The Institute of Medicine (IOM) reporting on the quality of health care in America recommends six aims for achieving the health care system we could have. Together with the Institute for Healthcare Improvement (IHI) Triple Aim initiative, a framework has emerged to challenge providers, educators, and policymakers to remake the health care system according to specific objectives: to provide care that is safe, effective, patient-centered, timely, efficient, and equitable to more people at a price we can afford. Complicating this mission of better prevention and better care at a lower cost is a daunting demographic: January of 2011 marked the month and year that the first of the baby boomers turned 65. The U.S. Census Bureau in May 2010 projected the number of Americans of this age and over to reach 88 million by 2050, more than double the current figure of 40.2 million. Parekh and Barton forecast in stark detail what it will be like to address these burgeoning numbers of older Americans with comorbidities, including the fact that over 20% of the population currently experiences at least two chronic medical conditions.

Abstract:
Jean-Emmanuel Gilibert (1741–1814) was a French botanist, physician, and politician who lived in the Polish-Lithuanian Commonwealth between 1775 and 1783, where he organized studies on medicine and natural sciences. During this time, he collected local plants, and in 1781–1782, published a two-volume compendium: Flora Litvanica inchoata. One hundred copies of the first volume and an unknown number of the second volume were issued. In 1925, Polish botanist Slawiński noted five libraries holding copies of Flora Litvanica. The current census shows at least 18 European libraries holding original copies of the book.

Many observers have noted that immigrants to the United States are highly concentrated in the largest metropolitan areas of a relatively few states. Though immigrants diffused into many places that had previously seen relatively few immigrants during the 1990s, as of the 2000 census, 77 percent of the nation's 31.1 million foreign born residents still lived in six states – California, New York, Texas, Florida, New Jersey, and Illinois. According to the 2000 census, the two largest metropolitan areas, Los Angeles and New York, accounted for one third of all immigrants ( http://www.census.gov/Press-Release/www/2002/demoprofiles.html ). While immigrants moved into many new areas during the 1990s, making the challenge of incorporating their children a national issue, their concentration in our largest cities remained pronounced.

This article presents arguments and data to show that the decennial census and annual Current Population Surveys include immigrants falling into four broad legal status groups: naturalized citizens; legal immigrants; legal nonimmigrants; and undocumented migrants. Since 1986, the relative rewards and penalties imposed on these four categories have shifted dramatically in response to U.S. policies, as have the relative number of foreigners in each group. In general, the relative share of foreigners in the most vulnerable status groups has increased, with the proportion of undocumented migrants and legal nonimmigrants rising and that of legal immigrants falling. Researchers using census and CPS data need to be aware of the shifting distribution of foreigners by legal status over time and of the changing profile of opportunities experienced by each status group, and they need to exercise caution in their interpretation of trends with respect to immigrant assimilation and the effects of immigration on U.S. society.

Using 1990 U.S. Census 5% PUMS and 1991 Canadian Census 3% public and 20% restricted microfiles, this article demonstrates the existence of a North American naturalization gap: immigrants living in Canada are on average much more likely to be citizens than their counterparts in the United States, and they acquire citizenship much faster than those living south of the border. Current theories explaining naturalization differences - focusing on citizenship laws, group traits or the characteristics of individual migrants - fail to explain the naturalization gap. Instead, I propose an institutional approach to citizenship acquisition. States' normative stances regarding immigrant integration (interventionist or autonomous) generate integrated or disconnected institutional configurations between government, ethnic organizations and individuals. Evidence from a case study of Portuguese immigrants living in Massachusetts and Ontario suggests that in Toronto government bureaucrats and federal policy encourage citizenship through symbolic support and instrumental aid to ethnic organizations and community leaders. In contrast, Boston area grassroots groups are expected to mobilize and aid their constituents without direct state support, resulting in lower citizenship levels.

The relevance of the research topic is due to the determining influence of social class appearance, the occupation and the previous geographical place of migrants’ residence on the further social and economic development of the populating region. On the example of the largest rural settlement of Tambov Governorate – Rasskazovo village – we show the features of state peas-ants migrations to the region of traditional agriculture (Central Black Earth Region) on the microhistorical level during the foundation and settlement by them the separate settlement in Zalesskiy stan of Tambov County in the late 17th – early 18th century. During the study we define the date and geographical location of the Rasskazovo village foundation – Lesnoy Tambov. We give brief information about the biography of its founder beekeeper S.A. Rasskaz – Vodyanov. We describe the nearest surroundings of the village with the indication of smallholders and the names of their owners. We analyze the course of home settlement of the village in the first years of its existence with the usage of data of census lists, census books and household censuses. We determine the range of settlement from which palace peasants moved to a new place of residence. We try to determine the class structure of the villagers in the first half of the 18th century. We also trace the demographic growth of the village in the period 1697–1744. We reveal a number of features of the primary (to the village) and secondary (to new places of residence) migrations of the first Rasskazovo settlers.

Abstract:
Background. Legal abortion is a challenge from the viewpoint of ethics and religion. The present study was conducted to investigate the frequency of fetal and maternal indications of legal abortion and also the maternal characteristics in Lorestan Province of Iran. Methods. As a descriptive cross-sectional study, all the cases with issued permits for legal abortion were selected by a census during 2017–2019. Descriptive data analysis was used to report the results. Event rates with Poisson 95% confidence intervals (CIs) were calculated based on the regional population. Results. A total of 305 cases were selected. The mean age of the mothers was 31.61 ± 7.48 years, and the mean of gestational age was 15.76 ± 2.80 weeks. Demographically, most cases were from Khorramabad city (101 cases) followed by Borujerd (51 cases) and Doroud (46 cases). The overall event rate was 1.732 per 10,000 individuals (95% CI: 1.543–1.938) of the general population of the region per 3 years. Fetal disturbance of the brain and spine was the most prevalent reason of abortion (24.92%, 95% CI: 19.63%–31.19%) followed by Down syndrome (19.34%, 95% CI: 14.73%–24.95), hydrops fetalis (12.79%, 95% CI: 9.09%–17.48%), and anencephaly (12.79%, 95% CI: 9.09%–17.48%). Conclusion. From each 10,000 individuals of the population, one to two cases of legal abortion were screened per 3 years. More than 90% of cases had fetal indication. In cities with lower event rates, we should plan for better screening.

Although the spatial assimilation of immigrants to the United States has important implications for social theory and social policy, few studies have explored the patterns and determinants of interneighborhood geographic mobility that lead to immigrants' residential proximity to the white, non-Hispanic majority. We explore this issue by merging data from three different sources – the Latino National Political Survey, the Panel Study of Income Dynamics, and tract-level census data – to begin unraveling causal relationships among indicators of socioeconomic, social, cultural, segmented, and spatial assimilation. Our longitudinal analysis of 700 Mexican, Puerto Rican, and Cuban immigrants followed from 1990 to 1995 finds broad support for hypotheses derived from the classical account of minority assimilation. High income, English language use, and embeddedness in Anglo social contexts increase Latino immigrants' geographic mobility into Anglo neighborhoods. U.S. citizenship and years spent in the United States are positively associated with geographic mobility into more Anglo neighborhoods, and coethnic contact is inversely associated with this form of mobility, but these associations operate largely through other predictors. Prior experiences of ethnic discrimination increase and residence in public housing decreases the likelihood that Latino immigrants will move from their origin neighborhoods, while residing in metropolitan areas with large Latino populations leads to geographic moves into “less Anglo” census tracts.

Résumé À partir de l’analyse de certains éléments du paratexte de la première traduction française d’un texte de Henry Fielding, soit Les Avantures de Joseph Andrews, et du ministre Abraham Adams de Pierre-François Guyot Desfontaines, cet article montre comment la censure, entendue dans tous les sens que ce terme prend à l’époque, influe sur la traduction et, simultanément, comment Desfontaines répond, dans le paratexte de sa traduction et dans d’autres textes, au phénomène de la censure. L’ensemble du paratexte du Joseph Andrews français semble faire appel à une série de procédés associés, au XVIIIe siècle, aux mécanismes de la censure politique et littéraire, qu’on lui donne le sens de critique ou d’interdiction d’un ouvrage. Le personnage de « La Dame Angloise » assumé par Desfontaines, critique dans ses « notes » et dans sa « Lettre » les usages décrits par Fielding et, par la même occasion, les coutumes de sa propre nation. De tels commentaires visent normalement à défendre le texte et les choix du traducteur contre les attaques des critiques et censeurs trop épris de morale.

Abstract. Background: Cardiovascular diseases are the leading cause of death in Germany. The knowledge of causal risk factors and their distribution is of utmost importance to design screening programs. Probands and methods: In this cross-sectional study design we used STROBE criteria to achieve the highest comparability possible. Anthropometric measures (height and weight), total cholesterol, glucose level, and blood pressure were measured. Probands’ history was collected by using a standardized questionnaire. The data was age- and gender-adjusted for the working population 16 to 70 years of age, derived from the micro census, the 1 %-sample census of the German statistical office. For each study year weight factors were calculated. Logistic regression analysis was conducted regarding the cardiovascular risk factors: smoking, arterial hypertension, diabetes, hypercholesterolemia, and obesity. Results: Between 2006 and 2015 a total of 28,293 employees took part in the ongoing company screenings. The mean age was 42.3 years for both sexes (median: 43 years). The mean body mass index (BMI) was 25.6 kg/m2 (men: 26.5 kg/m2, women: 24.7 kg/m2). A history of hypertension was present in 16 % of the employees (men: 17.8 %, women: 13.8 %). Of the respondents 2 % suffered from diabetes (men: 2.4 %, women: 1.6 %). Lipid-lowering drugs were taken by 2.8 % of all employees (3.6 % men and 1.9 % women). 23.3 % of the men and women indicated to be active smokers. In the regression analysis obesity was associated with a four times higher risk of hypertension and a three times higher risk of elevated glucose levels, thus manifesting as main contributor for vascular diseases. Meanwhile the risk for obesity was 140 % higher in probands who are former smokers. Conclusions: We regard obesity as the number one cardiovascular risk which should be assessed by various medical, legislative, and socio-economic actions to limit future mortality and health-care costs in Germany.

Abstract:
This study offers empirical evidence about how the structure of government and private ownership affected productivity in Chinese firms. It uses the microdata of China's last decennial industrial census, covering all of the 23,000 large and medium-sized industrial firms operating in China during 1995. The results show that government decentralization – ‘federalism’ – played an important role in improving the performance of not just collective firms, but also state-owned and mixed public/private ownership firms. This result is strongly confirmatory of much of the recent theoretical work on transition economies that posits a key role for government in the efficient operation of markets.
