Abstract:
Online counselling is a rapidly growing field and, while there is emerging evidence of its comparative effectiveness, there has been little research into what techniques are being applied in practice and which clients will most likely benefit from this medium. Using a focus group methodology, this study examines the experiences of 19 online clinicians employed by a youth mental health service, investigating their perception of online clients, views on their counsellor roles, the approaches and techniques they employ, and the unique aspects of counselling in an online environment. Overall, online clinicians perceived their clients as presenting with highly complex problems and a high level of psychological distress. They noted online clients would most often use the service once or twice, and that some would use online chat as an adjunct to face-to-face counselling. The online clinicians described various roles, including: assessments, gatekeeping, providing emotional support, and therapeutic interventions. According to the online clinicians, they used a variety of techniques online, but favoured person-centred techniques, as these helped keep the clients engaged with the service. Areas of further research and implications for practice are discussed.

Abstract:
Objective: Counselling within educational settings has now become commonplace. As with the advent of the use of new technologies in teaching, such developments are also impacting the broader support structures available to students. One development is the move of schools to offer pupils access to online counselling services. To date, such practices have received little attention, and this paper synthesises the existing empirical research literature in this area. Design: A systematic review of the literature was conducted which focused upon synchronous chat counselling for 11- to 25-year-olds. Method: Key bibliographic databases were searched for relevant papers. These were assessed for relevance and quality prior to being included into the analysis. The final selection of studies was analysed for key themes. Results: In all, 19 papers met the inclusion criteria. The thematic synthesis identified four higher order themes: (1) developing safe and youth-friendly online services, (2) online client characteristics, (3) in-session online processes and (4) session alliance and outcome. Conclusion: These themes outline the varied challenges and opportunities present within this developing practice. Each is initially discussed alongside existing literature related to online counselling before the implications for educational providers are explicitly considered. Ultimately, it is concluded that online counselling for students has much potential, however, those commissioning such services need to be mindful of common pitfalls.

Abstract:
Gambling has seen a significant increase in Italy in the last 10 years and has rapidly become a public health issue, and for these reasons the first National Helpline for Problem Gambling (GR-Helpline) has been established. The aims of this study are to describe the GR-Helpline users’ characteristics and to compare the prevalence rates of the users with those of moderate-risk/problematic gamblers obtained from the national survey (IPSAD 2010-2011). Statistical analysis was performed on data obtained from the counselling sessions (phone/e-mail/chat) carried out on 5,805 users (57.5% gamblers; 42.5% families/friends). This confirms that the problems related to gambling concern not only the gamblers but also their families and friends. Significant differences were found between gamblers and families/friends involving gender (74% of gamblers were male; 76.9% of families/friends were female), as well as age-classes and geographical area. Female gamblers had a higher mean age (47.3 versus 40.2 years) and preferred nonstrategy-based games. Prevalence rates of GR-Helpline users and of moderate risk/problematic gamblers were correlated (Rho = 0.58;p=0.0113). The results highlight the fact that remote access to counselling can be an effective means of promoting treatment for problem gamblers who do not otherwise appeal directly for services.

Abstract:
Background Speech recognition is a technology that enables machines to understand human language. Objective In this study, speech recognition of isolated words from a small vocabulary was applied to the field of mental health counseling. Methods A software platform was used to establish a human-machine chat for psychological counselling. The software uses voice recognition technology to decode the user's voice information. The software system analyzes and processes the user's voice information according to many internal related databases, and then gives the user accurate feedback. For users who need psychological treatment, the system provides them with psychological education. Results The speech recognition system included features such as speech extraction, endpoint detection, feature value extraction, training data, and speech recognition. Conclusions The Hidden Markov Model was adopted, based on multithread programming under a VC2005 compilation environment, to realize the parallel operation of the algorithm and improve the efficiency of speech recognition. After the design was completed, simulation debugging was performed in the laboratory. The experimental results showed that the designed program met the basic requirements of a speech recognition system.

Abstract:
IntroductionMany subfertile couples are diagnosed with (relatively) unexplained subfertility and a good prognosis. National professional guidelines (eg, the Netherlands and UK) advise ‘expectant management (EM)’ for 6–12 months, in which no interaction with healthcare staff is offered. Underpowered studies indicate that face-to-face sex-counselling increases the ongoing pregnancy rates of these couples. In patients with other conditions, web-based interactive educational programmes have the same effect on sexual functioning as face-to-face sex counselling. The ‘Pleasure&Pregnancy randomised controlled trial (RCT)’ will examine in couples with unexplained subfertility and a good prognosis whether a new web-based interactive educational programme results in a higher chance of naturally conceiving an ongoing pregnancy within 6 months as compared with EM.Methods and analysisA multicentre RCT with cost-effectiveness analysis will include heterosexual couples diagnosed with (relatively) unexplained subfertility and a good prognosis in Dutch and Belgian secondary or tertiary fertility clinics. Couples will be randomised between 6 months of EM and 6 months of the Pleasure&Pregnancy-programme. This new web-based interactive educational programme includes eight progressive modules of information (on the biology of conception and pleasurable sex) and sensate focus, couple communication and mindfulness exercises. Couples are offered interaction with their coaches via email and can take part in three moderated chat sessions with peers. The primary outcome of this RCT is the probability of naturally conceiving an ongoing pregnancy within 6 months after randomisation. Secondary outcomes include time-to-pregnancy, live birth rate, costs, sexual functioning and personal and relational well-being. Analysis will be according to intention to treat.Ethics and disseminationThis study has been approved by the Medical Ethical Committees of the Academic Medical Centre (the Netherlands) and the Leuven University Hospital (Belgium). The findings of this RCT will be disseminated through presentations at international scientific meetings and peer-reviewed publications.Trail registration numberNTR5709; Pre-results.

Abstract:
Background and Literature: Technology is slowly but surely finding its way into the therapy room. As such, it is moving from the position of being used to aid therapy, such keeping notes on a computer, to being used to facilitate therapy such as online counselling. The sub-type of online counselling known as internet relay chat is a form of counselling that relies solely upon text alone; having no visual or audial cues to aid the therapy and is what this study focuses on. The therapeutic use of self is a difficult concept to define even 55 years after being first introduced. It is a concept that pervades all therapeutic approaches in some way and can be referred to as the planned use of personality, insights, perceptions, and judgments as part of the therapeutic process. It is therefore an important part of therapeutic work with five types of use of self being identified in the literature: use of personality, use of belief system, use of relational dynamics, use of anxiety and use of self-disclosure. However, prior to completing this study it was unknown as to how this core skill could be translated to internet relay chat. Methodology: Eight participants provided transcripts from 20-minute long internet relay simulated counselling sessions. These sessions were part of a training exercise with participants being students from the University of Manchester who were practising online counselling with each other. This study was a mixed-methods study following an explanatory sequential design. The first quantitative stage was a directed content analysis that coded the transcripts for therapeutic uses of self. The second stage was a qualitative grounded theory analysis that analysed semi-structured interviews that looked at the reasons behind why the uses of self from the first stage were used and their perceived impact. Findings: The directed content analysis found that therapeutic uses of self could indeed be translated within this medium with 53 occurrences of self-disclosure, 45 uses of relational dynamics and 15 uses of personality. There were no examples of use of belief system or use of anxiety within this study. The grounded theory analysis resulted in 463 open codes, which were organised under the core category of therapeutic use of self online. This was succeeded by 7 axial codes which were, Conducting Research, Context of Internet Relay chat, Impact of Using Uses of Self, Patterns of Behaviour, Reasons for Using Uses of Self or Not, Therapeutic Use of Self and Impact of Training Exercise. Discussion and Conclusions: The use of self differed from the way it is reported to be used in the participant's face-to-face work and the compensatory techniques used proved similar to those found in the existing literature. It is recommended that that this is a useful exercise for trainee counselling psychologists to undertake as part of their training. This holds value as not all participants knew what a therapeutic use of self was, despite their importance within therapy. It is also beneficial due to the shift in communication we as a society are experiencing.

Abstract:
Tre stycken teman identifierades i resultatet.
Counselling through online chat has become a source of contact for youths who wants to seek help. Through the service youths can from their cell phones and computer chat with professionals and peers who can give them support to various problems. This study wants to give an overview on the research on the subject made the last ten years on why youths, 13 – 25 years, choose to use counselling through online chat. With a literature review 13 studies matched the inclusion criteria. Three themes were identified in the result. 1) The user, which revolves around what kind of person, problem and attitudes the user has, 2) Anonymity and control and 3) The Answerer, which revolves around the answerer's role. The three themes give an overview on the current research and shows that the users have a different kind of problems and that there are different dimensions of control compared to a physical session.

Abstract:
This master's thesis studies effective intervention in chat-based crisis support. This topic is especially relevant, as children, adolescents and young adults have been becoming more interested in such services in the Czech Republic. The theoretical part provides a window into the field of crisis support and elaborates on details of crisis intervention. It presents selected models, approaches, methods, and techniques of crisis intervention. The thesis also describes social support, effectivity, and effective intervention and connects these issues with the topic of crisis intervention. The main aim of this part is to focus on chat-based crisis intervention and its specific features when concerning children and young adults. The empirical part presents a mixed methods study conducted in the Safety line organization, which operates an important nation-wide hotline for children and young adults. In the study, clients and consultants were asked to evaluate the conversation that had just ended by filling out a questionnaire. The chat records were then analyzed in the next part of the study. Key words chat-based crisis intervention, chat counselling, children, adolescents, young adults, effectivity, effective approaches, Safety line / Child Helpline

AbstractIn the modern world, psychological problems like anxiety and depression are common phenomena in society. Dharma therapy is a Buddhist teaching-based therapeutic intervention which utilizes the Buddhist model of ending suffering to help clients to deal with psychological issues. In many cases, it has been shown to be effective. The present case study illustrates how the client, Mr. Peter Chan, who was suffering from anxiety, benefited from going through the intervention of Dharma Therapy. The article will describe what is Dharma Therapy, Mr. Chan’s psychological issues and how the seven steps of Dharma Therapy helped Mr. Chan to deal with his sufferings. Psychological tests administered before and after the therapeutic intervention showed improvement in all aspects of anxiety.

Abstract:
AbstractThe investigation of the four relief agencies’ organizational models – undertaken by combining analysis of websites, strategic documents and policy guidelines with fieldwork and interviews with NGO staffers – has shown the different ways in which each organization works. Exploration of the different sectors of intervention has highlighted the different roles NGOs want to have not only in the lives of their beneficiaries but more generally in the governance system of their communities. As illustrated in Chap. 10.1007/978-3-030-71143-6_5, the spectrum of activities is quite wide. Save the Children focuses on education and child protection (mainly through psychosocial support) complementary advocacy to secure policy change to enable a better world for children; Oxfam prioritizes ‘giving voice’ to the voiceless, water and sanitation, psychosocial support, legal counselling, combined also with a vigorous advocacy and influencing program to create lasting solutions to injustice and poverty. CARE has a similar focus on voice and empowerment especially for women and girls. Its gender transformative approach informs its work on protection, responses to gender-based violence) distribution of relief items, and, to a lesser extent, water and sanitation. As with Save the Children and Oxfam, CARE sets store by advocacy for policy reforms to end poverty and gender inequality. For its part, MSF operations focused on medical assistance, ranging from primary health care, surgery, mental health and psychosocial support, and medical evacuation. For MSF, belief in the power of témoignage has driven denunciations of those who hinder humanitarian action or divert aid and also critique of the wider disfunctionalities of the humanitarian system itself.

Abstract:
A substantial group of young people experience mental health problems, which impact their educational development and subsequent wellbeing. Of those who do suffer from mental health issues, a minority of these seek appropriate professional assistance. This paucity of help seeking behaviours among young people is a challenge for counsellors. Whereas adults who suffer mental health issues have increasingly turned to the internet for assistance, it is interesting that when young people, whose social lives are increasingly dependent on the communication technologies, are not catered to as much as adults by online counselling. One small online counselling pilot program conducted at a Queensland secondary school for three years (from 2005-2007) (Glasheen & Campbell, 2009) offered anonymous live-time counselling from the school counsellor (via a secure chat room) to students through the school’s website. Findings indicated that boys were more likely to use the service than girls. All participants transitioned to face-to-face counselling, and all reported it was beneficial. This pilot study attested to the potential of online counselling. However, school counselors, as a professional group, have been hesitant to utilise online counselling as part of their service delivery to young people in schools. This chapter concludes by identifying reasons for this reluctance and possible initiatives to increase online support for young people in schools.
