Abstract:
Renal damage secondary to deposition of monoclonal immunoglobulin can occur due to accumulation of either light chains, heavy chains, or both. These include myeloma kidney (cast nephropathy), light-chain and heavy-chain amyloidosis, and light- and heavy-chain deposition disease. Renal damage secondary to deposition of both chains is far less common. In the great majority of these cases the M-component is immunoglobulin G, but the spectrum of renal lesions associated with monoclonal gammopathy is extensive and depends on the physiochemical properties of the immunoglobulin produced.This chapter discusses two important, but less recognized disorders associated with monoclonal immunoglobulins: membranoproliferative glomerulonephritis and acquired Fanconi syndrome.

Investment is no longer a matter of individual savers directly choosing which shares or bonds to buy. Rather, most of their money flows through a ‘chain’: an often extended sequence of intermediaries. What goes on in that chain is of huge importance: the world’s investment managers, who are now almost as well paid as top bankers, control assets equivalent in value to around a year of total global economic output. In Chains of Finance, five social scientists (four of whom have worked in investment management) discuss the ways in which the intermediaries in the chain influence each other, channel the flows of savers’ money, enhance investment decisions, and form audiences for each other’s performances of financially competent selves. The central argument of the book is that investment management is fashioned profoundly by the opportunities and constraints this chain creates. Whether chains constrain or enable, however, they always entangle, tying intermediaries to each other—silently and profoundly shaping the investment management industry. Chains of Finance is a novel analysis that will make students, social scientists, financial professionals and regulators look at the workings of financial markets in a new light. Abstract:
Chapter 5 refines a solution previously proposed to the Gettier Problem focusing on proscribing certain roles for falsehoods in a ‘justification-explaining chain.’ The refinement is partly explained in contrast to Peter D. Klein’s defeasibility theory of knowledge, which focuses on proscribing certain relations of truths to ‘chains of justifiers.’ Klein eventually revised his defeasibility account in order to allow for instances of knowledge that depend on what he calls ‘useful falsehoods.’ His definition of the latter turns out to face counterexamples. An improved definition allows a role for useful falsehoods in justification-explaining chains. It also has the unexpected effect of pointing toward a solution to the Gettier Problem that obviates the need to appeal to either type of chain.
