Abstract:
As a species, we appear to be programmed to respond to the situations and emotions of others. However, there is wide variation in the ways doctors and other health professionals experience and express this capacity, and there is a need for effective training to enhance these skills. Unfortunately, systematic reviews suggest that many of our current training programmes do not improve the quality of communication in cancer and palliative care so as to limit the burden of professional burnout, and to improve patients’ mental or physical health and satisfaction. Our attempts to produce a generation of empathic clinical communicators are inconsistent and reviews of patient complaints reveal an increasing discontent with professional communication. So what is missing? How do we develop, sustain, and teach empathic communication? The answer, according to Lim and Dunn, is to shift the focus from empathy to authenticity.

The study of the Roman empire has changed dramatically in the last century. Emphasis is now placed on understanding the experiences of subject populations, rather than focusing solely on the Roman imperial elites. Local experiences, and interactions between periphery and centre are an intrinsic component in our picture of the empire’s function over and against the earlier, top-down model. But where does law fit in to this new, decentralized picture of empire? This volume brings together internationally renowned scholars from legal and historical backgrounds to study the operation of law in each region of the empire from the first century BCE to the end of the third century CE. Regional variation and specificity is explored alongside the emergence of common themes and activities by historical agents. When brought together, a new understanding of law in the Roman empire emerges that balances the practicalities of regional variation with the ideological construct of law and empire.

Scottish nineteenth mortality statistics are unusual in distinguishing death rates and causes grouped by the population size of localities, and also separately for many of the larger towns. Larger settlements tended to have higher death rates than smaller, and from most diseases, and, although these differences declined over time, the major towns of the West Central Belt (and Glasgow above all) show, with a few puzzling exceptions, persistent tendencies throughout our period to higher rates than other urban centres (other at some periods than Dundee). Infant mortality shows similar differences, but it remains hard to explain why Scotland had such relatively low infant mortality in the nineteenth century but so much higher than elsewhere for most of the twentieth. Various suggestions are explored.

Knowing what is normal and what is not is becoming a particular challenge in this era of molecular karyotyping. This chapter reviews the normal chromosome variation from classical times, now very well understood. This is followed by a discussion of the complexity and uncertainty that the molecular approach has, in this century, challenged researchers with. In particular, the chapter discusses the concept of the copy number variant (CNV) and how the harmlessness, or not, of a CNV may be assessed. Mention is made of CNVs potentially acting as “second hits,” such that, while nonpathogenic in one setting, they may contribute to an abnormal phenotype in the context of another, independent chromosome abnormality or CNV. The “sliding scale” of interpretation from “known pathogenic” through “known benign” Is noted. The chapter refers to useful databases to which the counselor may have access.

Abstract:
Concepts stand at the centre of human cognition. We use concepts in categorizing objects and events in the world, in reasoning and action, and in social interaction. It is therefore not surprising that the study of concepts constitutes a central area of research in philosophy and psychology. Since the 1970s, psychologists have carried out intriguing experiments testing the role of concepts in categorizing and reasoning, and have found a great deal of variation in categorization behaviour across individuals and cultures. During the same period, philosophers of language and mind did important work on the semantic properties of concepts, and on how concepts are related to linguistic meaning and linguistic communication. An important motivation behind this was the idea that concepts must be shared, across individuals and cultures. However, there was little interaction between these two research programs until recently. With the dawn of experimental philosophy, the proposal that the experimental data from psychology lacks relevance to semantics is increasingly difficult to defend. Moreover, in the last decade, philosophers have approached questions about the tension between conceptual variation and shared concepts in communication from a new perspective: that of ameliorating concepts for theoretical or for social and political purposes. The volume brings together leading psychologists and philosophers working on concepts who come from these different research traditions.

This chapter proposes that spontaneous variation has a central role in biological evolution, operant conditioning, creative thinking, and personal agency. But to support these advantageous outcomes, this spontaneity must be joined with some selection process or procedure that decides which alleles, behaviors, ideas, or choices are most adaptive or useful. The argument begins with spontaneous variations in evolutionary theory, and then turns to operant conditioning, with emphasis on the origins of spontaneous behaviors. That analysis leads directly to a discussion that introduces a three-parameter definition of both creativity and sightedness, two concepts that provide the foundation for the blind-variation and selective-retention model of creativity. The latter is then linked with the chance-then-choice theory of free will, a linkage that makes spontaneous choice generation the first of two steps leading to personal agency. In all four phenomena, spontaneity is defined as the production of variants in ignorance of their actual utilities.

This chapter provides an overview of the notion ofdiscourse-configurationality, a parametric property of languages in which at least one category of the Information Structural notions Topic and Focus is associated with a particular phrase structure configuration. The chapter clarifies the relation between discourse-configurationality and the concept of (non-)configurationality, and it compares discourse-configurationality to the more inclusive notion of discourse-prominence. A survey of the major parameters in cross-linguistic variation is presented, distinguishing different types of discourse-configurationality both within and across its two main manifestations: namely topic-configurationality and focus-configurationality. The concluding part outlines several prominent theoretical approaches to the syntax of discourse-configurationality, raising issues of grammatical architecture that centre around the hypothesis of the Autonomy of Syntax.

By confronting the singularity of the relationship between two exemplary writers of the last century, this book challenges and reinvigorates our notions of what art and criticism – literary or otherwise – can do. While it takes Roland Barthes’s encounters with Marcel Proust’s monumental masterpiece À la recherche du temps perdu as its specific focus, the implications of its argument are far-reaching. Indeed, the book argues that Barthes’s writing on Proust’s work between the early 1950s and 1980 (including a substantial set of unpublished notes for a seminar delivered at the University of Rabat in 1969–1970) proposes not only a critical culture of Proust that is productively inconsistent, but also, more generally, a fresh understanding of criticism as a creative activity that embraces insecurity and variation as it refuses to remain fixed upon reassuringly stable themes, meanings and interpretations.
