Abstract:

Enteric fever remains the most common febrile illness in urban Nepal. Some individuals may have recurrent infection and some may even progress to become long term chronic carriers. The aim of this thesis was to investigate the rate and factors leading to relapse with typhoid fever in patients who were enrolled in clinical treatment trials for acute enteric fever. The results show that relapses in enteric fever is a common complication and is more likely to be associated with the treatment antimicrobial, cefixime. I have investigated the possibility that specific conalbumin (ovotransferrin) iron saturation levels enable less virulent strains of Salmonella to become more virulent. Iron starved cells of two pathogenic Salmonella strains, S. paratyphi B var. java and S. thompson, were cultured in iron limited media at 3 different iron conalbumin saturation levels. Results indicate that strains differ significantly at both low and high iron saturation conalbumin. These differences depict a growth advantage for S. paratyphi B which correlates with reports by the Centers for Disease Control that S. paratyphi

In this thesis, comparative genomic analysis was used to detect variation within the Typhi and Paratyphi A populations, and to compare the evolution of these two pathogens. There was evidence in both populations of ongoing accumulation of inactivating mutations which result in loss of gene function. Detailed comparison of this functional gene loss in Typhi and Paratyphi A revealed that many of the same genes were inactivated in both serovars, but the mutations occurred independently and were not the result of horizontal transfer of DNA between their genomes. The O—antigens of Salmonella serogroups A, B and D differ structurally in their side-chain sugar residue. The genes encoding 0antigen biosynthesis are clustered in the rfb operon. This thesis describes the molecular cloning and analysis of the rfb genes from S. paratyphi A IMVSl316 (serogroup A) and S. typhi Ty21a (serogroup D). The rfb gene cluster from S. Typhimurium LT2 (serogroup B) has also been studied in our laboratory (Brahmbhatt et al 1986, 1988; P. Wyk, Ph.D. thesis, Univ. of Adelaide, 1988). The genetic basis of O-antigen variation in serogroups A, B and D of Salmonella is dscussed.

I performed two randomised controlled trials (RCTs) to determine the best treatments for enteric fever in Kathmandu, Nepal, an area with a high proportion of nalidixic acid resistant S. Typhi and S. Paratyphi A isolates. I recruited 844 patients with suspected enteric fever to compare chloramphenicol versus gatifloxacin. 352 patients were culture confirmed. 14/175 patients treated with chloramphenicol and 12/177 patients treated with gatifloxacin experienced treatment failure (HR=0.86 (95% CI 0.40 to 1.86), p=0.70). A total of 842 single patient isolates of Salmonella spp. from the New Territories East Cluster hospitals, Hong Kong, were collected during 2002 and 2004. The most common Salmonella enterica serotype isolated was S. Enteritidis (29.7%, 250 of 842) followed by S. Typhimurium (13.7%, 115 of 842).
