appearance of the hand. Several prosthetic implants have been used for the replacement of the proximal interphalangeal (PIP) joint. Pyrocarbon materials, a form of pyrolytic carbon, a ceramic-like material, have proven to be strong, durable, resistant to wear and nonreactive in the body. The Ascension PIP pyrocarbon total joint is a bicondylar, anatomically shaped, articulating implant that allows joint flexion–extension, while providing some restriction of adduction–abduction motion. A review of the literature of pyrocarbon PIP prosthesis reveals little clinical data. The case of a 33-year-old man with posttraumatic arthritis proximal interphalangeal joint right long finger is reported. The case was treated surgically using the Ascension PIP total joint. During the insertion of the implant, the proximal component fractured at the sub-articular collar. The component was removed, and a new implant was inserted without complication. Critical evaluation revealed that there was an inadequate resection of the volar lip resulting in a stress riser on the implant during impaction. Careful attention to this technical point will hopefully minimize the occurrence of this complication as the availability and use of these implants increases.

Polyarticular psoriatic arthritis is a chronic, progressive and disabling auto-immune disease often affecting the small joints of the hands in a symmetrical fashion. The disease can progress rapidly causing joint swelling and damaging cartilage and bone around the joints resulting in severe deformities. We report a very unusual case of a 49-year-old woman who presented with polyarticular psoriatic arthritis affecting all proximal interphalangeal (PIP) joints of both hands except the left ring finger PIP joint. On clinical examination there was no evidence of arthritis in the left ring finger PIP joint. We confirmed the paucity of joint damage in the PIP joint of the left ring finger using more modern imaging modalities such as musculoskeletal ultrasound and MRI scan of the small joints of the hands. All other PIP joints in both hands demonstrated advanced degrees of joint damage secondary to chronic psoriatic inflammatory arthritis. We postulated that wearing a gold wedding ring has helped protecting the PIP joint of the left ring finger from the damaging effect of inflammatory arthritis. The possible mechanisms by which metal jewellery (gold ring) confer protection to adjacent joints was discussed.

Cross-linking of the immunoglobulin E receptor on rat basophilic leukemia (RBL)1 cells by multivalent antigen activates phosphatidylinositol (PI) kinase and phosphatidylinositol 4-phosphate (PIP) kinase leading to the increased production of PIP and phosphatidylinositol 4,5-bisphosphate (PIP2). Activators of protein kinase C (PKC), such as phorbol myristate acetate (PMA) and the synthetic diacylglycerol, 1,2-dioctanoyl-sn-glycerol (diC8), were found to have the same effect even though PMA and diC8 do not cause the activation of phospholipase C. Although the kinetics are different depending on the stimulant, activation of PKC using multivalent antigen, PMA or diC8 also causes the polymerization of actin and an increase in the F-actin content of the cells. In all cases, a good correlation was observed between F-actin levels, activation of PI and PIP kinases, and the increased production of PIP and PIP2. However, in the case of antigen, there is no correlation between actin polymerization and the total amount of PIP and PIP2. Staurosporine, an inhibitor of protein kinases, blocks the F-actin response and the increased synthesis of PIP and PIP2 with similar dose dependencies. Furthermore, depletion of PKC activity through long-term exposure to PMA, inhibited both the F-actin response and the increased synthesis of PIP and PIP2 induced by either DNP-BSA or diC8. These results suggest that activation of PKC precedes the activation of PI and PIP kinases and that under certain circumstances activation of the kinases and the increased synthesis of PIP and PIP2 may be involved in the polymerization of actin in RBL cells, possibly through the interaction of the polyphosphoinositides with actin-binding proteins such as gelsolin and profilin.

We describe 4 cases of irreducible volar rotatory subluxation of the proximal interphalangeal (PIP) joint of the finger that required open reduction. All of the patients had radiographically proven (in lateral-view radiographs) volar rotatory subluxation of the PIP joint, without fracture. The causes of irreducibility were interposition of the lateral band about the condyle of the middle phalanx in 2 cases, interposition of the collateral ligament in 1 case, and scarring of the injured central slip in 1 case. Rupture of the collateral ligament of one side was found in all cases. Acceptable results were provided with all cases after restoration of the collateral ligaments and the damaged parts. Accurate early diagnosis by careful physical examination and obtaining true lateral radiographs of the PIP joint is important.

Radiographs showed volar dislocation of the DIP joint and dorsal lip fracture of the middle phalanx with volar subluxation of PIP joint of the little finger. This case was unique in terms of the mechanism of injury which was hyperflexion type in two adjacent joints of the same finger. The patient was treated by closed reduction of DIP joint dislocation and open reduction and internal fixation of the PIP joint fracture-subluxation and application of dorsal external fixator due to instability. Finally, full flexion of the PIP joint and full extension of the DIP joint were obtained but with 10 degree extension lag at the PIP joint and DIP joint flexion ranging from 0 degree to 30 degrees. Some loss of motion in small joints of the fingers after hyperflexion injuries should be expected.

OBJECTIVE: Divergent findings among prior studies on correlates of risk for postictal psychosis (PIP) suggest the value of a controlled study involving a relatively large number of patients. METHODS: The study population consisted of a consecutive series of 59 patients with partial epilepsy and a history of PIP, and 94 control patients with partial epilepsy and no history of PIP evaluated as inpatients with video-electroencephalography. The groups did not differ significantly regarding demographic features. Exact tests yielded a subset of variables and a tentative interpretation that were evaluated further utilizing principal components analysis and logistic regression. RESULTS: PIP was associated with extratemporal versus temporal ( p = 0.036) or undetermined ( p = 0.001) localization of seizure onset, bilateral interictal epileptiform activity ( p = 0.017), secondary generalization ( p = 0.049), and history of encephalitis ( p = 0.018). Interictal slow activity was more frequently absent in control patients ( p = 0.045). PIP was associated with family histories of psychiatric disorders ( p = 0.007) and epilepsy ( p = 0.042), which themselves were significantly intercorrelated (r 0.225; p = 0.006). Age of onset or duration of epilepsy and lateralized electroencephalographic or magnetic resonance imaging asymmetries did not differ significantly between control and PIP groups. The analysis indicated four underlying domains of risk for PIP: ambiguous/extratemporal localization, family neuropsychiatric history, abnormal interictal electroencephalographic activity, and encephalitis. Each unit increase on a simple additive scale composed of 9 dichotomous independent variables multiplied the odds ratio for PIP by 1.71 (95% confidence interval, 1.36–2.15; p < 0.0001). INTERPRETATION: PIP in partial epilepsy is associated with relatively broadly and bilaterally distributed epileptogenic networks, genetic determinants of psychiatric disorders and seizures, and encephalitis.

Background: Proximal interphalangeal (PIP) fracture dislocations remain a complex injury pattern to treat. There are several treatment methods available aimed to restore stability, preserve range of motion, and reconstitute the articular surface. This study looked at the mid-term clinical and radiographic results of open reduction internal fixation through a shotgun approach of comminuted PIP fracture dislocations. Methods: A retrospective review was conducted of all PIP fracture dislocations treated through a volar, shotgun approach at a single institution over a 15-year period. Patients identified were contacted and asked to return to the office for clinical and radiographic evaluation. Patient reported outcomes were assessed with the Michigan hand questionnaire (MHQ) and visual analog scale (VAS) for pain. Results: 5 patients returned to the office for further evaluation with average follow-up of 69 months (range, 33-133 months). 3 patients were found to have post traumatic arthritis on radiographs. 1 case had recurrent instability and one case had a deep infection, both necessitating further surgical intervention. Average PIP arc of motion was found to be 79°. Average VAS score of 0 and MHQ result of 95 (out of a possible score of 100) indicating no residual pain and excellent functionality of the affected hand. Conclusion: Open reduction internal fixation of comminuted PIP fracture dislocations utilizing the volar, shotgun approach provides excellent mid-term functional results despite the high incidence of post traumatic arthritis.

This article discusses the role literature plays in shaping 'home' as an ideological site of the spatial imaginary by drawing on Lloyd Jones' novel Mister Pip (2006) as a case study. Special emphasis is placed on analysing how ideals of home are rooted in 'cultures of subjectivity' (Reckwitz 2012). I systematically tease out two paradigms of home in Mister Pip and comment on their social implications within a postcolonial context. As I show, literature features as the privileged model of home in Mister Pip. This ideal of home is connected to a specific form of subjectivity, namely the 'creative self' (Reckwitz 2012). By shifting my attention to issues of reader response, I argue that Mister Pip 'trains' its readers in practices of the creative self, thereby contributing to a specific form of homemaking. I conclude with a critical assessment of Jones' literary achievement in the light of postcolonial power relations.

Background: This is a retrospective case series investigating the outcomes using a dynamic external fixator (DEF) for treatment on severe flexion contractures at the proximal interphalangeal (PIP) joint. Severe flexion contractures of the PIP joint occurring after multiple operations and neglected over a long period of time are difficult to treat. The recurrence of contracture, even after successful treatment, is inevitable in patients with severe cases. In this study, we defined the severity of PIP joint contracture based on the active range of motion (ROM), soft tissue condition, and duration of the contracture. We also illustrated the strategy, results, and complications of using a DEF with rubber bands in these severe cases. Methods: We studied 11 fingers of 10 patients with PIP joint contracture treated by DEF. These were fixed at a small arc and neglected for an average 4.1 years (range, 1–9 years). The temporal Kirshner wire (K-wire) fixation after achieving an extension via DEF was maintained for 9.1 weeks on average. We retrospectively reviewed the results of these patients with an average 2-year follow-up. Results: Our method yielded favorable results upon retrospective evaluation. The average active ROM of the affected PIP joint improved from 90/96° to 34/83° with a functional arc and good patient satisfaction. The elastic force induced by strong rubber bands was safe and effective. The first step of joint space widening was the key to obtaining a successful joint extension afterwards. Serious progression of osteoarthritis at the PIP joint and pin-site fracture were a complication in each one case. Conclusions: In this study, we evaluate the surgical strategy of using DEFs powered by elastic torque from rubber bands to treat severe cases of flexion contractures of fingers. We first created extension contracture intentionally, followed by promoting flexion movement during follow-up in this group of patients.

Purpose: To identify potentially inappropriate prescribing in ambulatory elderly patients and compare the appropriateness of guidelines; Beers' and Screening Tool of Older Person’s Prescription (STOPP)/Screening Tool to Alert Right Treatment (START) criteria to detect potentially inappropriate prescribing among the elderly.Methods: A retrospective study was conducted using case files of 335 elderly patients aged ≥ 60 years between 1st January and 31st December 2016, using a data extraction sheet. The 2015 American Geriatrics Society (AGS)-Beers Criteria, and version 2 of the STOPP and START were subsequently used to identify the Potentially Inappropriate Prescribing (PIP) and Potential Prescribing Omissions (PPOs).Results: Mean age of patients was 69 ± 0.4 years (range 60 - 85 years) and 219 (65.4 %) were females. An average of 4.2 medications per patient prescription was found. The Beers criteria identified 26.5 % PIP, while STOPP criteria identified 57.1 % PIP. START detected 29 PPOs in 15 (4.4 %) of the patient’s prescription. The most prevalent disease conditions were hypertension 235 (70.1 %) and osteoarthritis 64 (19.3 %). Polypharmacy was significantly associated with PIP in both Beers (p = 0.002) and STOPP (p = 0.001) criteria.Conclusion: The prevalence of PIP is high among elderly patients. The STOPP/START criteria identified a higher proportion of PIP among elderly patients compared with Beers criteria. The frequency of PIP should stimulate efforts to curtail potentially inappropriate prescribing and may require the need for advocating for a national criterion to be adopted by health care professionals in Nigeria. Context Long-term viability of wildlife populations may be influenced by the adaptive or maladaptive nature of behavioural shifts. Yet, in the short term, implications of novel behaviour are often uncertain, as they were for a newly formed urban nesting colony of pied imperial-pigeons (PIPs) on the mainland coast of north-eastern Australia. It represented unprecedented behaviour, as most of PIPs, also known as Torresian imperial-pigeons, Ducula bicolor/spilorrhoa, breed colonially on remote small islands. Aims The present study would (1) determine whether aggregated mainland nesting continued, (2) evaluate reproductive success, (3) evaluate spatial distribution of nests and (4) explore possible association of reproductive success with predator presence and broad indicators of food availability. Methods With assistance from volunteers, we found mainland PIP nests and revisited them intermittently to monitor progress. We calculated quantitative estimates of nest survival by the Mayfield method and evaluated reproductive output in relation to environmental data from independent sources. Key results During the 2012, 2013 and 2014 breeding seasons, we recorded 436, 387 and 417 PIP nest events at the new mainland colony. Daily nest-survival rates declined progressively and estimated fledgling output decreased over successive seasons from 0.66 to 0.44 per nest event. The highest mainland output was below that estimated for an island PIP colony (0.78) on the basis of sparse prior data. Across potential foraging grounds, there was no negative change in land use and no widespread adverse weather to account for diminishing success. We identified important causes of nest failure among mainland nesting PIPs to be predation, predominantly by birds, and anthropogenic hazards, including tree pruning and collisions with vehicles and windows of buildings. Conclusions With ongoing exposure to these hazards, mainland nesting PIPs cannot be expected to increase productivity; hence, the new colony may be a short-term phenomenon. We infer that the historic success of PIP populations in Queensland stems from their selection of breeding sites on remote islands that are largely free of relevant predators and anthropogenic activity. Implications Future conservation of extant PIP abundance will depend crucially on protection of island breeding sites, because multiple hazards of mainland nesting make it an unfavourable alternative strategy.

Purpose This paper aims to evaluate employee’s perception regarding information pollution and determined the factors that lead to perceived infollution. In the case of this study, a four-dimensional scale of perceived infollution is presented. In addition, this study quantified information pollution in contrast to using the measurement tools of information quality. Design/methodology/approach A sequential exploratory mixed-method design was used to validate the measurement scale. The population of the present study comprised of the employees who work in the operations and credit department of banking sector. In this study, a four-dimensional second-order scale of perceived information pollution with a total of 19 items or sub-dimensions managed to be developed using exploratory factor analysis and confirmatory factor analysis. Findings The measurement scale confirmed that perceived information pollution in the context of workplace environment consisted of four dimensions, namely, intrinsic PIP, accessible PIP, contextual PIP and representational PIP where PIP stands for Perceived Information Pollution. Research limitations/implications Management may use the four dimensions as a benchmark in revealing polluted information as well as enhancing information quality through information processing. Originality/value This is the first attempt of exploring the dimensions and validating the measurement scale of perceived infollution.

We report a rare case of open dislocation of the proximal interphalangeal (PIP) joint, associated with simultaneous avulsion of the central band and distal insertion of the extensor mechanism, rapture of the proximal volar plate, and rapture of the ulnar collateral ligament of the PIP joint. Although isolated IP joint injuries are common, they rarely occur simultaneously in a single finger. Even more rarely are they accompanied by a complete avulsion of the extensor mechanism. Operative restoration of the injured structures is a necessity in order to achieve a good functional outcome. An initial forceful flexion of the finger resulting to rapture of the extensor apparatus and then followed by finger hyperextension and PIP joint dislocation is our perception of the possible mechanism of this extremely rare injury.
